        전신진동운동이 건강한 성인의 가자미근 신경-근 활성화에 미치는 영향

        The purpose of this study was to investigate the effects of vibration frequency during whole-body vibration exercise on the soleus H-reflex in healthy adults. Fourteen Healthy adults was participated in this study. They performed whole-body vibration exercises in 4 vibration intensity conditions (No vibration, 10Hz 4 mm, 20Hz 4 mm and 27Hz 4 mm). The soleus H-reflex of right side was measured during the whole-body vibration exercise. The amplitude of H-reflex and M-wave, H/Mmax ratio were compared with one-way repeated measures of ANOVA (p<0.05) among four different vibration frequency. There were the significant differences in the amplitude of H-reflex according to vibration frequency (F=15.802, p<0.001). H-reflex was significantly reduced at 10Hz (p<0.01), 20Hz (p<0.001) and the 27Hz (p<0.01) than at No vibration. There were the significant differences in H/Mmax ratio according to vibration frequency (F=14.910, p<0.001). H/Mmax ratio was significantly reduced at 10Hz (p<0.05), 20Hz (p<0.01) and the 27Hz (p<0.01) than at No vibration. This study suggest that it might be not effective to apply an the vibration frequency of 10Hz or more during whole-body vibration exercise for proceeding of neuro-muscular activity in healthy adults.

        전신진동운동 시 진동자극강도가 뇌졸중 편마비 환자의 가자미근 H-reflex에 미치는 영향

        [PURPOSE] The purpose of this study was to investigate the acute effects of vibration stimulation intensity during whole-body vibration exercise on the soleus H-reflex in hemiplegic stroke patients. [METHODS] Sixteen hemiplegic stroke patients participated in this study and performed whole-body vibration exercises in 4 different vibration stimulation conditions (0Hz 0 mm, 10Hz 4 mm, 20Hz 4 mm and 27Hz 4 mm). The soleus H-reflex was measured at hemiplegic side during the whole-body vibration exercise. The amplitude of H-reflex, M-wave and H/Mmax ratio, were compared with one-way repeated measurement analysis of ANOVA (p<0.05) among four different vibration stimulation intensity. [RESULTS] There were the significant differences in the amplitude of H-reflex according to vibration stimulation intensity (F=9.295, p=0.000). H-reflex was significantly lower at 20Hz (p=0.015) and the 27Hz (p=0.006) than at 0Hz. The amplitude of M-wave was similar in all vibration stimulation intensities (F=1.628, p=0.184). There were the significant differences in the amplitude of H/Mmax ratio according to vibration stimulation intensity (F=9.921, p=0.000). H/Mmax ratio was significantly lower at 20Hz (p=0.006) and the 27Hz (p=0.001) than at 0Hz. [CONCLUSIONS] The results of this study suggest that it might be effective to apply an the vibration stimulation intensity of at least 20Hz or more during whole-body vibration exercise for improvement of neural control ability in hemiplegic stroke patients. Whole-body vibration is most recommend to hemiplegic stroke patients exercise method which provide a positive effect on improvement of neural control ability. [목적] 본 연구의 목적은 뇌졸중 편마비 환자를 대상으로 진동운동 수행 시 다양한 진동자극강도에서 가자미근(soleus muscle)의 H-reflex(호프만반사)를 측정하여 뇌졸중 환자들의 신경활성화에 긍정적인 영향을 미치는 최적화된 진동 자극강도를 알아보는 것이다. [방법] 본 연구에 참가한 대상자는 진동운동 수행과 H-reflex 측정이 가능한 뇌졸중 환자 16명이다(남=12명: 49.3±11.3yrs, 168.1±7.8cm, 69.3±12kg; 여=4명: 54.8±8.2yrs, 156.4±4.4cm, 59.7±9.8kg). 모든 대상자들은 진동운동 수행 중 다양한 진동자극강도(0Hz 0 mm, 10Hz 4 mm, 20Hz 4 mm, 27Hz 4 mm)에서 가자미근의 H-reflex를 측정하였다. 4가지 다른 진동자극강도에서 H-reflex 최대 진폭, M-wave 최대 진폭 및 H/Mmax 비율을 비교하기 위하여 일원반복측정분산분석을 적용하였다. [결과] H-reflex의 최대 진폭은 진동강도에 따른 유의한 차이가 나타났다(F=9.295, p=0.000). H-reflex는 진동자극강도 20Hzz가 0Hz 보다 통계적으로 유의하게 낮았으며(p=0.015), 27Hz가 0Hz 보다 통계적으로 유의하게 낮았다(p=0.006). M-wave는 모든 진동자극강도에서 유의한 차이가 나타나지 않았다(F=1.628, p=0.184). H/Mmax 비율은 진동강도에 따른 유의한 차이가 나타났다(F=9.921, p=0.000). H/Mmax 비율은 진동자극강도 20Hz가 0Hz 보다 통계적으로 유의하게 낮았으며(p=0.006), 27Hz가 0Hz 보다 통계적으로 유의하게 낮았다(p=0.001). [결론] 본 연구의 결과를 통해 뇌졸중 편마비 환자들의 전신진동운동 시 20Hz 이상의 강도를 적용하는 것은 신경 조절 능력 개선에 효과적인 방안이 된다는 것을 제안한다. 결론적으로 전신진동운동은 뇌졸중 편마비 환자들의 신경조절 능력 개선에 효과적인 운동방안으로 권장된다.

        일시적인 전신진동운동이 준비운동으로서 효과

        The purpose of this study was attempted to quantify the acute effect of intermittent whole body vibration exercise for the effects of warm-up. The subjects of this study were 20 university students were participated. Subjects performed 6 set of vibration exercise that have 1 min vibration exercise(40Hz, ±5mm) and 2min rest, maintaining an half-squat position on vibratory platform. vertical jump and whole body reaction time test were administered before and after each vibration exercises. One-way analysys of variance(ANOVA) were performed to see the differences of jump height and reaction time between pre and post exercise sets. The result showed an increase(not significant) in vertical jump height when exercise time was further, and maximum height was found at after 5set. After 3 or 4set vibration exercise Significantly higher than pre(p<.001), and over 5 or 6set were none. The results indicated that whole body vibration may be a potential warm-up procedure for increasing vertical jump height and whole body reaction time, but optimum exercise time were important factor. Future research is warranted addressing the influence of various protocols of duration and frequency on whole-body vibration exercise.

        전신진동운동이 APPsw/PS2m 알츠하이머 질환 모델동물 해마의 자식작용에 미치는 영향

        The purpose of this study is to investigate whether whole body vibration is effective in improving Alzheimer’s disease through autophagy and cell death control. Experimental animals were classified into a 24-month-old Alzheimer’s disease model mouse comparison group (SED-Tg) a treadmill exercise group (TE-Tg), and a whole-body vibration exercise group (VE-Tg, n=8) for 5 minutes of exercise, 11m/min for the next 5 minutes, and 14m/min for the last 20 minutes, and the whole-body vibration was performed at a speed of 4Hz for 5 minutes, 6Hz for 5 minutes, and 8Hz for 12 weeks for 30 minutes for 5 days. Analysis was performed by watermaze test, Rota-rod test, serum analysis, protein analysis, and immunohistochemistry. As a result, the behavioral test showed a difference in cognitive function improvement, and the serum analysis showed a difference in decreasing creatin kinase and lactate dehydrogenase. In protein analysis, Aβ42 level was reduced and there was a significant difference in the indicators related. BNIP3 and LC3-2. In addition, there was a difference in the p-AKT/AKT ratio, which is an indicator of the signal cutting path. Taking the results of this study together, it is considered that whole-body vibration can be effectively used to improve Alzheimer’s disease as child action and cell death are controlled through whole-body vibration.

        단기간의 슬링운동 시 전신진동의 적용이 근 기능 회복에 미치는 영향

        The purpose of this study was to investigate for an effective recovery method on muscle function maximal isotonic exercise. 36 adult males were selected for this study. The each group were divided into three groups. Sling exercise group(SG, n=12), sling exercise with all time whole body vibration group(n=12, SAVG) and sling therapy with intermittent time whole body vibration group(n=12, SIVG) were randomly composed. Sling exercise therapy using supine position and whole body vibration of 10Hz, 5mm were performed. After 15 minutes, sling with whole body vibration was conducted in two groups, respectively. Surface electromyography(EMG) was used to measure muscle activation levels of lliocostalis thoracis and multifidus. Also isokinetic of lumber joint extension and flexion peak torque(N/m), total work(J) and average power(W) was measured using biodex system 3 dynamometer. The recovery method of sling with whole body vibration showed positive changes in physical characteristics(muscle function). EMG results show that SIVG was effectively increased compared with SG and SAVG. Also isometric contraction of peak torque(Nm), average power(watts) and total work(J) was significantly increased in recovery step for SIVG. 본 연구의 목적은 최대운동 후 효과적인 회복방법을 통해 근 기능 변화를 비교 분석하고자 하였다. 36명의 20대 성인남성을 대상으로 슬링과 진동제공 시간에 따라 슬링그룹(n=12), 슬링 및 지속적 진동그룹(n=12), 슬링 및 간헐적 진동그룹(n=12)을 무작위로 선정하였다. 슬링운동은 양외위자세로 진행하였으며, 진동강도는 10Hz, 5mm로 설정하였다. 지속적 진동 그룹은 15분간 진동을 제공하였고, 간헐적인 진동 그룹은 1분씩 총7회를 제공하였다. 표면근전도(EMG)를 활용하여 흉장늑근(iliocostalis thoracis) 및 다열근(multifidus)을 측정하였다. 또한, 등속성 근기능 평가는 요추관절 굴곡 신전 시 peak torque(N/m), total work(J) 및 average power(W)를 측정하여 비교분석 하였다. 근전도 비교분석 결과 간헐적인 진동자극을 슬링과 복합적으로 제공한 그룹에서 안정 시, 운동직후(피로유발), 회복구간에서 근 활성도가 좀 더 높은 것으로 나타났다. 또한, 등속성 근기능 평가결과에서도 진동을 지속적으로 제공한 그룹보다 간헐적으로 제공받은 그룹에서 최대토트(N/m), 총일량(J) 및 평균파워(W)의 안정 시와 회복 후 비교에서 근기능이 좀 더 향상되는 효과적인 방법으로 나타났다.

        준비운동으로서의 Whole-Body Vibration Exercise가 슬관절 등속성 근기능에 미치는 영향

        본 연구는 준비운동으로서의 전신진동운동이 등속성 근기능에 미치는 영향을 조사하는데 있다. 이를 위해 K대학에 재학 중인 남학생 12명(21.50±0.85)을 모집하여 운동부하검사(Bruce protocol, Treadmill)를 통해 피험자의 최대산소섭취량(VO2max)을 측정한 후, Treadmill, Squat, WBVE(Whole-Body Vibration Exercise)운동 방법을 이용하여 VO2max30%에 도달했을 때 하지 근 기능검사를 실시하였다. 본 연구의 결과 우측 슬관절 180°/sec 등속성 신전근력에서 집단 간 차이가 나타났으며(p<.05), 사후검증 결과 WBVE가 Control에 비해 12.99% 높은 것으로 나타났다(p<.01). 좌측 슬관절 180°/sec 등속성 신전근력에서도 집단 간 차이가 나타났다 (p<.01). 사후검증 결과 WBVE가 Control에 비해 16.01% 높게 나타났고(p<.01), Squatting이 Control에 비해 11.99% 높은 것으로 나타났다(p<.05). 목표산소섭취량(VO2max30%) 도달 시 심박수는 집단 간 유의한 차이가 나타났으며(p<.05), 사후검증 결과 Squatting이 Treadmilll에 비해 10.14% 높은 것으로 나타났다(p<.01). 도달시간의 차이는 집단 간 유의한 차이가 나타났으며(p<.001), 사후검증 결과 Treadmill이 Squatting과 WBVE에 비해 각각 77.86%, 107.50% 높은 것으로 나타났다(p<.001). 이상의 결과에 비추어 볼 때, 전신진동운동은 순발력을 요하는 종목의 준비운동으로 활용될 수 있을 것이다. 또한 정규 훈련 프로그램이나 기존의 준비운동에 추가적으로 전신진동운동을 실시한다면 선수들의 운동 수행력을 향상시킬 수 있다고 사료된다. The purpose of this study was to investigate the effects of whole body vibration exercise, as warning-up method, on isokinetic muscle function. Participants were 12 male students of K college(21.50±0.85). We measured VO2max(maximal oxygen uptake) by Quinton metabolic cart using Bruce protocol, Treadmill and they performed Treadmill, Squat, WBVE(Whole-Body Vibration Exercise) then lower extremity muscle strength was tested concordance with criterion of 30% VO2max. There were significant differences in right knee extensor strength at 180°/sec among groups(p<.05) and as a result of post hoc test, strength of right knee extensor in WBVE were higher than Control(12.99%, p<.01). There were significant differences in left knee extensor strength at 180°/sec among groups(p<.01), and strength of left knee extensor in WBVE(16.01% p<.01) and squatting(11.99%, p<.05) were higher than Control. There were significant differences in HR(heart rate) of 30% VO2max among groups(p<.05), and Squatting were higher than Treadmill in post hoc test(10.14%, p<.01). There were significant differences in reached time to 30% VO2max among groups(p<.001), and Treadmill were higher than Squatting(77.86%, p<001) and WBVE(107.50%, p<001). In conclusion, whole body vibration exercise could be used as warming-up method in sports requiring muscle power. Also, additional whole body vibration exercise in existed warming-up methods or regular training program may help to improve the performance of players.

      • 고령자용 인지 및 신체기능 증진을 위한 진동기반 원코스 운동 플랫폼 개발

        With the acceleration of the aged society, the number of patients with various physical and mental degenerative diseases is increasing. In order to prevent this, various studies are being conducted on exercise methods and devices that can enhance cognition and physical function in the elderly for the purpose of preventing diseases. However, prior research on the development of a vibration-based exercise protocol and system that maximizes the exercise effect without putting any burden on the human body is insufficient. Therefore, in this study, a one-course exercise system based on whole-body vibration was developed for the purpose of improving the cognitive and physical functions of the elderly. One-course exercise system based on sonic vibration consisting of ball-type, dumbbell-type, and mat-type exercise equipment for warm-up exercise, plate-type exercise equipment for main exercise, and chair-type exercise equipment for finishing exercise and exercise that coordinates recognition and body functions by stage A program was developed to present the protocol to the monitor. All devices can be operated in conjunction with the smart app, and the exercise program is linked to the frequency and intensity of the whole body vibrating device for each unit exercise and presented to the user through the monitor. In the future, we are developing an exercise service platform through the development of a web server and Android app with an exercise trainers. In addition, we plan to conduct a study to verify the effectiveness of the vibration-based one-course exercise service platform for cognitive and physical functions for the elderly.

      • ELSEVIER : Irisin in response to acute and chronic whole-body vibration exercise in humans

        Objective. Irisin is a recently identified myokine, suggested to mediate the beneficial effects of exercise by inducing browning of white adipocytes and thus increasing energy expenditure. In humans, the regulation of irisin by exercise is not completely understood. We investigated the effect of acute and chronic whole-body vibration exercise, a moderateintensity exercise that resembles shivering, on circulating irisin levels in young healthy subjects. MaterialslMethods. Healthy untrained females participated in a 6-week program of wholebody vibration exercise training. Blood was drawn before and immediately after an acute bout of exercise at baseline (week 0) and after 6 weeks of training. Results. The resting irisin levels were not different at baseline (week 0) and after 6 weeks of training. At both 0 and 6 weeks of training, an acute bout of vibration exercise significantly elevated circulating irisin levels by 9.5% and 18.1%, respectively (p = 0.05 for the percent change of irisin levels). Conclusions. Acute bouts of whole-body vibration exercise are effective in increasing circulating irisin levels but chronic training does not change levels of baseline irisin levels in humans. @ 2014 Elsevier Inc.       • 작업치료 전 앉은 자세 전신진동운동이 만성 뇌졸중 환자의 상지기능 및 일상생활수행능력에 미치는 영향

        서론: 본 논문은 뇌졸중 환자의 작업치료 전 앉은 자세 전신진동운동이 상지기능 및 일상생활수행능력에 미치는 영향을 보고자 한다. 연구방법: 본 논문은 만성 뇌졸중 환자 28명을 대상으로 작업치료 전 10분간 앉은 자세 전신진동운동을 적용후 20분간 일반적인 작업치료를 시행한 연구군 14명과 30분간 일반적인 작업치료를 시행한 대조군 14명간에 상지기능의 변화, 일상생활수행능력을 비교하기 위해 연구군과 대조군의 상지기능(MFT) 및 일상생활수행능력(MBI) 전, 후 차이를 SSPSS(ver.18.0)를 사용하여 통계 분석하였다. 연구결과: 작업치료 전 앉은 자세 전신진동운동이 만성 뇌졸중 환자의 상지기능에 대한 연구군은 실험 전 14.14±8.59점에서 실험 후 14.50±8.72점으로 점수의 향상을 보여 통계적으로 유의한 차이를 보였고, 일상생활수행능력에 대한 연구군 실험 전 65.43±19.85점에서 실험 후 67.07±19.71점으로 점수의 향상을 보여 통계적으로 유의한 차이를 보였다(p<0.05). 논의 및 결론: 작업치료 전 앉은 전신진동운동은 뇌졸중 환자의 상지기능 증진, 일상생활수행능력의 증진시키는데 긍정적인 효과를 확인하였다. 따라서 작업치료 전 앉은 자세 전신진동운동은 작업치료 전 준비운동으로써 효과적인 치료방법 중의 하나로 적용될 수 있다. Abstract : This research aims to examine the effects of whole body vibration exercise in a sitting position on chronic CVA patients’ upper extreme function and activity of daily living before they go through occupational therapy. Method : This research applied a whole body vibration exercise in a sitting position on 28 chronic CVA patients for 10 minutes before conducting occupational therapy. After, In order to compare the changes in upper extreme function and activity of daily living between the research groups of 14 patients, who were carried out a general occupational therapy for 20 minutes, and a control group of 14 patients that had 30 minutes of general occupational therapy, the before and after of the research and control groups’ upper extreme function (MFT) and activity of daily living (MBI) were statistically analyzed using SSPSS (ver.18.0). Result : For the research group of chronic CVA patients that had a whole body vibration exercise in a sitting position before occupational therapy, their score on upper extreme function increased from 14.14±8.59 before the experiment to 14.50±8.72 after the experiment, showing a statistically significant difference. Also, the research group had an increased score on activity of daily living from 65.43±19.85 before the experiment to 67.07±19.71 after the experiment, proving that it had a statistically significant difference (p<0.05). Discussion and Conclusion : The whole body vibration exercise in a sitting position before occupational therapy have been proved to have a positive effect in enhancing chronic CVA patients’ upper extreme function and activity of daily living. Thus, whole body vibration exercise in a sitting position before occupational therapy can be applied as an effective treatment method as a warm-up exercise.

        라이프케어 증진을 위한 전신진동운동이 낙상위험 뇌졸중 환자의 균형 및 보행능력에 미치는 영향

        본 연구는 라이프케어 증진을 위한 전신진동운동이 낙상위험 뇌졸중 환자의 균형 및 보행능력에 미치는 영향을 알아보았다. 본 연구의 대상자는 뇌졸중 환자 30명을 대상으로 전신진동운동군과 오타고운동군으로 15명씩 할당하여 균형능력은 버그균형척도(BBS)와 일어서서 걷기 검사(TUG), 보행능력은 10m 보행속도검사와 Tinetti 운동성 검사를 측정하여 운동전과 운동 후, 군 간을 비교․분석하였다. 집단 내 운동 전·후의 차이를 비교하기 위해 대응표본 t-검정을 사용하였고, 집단 간의 차이를 비교하기 위하여 독립표본 t-검정을 실시하였다. 실험결과 두 군 모두 운동 전후 BBS와 Tinetti 운동성 검사는 유의하게 증가하였고, TUG와 10m 보행속도는 유의하게 감소하였다. 군 간 비교에서는 BBS, TUG, 10m 보행속도, Tinetti 운동성 검사 모두 유의한 차이가 있었다. 본 연구결과를 통해 라이프케어 증진을 위한 전신진동운동은 낙상위험 뇌졸중 환자의 균형능력과 보행능력을 개선하는데 영향을 미치는 것으로 사료된다. 따라서 낙상위험 뇌졸중 환자의 균형능력과 보행능력을 향상을 통한 라이프케어 증진을 위해 전신진동운동 프로그램을 권장한다. The purpose of this study was to examine the effects of whole-body vibration exercise for life care on balance and walking ability in the fall risk stroke patients. 30 stroke patients were divided into two groups, Otago exercise group(n=15) and whole-body vibration exercise group(n=15). Walking ability were measured by 10m walking and Tinetti mobility test with stroke patients and balance ability were measured by BBS and TUG on comparative analysis of pre, post exercise. To compare differences before and after the program, a paired t-test was used and to compare differences between both groups, an independent samples t-test was used. As a result of the test, it was discovered that BBS and Tinetti mobility test of the both groups were increased statistically significantly and TUG and 10m walking test of the both groups were decreased statistically significantly. balance ability and walking ability of the whole-body vibration exercise group were improved statistically significantly in comparison with those of the Otago exercise group. The results of this study showed that whole-body vibration exercise for the improvement of life care had significant effects on improving BBS, TUG, 10m walking and Tinetti mobility test of fall risk stroke patients. Therefore, it is recommended to apply whole-body vibration exercise to improve life-care through improving balance ability and walking ability of fall risk stroke patients.
