It has been asserted that per diem payment system should be introduced, in place of the current fee-for-service system, for payment of the inpatient services of the geriatric hospitals, Based on the assentation, this study aims at calculating costs and profits per inpatient-day of the geriatric hospitals, and thereby at contributing to the managerial improvement from the both sides of the Government and the hospitals. Relevant data of the three months, May to August, 2002 were collected from the five geriatric hospitals, and per inpatient-day costs and profits were calculated for the three disease groups. Major results and conclusions are as follow : Firstly, total costs per insured inpatient-day of the geriatric hospitals are 65,389 won for dementia (including optimal profit of 3,838 won), 69,730 won for stroke (including optimal profit of 4,117 won), and 70,085 won for other diseases (including optimal profit of 4,134 won). Secondly, the amount of the non-insured costs per inpatient-day occupies 34.5% of the total costs for dementia, 30.3% for stroke, and 30.1% for other diseases. Thirdly, the total amount of the per inpatient-day costs calculated including the optimal profits is, on the average, higher by 12% than the present price level calculated for the current fee-for-service system. This implies that the present price level should rise by 12% when the current fee-for-service payment system be maintained, and Finally, introduction of a sliding-scale payment system should be considered for the inpatient medical management fees for the length of stay over six months or more that are being cut in the claim examination process by the insurance corporation.

The success or failure of the build up of military force makes a significant impact on the survival strategy of an individual country. As the ROK has made every effort to reinforce the weapon system and equipment by investing a couple of billion won so far since 1947, it obtains a remarkable success in various fields. Above all, in addition to deterring NK's military threats in a substantive manner, this investment contributes, to a great degree, to acquiring some state of the art technology and personnel with expertise, vitalizing national economy, developing dual use technology and so forth. However, if we look into the whole process in detail, its substantial efficiency falls down. in comparison with investment cost since the most portion of the military force reinforcement is in conjunction with direct introduction of foreign weapon systems and technology-import production. In particular, if we examine 'Defense Reform 2020' which is now under controversy, it is certain that key weapon systems are primarily dependent upon foreign introduction. In a few words, it makes acquisition of key technologies be merely ideal and only deepens the degree subject to the advanced country in the defense industry. Thus it is of my view that we should set up, even now, general acquisition policy which is to invest the budget intensively on domestic R&D in the long term, and also invest more than one third of defense investment budget in R&D in addition to seeking for the policy centered on not threat-based but capabilities-based acquisition. Further, the Defense Ministry should take advantage of the ROK-US alliance as much as necessary to minimize the security instability that might be caused by the lack of force. Although it results in the occurrence of some difficulties thereby, the Defense Ministry should proceed with aforementioned proposition. Unless doing it, we are not able to enter the circle of advanced defense industry nations forever, leading to falling behind the others in national defense science & technology. It is the right time that we have to change our way of thinking so as to make such an astronomical budget investment give birth to more positive impact to national development.

피부 심재성 진균증의 임상조직병리학적 감별진단

The awareness of classification and differential diagnosis of deep mycoses, particularly systemic infections due to pathogenic fungal infections, is essential for the early detection and appropriate therapy. These systemic pathogenic fungal infections such as histoplasmosis, African histoplasmosis, paracoccidioidomycosis, blastomycosis, are usually endemic in limited areas of continents of America, and Africa. A newly recognized penicilliosis due to Penicillium marneffei, a dimorphic fungus, used to be endemic in Southeast Asia, is now world-wide new threat because the increased numbers of cases have been reported in Australia, France, Italy, the Netherlands, United Kingdom, and the United States. Because of the increased international vistiors to the endemic areas and significant enlargement of the populations existing in immunocompromized state, thus susceptible to systemic mycoses, we, the primary care physicians should be on the alert for the clinicopathologic aspects of the systemic mycoses to make a diagnosis early enough to institute effective therapy.

Tufted Angioma와 유사한 소견을 보인 화농성 육아종

Tufted angiomas are rare slowly progressive vascular lesions found typically in children and young adults, usually involving the trunk and neck. Microscopically, groups of capillary tufts, many uncanalized, were found dispersed at various levels in the dermis. Some authors regarded tufted angioma as a member of lobular capillary hemangiomas. We describe a pyogenic granuloma mimicking acquired tufted angioma on the lip in a 59-year-old woman. We suggest that a term lobular capillary hemangioma$quot; may be preferable in this case.

항공기 노즈 랜딩기어의 쉬미 해석

본 연구를 통하여 항공기 노즈 랜딩기어의 동안정성을 지배하는 운동방정식을 유도하고, 이를 수치적으로 해석하는 프로그램을 개발하였다. 개발된 프로그램은 어떠한 초기조건 및 파라메타의 변화가 착륙장치의 동안정성에 미치는 영향을 해석하는데 매우 유용한다. 노즈 랜딩기어의 운동 방정식을 해석한 결과, 동적 불안정을 유발시키는 중요한 파라메타를 결정하였고, 이 파라메타들을 적적히 조절하면 지배장정식은 안정된 해를 갖게 된다. 그 중 대표적인 파라메타는 K?,K₃,C이다. 해의 두번째 특징으로 발산하지 않는 해 가운데서 쉬미(shimmy)현상을 나타내는 해가 있다. 쉬미현상은 비선형 시스템에서 나타나는 리미트사이클로서 고정된 진폭과 주파수를 갖고 진동하는 해를 뜻한다. 이러한 쉬미현상을 해석한 결과, 조향계통의 프리플레이(free play)가 쉬미 발생에 결정적인 원인이 됨을 확인 하였다. In this study, the equations of motion of an nose landing gear system for shimmy analysis has been derived and then the equations have been solved by numerical method. The developed program in this study is useful to evaluate the effect of the initial conditions and parameters of the nose landing gear on the its dynamic stability. It has been found that the parameters such as K?K₃,C? play the most prominent role on the dynamic stability of the nose landing gear. Shimmy is a kind of limit cycle which can be occurred in the nonlinear dynamic systems. Shimmy has been analyzed numerically in this paper. It turns out that the free play in the steering system is the most important factor to generate shimmy.

조갑진균증에서 Ciclopirox Nail Lacquer의 치료효과

배경 및 목적:조갑진균증의 치료에 있어 국소용 항진균제는 조갑판을 통과하지 못하기 때문에 치료율이 낮은 것으로 알려졌으나 최근 조갑판을 통과하여 조상에서도 높은 치료농도를 유지할 수 있는 국소용 항진균제인 ciclopirox nail lacquer가 개발되었다. 이에 본 저자들은 8% ciclopirox nail lacquer 단독 도포의 조갑진균증에서의 치료 효과와 안전성을 알아 보기 위한 연구를 수행하였다. 방법:표적 조갑판의 감염된 면적의 비율이 50%이하인 경증의 조갑진균증 환자 27명을 평가 대상으로 하였으며 8% ciclopirox nail lacquer 1일 1회 도포요법으로 6개월간 치료하였다. 매 방문시마다 감염면적 비율을 기록하였고 치료 전과 치료 후 진균 검사를 시행하였다. 결과: 1)표적조갑판의 감염 면적이 치료 전 평균 28.9%에서 치료 종료시 4.7%로 통계적으로 유의하게 감소하였다(p<0.001). 2)KOH 직접 도말 검사와 진균 배양 검사에서 모두 음성인 진균학적 완치율은 치료 종료시 77.8%였다. 3)부작용으로는 3명(11.1%)에서 일시적 작열감, 조갑 감입 등을 호소하였다. 결론:경증의 조갑진균증 치료에 있어 8% ciclopirox nail lacquer는 비교적 좋은 치료 효과를 보이며 부작용이 적은 약제로 생각된다. Background: Topical antifungal agents have been ineffective in the treatment of onychomycosis because they could not reach the infected nail bed. 8% ciclopirox nail lacquer is a new topical antimycotic which is characterized by excellent penetration into the nail plate. We performed a study to assess the therapeutic efficacy and safety of 8% ciclopirox nail lacquer in the treatment of onychomycosis. Methods: We assessed 27 onychomycotic patients whose affected area of target nail plate was not more than 50%. The patients were instructed to apply 8% ciclopirox nail lacquer once daily and evaluated monthly for 6 months. Mycological examinations were done before and after the treatment. Overall efficacy, tolerability, cosmetic acceptance and adverse effects were assessed at the end of 6 months' treatment. Results: The mean percentage of affected area decreased from 28.9% to 4.7%. Mycological cure rate was 77.8% at the end of the treatment. Overall efficacy assessed by physician and patients were similar and evaluated as complete cure in 48% and 44%, respectively. Tolerability and cosmetic acceptance were excellent or good in most of the patients. No systemic adverse effects were found, but 3 patients complained of mild prickling sensation or ingrowing toe nail. Conclusions: These data suggest that 8% ciclopirox nail lacquer is safe and effective in the treatment of mild onychomycosis.

고정층에서 GAC를 이용한 페놀화합물의 제거

Adsorption of phenolic compounds from aqueous solution by GAC was studied. Batch tests were carried out to get isotherms. Flow experiments were carried out in a fixed bed. and the operating parameters for the flow reactors are the amount of GAC, concentration of phenolic compounds and liquid flowrate. Adsorption isotherm data are best represented by Freundlich equation and the amounts of equilibrium adsorption by GAC were: o-chlorophenol > p-chlorophenol > phenol. Stirring and aeration in a batch tests increased diffusion coefficient slightly. Breakthrough time deceased as the amount of GAC decreased and liquid flowrate increased. Adsorption data could be predicted by the surface diffusion and pore diffusion models.
