        탈접착 후처치시 핸드피스(handpiece) 속도가 치아에 미치는 영향에 대한 비교 연구

        This study was, performed to examine treatment efficiency and patient discomfort rate according to used handpiece speed in clean-up technique. Brackets were bonded to extracted human premolar(50 teeth). After debonding, 50 extracted human premolar were divided into each two groups( low speed handpiece group with tungsten carbide bur and high speed handpiece group with ultra-fine diamond finishing bur ) of 25 according to used handpiece speed in clean-up technique. In clean-up procedure, teeth vibration and pulp thermal changes were measured. After clean-up procedure, the enamel surfaces of randomly selected 10 teeth from each two groups were taken by SEM and evaluated. The findings of this study were as follows ; 1. During resin removal, tooth vibrations of various amplitude in low speed handpiece group were more than those of high speed handpiece. 2. The pulpal thermal changes of high speed handpiece group were significantly higher than those of low speed handpiece group, also the resin removal time in high speed handpiece group was almost as twice as in low speed handpiece group. 3. The figures of SEM to enamel surfaces after resin removal showed that notches and resin remnants in high speed handpiece group were more than those in low speed handpiece group. 본 연구는 탈접착 후처치시 사용된 핸드피스(handpiece) 속도에 따른 처치 효율성과 환자 불편감 정도를 알아보기 위하여 시행되었다. 발거된 소구치(50개)에 브라켓을 접착하고 제거한 후 tungsten carbide bur를 장착한 저속 핸드피스와 ultra-fine diamond finishing bur를 장착한 고속 핸드피스로 각각 25개씩 탈접착 후처치를 시행했으며, 이때 각 실험군내 치아의 진동정도와 치수강내 온도 변화를 측정하였다. 이후 각 실험군(저속, 고속 핸드피스군)에서 임으로 10개씩의 치아를 선택하여 주사전자현미경으로 법랑질 표면에 대한 평가를 시행하여, 다음과 같은 결론을 얻었다. 1. 저속 핸드피스를 사요한 레진 제거시 다양한 진폭의 치아 진동이 고속 핸드피스를 이용한 경우보다 많이 나타났다. 2. 고속 핸드피스를 이용한 경우의 치수강내 온도변화가 저속 핸드피스를 사용한 경우보다 유의성 있게 높았으며, 레진 제거 시간 역시 두배 정도 길게 걸렸다. 3. 레진 제거 후 법랑질 표면에 대한 주사전자현미경상은 고속 핸드피스를 사용한 경우에서 절흔들(notches)과 잔여 레진들이 저속 핸드피스를 사용한 경우보다 많은 것을 보여주었다.

        Effect and Safety of Pressure Sensor-equipped Handpiece in Phacoemulsification System

        Purpose: To evaluate the effect of the Active Sentry handpiece of the Centurion Vision System compared to the Centurion Ozil handpiece for phacoemulsification in cataract surgery. Methods: A retrospective study was conducted on 281 patients (449 eyes) who underwent cataract surgery between August 2020 and June 2021. Preoperative measurements, intraoperative parameters, complication rate, and postoperative outcomes were compared between the Active Sentry handpiece and the Centurion Ozil handpiece groups. Additionally, the parameters were compared in different cataract severity groups and multiple predictive factors for the number of active surge mitigation (ASM) actuations were assessed with the Active Sentry handpiece. Results: There were 198 eyes in the Active Sentry group and 251 eyes in the Centurion Ozil group. There were no statistically significant differences between the two groups, as the cumulative dissipated energy in the Active Sentry and Centurion Ozil groups were 8.32 ± 7.74 and 7.87 ± 9.25 μJ, respectively (p = 0.576). Total surgery time, ultrasound usage time, aspiration time, amount of fluid aspirated, postoperative corrected distant visual acuity, and postoperative decrease in corneal endothelial cell density were comparable between the two groups. The significant contributors to the number of ASM actuations were age, preoperative corrected distant visual acuity, axial length, and total ultrasound time. Conclusions: There was no clear advantage of the Active Sentry handpiece compared to the Centurion Ozil handpiece. ASM actuation increases with age, poor visual acuity before surgery, short axial length, and prolonged ultrasound usage time. It is expected that in more severe and high-risk cataract surgery, the Active Sentry handpiece functions more effectively, possibly affecting the safety and prognosis.

      • 의료기기 Handpiece의 형태에 따른 사용성 평가

        본 연구에서는 각각 모양 및 색상이 다른 4가지의 피부 의료기기 Handpiece를 사용하여 선호하는 형태 및 색상, 사용성평가를 일대비교법을 통해 규명하고자 한다. 따라서 색상이 선호도에 영향을 끼지지 않는 폐안 상태일 때, 색상이 선호도에 영향을 미치는 개안 상태일 때로 각각 2번씩 20대 학생들을 중심으로 실험을 진행 하였다. 그 결과, 굴곡이 존재하는 Handpiece A가 페안, 개안일 때 가장 높은 선호도를 나타내었고, 굴곡이 존재하지 않는 Handpiece B가 가장 낮은 선호도를 나타내었다. 이를 통해 피부용 의료기기 Handpiece는 굴곡진 형태가 직선 형태보다 사용성이 높고, 색상 또한 파란계열보다 하얀계열을 선호하는 것을 확인 하였다. 결론적으로, 본 연구에서 의료기기 Handpiece는 밝은 이미지의 하얀색인 굴곡이 있고 잡았을 때 그립감이 좋은 형태로 설계를 한다면 실제 사용자인 의사들이 사용하기 더 편리하고 근골격계질환에 예방할 수 있다고 판단된다.

        외과 수술용 Air-Powered Handpiece 개발

        The purpose of this paper is concerned with a development of an air-powered handpiece for surgical operation. The handpiece is the tool of surgical instruments and it can be used to interchange multiple attachments for drilling, pinning, sawing, driving screws and reaming. Most of domestic medical instruments bring in overseas and the air-powered handpiece imported from foreign countries at 100% too. Therefore we develop new air powered handpiece. we research in 2D and 3D modeling, design of air line, analyze structure. The air-powered handpiece composes of body, power supply air-line, elements for mechanical power transmission, attachment, and surgical tools. The handpiece is developed by several processes that 3D design, machining, heat treatment and coating. The developed handpiece is experimented to confirm check the efficiency.

        Designing High-Speed Dental Air-Turbine Handpiece by Using a Computational Approach

        The high-speed dental air-turbine handpieces are widely used in the dentistry as a tool for cutting and drilling a tooth. The impeller of the air-turbine handpiece rotates a cutting bur through the compressed-air. Two dental handpieces which have standard type head (Model 1) and torque type head (Model 2) were designed using computational analysis. The advanced existing high-speed dental airturbine handpieces were investigated to compare with the designed dental handpieces. The computational domain of the air-turbine was investigated using ANSYS CFX software. The impeller blade type which has high torque value was selected to design the airturbine impeller. The performance of the designed handpieces was verified through the measurements and the pre-clinical tests. The results showed that the newly designed handpieces had excellent performance which is compatible with existing advanced handpieces.

        치과용 고속 에어터빈 핸드피스의 공급관로 분석 및 성능평가

        The dental high-speed air turbine handpiece is one of the most popular devices that have been widely used as the main means of cutting tooth structure and restorative material in dentistry. In consideration of usage and marketability of the dental handpiece, it is obviously worthy of investigating it. The goal of this paper is to establish the relationship between the air turbine speed and the supply route inside the handpiece. To do this, the Computational Fluid Dynamics (CFD) tool, Fine<SUP>TM</SUP>/Turbo is used and the optimal supply route position is suggested from the simulation results. In addition, as an attempt for domestic product, the reverse engineering process of a high speed dental handpiece by 3D X-Ray CT equipment and wire cutting is presented for the Mark II model in NSK. In doing so, the 3D modeling of the handpiece parts is carried out with CATIA V5, and the interference between parts is examined. Finally, the result of performance test for the prototype produced in this research is presented.

        귀 모형을 이용한 치과 핸드피스에 대한 소음 저감 장치의 효과 분석

        This study intended to evaluate noise level of dental handpieces and the effects of noise cancelling devices. An ear model was designed to measure the level of the noise delivered to the inner ear during dental handpiece operation. The level of noise was measured in various conditions using the ear model and the portable noise meter. Noise level was measured again after applying of 4 different noise cancelling devices to the model ear. The noise level of dental handpieces was 82.5 - 84.4 dB. When 4 types of noise canceling devices were applied, the noise level reduced to 67.4 - 73.8 dB. All 4 devices had statistically significant effect in reducing the noise of the handpiece. Considering the intensity and exposure time, noise in dental clinics can cause hearing damage to dentists. For protection from the risk of occupational hearing damages, noise canceling devices can be recommended. 이 연구는 치과 핸드피스의 소음 수준과 소음 저감 장치의 효과를 평가하기 위한 것이다. 귀 모형은 치과용 핸드피스 작동 중에 내이에 전달되는 소음의 수준을 측정할 수 있도록 설계되었다. 소음의 크기는 귀 모형과 휴대용 소음 측정기를 사용하여 다양한 조건에서 측정되었다. 시판되는 4종의 소음 저감 장치를 귀 모형에 적용한 후 같은 조건에서 소음 수준을 측정한 뒤 비교하였다. 치과 핸드피스의 소음 수준은 조건에 따라 약 82.5 - 84.4 dB이었다. 4종의 소음 저감 장치를 적용했을 때 소음은 67.4 - 73.8 dB로감소했다. 4종의 장치 모두 핸드피스의 소음을 줄이는데 유의한 효과가 있었다. 강도와 노출 시간을 고려할 때, 치과 소음은 치과 의료진에게 청력 손상을 일으킬 수 있다. 직업적 청력 손상 위험을 예방하기 위해소음 저감 장치 사용을 고려할 수 있다.

        Pigmented and Depressed Facial Scar after an External Injury: Successful Treatment with 1,064-nm Picosecond Neodymium: Yttrium-Aluminum-Garnet Laser Simply Combined with a Fractional Beam-Splitter Handpiece

        A 29-year-old man presented with facial scars one and half years after an external injury. A physical examination revealed a slightly depressed linear scar with bluish brown pigments from traumatic tattoos on the nasal root. He was treated with a 1,064-nm picosecond neodymium:yttriumaluminum- garnet (Nd:YAG) laser combined with a fractional picosecond mode by integrating a holographic beam splitter handpiece. After five sessions of this simple combined treatment modality, the patient achieved significant clinical improvement with no downtime period. To the best of the authors’ knowledge, this is the first case of the effective treatment of facial scars after a skin injury using a 1,064-nm picosecond Nd:YAG laser together with a simple combination of a fractional beam splitter handpiece, successfully targeting both the traumatic tattoos and depressed scars.

        치과용 핸드피스에 의한 일시적 진동감각역치 변화

        치과용 핸드피스에서 발산하는 고주파수의 진동에 의한 일시적 진동감각역치 변화를 관찰하고자 치과위생병 28명을 대상으로 34,000rpm의 low-speed handpiece를 진동기구로 사용하였다. 진동에 폭로시키기 전과 후, 치과의사가 스케일링시 취하는 작업자세에 5분간 폭로시킨 후의 진동감각역치를 청력계기의 골전도진동기를 사용하여 250 Hz에서 그들이 주로 사용하는 손의 제2지두수에서 측정한 결과, 각각 $23.5{\pm}3.5dB,\;30.8{\pm}4.2dB,\;23.7{\pm}4.6dB$로 진동에 5분간 폭로시킨 후의 역치가 폭로 전에 비해 7.3dB의 유의한 증가가 있었다(p<0.001). 진동폭로 전후의 차이에 따른 분포를 보면, 10 dB 내외로 증가한 사람이 71.4%로 대부분이었고 16 dB의 증가가 가장 높았다. 진동 폭로 전의 진동감각역치와 신장, 체중, 체질량 지수와의 상관관계는 각각의 상관계수가 0.0360, -0.1657, -0.2641로 통계적 유의성은 없었으며, 흡연에 따른 차이도 흡연군(N=20)과 비흡연군(N=8)의 진동감 각역치가 각각 23.1 dB, 24.6 dB로 그 차이 가 없었다. 본 연구 결과로 볼 때 핸드피스와 같은 고주파-진동기구를 사용하는 치과의사 및 치과위생사에게서 진동감각역치의 변화가 예상되며, 말초신경병증의 원인이 될 수 있는 작업자세에 의한 장기간의 영향 및 아말감 충전시 사용하는 수은의 영향에 대해서도 연구가 필요할 것으로 생각된다. This study was performed to investigate the possibility of temporary threshold shift (TTS) of vibration sense could induced by exposure to high-frequency vibration or by work position taking by dentists in drilling or polishing. The vibratory perception thresholds (VPT) of 28 healthy men were measured on the index fingertip pulp of dominant hand at 250 Hz. The vibrating tool used in test was a low-speed handpiece of 34,000 rpm. For the TTS test, the dominant hand was exposed to high-frequency vibration and to work position for five minutes, respectively. The VPTs before and after vibratory exposure were $23.5{\pm}3.5dB,\;30.8{\pm}4.2dB$, respectively and VPT after work position was $23.7{\pm}4.6dB$. The difference between before and after vibartory exposure was statistically significant (p<0.001). The correlation of height, weight and BMI with baseline VPTs was not statistically significant. Also, there was no difference of VPTs by smoking. These results suggest that high-frequency vibration from dental handpieces might cause the impairment of vibration sensation.

        Pore and Acne Scar Treatment with a Q-switched Laser Using a Novel Fractional MDF (Multi-depth Focusing) Handpiece: A Pilot Study

        Background and ObjectivesAlthough skin rejuvenation is not within the usual repertoire of the Q-switched 1064 nm Nd:YAG (Q-NDY) laser, the development of a multi-depth focusing (MDF) handpiece allowing user-selectable depths of focus has changed this. The current in-house pilot study was designed to assess the safety and efficacy of a Q-NDY laser fitted with an MDF handpiece in the treatment of enlarged pores and acne scars.Materials and MethodsTen patients with enlarged pores and acne scars were recruited into the study as volunteers. Clinical photographs were taken at baseline and then at six weeks (A1) and three months (A2) after the final treatment, based on which a quintile scale physician global assessment (PGA) was obtained at A2 scored by two independent blinded dermatologists. Patients subjectively scored their satisfaction with the treatment in a questionnaire at A2, also on a quintile scale.ResultsIn general, patients showed a statistically significant improvement in both the pores and acne scars at both A1 and A2, with a trend towards continued improvement between the first and second assessments, more particularly for the acne scars. The average percentage improvements for the pores at A1 and A2 were 48.45 ± 16.1% and 57.8 ± 15.2% (p = 0.001 for both), respectively, for acne scars, the respective values were 27.8 ± 15.0% and 34.2 ± 24.3% (p = 0.0008 for A1, 0.0004 for A2). No difference was seen between the subjects who had three sessions and those who had four, suggesting that the 3-treatment protocol was ideal, although numbers were too small to make any generalization. The average PGA score was 3.3/4 and the average subject satisfaction score was 3.4/4. The average pain score was 3.7 (range 3-5), and no adverse events were reported during both the treatment and follow-up periods.ConclusionFurther controlled studies in larger populations are warranted to confirm the optimistic results of this pilot study.
