      • $^{18}F$-FDG PET/CT검사에서 지연영상을 이용한 갑상선 우연종 감별의 유용성 평가

        본 연구의 목적은 $^{18}F$-FDG PET/CT검사 시 우연히 발견된 갑상선병변의 악성여부를 평가하기 위하여 위양성으로 판독되어지는 양성종양이나 염증질환과 악성 종양의 감별이 가능한 검사법으로 지연영상(Delay scan)을 적용하고 결과 값(SUV)의 기준안을 마련하며 이 연구에서 제시되는 검사법의 유용성을 평가하는데 그 목적이 있다. 2008년 1월부터 2008년 4월까지 E대학병원에 내원하여 $^{18}F$-FDG PET/CT를 시행한 환자 800명을 대상으로 후향적 분석을 실시하였다. 같은 연구기간 중에 갑상선의 원발암으로 $^{18}F$-FDG PET/CT을 실시한 1명은 연구에서 제외하였다. 최종적으로 799명이 연구대상이었으며 $^{18}F$-FDG PET/CT검사를 실시한 이유는 기존에 암이 증명되어 예후 관찰로 검사하거나 암이 의심되는 경우가 696명, 건강 검진 목적으로 실시한 경우가 103명이었다. $^{18}F$-FDG PET/CT영상 촬영은 SIEMENS사에서 만든 Biograph-Duo에서 시행하였으며, $^{18}F$-FDG PET/CT 정상영상(1 hr)을 취득한 후 갑상선 이상섭취가 있고 SUV 2.0 이상인 환자를 대상으로 1시간 후에 갑상선부위를 1 bed 지연영상을 촬영하였고, 한 bed에 2분 동안 영상을 얻었다. 갑상선 이상섭취가 있고 SUV 2.0 이상인 환자를 대상으로 이상섭취가 있는 부위의 1시간 영상과 2시간 지연영상의 SUVmax를 측정하여 비교 분석하였다. 본 연구에서 $^{18}F$-FDG PET/CT 검사 시 우연히 갑상선에 $^{18}F$-FDG 섭취를 보인 환자 중 갑상선 우연종(Thyroid incidentaloma)이 5명으로 0.63%로 나타났으며 모두 양성병변을 시사하는 소견을 나타냈다. 그리고 우연히 갑상선에 $^{18}F$-FDG섭취가 있는 경우 먼저 갑상선부위의 2시간 지연검사를 통해 $SUV_{max}$의 변화치를 이용하여 간접적으로 양성 혹은 악성여부를 판단하는 하나의 기준이 될 수 있고 판독의 오류도 줄일 수 있다. 발견된 갑상선 우연종(Thyroid incidentaloma)의 경우 1) 국소부위의 갑상선병변 SUVmax가 5.0 이상이고, 2) $^{18}F$-FDG PET/CT 정상검사(1 hr)와 지연검사(2 hr)의 갑상선병변 $SUV_{max}$차이가 $1.0{\pm}0.5$인 경우 Malignant를 의심하고 조직학적 확진을 진행한다. 그렇지 않은 경우에도 추후 PET이나 CT를 이용한 면밀한 영상검사로 추적 관찰하는 것이 병변의 진단을 위한 합리적인 접근방법이라고 생각한다. Purpose: To evaluate the degree of malignancy of incident thyroid lesion found in $^{18}F$-FDG PET/CT findings and the usefulness of the method suggested in this study, we applicate the Delay Scan Method that differentiate a false positive benign tumor, inflammation and malignancy, as well as make the criteria of SUV. Materials and Methods: A retrograde study was conducted of 800 patients who were admitted in E hospital to receive $^{18}F$-FDG PET/CT examination. One patient who was diagnosed as primary thyroid cancer and received $^{18}F$-FDG PET/CT examination was excluded. The number of final patients of this study was 799, the reasons of $^{18}F$-FDG PET/CT examination of these patients were follow-up of old cancer or suspicious tumorous lesion in 696 and disease screening in 103. $^{18}F$-FDG PET/CT image photographing was taken in Biograph-Duo made by SIEMENS, after taking normal $^{18}F$-FDG PET/CT image (1 hr) and then 1 hr later we took the thyroid 1 bed-delayed image for the patients who showed abnormal thyroid $^{18}F$-FDG uptake and above 2.0 SUV for 2 minutes every 1 bed. For the patients who showed abnormal thyroid uptake and above 2.0 SUV, 1 hr later, we took a 1 bed-delayed image and then made a comparative study between measured $SUV_{max}$ of 1 hr-abnormal uptake image and that of 2 hr-delayed image. Results and Conclusion: In this $^{18}F$-FDG PET/CT study among the patients who showed incidental $^{18}F$-FDG thyroidal uptake the number of thyroid incidentaloma was 5 (0.63%), all of then showed benign findings. And in the case of incidental $^{18}F$-FDG uptake in thyroid, $SUV_{max}$ variance obtained from 2 hr delayed image can be a indirect criteria in differentiating benign tumor from malignancy and decrease finding error. In the cases found thyroid incidentaloma when 1) $SUV_{max}$ of focal thyroid lesion is above 5.0 and 2) $SUV_{max}$ variance between normal $^{18}F$-FDG PET/CT exam and 2 hr delayed is $1.0{\pm}0.5$, they are suspected as malignancy and confirming biopsy is to be followed. Otherwise, I also think that distinct follow-up PET or CT image study is a reasonable diagnostic method.

        갑상선유두암 재발 진단에서 18F-FDG PET/CT와 경부초음파검사 병용의 유용성: PET, 경부초음파검사 그리고 혈청 티로글로불린 (thyroglobulin)의 비교

        목적: 이 연구의 목적은 갑상선유두암 환자를 대상으로 재발의 진단에서 18F-FDG PET/CT와 경부초음파검사를 함께 시행하는 것에 대한 유용성을 평가하고자 한다. 대상 및 방법: 2006년 12월부터 2008년 4월까지 갑상선유두암으로 갑상선 전절제술 및 고용량의 방사성옥소 치료를 시행 받은 환자 중에서 재발이 의심되어 18F-FDG PET/CT를 시행한 61명(91건)을 대상으로 후향적으로 연구를 진행하였다. 의심되는 병소는 조직검사를 시행하였고, 조직 검사결과와 PET 검사결과, PET/CT 검사결과, 경부초음파검사 결과를 비교하였다. 결과: 18F-FDG PET/CT (여자 57명, 남자 4명; 연령 범위, 24-81세, 평균 49세; 유두암 61개)의 민감도, 특이도, 정확도는 각각 환자 기준으로 87.2%, 59.0%, 77.0%였고, 병소 기준으로 92.3%, 64.1%, 80.2% 였다. 18F-FDG PET의 민감도, 특이도, 정확도는 각각 환자 기준으로 71.8% (p=0.03), 59.0% (p=1.00), 67.2% (p=0.03)였고, 병소 기준으로 78.8% (p=0.01), 64.1% (p=1.00), 72.5% (p=0.02)였다. 경부초음파검사의 경우 민감도, 특이도, 정확도는 각각 환자 기준으로 71.1% (p=0.07), 52.2% (p=0.75), 63.9% (p=0.08)였고, 병소 기준으로 71.2% (p<0.01), 61.5% (p=1.00), 67.0% (p=0.06)였다. 18F-FDG PET/CT와 경부초음파검사를 함께 판독한 경우 민감도, 특이도, 정확도는 각각 환자 기준으로 94.7% (p=0.50), 82.6% (p=0.13), 90.2% (p=0.03)였고, 병소 기준으로 96.2% (p=0.50), 89.7% (p<0.01), 93.4% (p<0.01)였다. 결론: 18F-FDG PET/CT는 갑상선유두암의 재발 진단에서 18F-FDG PET와 경부초음파검사와 비교하여 높은 민감도, 특이도, 정확도를 보였다. 게다가 18F-FDG PET와 경부 초음파를 함께 시행한 경우 갑상선유두암의 재발 진단에 있어서 더욱 향상된 민감도, 특이도, 정확도를 보였다. Purpose: The aim of this study was to investigate the usefulness of 18F-FDG PET/CT with neck ultrasonography (neck US) in patients with recurrent, papillary thyroid cancer. Material and methods: This retrospective study (December 2006 to April 2008) enrolled sixty-one patients (ninety-one lesions) who underwent high-dose 131I-ablation therapy after total thyroidectomy, and evaluated recurred papillary thyroid cancer. All lesions were confirmed by histopathology and compared histopathologic findings to PET, PET/CT, and neck US findings. Results: In sixty-one patients (57 women, 4 men; age range, 24-81 years, mean 49 years; 61 papillary carcinomas), the sensitivity, specificity, accuracy of 18F-FDG PET/CT was 87.2%, 64.0%, 78.1% on a patient basis and 92.3%, 66.7%, 80.9% on a lesion basis, respectively. The sensitivity, specificity, accuracy of 18F-FDG PET was 71.8% (p=0.03), 59.0% (p=1.00), 67.2% (p=0.03) on a patient basis and 78.8% (p<0.01), 64.1% (p=1.00), 72.5% (p=0.02) on a lesion basis, respectively. The sensitivity, specificity, accuracy of neck US was 71.1% (p=0.07), 52.2% (p=0.75), 63.9% (p=0.05) on a patient basis and 71.2% (p<0.01), 61.5% (p=1.00), 67.0% (p=0.06) on a lesion basis, respectively. Combined 18F-FDG PET/CT with neck US improved the sensitivity, specificity, accuracy to 94.7% (p=0.50), 82.6% (p=0.13), 90.2% (p=0.03) on a patient basis and 96.2% (p=0.50), 89.7% (p<0.01), 93.4% (p<0.01) on a lesion basis, respectively. Conclusion: 18F-FDG PET/CT demonstrated significantly higher sensitivity than neck US for the detection of recurred papillary thyroid cancer lesions. Furthermore, combined 18F-FDG PET/CT with neck US showed more improved sensitivity, specificity, accuracy for diagnosis of recurrent papillary thyroid cancer.         처음 진단된 두경부 선양낭성암종에서 18F-FDG PET/CT: 임상상 및 병리소견과의 상관성

        목적: 처음 진단된 두경부 선양낭성암종의 18F-FDG PET/CT의 영상소견을 선양낭성암종의 병리적 소견, 병기 결정, 가장 흔한 두경부암인 편평세포암종의 18F-FDG 섭취양상, 예후와 상관 지어 알아보았다. 대상 및 방법: 두경부 선양낭성암종으로 처음 진단되어 18F-FDG PET/CT를 시행한 16명의 환자가 대상이었다. PET/CT의 병기 결정 능력을 알아보고, 두경부 선양낭성암종의 SUVmax를 원발종양의 조직학적 아형(고형 대 관상형/사상형상), 병변의 크기와 일치하는 두경부 편평세포암종의 SUVmax, 무병생존율과 비교하였다. 결과: 16명의 환자들 중, 관상형 또는 사상형을 가진 군은 10명이었고 나머지 6명은 고형을 가진 군이었다. 고형 선양낭성암종의 SUVmax는 관상형 또는 사상형 선양낭성암종 보다 유의하게 높았다(6.7±3.2 vs. 4.2±0.9, p=0.03). 18F-FDG PET/CT로 기존 영상법에서 발견하지 못한 원격 전이를 18.7%의 환자에서 발견하여, 치료방침 변경에 기여하였다. 선양낭성암종의 SUVmax는 병변 크기가 같은 편평세포암종과 비교해 보았을 때 유의하게 낮았다(5.1±2.4 vs. 13.6±6.0, p<0.001). 조직학적 아형에 따른 무병생존율은 통계적으로 유의한 차이를 보이지 않았다. 반면에, 원발종양의 SUVmax가 6.0 이상인 환자들의 무병생존율은 SUVmax가 6.0 미만인 환자들보다 유의하게 낮았다(p=0.002). 결론: 두경부 선양낭성암종의 18F-FDG 섭취양상은 조직학적 아형과 무병생존율과 유의한 관계가 있다. 18F-FDG PET/CT는 기존 영상법에서 발견하지 못한 원격전이 진단에 유용한 것으로 보인다. 사상형상 또는 관상형 선양낭성암종의 18F-FDG 섭취는 낮을 수 있어, 선양낭성암종이 의심되지 않은 환자들의 PET 영상 판독에 주의가 필요하다. Purpose: We evaluated 18F-FDG PET/CT findings in initially diagnosed adenoid cystic carcinoma (ACC) of the head and neck in association with pathological subtype, staging, uptake comparison with squamous cell carcinoma (SqCC) and prognosis. Materials and Methods: The subjects were 16 patients with initially diagnosed ACC of head and neck who underwent pretreatment 18F-FDG PET/CT. Histological subtype (solid pattern vs. tubular/cribriform pattern), SUVmax of size-matched SqCC of the head and neck as control group, disease-free survival (DFS) were compared with the SUVmax of ACC of the head and neck. Results: Of total 16 patients, 6 had solid pattern and the remaining 10 had tubular/cribriform pattern. The SUVmax were significantly higher in solid pattern group than in tubular/cribriform pattern group (6.7±3.2 vs. 4.2±0.9, p=0.03). PET/CT found unexpected distant metastasis in 18.7% of patients (3/16) and changed the therapeutic plan in those patients. The SUVmax of ACC was significantly lower than that of size-matched SqCC (5.1±2.4 vs. 13.6±6.0, p<0.001). DFS was not significantly different according to the histological subtype. In contrast, patients with high 18F-FDG uptake (SUVmax≥6.0) had significantly shorter DFS than those with low 18F-FDG uptake (SUVmax<6.0). Conclusion: 18F-FDG uptake of ACC of the head and neck is significantly associated with histological subtype and DFS. 18F-FDG PET/CT may be useful for detecting unexpected metastasis. Since 18F-FDG uptake of tubular/cribriform ACC compared with SqCC is relatively low, it is necessary to interpret PET images carefully in patients without alleged ACC.         Usefulness of $^{18}F$-FDG PET/CT for the Evaluation of Bone Marrow Involvement in Patients with High-Grade Non-Hodgkin's Lymphoma

        Purpose To assess the usefulness of $^{18}F$-fluorodeoxyglucose PET/CT in the detection of bone marrow (BM) involvement of high-grade non-Hodgkin's lymphoma (NHL). Methods One hundred twenty patients with newly diagnosed diffuse large B-cell lymphoma or peripheral T-cell lymphoma between January 2007 and June 2011, who received BM trephine biopsy and $^{18}F$-FDG PET/CT before chemotherapy, were included in this retrospective study. We reviewed their $^{18}F$-FDG PET/CT images and bone marrow biopsy (BMB) results. After reviewing the images, we reviewed the medical records and radiological findings of interesting patients. Results There were 23 $^{18}F$-FDG PET/CT scans in which the marrow was considered to be abnormal (either positive or equivocal), and 97 $^{18}F$-FDG PET/CT scans were regarded as having negative FDG uptake. Of 120 patients, 100 (83.3 %) had a concordant result of BM interpretation between $^{18}F$-FDG PET/CT and BMB, and the remaining 20 patients had discordant results. Among 23 patients with either positive or equivocal $^{18}F$-FDG PET/CT scans, 1 of 12 patients with 'positive' $^{18}F$-FDG PET/CT had a lymphomatous involvement on BMB. In contrast, 10 of 11 patients with 'equivocal' BM hypermetabolism were reported as having positive involvement by BMB. Patients with abnormal $^{18}F$-FDG PET/CT had significantly higher $mSUV_{highest}$ than those with normal FDG-PET/CT. Conclusions $^{18}F$-FDG PET/CT and BMB are complementary techniques in assessing the presence of BM involvement in patients with high-grade NHL. The increasing availability of $^{18}F$-FDG PET/CTwill raise the need for additional biopsy for FDG-avid lesions, especially in patients with negative standard BMBs. $^{18}F$-FDG PET/CT can be useful as a decision-making tool for determining whether to perform a standard BMB or targeted biopsy to the FDG-avid lesion as an initial staging procedure.A direct bone biopsy for FDGpositive bone lesions should be included in staging guidelines in future. In $^{18}F$-FDG PET/CT-negative cases, BMB is still a powerful procedure, but BMB alone is insufficient for full evaluation of BM.

        <sup>18</sup>F-FDG PET/CT검사에서 지연영상을 이용한 갑상선암 진단의 유용성 평가

        목적: 본 연구는 <sup>18</sup>F-FDG PET/CT검사 시 우연히 발견된 갑상선병변의 악성여부를 평가하기 위하여 위양성 으로 판독되어지는 양성종양이나 염증질환과 악성 종양의 감별이 가능한 검사법으로 지연영상(Delay scan)을 적용하 고 결과 값(SUV)의 기준안을 마련하며 이 연구에서 제시되는 검사법의 유용성을 평가하는데 있다. 재료 및 방법 : 2008년 1월부터 2008년 4월 까지 E대학병원에 내원하여 <sup>18</sup>F-FDG PET/CT를 시행한 환자 25명(1명 제 외)을 대상으로 후향적 분석을 실시하였다. 18F-FDG PET/CT영상 촬영은 SIEMENS사에서 만든 Biograph-Duo에서 시행 하였으며, <sup>18</sup>F-FDG PET/CT 정상영상(1hr)을 취득한 후 갑상선이상섭취가 있고 SUV 2.0이상인 환자를 대상으로 1시간 후에 갑상선부위를 1bed 지연영상을 촬영하였고, 한 bed에 2분 동안 영상을 얻었다. 갑상선이상섭취가 있고 SUV 2.0이 상인 환자를 대상으로 이상섭취가 있는 부위의 1시간영상과 2시간지연영상의 maxSUV를 측정하여 비교분석하였다. 결과: <sup>18</sup>F-FDG PET/CT 검사 시 우연히 갑상선에 <sup>18</sup>F-FDG 섭취를 보인 환자 24명중 갑상선 암(Thyroid cancer)이 5 명으로 20.8%로 나타났으며 모두 양성병변을 시사하는 소견을 나타냈다. 갑상선기능이상이나 갑상선염 등 단순 갑상 선 이상섭취로 판독된 19명의 1시간영상과 2시간 영상의 표준섭취계수(SUV)값은 나타낸 표로 1시간영상의 18F-FDG 표준섭취 평균값은 5.06maxSUV이었고, 2시간 평균값은 5.23maxSUV였다. 측정된 두 값의 차이는 0.19maxSUV이었 다. 갑상선암으로 판독된 5명의 1시간 PET/CT검사에서 갑상선국소부위의 <sup>18</sup>F-FDG 표준섭취 평균값은 9.63maxSUV 이었고 2시간 평균값은 10.65maxSUV이였다. 두 값의 차이는 1.02maxSUV로 나타났다. 결론 : 본 연구는 우연히 발견된 갑상선 국소부위에 5.0이상 maxSUV의 이상섭취병변이 발견된 경우에는 <sup>18</sup>F-FDG PET/CT 지연영상을 추가로 검사한다면 병소의 악성 여부 감별에 도움을 줄 수 있는 유용한 촬영방법임을 확인할 수 있었다.       • Diagnostic Value of <sup>18</sup>F-FDG PET/CT in Comparison to Bone Scintigraphy, CT and <sup>18</sup>F-FDG PET for the Detection of Bone Metastasis

        Purpose: To evaluate the diagnostic value of $^{18}F$-FDG PET/CT for detection of bone metastasis in comparison with the efficacies of $^{18}F$-FDG PET/CT, CT, $^{18}F$-FDG PET and conventional planar bone scintigraphy in a series of cancer patients. Methods: Five hundred and thirty patients who underwent both $^{18}F$-FDG PET/CT and bone scintigraphy within 1 month were retrospectively analyzed. The skeletal system was classified into 10 anatomic segments and interpreted blindly and separately. For each modality, the sensitivity, specificity, accuracy, PPV and NPV were calculated and the results were statistically analyzed. Results: Bone metastases were confirmed in 117 patients with 459 positive segments. On patient-based analysis, the sensitivity, specificity, accuracy, PPV and NPV of $^{18}F$-FDG PET/CT were significantly higher than bone scintigraphy, CT and $^{18}F$-FDG PET (P<0.05). On segment-based analysis, the sensitivity of CT, bone scintigraphy, $^{18}F$-FDG PET and $^{18}F$-FDG PET/CT were 70.4%, 89.5%, 89.1% and 97.8%, respectively (P<0.05, compared with $^{18}F$-FDG PET/CT). The overall specificity and accuracy of the four modalities were 89.1%, 91.8%, 90.3%, 98.2% and 90.3%, 90.9%, 89.8%, 98.0%, respectively (P<0.05, compared with $^{18}F$-FDG PET/CT). The PPV and NPV were 89.8%, 87.6%, 85.6%, 97.2% and 85.6%, 93.2%, 92.8%, 98.6%, respectively. Three hundred and twelve lesions or segments were presented as lytic or sclerotic changes on CT images at the corresponding sites of increased $^{18}F$-FDG uptake. In lytic or mixed lesions, the sensitivity of $^{18}F$-FDG PET/CT and $^{18}F$-FDG PET were better than bone scintigraphy, while in osteoblastic lesions bone scintigraphy had a similar performance with $^{18}F$-FDG PET/CT but better than $^{18}F$-FDG PET alone. Conclusion: Our data allow the conclusion that $^{18}F$-FDG PET/CT is superior to planar bone scintigraphy, CT or $^{18}F$-FDG PET in detecting bone metastasis. $^{18}F$-FDG PET/CT may enhance our diagnosis of tumor bone metastasis and provide more information for cancer treatment.

        Opportunities for 2-[18F] Fluoro-2-Deoxy-D-Glucose PET/CT in Cervical-Vaginal Neuroendocrine Carcinoma: Case Series and Literature Review

        Objective: Neuroendocrine cervical carcinoma is a rare subtype of cervical cancer. These tumors exhibit an aggressive behavior with early regional lymph node and distant metastases. The purpose of our study was to describe five cases of neuroendocrine cervical-vaginal carcinoma and to discuss the potential of the 2-[18F] fluoro-2-deoxy-D-glucose positron emission tomography/computed tomography (18F-FDG PET/CT) scan for the detection of this rare malignancy. Materials and Methods: Five cases of cervical-vaginal neuroendocrine tumor were retrospectively collected, during a two year (from September 2009 to August 2011) period in our hospital. The clinical staging distributions were International Federation of Gynecology and Obstetrics (FIGO) stage IB2 (1 of 5), stage IIA (3 of 5) and stage IVA (1 of 5). Results: Two cases (cases 1 and 4) were restaged after 18F-FDG PET/CT scan in the initial staging process. Post-treatment 18F-FDG PET/CT scans, in three patients, revealed positive findings for tumor recurrence or lymph node metastases. Two patients (cases 2 and 3) died of tumor within two years. Conclusion: 18F-FDG PET/CT scan is a useful tool in cervical-vaginal neuroendocrine tumor. In its initial staging, the 18F-FDG PET/CT scan may help assess the possible nodal involvement or early hematogeneous spreading. We can also use the 18F-FDG PET/CT to detect local recurrence and to evaluate the treatment response after clinical manipulation.       • 월경주기에 따른 $^{18}F$-FDG PET/CT의 자궁 내 섭취에 관한 연구

        $^{18}F$-FDG PET/CT는 암세포에서 당 대사가 항진되는 현상을 이용하여 암환자의 진단에 폭넓게 사용 되고 있으며, 악성 종양에 대부분 섭취 되는 것이 일반적이지만, 정상 조직에도 섭취되는 경우도 있다. 이러한 현상은 보통 소화기계에서 빈번히 일어나며, 월경을 하는 가임기 여성에게서 자궁 주위에서의 섭취가 일어나기도 한다. 특히 가임기 여성은 월경에서 각 주기 동안 난소는 월경기, 증식기, 배란기, 분비기 이렇게 4단계의 주기를 거친다. 이러한 월경 주기에 따른 $^{18}F$-FDG 섭취는 영상에 다양한 영향이 미치기 때문에, 본 연구는 자궁의 $^{18}F$-FDG 섭취와 월경주기와의 관계를 알아보고자 하였다. 2008년 1월부터 2009년 3월까지 부인과 질환이 없고 월경이 규칙적인 $38{\pm}7.93$세 여성 환자 200명을 대상으로 하였다. 검사 전 6~8시간 금식 후 $^{18}F$-FDG 370 MBq를 정맥주사하고, 1시간 동안 충분한 휴식 후 전신 PET/CT 검사를 시행하였다. 영상 분석은 CT 영상으로 난소와 자궁의 위치를 정확히 파악한 후 같은 위상의 영상에서 표준화섭취계수를 측정하여 분석하였다. 월경주기에 따라 SUV의 변화가 크지는 않았지만, 분명 차이가 있었으며, 월경기 $SUV_{max}$는 $3.17{\pm}1.59$, $SUV_{avg}$는 $2.89{\pm}1.04$였고, 증식기 $SUV_{max}$는 $2.98{\pm}1.14$, $SUV_{avg}$는 $2.40{\pm}0.88$였다. 그리고 배란기 $SUV_{max}$는 $3.71{\pm}1.67$, $SUV_{avg}$는 $3.59{\pm}1.76$였고, 분비기 $SUV_{max}$는 $3.1{\pm}1.80$, $SUV_{avg}$는 $2.58{\pm}1.39$였다. 가임기 여성은 월경 주기에 따라 자궁과 난소에서 일정한 주기로 변화가 일어나며, 이에 따라 $^{18}F$-FDG PET/CT에서도 변화가 일어난다. 월경기, 증식기, 배란기, 분비기 이렇게 4단계의 주기를 가지고 영상에서 변화가 일어나므로, 검사 전 가임기 여성의 월경 주기를 파악하여 검사 시에 활용한다면, 자궁주변의 병변의 진단에 큰 도움이 될 것으로 사료된다. Purpose: The menstrual cycle of normal premenopausal patients was divide into menstrual flow phase, proliferative phase, ovulatory phase, secretory phase. The aim of this study was to ovarian and endometrial $^{18}F$-FDG uptake during the menstrual cycle in normal premenopausal patients. Materials and Methods: We identified 200 incidental $^{18}F$-FDG uptake in the ovary. The patient fasted at least 6 hours before receiving an intravenous injection of 370-592 MBq (10-16mCi) of $^{18}F$-FDG. Scanning from the base of the skull though the mid thigh was performed using the Discovery Ste PET/CT system (GE Healthcare, Milwaukee, WI, USA). Ovarian and endometrial $^{18}F$-FDG uptake (expressed as standardized uptake value) was measured on PET/CT image. Results: Two peaks of increased endometrial $^{18}F$-FDG uptake were identified during the menstrual cycle. The $SUV_{avg}$ and $SUV_{max}$ was $2.89{\pm}1.04$ and $3.17{\pm}1.59$ in menstruating patients, $2.4{\pm}0.88$ and $2.98{\pm}1.14$ in proliferative phase patients, $3.59{\pm}1.76$ and $3.17{\pm}1.67$ in ovulatory phase patients, $2.58{\pm}1.39$ and $3.1{\pm}1.8$ in secretory phase patients. Conclusions: Increased ovarian and endometrial $^{18}F$-FDG uptake could be found the time of menstrual flow and ovulatory phase of menstrual cycle. Increased uptake in endometrial adjacent to a cervical tumor does not necessarily reflect endometrial tumor invasion. Since increased uptake was dependent on the menstrual cycle, it can be avoided by scheduling PET/CT just after menstruation. Non-menstrual-related endometrial uptake may be instrumental in establishing a diagnosis in a premenopaual patient.

        The Role of 18^F-FDG PET/CT in the Evaluation of Gastric Cancer Recurrence after Curative Gastrectomy

        Purpose: 18^F-fluorodeoxyglucose positron emission tomography/computed tomography (18^F-FDG PET/CT) scans are frequently performed for the screening or staging of malignant tumors. This study aimed to assess the usefulness of 18^F-FDG PET/CT in detection of gastric cancer recurrence after curative gastrectomy. Materials and Methods: Eighty nine patients who had undergone curative gastrectomy due to gastric cancer and had 18^F-FDG PET/CT and contrast CT scans within 2 weeks for surveillance in asymptomatic patients (n = 11) or to clarify suspected recurrence (n = 78) were consecutively collected and retrospectively analyzed. They had clinical follow-up for at least 12 months after PET/CT and CT scans. Results: Fifteen of the 89 patients (16.9%) were diagnosed with recurrent gastric cancer in 21 organs. Forty one organs showed an increase in FDG uptake, and only 9 of these organs were diagnosed with recurrent gastric cancer by 18^F-FDG PET/CT. The sensitivity,specificity, positive predictive value, negative predictive value, and diagnostic accuracy of the 18F-FDG PET/CT were 42.9%, 59.7%, 29.3%, 78.2%, and 57.3%, respectively. On the CT scan, 18 of 21 recurrent gastric cancers were detected,and 7 cases were in agreement with the 18^F-FDG PET/CT. The sensitivity and specificity of the CT scan were 85.8% and 87.3%, respectively, which are superior to the 18^F-FDG PET/CT. When we diagnosed a recurrence based on either 18^F-FDG PET/CT or CT scans, the sensitivity increased to 95.2% and the specificity decreased to 45.6%, when compared with the contrast CT scan alone. Conclusion: 18^F-FDG PET/CT is an insufficient diagnostic method in detection of recurrence after curative gastrectomy, and even less accurate than contrast CT scan alone.

        18F-FDG PET/CT에서 우연히 관찰되는 구개편도의 비대칭적 섭취증가의 임상적 의의

        목적: 이 연구는 다양한 임상 적응증으로 시행된 전신 18F-FDG PET/CT 영상에서 구개편도의 비대칭적인 섭취증가의 발견빈도와 악성여부를 조사하고, 악성병변을 시사하는 임상 및 PET/CT 영상소견들을 알아보고자 하였다. 대상 및 방법: 1년 동안 18F-FDG PET/CT를 시행한 2,901명(평균연령 58.4±12.3세, 연령범위 20~88세, 남성:여성=1,841:1,060)의 PET/CT영상에서 구개편도의 비대칭적 섭취증가 여부 및 형태를 육안적으로 분석하였다. 구개편도에 비대칭적인 섭취증가를 보이는 빈도와 악성률을 조사하였고 관련된 임상적 소견 및 PET/CT 영상에서 동반된 이상소견과의 연관성을 평가하였다. 결과: 2,901명 중 10.0%(291명)에서 구개편도에 비대칭적인 섭취증가를 보였으며 겨울에 가장 빈번하게 관찰되었다(12.1%). 임상 추적관찰 혹은 조직학적 검사를 통해 최종적으로 악성여부가 확인된 209명 중 2.4%(5명)가 악성병변으로 진단되었고 불규칙한 두개편도의 섭취증가나 경부림프절의 섭취증가 소견이 동반된 경우, 원발미상의 경부림프절전이 환자에서 PET/CT를 시행한 경우 악성률이 높았다. 결론: 전신 18F-FDG PET/CT에서 우연히 관찰되는 구개편도의 비대칭적인 섭취증가는 대부분이 양성으로 악성률은 낮았다. 그러나 원발미상의 경부림프절 전이환자나 PET/CT에서 구개편도의 섭취증가가 불규칙할 경우에는 악성의 가능성이 높기 때문에 적극적인 조직학적 검사가 필요하리라 생각된다. Purpose: The aim of this study was to determine the incidence and malignant rate of incidental asymmetric palatine tonsillar uptake (ATU) on 18F-FDG PET/CT in various clinical indications and to evaluate the clinical and PET/CT findings suggesting malignancy. Materials and Methods: We retrospectively reviewed a total of 2,901 patients (58.4±12.3 yrs, range 20~88 yrs, M:F=1,841:1,060) who underwent 18F-FDG PET/CT during an 1-year period with various indications except primary tonsillar cancer and lymphoma evaluation. On 18F-FDG PET/CT, metabolic abnormality of the palatine tonsil and cervical lymph node were visually assessed. ATU was defined as increased palatine tonsillar uptake with diffuse, focal, or irregular pattern compared to contralateral side. The incidence and malignant ratio of ATU were evaluated according to clinical and PET/CT findings. Results: Of 2,901 cases, 290 (10.0%) showed ATU. The incidence of ATU showed seasonal variation and was high in the winter (12.1%). Of 209 cases with ATU confirmed pathologically and/or clinically, five (2.4%) were malignant lesions. ATU with irregular uptake pattern (2/2) and in cases referred for cervical lymph node metastasis of unknown origin (3/5) were frequently associated with malignant lesion (p<0.05). Conclusion: ATU was not infrequently observed on 18F-FDG PET/CT, and the malignant risk of ATU was low. However, ATU with cervical lymph node metastasis or with irregular pattern on PET/CT would be further evaluated by the histopathologic examination. (Nucl Med Mol Imaging 2009;43(5):402-410)
