Background and Objectives: Quantitative flow ratio (QFR) is an angiography-based technique for functional assessment of coronary artery stenosis. This study investigated the response of QFR to different degree of stenosis severity and its ability to predict the positron emission tomography (PET)-defined myocardial ischemia. Methods: From 109 patients with 185 vessels who underwent both 13N-ammonia PET and invasive physiological measurement, we compared QFR, fractional flow reserve (FFR) and instantaneous wave-free ratio (iFR) for the responses to the different degree of anatomical (percent diameter stenosis [%DS]) and hemodynamic (relative flow reserve [RFR], coronary flow reserve, hyperemic stenosis resistance, and stress myocardial flow) stenosis severity and diagnostic performance against PET-derived parameters. Results: QFR, FFR, and iFR showed similar responses to both anatomic and hemodynamic stenosis severity. Regarding RFR, the diagnostic accuracy of QFR was lower than FFR (76.2% vs. 83.2%, p=0.021) and iFR (76.2% vs. 84.3%, p=0.031). For coronary flow capacity (CFC), QFR showed a lower accuracy than iFR (74.1% vs. 82%, p=0.031) and lower discriminant function than FFR (area under curve: 0.74 vs. 0.79, p=0.044). Discordance between QFR and FFR or iFR was shown in 14.6% of cases and was driven by the difference in %DS and heterogeneous distribution of PET-derived RFR and stress myocardial blood flow. Conclusions: QFR demonstrated a similar response to different anatomic and hemodynamic stenosis severity as FFR or iFR. However, its diagnostic performance was inferior to FFR and iFR when PET-derived RFR and CFC were used as a reference.

Background Sepsis-associated encephalopathy (SAE) is characterized by blood–brain barrier (BBB) disruption, neuron apoptosis and infl ammation. Studies show that miR-370-3p can be a new diagnostic biomarker for SAE. Objective This study aims to investigate the role of miR-370-3p in SAE and the underlying mechanism. Result An in vitro model of BBB disruption was established in brain microvascular endothelial cells (BMECs) with lipopolysaccharide (LPS). The expression level of RNAs was tested via RT-qPCR. Protein levels were detected by western blotting. Annexin V/PI double staining and CCK-8 assays were conducted to assess the apoptosis and viability of brain microvascular endothelial cells (BMECs). The migration of BMECs was detected by Transwell assay. BBB permeability analysis was performed to assess the relative permeability coeffi cient of BMECs. The in vitro barrier integrity was detected by trans-epithelial/ endothelial electrical resistance (TEER). The binding between miR-370-3p and SMAD specifi c E3 ubiquitin protein ligase 1 (SMURF1) was identifi ed by luciferase reporter assay. In this study, miR-370-3p was upregulated in LPS-treated BMECs. MiR-370-3p knockdown alleviated LPS-induced BBB disruption and apoptosis of BMECs, as well as recovered cell viability and migration. SMURF1 was directly targeted by miR-370-3p. SMURF1 silencing reversed the eff ects of miR-370-3p defi ciency in LPS-stimulated BMECs. Moreover, miR-370-3p activated the TLR4/MyD88/NF-κB signaling via regulating SMURF1 in LPS-stimulated BMECs. Conclusion MiR-370-3p aggravates BBB disruption and neuron apoptosis by targeting SMURF1 to activate the TLR4/ MyD88/NF-κB signaling in SAE.

Inflammatory bowel disease (IBD), which comprises of Crohn’s disease and ulcerative colitis, is an idiopathic relapsing and remitting disease in which the interplay of different environment, microbial, immunological and genetic factors that attribute to the progression of the disease. Numerous studies have been conducted in multiple aspects including clinical, endoscopy and histopathology for the diagnostics and treatment of IBD. However, the molecular mechanism underlying the aetiology and pathogenesis of IBD is still poorly understood. This review tries to critically assess the scientific evidence at the transcriptomic level as it would help in the discovery of RNA molecules in tissues or serum between the healthy and diseased or different IBD subtypes. These molecular signatures could potentially serve as a reliable diagnostic or prognostic biomarker. Researchers have also embarked on the study of transcriptome to be utilized in targeted therapy. We focus on the evaluation and discussion related to the publications reporting the different approaches and techniques used in investigating the transcriptomic changes in IBD with the intention to offer new perspectives to the landscape of the disease.

Introductions: Pneumothorax is the most frequent severe acupuncture-related adverse event occurring at the GB 21 acupoint. This study used ultrasonography (US) to measure the depth of the GB 21 acupoint in adults and assessed its applicability for ensuring safe acupuncture practices. Materials and methods: A US probe was used to measure the vertical depth from the skin to the pleural line of the apex of the lungs at the right and left GB 21 acupoints. The sex, age, body weight, height, and body mass index of each person were recorded. Student t test and generalized estimating equations were used for statistical analysis. Results: Sixty women and 41 men with a median age of 29 years were included in this study. The depth of GB 21 increased with body weight, height, and body mass index (p < 0.001). The mean depth of GB 21 in men (17.4 mm) was greater than that in women (14.6 mm;p < 0.001). The depth difference between the right and left shoulders was significant in men (p < 0.001) but not in women. Conclusions: US can be used to measure GB 21 acupoint depth. Further control studies on its use to ensure safe clinical practices, including preventing acupuncture-related pneumothorax, are warranted.

Tetanus is a specific infectious disease, which is often associated with catastrophic events such as earthquakes, traumas, and war wounds. The obligate anaerobe Clostridium tetani is the pathogen that causes tetanus. Once the infection of tetanus progresses to an advanced stage within the wounds of limbs, the rates of amputation and mortality increase manifold. Therefore, it is necessary to devise a rapid and sensitive point-of-care detection method for C. tetani so as to ensure an early diagnosis and clinical treatment of tetanus. In this study, we developed a detection method for C. tetani using loop-mediated isothermal amplification (LAMP) assay, wherein the C. tetani tetanus toxin gene was used as the target gene. The method was highly specific and sensitive, with a detection limit of 10 colony forming units (CFU)/ml, and allowed quantitative analysis. While detecting C. tetani in clinical samples, it was found that the LAMP results completely agreed with those of the traditional API 20A anaerobic bacteria identification test. As compared with the traditional API test and PCR assay, LAMP detection of C. tetani is simple and rapid, and the results can be identified through naked-eye observation. Therefore, it is an ideal and rapid point-of-care testing method for tetanus.
