        건강정보 탐색의도 예측에 있어 태도의 매개효과 및 주관적 건강지식의 조절효과 분석 : 조혈모세포 기증희망등록 캠페인에의 적용

        Health information-seeking behaviors, as forms of communication activities, can be expected to have direct and indirect effects on health promotion and disease prevention. Among the various theories related to individual and social health promotion, the antecedents of health information seeking behavior have also been examined. Focusing on the importance of information related to hematopoietic stem cell donation, this study aimed to examine the relationship between past behavior, attitude, and intention to seek health information among various antecedents of health information seeking. Specifically, this study examined whether past information-seeking behaviors related to hematopoietic stem cell donation and transplantation can influence the intention to seek information on hematopoietic stem cell donation and transplantation via related attitudes. Moreover, the study tested the moderating effect of subjective health knowledge on the relationship between attitude and behavioral intention to seek health information. A survey was conducted among conveniencesampled university students. The results showed that past information seeking behavior on hematopoietic stem cell donation and transplantation had a positive effect on intention to seek related information through attitude toward information seeking. Also, a significant moderating effect of subjective health knowledge was found, with a tendency for attitude toward information seeking to have a greater effect on intention to seek in groups with low subjective health knowledge. The theoretical and practical implications of the study suggest that health communication scholars and campaign practitioners should pay attention to the provision of experiences related to seeking information on stem cell donation and transplantation, the formation of positive attitudes, and the effective use of subjective knowledge in developing campaign strategies for stem cell donor registration.

        건강정보 접근방법별 소비자 평가 결과 비교

        Objective: Each consumer uses different consumer evaluation by sources of health information, considering the influences of consumers' socio-demographic characteristics, health status, attitude to health, and types of health information needed. Methods: Sources of health information include web-based resources, health professionals, and publications such as medical books, magazines, newspapers, etc. Data were collected from 1,189 respondents through a telephone survey, among whom 513(43%) responses were finally included in the analysis. The results were analysed by statistical methods including Chi-square, multiple regression, and correlation analyses. Results: Key findings are as follows. First, there were significant differences in evaluation results by different sources of health information. Access to health professionals shows positive outcomes in general, followed by web-based resource. Second, the significant factors related to the access outcomes were different in each domains such as easiness, amount, understanding, reliability, and helpfulness. In addition, while access to health professionals was found to have significant influence on health behavior by improving the ability of health management, access to internet improved only the understanding of health information. Conclusion: Access to health professionals for health information was positively related to utilization outcomes in general, but access to publications was negatively related to outcomes. Despite the increase in the number of web-based users, they could not rely on the information they found. To effectively utilize various sources of health information and improve the nation's health by providing appropriate health information, quality assurance strategy for contents in health information websites and active intervention by health professionals should be pursued.

        u-Health 환경에서의 정보보호 수준제고를 위한 보안 표준 개발

        e-Business in healthcare sector has been called e-Health, which is evolving into u-Health with advances of ubiquitous technologies. Seamless information sharing among health organizations is being discussed in many nations including USA, UK, Australia and Korea. Efforts for establishing the electronic health record (EHR) system and a nation-wide information sharing environment are called NHII (National Health Information Infrastructure) initiatives. With the advent of u-Health and progress of health information systems, information security issues in healthcare sector have become a very significant problem. In this paper, we analyze several issues on health information security occurring in u-Health environment and develop an information security standard for protecting health information. It is expected that the standard proposed in this work could be established as a national standard after sufficient reviews by information security experts, stakeholders in healthcare sector, and health professionals. Health organizations can establish comprehensive information security systems and protect health information more effectively using the standard. The result of this paper also contributes to relieving worries about privacy and security of individually identifiable health information brought by NHII implementation and u-Health systems.

        인터넷 건강정보의 질적 개선을 위한 관리 방안

        The Internet has given rise to an information revolution. Through the Internet the public in South Korea has access to a growing supply of information on health and disease. In South Korea, 92% of those who can use the computer use the Internet in 2001. An estimated 22.23 million number used the Internet in 2001. The number has increased rapidly compared to 1.63 million in 1997. Health information is often said to be one of the most retrieved types of information on the Internet However, a concern has emerged for the quality of health information documents contained on the World Wide Web. Lack of evaluation and oversight, and ease of publication, have led to inaccurate and misleading health-related publications on the Internet For those seeking easy ways to identify high-quality and reliable information rating systems to evaluate the quality of health information on the internet should be provided and developed. Given this background, the purpose of this study was to provide criteria and quality management for health information sites on the internet. This article reviews and discusses elements to evaluate health information sites on the internet. The criteria include quality of content (currency, references, readability, etc.), authorship (identification of authors and contributors, author credentials), purpose, design and aesthetics, functionality, contact address and feedback mechanism, and privacy. The criteria suggested may be useful to providers and users of health information on the Internet. The criteria have been developed with the understanding that they will require ongoing reevaluation and frequent revision to keep pace with evolving technology and practices on the internet. There is a need to develop simple criteria that the general people can understand and use. To achieve quality assurance of health information, a combination of centralized and decentralized approaches can be used. The Government should make an effort to make health information policies on the Internet. Third parties or peer review should evaluate and rate health information. Labeling and filtering software programs such as PICS (platform for internet content selection) should be developed to help for professionals and customers to find easily valuable health information.

        건강정보의 이차적 이용

        Traditionally medical information has used for the purpose of clinical care. Also it has used for the accumulation of medical knowledge and experiences. However, because of the rapid development of biomedical science, the situation has been changing. Besides the traditional medical information, genetic information and other information which has not been included in traditional medical information are attracted attention. So, the term “health information”, instead of medical information is appropriate for referring the information which is related to human health condition. Health information should be regulated properly because it contains patient’s or subject’s private information. If health information is abuses or misused, the privacy of individuals who provide their information. There are some Acts which regulate the use of health information in Korea. Use of medical information is regulated by <Medical Act>. Collection and Use of genetic information is regulated by <Bioethics and Biosafety Act>. However, there are great needs for secondary use of health information. there are many situations which may use health information which is collected and created during medical practices or researches, such as for the purposes of another researches, health insurance or cohort studies. In using health information, respect of individual’s autonomy is the most important principle to be considered. Informed consent of patient or subject is the means to respect individual’s autonomy. Another issue which is considered whether we allow secondary use of health information is the benefit when we use health information. If the risk of secondary use of health information is trivial and the benefit is very big, we can allow it under appropriate regulation. Also, we should use health information for controlling very severe infectious diseases. Medical practitioners contribute to make health information. When medical doctors examine patients and collect health information from patients, they also evaluate and manipulate raw information which they gain. Also, they create health information during the procedure of medical practice. For example, medical practitioners decide which lab they will order or which process they will do. Because of these aspects, we should consider medical practitioners’ right when we regulate the secondary use of health information.

        국가건강정보포털의 의미와 역할

        The National Health Information Portal (http://health.mw.go.kr), a collaborative project by the Ministry of Health and Welfare and the Korean Academy of Medical Sciences, was officially launched in January 2011. In the modern society, the internet and smart phones have been flooded with health information, in addition to the information available through traditional media such as newspapers, television, and radio broadcasting. The Korean Medical Association and the Korean Academy of Medical Sciences have expanded their work in several directions,including a monitoring project to correct wrong health information that is commonly used. Nevertheless, they are confronted with the fundamental problem that they are unable to keep up with the speed of production and diffusion of health information in modern society. This initiative of the National Health Information Portal led by the Ministry of Health and Welfare and the Korean Academy of Medical Sciences is significant in that anyone with internet access can easily find proven health information. Unlike existing health information, the National Health Information Portal is structured to help improve understanding by presenting a variety of materials, such as cartoons, pictures, photos, and videos, as compared to simple documents or texts. In addition, it is constructed with a feedback system so that the consumer of the health information can directly and easily access specific content. Furthermore, the National Health Information Portal continues to strive to provide practical contents with mobile services like smart phone applications. We expect the National Health Information Portal to make a contribution to the prevention and treatment of disease by reducing unnecessary medical expenditures, and providing quality health information to society.

        노인들의 건강 정보 수용: 임파워먼트의 역할

        본 연구는 노인들의 온라인 건강 정보 수용의 경로에 대한 이해를 위하여 건강 관심도가 임파워먼트와 온라인 건강 정보 탐색을 통해 온라인 건강 정보 활용에 미치는 영향을 살펴보았다. 이를위해 65세 이상 노년층 240명을 대상으로 1:1 면접을 통한 설문 조사를 실시하였다. 연구 결과노인들의 건강 관심도는 임파워먼트에 정적 영향을 미쳤으며 임파워먼트는 온라인 건강 정보 탐색과 온라인 건강 정보 활용을 증가시키는 것으로 나타났다. 반면, 임파워먼트를 거치지 않은 건강 관심도는 온라인 건강 정보 탐색에 부적 영향을 미치는 것으로 나타났다. 이는 건강 관심도가임파워먼트를 매개할 경우 온라인 건강 정보 탐색과 활용을 증가시키지만, 건강 관심도가 임파워먼트를 매개하지 않을 경우 온라인 건강 정보 탐색에 부적 영향을 미침으로서 결과적으로 온라인 건강 정보 활용을 감소시키는 결과로 볼 수 있다. 본 연구의 결과는 노년층의 건강 정보 행동증진을 위한 전략 수립에 이론적, 실무적 기여를 할 것으로 기대된다. To improve our understanding of seniors’ health-related information behaviors online, this study examines how health consciousness and empowerment relates to online health information behaviors. Data comes from the survey based on a sample of 240 elderly people who are over 65 years old. Results show that health consciousness is positively associated with empowerment, and empowerment is positively related to both information behaviors (i.e., online health information seeking and use of online health information). However, in the absence of mediating role of empowerment, health consciousness is negatively associated with online health information seeking. The results suggest that the relationship between health consciousness and online health information seeking turns out to be in a negative direction unless empowerment mediates the relation. In contrast, health consciousness is related to increased levels of online health information seeking via empowerment, which, in turn, is positively related to use of online health information. Results highlight the significance of empowerment as a means of fostering online health information behaviors. The findings have significant theoretical and practical implications for the design of health promotion strategies to improve health outcomes for elderly people.

        이민자의 건강정보이용 실태 분석

        본 연구는 한국에 거주하는 이민자의 건강정보 이용실태를 파악하기 위한 서술적 조사연구이다. 서울과 경기도 소재의 ‘이주노동자 무료 진료소’를 이용하는 이민자 171명을 대상으로 설문 조사하였다. 수집된 자료는 기술통계, Chi-square test, Fisher’s exact test로 분석하였다. 이민자의 건강정보 이용빈도는 낮은 편으로 월 1회 미만으로 건강정보를 이용한다는 응답자가 가장 많았다. 건강정보 이용빈도는 나이가 많거나, 직업이 없거나, 질병이 있는 대상자에서 더 높았다. 주요한 건강정보 출처는 인터넷이었고, 나이와 인터넷 사용 여부에 따라 건강정보 출처에 차이가 있었다. 나이가 증가하거나 인터넷을 사용하지 않은 응답자의 건강정보 출처로 인터넷 사용 비율이 낮았다. 건강정보의 주요한 활용대상자는 본인이었고, 지각된 건강상태가 건강하지 않은 대상자는 본인을 위하여 건강정보를 더 많이 사용하였다. 본 연구의 결과에서처럼 이민자의 대부분이 건강정보를 활발히 사용하지 않는 것으로 나타났다. 그러나 나이가 증가하거나 건강하지 못한 경우에는 건강문제를 해결하기 위하여 건강정보 사용이 증가하면서 건강정보의 필요성이 나타났다. 그러므로 이민자의 건강정보 이용 증진과 효과적인 건강정보 제공을 위해서는 대상자의 특성을 고려하여 건강정보를 다양한 방법으로 제공하는 것이 중요하고, 이를 위하여 보건의료의 정책이나 서비스 개발이 필요하다. This study was designed to investigate the health information usage of immigrants. A questionnaire survey was conducted on 171 immigrants. Data was analyzed for descriptive statistics, chi-square test, and Fisher"s exact test. The frequency of migrants" use of health information is low, and the number of respondents who use health information less than once a month is highest. There were statistically significant differences in the frequency of use of health information according to age, occupation, and those who contracted diseases. The main source of health information was the Internet, and there were differences in the sources of health information according to age and whether the Internet was used. Most of the respondents used health information for themselves, and there was a difference in the targets of using health information according having a cohabitee and the perceived health status. It was found that the majority of immigrants do not actively use health information. However, when the age increased or the immigrant was unhealthy, the use of health information increased to solve health problems. Therefore, it is important to provide health information in a variety of ways according to the characteristics of immigrants.

        수요자 의료정보시스템의 정보제공형태가 의료성과에 미치는 영향에 대한 연구

        헬스 케어 산업이 부각되고 있는 시점에서 성공적인 의료성과를 얻기 위해, 의료성과에 영향을미치는 주요 요인을 정의하는 연구는 매우 중요하다. 기존의 의료분야에서는 치료 전 정보제공을 주요 요인으로 정의하고 증명한 연구는 다수 있으나, 정보의 유형이나 정보제공방법에 초점을 둔 연구로 e-Health System을 통한 정보제공이나 제공되는 정보의 표현형식에 관한 연구는 전무한 상황이다. 특히 e-Health System으로 정의되는 인터넷 웹 사이트를 통한 실증연구가 매우 부족하다. 본 연구에서는 의료분야와 IS분야의 각각의 연구초점을 아울러 환자의 정보인지도와 의료성과인 환자만족도에 영향을 미치는 정보제공형태를 정의하고, e-Health System의 성과에 직접적인 관계가 있는 재사용의도와 정보만족도에 영향을 미치는 정보제공형태를 정의하였다. 연구 결과, 정보제공형태는 환자의 정보인지도, 환자만족도, e-Health System(인터넷 웹 사이트)의 재사용의도와 정보만족도에 통계적으로 유의한 긍정적인 영향을 미치는 것으로 나타났다. Study results show that information types had positive effect on patient cognition onhealth information, patient satisfaction, patient's intention to reuse provided health information,and satisfaction on health information. Patient satisfaction was significantly higher for the patientgroup given structured health information, both interactive and text-based information, than thegroup given unstructured health information. The effect of interactive health information onpatient cognition was larger than the effect of text-based health information, and the differencewas statistically significant. The study implied that structured health information, especially theinteractive types of information, is important for patient satisfaction.

        인터넷 건강정보 이용 후의 건강관련 행태 경로 분석

        Objectives : This study investigated the health information such as the general health information, the health product information, and the hospital information, and we wanted to identify the association between internet health information and the health related behavior by analyzing the process after people search the Internet. Methods : A telephone survey with structured questionnaire was performed by trained surveyors. The respondents were sampled proportionate to the Korean demographic distribution with considering the city size and the populations' ages and gender. The survey was conducted from October 2006 to November 2006. Results : Out of 3,758 successfully connected persons of age 20 or more, 871(23.2%) respondents had used Internet health information during the last year. The purposes of searching the Internet for health was, 1) to get general health information (717 cases, 81.0%), 2) shopping for health product (109 cases, 12.3%) and 3) seeking information about hospital selection (59 cases, 6.7%). Our research showed that the process after searching the Internet for health information depends on the purpose of the search. 68.8% of the searchers for general health information, 67% of the searchers for health product shopping and 64.4% of the searchers seeking information to guide hospital selection were satisfied with their Internet search. However one third of the respondents reported not being satisfied with the result of the search. Conclusions : Unsatisfied consumers with internet health information tended to ask lay referrals from others or they gave up seeking health information. The health information system should be improved to increase the accessibility and to provide reliable and effective information. Also, a more user-centric community is needed in order to strengthen the effective role of lay referrals among the internet users.
