        Investigation of Data Representation Issues in Computerizing Clinical Practice Guidelines in China

        Objectives: From the point of view of clinical data representation, this study attempted to identify obstacles in translating clinical narrative guidelines into computer interpretable format and integrating the guidelines with data in Electronic Health Records in China. Methods: Based on SAGE and K4CARE formulism, a Chinese clinical practice guideline for hypertension was modeled in Protégé by building an ontology that had three components: flowchart, node, and vMR. Meanwhile, data items imperative in Electronic Health Records for patients with hypertension were reviewed and compared with those from the ontology so as to identify conflicts and gaps between. Results: A set of flowcharts was built. A flowchart comprises three kinds of node: State, Decision, and Act, each has a set of attributes, including data input/output that exports data items, which then were specified following ClinicalStatement of HL7 vMR. A total of 140 data items were extracted from the ontology. In modeling the guideline, some narratives were found too inexplicit to formulate, and encoding data was quite difficult. Additionally, it was found in the healthcare records that there were 8 data items left out, and 10 data items defined differently compared to the extracted data items. Conclusions: The obstacles in modeling a clinical guideline and integrating with data in Electronic Health Records include narrative ambiguity of the guideline, gaps and inconsistencies in representing some data items between the guideline and the patient’ records, and unavailability of a unified medical coding system. Therefore, collaborations among various participants in developing guidelines and Electronic Health Record specifications is needed in China.

        The Development of Clinical Document Standards for Semantic Interoperability in China

        Objectives: This study is aimed at developing a set of data groups (DGs) to be employed as reusable building blocks for the construction of the eight most common clinical documents used in China’s general hospitals in order to achieve their structural and semantic standardization. Methods: The Diagnostics knowledge framework, the related approaches taken from the Health Level Seven (HL7), the Integrating the Healthcare Enterprise (IHE), and the Healthcare Information Technology Standards Panel (HITSP) and 1,487 original clinical records were considered together to form the DG architecture and data sets. The internal structure, content, and semantics of each DG were then defined by mapping each DG data set to a corresponding Clinical Document Architecture data element and matching each DG data set to the metadata in the Chinese National Health Data Dictionary. By using the DGs as reusable building blocks, standardized structures and semantics regarding the clinical documents for semantic interoperability were able to be constructed. Results: Altogether, 5 header DGs, 48 section DGs, and 17 entry DGs were developed. Several issues regarding the DGs, including their internal structure, identifiers,data set names, definitions, length and format, data types, and value sets, were further defined. Standardized structures and semantics regarding the eight clinical documents were structured by the DGs. Conclusions: This approach of constructing clinical document standards using DGs is a feasible standard-driven solution useful in preparing documents possessing semantic interoperability among the disparate information systems in China. These standards need to be validated and refined through further study.

        Secure and Fine-grained Electricity Consumption Aggregation Scheme for Smart Grid

        Currently, many of schemes for smart grid data aggregation are based on a one-level gateway (GW) topology. Since the data aggregation granularity in this topology is too single, the control center (CC) is unable to obtain more fine-grained data aggregation results for better monitoring smart grid. To improve this issue, Shen et al. propose an efficient privacy-preserving cube-data aggregation scheme in which the system model consists of two-level GW. However, a risk exists in their scheme that attacker could forge the signature by using leaked signing keys. In this paper, we propose a secure and fine-grained electricity consumption aggregation scheme for smart grid, which employs the homomorphic encryption to implement privacy-preserving aggregation of users’ electricity consumption in the two-level GW smart grid. In our scheme, CC can achieve a flexible electricity regulation by obtaining data aggregation results of various granularities. In addition, our scheme uses the forward-secure signature with backward-secure detection (FSBD) technique to ensure the forward-backward secrecy of the signing keys. Security analysis and experimental results demonstrate that the proposed scheme can achieve forward-backward security of user’s electricity consumption signature. Compared with related schemes, our scheme is more secure and efficient.

        Generalized Stability Criterion for Multi-module Distributed DC System

        The stability issues of a multi-module distributed DC power system without current-sharing loop are analyzed in this study. The physical understanding of the terminal characteristics of each sub-module is focused on. All the modules are divided into two groups based on the different terminal property types, namely, impedance (Z) and admittance (Y) types. The equivalent circuits of each group are established to analyze the stability issues, and the mathematical equations of the equivalent circuits are derived. A generalized criterion for multi-module distributed systems is proposed based on the stability criterion in a cascade system. The proposed criterion is independent of the power flow direction.

        The cytoplasmic loops of AgrC contribute to the quorum-sensing activity of Staphylococcus aureus

        In Staphylococcus aureus, the accessory gene regulator (agr) quorum-sensing system is thought to play an important role in biofilm formation. The histidine kinase AgrC is one of the agr system components and activated by the self-generated auto-inducing peptide (AIP), which is released continuously into the extracellular environment during bacterial growth. The extracellular loops (Extra-loops) of AgrC are crucial for AIP binding. Here, we reported that the cytoplasmic loops (Cyto-loops) of AgrC are also involved in Agr activity. We identified S. aureus ST398 clinical isolates containing a naturally occurring single amino acid substitution (lysine to isoleucine) at position 73 of an AgrC Cyto-loop that exhibited significantly stronger biofilm formation and decreased Agr activity compared to the wild-type strain. A constructed strain containing the K73I point mutation in AgrC Cyto-loop continued to show a growth dependent induction of the agr system, although the growth dependent induction was delayed by about 6 h compared to the wild-type. In addition, a series of strains containing deletion mutants of the AgrC Cyto- and Extra-loops were constructed and revealed that the removal of the two Cyto-loops and Extra-loops 2 and 3 totally abolished the Agr activity and the growth-dependence on the agr system induction. Remarkably, the Extra-loop 1 deletion did not affect the Agr activity. In conclusion, the AgrC Cyto-loops play a crucial role in the S. aureus quorum-sensing activity.

      • Is there chiral correlation between graphitic layers in double-wall carbon nanotubes?

        <P><B>Abstract</B></P> <P>Because of the unique concentric structure, double-wall carbon nanotubes (DWNTs) possess fascinating properties which depend on the respective chirality of both the outer and the inner tubes. In this work, we study the chiral correlations of DWNTs synthesized by chemical vapor deposition on Fe nanoparticles. Contrary to some recent report, the distribution of chiral angle difference between inner and outer tubes in our work agrees with that calculated for all possible configurations. Remarkably, DWNTs with commensurate structures, <I>i.e</I>. outer and inner tubes have same chiral angles, are observed. The mechanism for the formation of DWNTs with approximately commensurate structures is discussed on the basis of layer by layer model. Furthermore, the interactional energies between the inner and outer walls are calculated to address the stability of different DWNT configurations. This work helps understand the growth mechanism of DWNTs and comprehend their structure stabilities with different configurations.</P> <P><B>Graphical abstract</B></P> <P>Double-wall carbon nanotubes (DWNTs) synthesized by chemical vapor deposition show randomly distributed chiral angle differences between inner and outer tubes.         LDL Coating pVEGF/polyethylenimine Complex Enhances Vascular Endothelial Growth Factor Expression

        The major limitations to non-viral gene delivery are relatively low efficiency and cytotoxicity, which need to be addressed in the design of new vectors. In this study,negatively charged low density lipoproteins (LDL) were coated onto positively charged pVEGF/PEI complexes to form pVEGF/PEI/LDL terplexes by a two-step procedure. The biocompatible LDL was introduced to reduce the cytotoxicity of the gene delivery system and increase its affinity to cells. The successful formation of pVEGF/PEI/LDL terplexes was confirmed by their near-neutral and slightly negative surface charges. The pVEGF/PEI/LDL terplexes were well-defined sub-micron spherical particles. On the cell viability assay, both of the PEI/LDL combined vector and pVEGF/PEI/LDL terplexes exhibited much lower cytotoxicity to HeLa cells and HUVE cells than those of PEI and pVEGF/PEI complexes, attributed to the shielding effect of the LDL. pEGFP/PEI/LDL terplexes showed significantly higher transfection efficiency in comparison to pEGFP/PEI complexes in serum-containing medium. pVEGF/PEI/LDL terplexes at their optimal N/P ratio and LDL/PEI weigh ratio induced higher expression levels of VEGF protein in HUVE cells than those of pVEGF/PEI complexes. Therefore, the pVEGF/PEI/LDL terplexes could be used as a promising gene delivery system to enhance VEGF protein expression.

        20(S)-ginsenoside Rh2 induces caspase-dependent promyelocytic leukemia-retinoic acid receptor A degradation in NB4 cells via Akt/Bax/caspase9 and TNF-α/caspase8 signaling cascades

        Background: Acute promyelocytic leukemia (APL) is a hematopoietic malignancy driven by promyelocytic leukemia―retinoic acid receptor A (PML-RARA) fusion gene. The therapeutic drugs currently used to treat APL have adverse effects. 20(S)-ginsenoside Rh2 (GRh2) is an anticancer medicine with high effectiveness and low toxicity. However, the underlying anticancer mechanisms of GRh2-induced PML-RARA degradation and apoptosis in human APL cell line (NB4 cells) remain unclear. Methods: Apoptosis-related indicators and PML-RARA expression were determined to investigate the effect of GRh2 on NB4 cells. Z-VAD-FMK, LY294002, and C 87, as inhibitors of caspase, and the phosphatidylinositol 3-kinase (PI3K) and tumor necrosis factor-α (TNF-α ) pathways were used to clarify the relationship between GRh2-induced apoptosis and PML-RARA degradation. Results: GRh2 dose- and time-dependently decreased NB4 cell viability. GRh2-induced apoptosis, cell cycle arrest, and caspase3, caspase8, and caspase9 activation in NB4 cells after a 12-hour treatment. GRh2-induced apoptosis in NB4 cells was accompanied by massive production of reactive oxygen species, mitochondrial damage and upregulated Bax/Bcl-2 expression. GRh2 also induced PML/PML-RARA degradation, PML nuclear bodies formation, and activation of the downstream p53 pathway in NB4 cells. Z-VAD-FMK inhibited caspase activation and significantly reversed GRh2-induced apoptosis and PML-RARA degradation. GRh2 also upregulated TNF-a expression and inhibited Akt phosphorylation. LY294002, an inhibitor of the PI3K pathway, enhanced the antitumor effects of GRh2, and C 87, an inhibitor of the TNF-α pathway, reversed NB4 cell viability, and GRh2-mediated apoptosis in a caspase-8-dependent manner. Conclusion: GRh2 induced caspase-dependent PML-RARA degradation and apoptosis in NB4 cells via the Akt/Bax/caspase9 and TNF-α/caspase8 pathways.
