      • A Case of a Gallbladder Infarction after Transcatheter Arterial Chemoembolization, Recovered with Only Medical Treatment

        Transcatheter arterial chemoembolization (TACE) is suitable treatment method of hepatocellular carcinoma (HCC) for patients with decreased underlying liver function or unresectable disease. But this procedure can induce some embolic events. We report a case of a gallbladder infarction caused by the embolic event in TACE procedure. A 61-year-old man had been going to undergo 6th TACE for marginal recurrence of HCC. After the procedure, gallbladder infarction occurred and was detected by abdominal computed tomography (CT) scan for response evaluation of the treatment at out-patient-clinic. Then, he was treated with oral antibiotics and conservative therapy at out-patient clinic. In 6th week after the event, abdominal CT scan showed improved state of gallbladder.

        국내 애견 관련 산업은 2012 년 기준 이미 2 조원이라는 큰 시장을 형성하였고, 국내의 애완동물을 키우는 인구수는 1,000 만 명에 육박한다. 앞으로도 애견 관련 산업은 지속적으로 성장할 것으로 예상된다. 그런데 많은 애견인들은 바쁜 라이프 스타일 때문에 애견을 충분히 돌보지 못하고 있다. 애견의 분리불안 증세나 우울증 등의 문제를 해결하기 위하여 애견 유치원, 호텔, 애견용 TV 채널 등 다양한 해결책이 나왔으나 아직 획기적인 해결책은 나오지 않았다. 우리는 이 문제에 주목하여 ‘애견의 정서발달을 쉽게 관리할 수 있는 웨어러블 디바이스’인 GATWALK (개 + Network + Walk 의 합성어)를 기획하였다. The market of pet industry is formed as big as about 16 billion dollars, and the population of pet holder reaches 10 million in Korea. And it’s expected to be growing. But many of pet holders are not able to take good care of their pets because of hectic lifestyle. In this context, we designed GATWALK, a wearable device that helps pet to have better emotional status and relationship with the pet owner.

      • EEG and EMG recording system for sleep study with a freely moving rat

        Sleep is known to play an important role in cognitive functions as well as in physical recovery. Especially, high-quality sleep is closely associated with smooth sleep cycles between NREM (non-rapid eye movement) and REM (rapid eye movement) sleep. It is well known that both sleep stages show clear distinction in electroencephalogram (EEG) and electromyogram (EMG) signals. Therefore, as a first step for sleep study with rodents, a sleep monitoring system must be established for stable measurement of EEG and EMG. There are two challenges for the setup. First, animals should be able to freely move without interruption of EEG or EMG recording cable, so that their natural behavior and sleep can occur. Second, long-term EEG and EMG should be recorded stably. Also, the movements of animals can cause a cable tension and moving artifacts. The movement-related artifacts can interfere with judgment of sleep stages. Also, rodent gnawing of the recording cable frequently leads to early termination of sleep study. In this study, in order to overcome these problems, acrylic cages were developed for sleep monitoring which can minimize the interruption of cables and the artifacts. Principle of lever mechanics is applied to rotational cover of the cage, and it rotates vertically in synchronization with rat’s movement. When the rat stand up, the cage cover opens up to prevent the cable from loosening. Therefore, cable tension can be stably maintained, allowing the rat to move freely without causing artifact. As a result, long-term EEG and EMG recordings are successful performed, and specific characteristics of each sleep stages can be extracted due to the high-quality recorded signals. We expected that the developed sleep monitoring setup can be applied to various long-term in vivo experiments with rats.

        Biomonitoring using sample derived from human has been conducted in several countries. Most researches have traced organochloride insecticides which were banned since 1970s as endocrine distributor. Recently, researchers has conducted biomonitoring using urine and blood in human. Urine sample is difficult to calculate quantitative amount of pesticide because of metabolism in the kidney. It is also influenced by intake of waters or other factors. Blood is a regulated fluid and its volume does not vary virtually with water intake or other factors. Pesticides in the blood measured at a specified time interval after exposure will remain the same as long as the absorbed amounts are constant. Blood measurement provide and estimation of the dose available for the target site, allowing for prediction of dose-responserelationships. Blood concentrations of the pesticides are usually at a maximum directly after exposure, so if exposure events are known, the preferred time range for sampling may be clearer than with urine. But It has disadvantage also, when we sampling the human blood, it is needed to venipuncture. The amount of blood is influenced by collector’s techniques. In this study, we indicated that the results of biomonitoring researches and its methods using human blood. We also compared abroad and domestic research and its methods. It will contribute to improve domestic researches of residual amounts of pesticides in biomonitoring using human blood.

        Disappearance of contralateral dominant neural activity of auditory cortex after single-sided deafness in adult rats

        <P><B>Abstract</B></P> <P>Hearing loss in mature ears can cause functional reorganization of the auditory cortex. The functional reorganization is speculated to negatively affect the outcome of hearing rehabilitation. Therefore, once hearing loss occurs, it is important to provide auditory input before extensive reorganization in the auditory pathways. We investigated the neural plasticity in auditory cortex after single-sided deafness (SSD) in an adult rat model. The animals were divided into two groups: a normal hearing (NH) and the SSD group. The neural recordings of the SSD group were conducted at different time points (2, 4, 6 and 8 weeks) after cochlear ablation. The multi-unit activity was discriminated on the sum of spikes, peak amplitude, onset latency, peak latency, and responsive area based on the peak amplitude. The auditory cortical reorganization was observed after SSD. The contralateral dominance of peak amplitude and latency that normally occur in NH group were not present in the SSD group, replaced by higher amplitude and faster response in ipsilateral cortex. According to serial recordings at different time points after SSD, different phases in the response of the auditory cortex were speculated. Compared with normal hearing, alteration of contralateral dominance was observed because of the functional reorganization of the auditory cortex after SSD.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Adult rats show plastic changes of both brain hemispheres after single side deafness. </LI> <LI> The plastic change can be summarized as the loss of normal contralateral dominance. </LI> <LI> Serial time point analysis suggests the two different phase after single side deafness. </LI> </UL> </P>

      • Puzzle Taxi: How to Share the Time and Space for Everyone in the Road

        This study focuses on the challenge in Seoul's public transportation system where taxis are increasingly used for short distances due to inconvenience during peak times. The rise in taxi fares, dissatisfaction, and traffic congestion caused by singleperson taxis add strain to the system. Public dissatisfaction with high-cost taxis is revealed, leading to a decline in taxi usage, and the study emphasizes the demand for both long and short-distance taxis during rush hours. It proposes the Puzzle Taxi as an innovative solution to reduce costs and traffic congestion. The envisioned service involves shared taxis with flexible boarding, aiming to improve efficiency, service quality, and contribute to a sustainable transportation system. In conclusion, revitalizing public transportation with shared services and introducing additional services in taxis for public benefit is suggested to enhance convenience and satisfaction while addressing issues related to private sharing reluctance.

        Clinical, endocrinological, and molecular features of four Korean cases of cytochrome P450 oxidoreductase deficiency

        Purpose: Cytochrome P450 oxidoreductase (POR) deficiency is a rare autosomal recessive disorder caused by mutations in the POR gene encoding an electron donor for all microsomal P450 enzymes. It is characterized by adrenal insufficiency, ambiguous genitalia, maternal virilization during pregnancy, and skeletal dysplasia. In this study, we investigated the clinical, hormonal, and molecular characteristics of patients with POR deficiency in Korea. Methods: This study included four patients with POR deficiency confirmed by biochemical and molecular analysis of POR. Clinical and biochemical findings were reviewed retrospectively. Mutation analysis of POR was performed by Sanger sequencing after polymerase chain reaction amplification of all coding exons and the exon-intron boundaries. Results: All patients presented with adrenal insufficiency and ambiguous genitalia regardless of their genetic sex. Two patients harbored homozygous p.R457H mutations in POR and presented with adrenal insufficiency and genital ambiguity without skeletal phenotypes. The other two patients with compound heterozygous mutations of c.[1329_1330insC];[1370G>A] (p.[I444Hfs*6];[R457H]) manifested skeletal abnormalities, such as craniosynostosis and radiohumeral synostosis, suggesting Antley-Bixler syndrome. They also had multiple congenital anomalies involving heart, kidney, and hearing ability. All patients were treated with physiologic doses of oral hydrocortisone. Conclusion: We report the cases of 4 patients with POR deficiency identified by mutation analysis of POR. Although the study involved a small number of patients, the POR p.R457H mutation was the most common, suggesting founder effect in Korea. POR deficiency is rare and can be misdiagnosed as 21-hydroxylase or 17α-hydroxylase/17,20-lyase deficiency. Therefore, molecular analysis is critical for confirmatory diagnosis.

      • Coexistence of ZigBee-Based WBAN and WiFi for Health Telemonitoring Systems

        <P>The development of telemonitoring via wireless body area networks (WBANs) is an evolving direction in personalized medicine and home-based mobile health. A WBAN consists of small, intelligent medical sensors which collect physiological parameters such as electrocardiogram, electroencephalography, and blood pressure. The recorded physiological signals are sent to a coordinator via wireless technologies, and are then transmitted to a healthcare monitoring center. One of the most widely used wireless technologies in WBANs is ZigBee because it is targeted at applications that require a low data rate and long battery life. However, ZigBee-based WBANs face severe interference problems in the presence of WiFi networks. This problem is caused by the fact that most ZigBee channels overlap with WiFi channels, severely affecting the ability of healthcare monitoring systems to guarantee reliable delivery of physiological signals. To solve this problem, we have developed an algorithm that controls the load in WiFi networks to guarantee the delay requirement for physiological signals, especially for emergency messages, in environments with coexistence of ZigBee-based WBAN and WiFi. Since WiFi applications generate traffic with different delay requirements, we focus only on WiFi traffic that does not have stringent timing requirements. In this paper, therefore, we propose an adaptive load control algorithm for ZigBee-based WBAN/WiFi coexistence environments, with the aim of guaranteeing that the delay experienced by ZigBee sensors does not exceed a maximally tolerable period of time. Simulation results show that our proposed algorithm guarantees the delay performance of ZigBee-based WBANs by mitigating the effects of WiFi interference in various scenarios.</P>

      • The association between insulin resistance and depression in the Korean general population

        <P><B>Abstract</B></P> <P><B>Background</B></P> <P>Previous studies showed that the insulin resistance (IR) could be related to depression. However, this association is still equivocal in the general population. Herein, we aimed to investigate the association between IR and depressive symptoms in a large sample in South Korea.</P> <P><B>Methods</B></P> <P>A cross-sectional study was carried out for 165,443 Korean men and women who received a health checkup including various clinical parameters and the Center for Epidemiologic Studies Depression scales (CES-D). Subjects were stratified into subgroups by CES-D score, sex, age, and presence of diabetes. The odd ratios (ORs) for homeostasis model assessment of insulin resistance (HOMA-IR) were compared between groups using multivariable logistic regression analyses.</P> <P><B>Results</B></P> <P>After adjusting covariates (e.g. smoking, family income, marriage state, unemployment status, average alcohol use, BMI, physical activity, systolic blood pressure, diabetes), increased IR was weakly associated with greater depressive symptoms (adjusted OR=1.01 [95% CI 1.0001–1.03]). Subgroup analysis revealed this association was statistically significant in females (adjusted OR=1.03, [95% CI 1.001–1.06]), non-diabetic group (adjusted OR=1.04, [95% CI 1.02–1.06]), and young participants under the age of thirty (adjusted OR=1.17, [95% CI 1.07–1.27]). But we couldn’t find significant association in diabetic and middle to elderly participants.</P> <P><B>Conclusions</B></P> <P>This study demonstrates that there is a relationship between IR and depressive symptoms in the Korean general population. Results from this epidemiological study revealed that young adults and non-diabetic individuals with increased IR may be related with depressive symptoms.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Increased insulin resistance was associated with depressive symptoms in the Korean general population (N=165,443). </LI> <LI> The subgroup analysis revealed the association was statistically significant in female, non-diabetic and young groups. </LI> <LI> Increased insulin resistance in non-diabetic participants was more strongly associated with depressive symptoms. </LI> <LI> These findings suggest that early management of insulin resistance may prevent progression toward depressive symptoms. </LI> </UL> </P>
