MEF2C-Related 5q14.3 Microdeletion Syndrome Detected by Array CGH: A Case Report

Genetic screening is being widely applied to trace the origin of global developmental delay or intellectual disability. The 5q14.3 microdeletion has recently been uncovered as a clinical syndrome presenting with severe intellectual disability, limited walking ability, febrile convulsions, absence of speech, and minor brain malformations. MEF2C was suggested as a gene mainly responsible for the 5q14.3 microdeletion syndrome. We present the case of a 6-year-old girl, who is the first patient in Korea with de novo interstitial microdeletions involving 5q14.3, showing the typical clinical features of 5q14.3 microdeletion syndrome with a smaller size of chromosomal involvement compared to the previous reports. The microdeletion was not detected by subtelomeric multiplex ligation-dependent probe amplification, but by array comparative genomic hybridization, which is advisable for the detection of a small-sized genetic abnormality.

Purpose: In order to overcome the limitations of the conventional cryopreserved fibroblast or keratinocyte allograft method used in the treatment of diabetic foot ulcers, we reported a pilot study in 2004 demonstrating promising results of a fresh fibroblast allograft method in eight patients. However, the number of cases was insufficient for full evaluation and the follow-up duration was not long enough to determine the efficacy and safety of the method. This encouraged us to conduct this follow-up study to fully evaluate the use of noncryopreserved fresh human fibroblast allografts in treating diabetic foot ulcers. Methods: Thirty-seven patients with diabetic foot ulcers were treated using fresh fibroblast allografts. Human dermal fibroblasts from healthy teenagers were cultured in DMEM/F-12 medium supplemented with 10% serum. The cultured cells were applied on the wounds immediately following debridement, with fibrin being used as a cell carrier. In eight weeks, percentages of complete healing, mean healing time, and patient satisfactions were assessed, with follow-up time ranging from 6 to 40 months. Results: Our study showed that 83.8% of the treated patients were complete healed. The time required for complete healing was 30.9±10.1 days. Patient satisfaction scores for the experimental treatment were higher than those for the conventional method(mean scores of 8.1±1.1 and 4.8±1.4, respectively). No adverse events related to the study treatment occurred. Conclusion: The use of fresh human fibroblast allografts was found to be a safe and effective treatment for diabetic foot ulcers.

For practical application of the Walsh function method for parameter estmation of time-invariant linear system, some problems are discussed. Since Walsh functions are relatively easy to handle in computations, the approach to fast Walsh transform for parameter estimation of linear systems is quite simple to implement in practical situations. The experiment indicates that acceptable parameter estimates can be obtained even incases where the linear system is subject to small modeling and measurement errors.

This thesis deals with an state observer of DC motor via orthogonal functions. The state estimation of DC motor is studied based on single term expansion of orthogonal functions. Thus the design task becomes essentially a problem o finding a class of observer feedback matrices. Once an observer feedback matrix is determined such that the error dynamic is uniformly and asymptotically stable, the observation converges to the true state. A classical Liapunov method is applied to show the uniform and asymptotic stability of the observation error dynamics. Using this method the estimated speed is always acceptable because the parameters converge to the ideal values.

This thesis deals with an analysis of linear systems via Haar functions. It is introduced to a new operational martix which relates Haar functions and their integrations. Using the operational martix for integration and the product operational matrix, the state equation of linear system is reduced to a set of simulateous linear algebraic equation. This method is compared with the single term expansion. Examples are presented to illustrate the simplicity of the method.

We have developed computer software to store data on all anesthetics administered by our department. Our residents transcribe data from the anesthetic records into the computer file, which is then available for producing monthly reports of method, agent, age, and duration statistics. Also another back up floppy disk is made for yearly statistics. Capacity of a floppy disk is 3000 cases of records in a month and 30000 cases in a year. It imposes little additional workload on our clinical personnel, who use simple codes to cumulation and analyze anesthetic records monthly with emphasis on minimal cost, accuracy of data and economy of time.

The Kalman filter is a standard filter for state estimation in linear systems and provides the vector modeling of the random processes and the recursive processing of noisy measurement data. The state estimation of stochastic continuous systems can be obtained by solving the n×n matrix variance Riccati differential equation. This paper presents the algebraic Kalman filter for the stochastic system, which is based on rational Harr transformation.

- This paper presents a new method for finding the Block Pulse series coefficients, deriving the Block Pulse integration operational matrices and generalizing the integration operational matrices which are necessary for the control fields using the Block Pulse functions. In order to apply the Block Pulse function technique to the problems of state estimation or parameter identification more efficiently, it is necessary to find the more exact value of the Block Pulse series coefficients and integral operational matrices. This paper presents the method for improving the accuracy of the Block Pulse series coefficients and derives the related integration operational matrices and generalized integration operational matrix by using the Lagrange second order interpolation polynomial

AHP를 이용한 공공병원 평가지표의 우선순위 및 가중치 개발

This study looks at priorities in managing public hospitals by figuring out categories needed to examine the quality of each hospital. In order to analyze priorities and relative importance of valuation indicator in Seoul municipal hospitals, surveys were sent via e-mail to medical professionals who have participated in evaluation of municipal hospitals. The analytic hierarchy process (AHP) was conducted using the Expert Choice 11.5 program. The results show that ‘providing public service’ is considered to be the most important category for all kinds of public hospitals, followed by ‘improving the quality of medical care,’ ‘hospital management and governance,’ and ‘efficient hospital management.’ The importance of ‘hospital management and governance’ and ‘efficient hospital management’ is different depending on the types of hospitals, but the importance of ‘providing public service’ and ‘improving the quality of care’ remains the same regardless of the types of hospitals. Based on these results, the study comes to a conclusion that public hospitals should place high value on certain categories and their management purposes should differ depending on their specific fields and characteristics. This study will help furthering discussions on the identity and roles of public hospitals.
