      • Topic-Map을 이용한 사출성형 유동해석 결과 기반의 지식관리시스템

        Nowadays, much attention is being paid to topic map - a kind of searching method based on meanings - for a solution which helps people manage and integrate huge amount of technical information and retrieve correct ones fast and effectively. In order to achieve win-win cooperations between the large enterprises and SMEs (Small and Medium Enterprises), engineering collaborative hub was developed and lots of CAE service - injection-mold flow analysis - were conducted. This paper proposed the integrated KMS(Knowledge Management System) of technical information needed when developing injection molds. Based on the results of injection-mold flow analyses and the use-case models of the people who develop injection-molds, the technical knowledge management system was designed and constructed in the engineering collaborative hub. This system consists of the integrated ontology model of technical information utilizing topic map approach and the retrieval system based on meanings in the field of injection mold development

        Comparative efficacy of three topical anesthetics on 7-11-year-old children: a randomized clinical study

        Background: This study evaluated the efficacy of three intraoral topical anesthetics in reducing the injection needle prick pain from local anesthetic among children aged 7-11 years old. Methods: It is a prospective, Interventional, parallel design, single-blind, randomized clinical trial in which subjects (n=90) aged 7-11 years were included in the study based on an inclusion criteria. Subjects were divided into three groups based on computer-generated randomization with an allocation ratio of 1:1:1. Groups A, B, and C received benzocaine 20% jelly (Mucopain gel, ICPA health products Ltd, Ankleshwar, India), cetacaine anesthetic liquid (Cetylite Industries, Inc, Pennsauken, NJ), and EMLA cream (2% AstraZeneca UK Ltd, Luton, UK), respectively, according to manufacturer's instructions, for 1 minute prior to local anesthetic injection. After application of topical anesthetic agent, for all the groups, baseline pre-operative (prior to topical anesthetic administration) and post-operative scores (after local anesthetic administration) of pulse rate was recorded using Pulse oximeter (Gibson, Fingertip Pulse Oximeter, MD300C29, Beijing Choice Electronic). Peri-operative (i.e., during the administration of local anesthesia) scores were recorded using Face, Legs, Activity, Cry, Consolability (FLACC) Scale, Modified Children hospital of Eastern Ontario Pain Scale (CPS) behavior rating scale, and Faces Pain Scale (FPS-R) - Revised (For self-reported pain). Direct self-reported and physiological measures were ascertained using FPS-R - Revised and Pulse oximeter, respectively, whereas CPS and FLACC scales assessed behavioral measures. To test the mean difference between the three groups, a one way ANOVA with post hoc tests was used. For statistical significance, a two-tailed probability value of P < 0.05 was considered as significant. Results: The Cetacaine group had significantly lower pain scores for self-report (P < 0.001), behavioral, and physiological measures (P < 0.001) than the other two groups. However, there was no significant difference between the Benzocaine group and EMLA group during palatal injection prick. Conclusion: Cetacaine can be considered as an effective topical anesthetic agent compared to benzocaine 20% jelly (Mucopain gel) and EMLA cream.

        Comparing Peribulbar and Topical Anesthesia in Cataract Surgery among Patients with Cardiovascular Disease

        Purpose: To compare topical and peribulbar anesthesia in cataract surgery for hemodynamic changes, rate of complications and pain score in patients with cardiovascular disease. Methods: A prospective comparative study at a tertiary care center in India. Patients >40 years old with treated/controlled hypertension and cardiovascular disease scheduled for cataract surgery under topical or peribulbar anesthesia were recruited. Heart rate, blood pressure, and ophthalmic and systemic complications were noted: preoperatively, immediately after block, intraoperatively, immediately postoperatively and 1 hour postoperatively. A visual analog scale was used to assess the pain score. Results: A total of 150 patients (75 in each group) underwent cataract surgery. There was a significant rise in pulse rate and blood pressure after peribulbar injection and intraoperatively, which gradually reduced to baseline 1 hour after surgery in both groups (p < 0.001), with systolic blood pressure intraoperatively being significantly greater in the peribulbar group (155.49 ±18.14 mmHg vs. 147.95 ±17.71 mmHg, p = 0.01). The topical group had slightly lower visual analog scale scores (1.12 ± 0.99) than the peribulbar group (1.44 ± 0.90, p = 0.04). Conclusions: Cataract surgery appears safe in patients with adequately controlled cardiovascular disease, and topical anesthesia may be preferable due to noninvasiveness, adequate analgesia, and minimal effect on hemodynamic parameters. Therefore, hemodynamically stable patients of cardiovascular disease undergoing uncomplicated cataract surgery may be counselled for topical anesthesia.

        스테로이드 병변내 주사와 Tacrolimus 국소도포로 치료한 얼굴육아종 1예

        Granuloma faciale is an uncommon disease of an unknown origin, and it is characterized by single or multiple erythematous facial skin lesions. Histologically, a narrow Grenz zone of normal dermis is observed between the epidermis and the dense dermal polymorphous infiltrates. The course of granuloma faciale is chronic and the response to therapy is usually poor. We report here a case of granuloma faciale in a 37-year-old male who presented with a 13-year history of a solitary, red-brown, soft, elevated, irregular shaped plaque on the chin. The biopsy specimen showed a Grenz zone and a dense, dermal infiltrate of mononuclear cells mixed with numerous eosinophils and neutrophils that displayed a diffuse and perivascular pattern. After 4 courses of intralesional steroid injection, the lesion almost became flattened, although it remained a pinkish patch. The lesion almost cleared up after 4 months of applying 0.03% topical tacrolimus. (Korean J Dermatol 2009;47(1):111~114)

        There are many methods for treatment of solitary bone cyst. But the recurrence rate was relatively high and beside, serious complications may be followed. Recently, simple topical of steroid into cyst was contrived and the result was know as same or even better than surgical management. A patient of solitary bone cyst who had been failed two times by surgical intervention was treated with topical steroid injection for several times. Two years following up, the result was good. No evidence of recurrency was seen and was shown good healing process.

      • Study on bioresorbable, flexible porous silicon needle patch

        Miniaturized needles enable minimally invasive topical delivery of drug molecules to biological targets. Recently, miniaturized porous-silicon (p-Si) needles are emerging as an attractive candidate for intra-tissue drug injection due to their biocompatibility and controlled biodegradability. Here, we discuss the fabrication method and characteristics of bioresorbable miniaturized p-Si needles. To create a bioresorbable drug delivery platform, vertically aligned p-Si needles arrays are integrated into a water-soluble and flexible substrate by using a transfer printing technique. The p-Si needles remain inserted into the tissue and gradually dissolve, allowing for the sustained release of the drug molecules.
