        Human milk is the best source of nutrients for the human infant, and its nutritional and immunologic benefits are now established, In spite of these advantages, the incidence and duration of breast feeding- is decreasing in many part of the world. The purpose of the study is to collect the data on breast feeding practice in rural and urban area of Chullanamdo, Korea and to follow the trend toward breast feeding in fuutre. Between July 1981 and January 1982, 674 mothers with the children under 2 years of age have been interviewed at Out-Patient Department. The breast feeding situation at the time of interviewed was recorded. The percentage of wholly breast-fed in the age group of 0~1, 1-2, 2~3 ,3~4, 4~5, 5~6, 6-12 and 12~24 months were 61.8%, 50.0%, 51.6%, 65.2%, 50.0%, 45.7%, 23.0% and 17.3% respectively. And the percentage of partially breast-fed in the age group of 0~1, 1~2, 2~3, 3~4, 4~5, 5~6, 6~12, and 12~24 months were 29.4%, 33.3%, 25.8%, 17.4%, 34.2%, 39.1%, 8.6% and 80.0% respectively, In the incidence of wholly breast feeding, there was no correlation between deliveries in hospital and deliveries at home attended by midwives or traditional midwives. The most frequent reasons for partially breast-fed in the age group of 0~1, 1~2, 2~3, and 3~4 months were not enough breast milk.

        Phase II trial of preoperative sequential chemotherapy followed by chemoradiotherapy for high-risk gastric cancer

        <P><B>Abstract</B></P> <P><B>Background and purpose</B></P> <P>To evaluate the safety and efficacy of preoperative chemotherapy (CTx) followed by chemoradiotherapy (CCRT) for high-risk gastric cancer (GC).</P> <P><B>Methods and materials</B></P> <P>The inclusion criteria were as follows: (1) Borrmann type 4; (2) large Borrmann type 3 (≥8 cm); (3) single bulky (≥3 cm × 1) or multiple lymph nodes (≥1.5 cm × 3). Patients received two 21-day courses of induction CTx of TS-1 (35 mg/m<SUP>2</SUP>, p.o, twice daily on days 1–14), docetaxel (30 mg/m<SUP>2</SUP>, i.v., days 1 and 8), and cisplatin (30 mg/m<SUP>2</SUP>, i.v., days 1 and 8) followed by CCRT (two courses of TS-1 and cisplatin in combination with 45 Gy radiation).</P> <P><B>Results</B></P> <P>Forty-two patients were enrolled between March 2014 and February 2016, and 39 of these completed sequential CTx and CCRT. Among the 33 patients who underwent R0 resection, the pathologic response rate was 39.4% [no residual carcinoma (<I>n</I> = 5, 15.2%), with 1–10% residual carcinoma (<I>n</I> = 8, 24.2%)]. Overall, 4 patients (12.1%) were pathologic stage 0, 7 (21.2%) were stage I, 10 (30.3%) were stage II, and 12 (36.4%) were stage III. The overall survival rate at 3 years was 77.9% for stages 0 and I, 66.8% for stages II–III, and 33.3% for unresectable cases (<I>P</I> = 0.001). Toxicity was mild to moderate with grade 4 neutropenia (<I>n</I> = 1) and neutropenic fever (<I>n</I> = 1) as the most prominent side-effects.</P> <P><B>Conclusions</B></P> <P>Sequential CTx and CCRT prior to surgery are feasible and effective for high-risk GC.</P> <P>Trial registration number: NCT02495493.</P> <P><B>Highlights</B></P> <P> <UL> <LI> A complex preoperative staging was adopted for high-risk GC. </LI> <LI> We adopted an intensive, weekly-split, triplet regimen with favorable toxicities. </LI> <LI> Induction chemotherapy and chemoradiotherapy indicate a promising outcome. </LI> </UL> </P>

        Cognitive and Behavioral Outcomes of School-aged Children Born Extremely Preterm: a Korean Single-center Study with Long-term Follow-up

        Background: School-aged children born very preterm have been suggested to have worse cognitive and behavioral outcomes than children born full-term. Executive function (EF) is a higher level of cognitive function related to academic achievement. The present study aimed to evaluate the cognitive (including EF) and behavioral outcomes of Korean children born extremely preterm (EP) and to analyze any biological or socioeconomic risk factors for poor cognitive outcomes in this population. Methods: A total of 71 infants weighing < 1,000 g at birth or born before 30 weeks of gestation (EP group) who were admitted to the neonatal intensive care unit from 2008 to 2009 were included in this study and compared with 40 term-birth controls. The Korean Wechsler Intelligence Scale for Children-Fourth Edition, Advanced Test of Attention (ATA), Stroop test, Children's Color Trails Test (CCTT), and Wisconsin Card Sorting Test (WCST) were used. Additionally, the Korean Child Behavior Checklist (K-CBCL) and Korean ADHD Rating Scale (K-ARS) were completed. Perinatal and demographic data were collected and analyzed. Results: The mean full-scale intelligence quotient (FSIQ) score in the EP group was significantly lower than that of the term control group (89.1 ± 18.3 vs. 107.1 ± 12.7; P < 0.001). In the EP group, 26 (37%) children had an FSIQ score below 85, compared to only one child (3%) in the control group. Furthermore, the EP group showed significantly worse EF test results (ATA, Stroop test, CCTT, WCST). Except for the higher social immaturity subscore in the EP group, the K-CBCL and K-ARS scores were not different between the two groups. EP children who received laser treatment for retinopathy of prematurity (ROP) had an 8.8-fold increased risk of a low FSIQ score, and a 1-point increase in the discharge weight Z-score decreased the risk of a low FSIQ score by approximately half in this EP cohort. Conclusion: This is the first Korean study to investigate the cognitive and behavioral outcomes of school-aged children born EP. In the study cohort, EP children exhibited significantly lower FSIQ scores and EF than their full-term peers, and 37% of them had cognitive problems. Nonetheless, except for social immaturity, the behavioral problems were not different in EP children. Severe ROP and low discharge weight Z-score were identified as independent risk factors for low FSIQ score after adjusting for birth weight.

        Polyurethane-silica hybrid foams from a one-step foaming reaction, coupled with a sol-gel process, for enhanced wound healing

        <P><B>Abstract</B></P> <P>Polyurethane (PU)-based dressing foams have been widely used due to their excellent water absorption capability, optimal mechanical properties, and unequaled economic advantage. However, the low bioactivity and poor healing capability of PU limit the applications of PU dressings in complex wound healing cases. To resolve this problem, this study was carried out the hybridization of bioactive silica nanoparticles with PU through a one-step foaming reaction that is coupled with the sol-gel process. The hybridization with silica did not affect the intrinsically porous microstructure of PU foams with silica contents of up to 10wt% and where 5–60nm silica nanoparticles were well dispersed in the PU matrix, despite slight agglomerations. The incorporated silica enhanced the mechanical performance of PU by proffering better flexibility and durability as well as maintaining good water absorption capabilities and the WVTR characteristics of pure PU foam. The silica of PU-10wt% Si foams was gradually dissolved and released under physiological conditions during a 14-day immersion period. The <I>in vitro</I> cell attachment and proliferation tests showed significant improvements in terms of the biocompatibility of PU-Si hybrid foams and demonstrated the effects of silica on cell growth. More significantly, the superior healing capability of PU-Si as a wound dressing in comparison to PU-treated wounds was verified through <I>in vivo</I> animal tests. Full-thickness wounds treated with PU-Si foams exhibited faster wound closure rates as well as accelerated collagen and elastin fiber regeneration in newly formed dermis, which was ultimately completely covered by a new epithelial layer. It is clear that PU-Si hybrid foams have considerable potential as a wound dressing material geared for accelerated, superior wound healing.</P> <P><B>Highlights</B></P> <P> <UL> <LI> PU-silica hybrid foams successfully fabricated using a one-step foaming reaction involving the sol-gel process. </LI> <LI> PU-silica hybrid foams with up to 10wt% silica, the hybrid foams exhibited significantly improved mechanical performance. </LI> <LI> Regardless of silica contents, hybrid foams maintain water absorption capabilities of pure PU. </LI> <LI> PU-silica hybrid foams have significantly enhanced biocompatibility and wound healing capabilities compared to pure PU foams. </LI> </UL> </P>

        Regulation of mouse steroidogenesis by WHISTLE and JMJD1C through histone methylation balance

        <P>The dynamic exchange of histone lysine methylation status by histone methyltransferases and demethylases has been previously implicated as an important factor in chromatin structure and transcriptional regulation. Using immunoaffinity TAP analysis, we purified the WHISTLE-interacting protein complexes, which include the heat shock protein HSP90α and the jumonji C-domain harboring the histone demethylase JMJD1C. In this study, we demonstrate that JMJD1C specifically demethylates histone H3K9 mono- and di-methylation, and mediates transcriptional activation. We also provide evidence suggesting that both WHISTLE and JMJD1C performs functions in the development of mouse testes by regulating the expression of the steroidogenesis marker, <I>p450c17</I>, via SF-1-mediated transcription. Furthermore, we demonstrate that WHISTLE is recruited to the <I>p450c17</I> promoter via SF-1 and represses the transcription of prepubertal stages of steroidogenesis, after which JMJD1C replaces WHISTLE and activates the expression of target genes via SF-1-mediated interactions. Our results demonstrate that the histone methylation balance mediated by HMTase WHISTLE and demethylase JMJD1C perform a transcriptional regulatory function in mouse testis development.</P>

      • Monodisperse Microshell Structured Gelatin Microparticles for Temporary Chemoembolization

        <P>Embolization is a nonsurgical, minimally invasive procedure that deliberately blocks a blood vessel. Although several embolic particles have been commercialized, their much wider applications have been hampered owing mainly to particle size variation and uncontrollable degradation kinetics. Herein we introduce a microfluidic approach to fabricate highly monodisperse gelatin microparticles (GMPs) with a microshell structure. For this purpose, we fabricate uniform gelatin emulsion precursors using a microfluidic technique and consecutively cross-link them by inbound diffusion of glutaraldehyde from the oil continuous phase to the suspending gelatin precursor droplets. A model micromechanic study, carried out in an artificial blood vessel, demonstrates that the extraordinary degradation kinetics of the GMPs, which stems from the microshell structure, enables controlled rupturing while exhibiting drug release under temporary chemoembolic conditions</P>

        Objective : The purpose of this study was to investigate the anti-cancer effects of Carthami Flos in some kinds of human colorectal adenocarcinoma cells. Method : We used two kinds of human colorectal adenocarcinoma cell lines, such as HT-29 and WiDr cells. We examined cell death by MTT assay and observed the morphological changes with Carthami Flos. Result : We showed that the combination of sub-optimal doses of Carthami Flos and cisplatin noticeably suppresses in HT-29 cells and doxorubicin in WiDr cells. Furthermore, we studied the caspase 3 activity to identify the apoptosis. Conclusion : Our findings provide insight into unraveling the effects of Carthami Flos in human colorectal adenocarcinoma cells and developing therapeutic agents against colorectal cancer.

        Human Organic Anion Transporting Polypeptide 1B3 Applied as an MRI-Based Reporter Gene

        Objective: Recent innovations in biology are boosting gene and cell therapy, but monitoring the response to these treatments is difficult. The purpose of this study was to find an MRI-reporter gene that can be used to monitor gene or cell therapy and that can be delivered without a viral vector, as viral vector delivery methods can result in long-term complications. Materials and Methods: CMV promoter-human organic anion transporting polypeptide 1B3 (CMV-hOATP1B3) cDNA or CMV-blank DNA (control) was transfected into HEK293 cells using Lipofectamine. OATP1B3 expression was confirmed by western blotting and confocal microscopy. In vitro cell phantoms were made using transfected HEK293 cells cultured in various concentrations of gadoxetic acid for 24 hours, and images of the phantoms were made with a 9.4T micro-MRI. In vivo xenograft tumors were made by implanting HEK293 cells transfected with CMV-hOATP1B3 (n = 4) or CMV-blank (n = 4) in 8-week-old male nude mice, and MRI was performed before and after intravenous injection of gadoxetic acid (1.2 μL/g). Results: Western blot and confocal microscopy after immunofluorescence staining revealed that only CMV-hOATP1B3-transfected HEK293 cells produced abundant OATP1B3, which localized at the cell membrane. OATP1B3 expression levels remained high through the 25th subculture cycle, but decreased substantially by the 50th subculture cycle. MRI of cell phantoms showed that only the CMV-hOATP1B3-transfected cells produced a significant contrast enhancement effect. In vivo MRI of xenograft tumors revealed that only CMV-hOATP1B3-transfected HEK293 tumors demonstrated a T1 contrast effect, which lasted for at least 5 hours. Conclusion: The human endogenous OATP1B3 gene can be non-virally delivered into cells to induce transient OATP1B3 expression, leading to gadoxetic acid-mediated enhancement on MRI. These results indicate that hOATP1B3 can serve as an MRI-reporter gene while minimizing the risk of long-term complications.
