Study Design: Retrospective study.Purpose: This study aimed to evaluate the incidence, characteristics, and risk factors for clinical L5–S1 adjacent segment degeneration (ASD) after L5 floating lumbar fusion.Overview of Literature: ASD is known to occur after lumbar spine fusion at a certain frequency. Several studies on radiological L5– S1 ASD have been reported. However, there are only a few studies on L5–S1 ASD with clinical symptoms, including back pain and/or radiculopathy.Methods: In total, 306 patients who received L5 floating lumbar fusion were included in this study. Clinical L5–S1 ASD was defined as newly developed radiculopathy in relation to the L5–S1 segment. Patients’ medical records and imaging data were retrospectively analyzed. The risk factors for clinical ASD were assessed by an inverse probability of treatment weighting-adjusted logistic regression analysis.Results: Clinical L5–S1 ASD occurred in 17 patients (5.6%). The mean onset time of L5–S1 ASD was 12.9±7.5 months after the primary surgery. Among these patients, 10 (58.8%) presented with clinical L5–S1 ASD within 12 months. Reoperation was performed in three patients (1.0%). The severity of L5–S1 disk degeneration did not affect the occurrence of L5–S1 ASD. Logistic regression analysis showed that the number of fusion levels was a significant risk factor for clinical L5–S1 ASD.Conclusions: The incidence and characteristics of clinical L5–S1 ASD after L5 floating lumbar fusion were retrospectively investigated. This study established that the number of fusion levels was a significant candidate factor for clinical L5–S1 ASD. Careful clinical follow-up is deemed necessary after L5 floating lumbar fusion surgery, especially for patients who received multiple-level fusions.

If the lines which connect the output terminal of DC-DC converter and the load are long, the voltage actually across load will be changed largely to produce the voltage drops by the impedances of the lines. Then, the terminalwhich senses the voltage across load is prepared apart from the output terminal of DC-DC converter, and it leads to the load, and the voltage across load is adjusted so that it is not changed largely. This method is called remote sensing.When the lines becomes long, the usual method of adjusting the voltage across load cannot suppress enough the change of it. In this paper, the robust digital controller for suppressing the change of the voltage across load is proposed.Experimental studies using DSP demonstrate that this type of digital controller is effective to suppress it.

The environment regarding low temperature equipments bas been changing a lot. There are varieties of demanded functions not only stabilization of temperature inside the warehouse or high reliability, but also saving energy, simplification of construction, prevention against global warming, and so on. We have developed various technologies in order to meet those customers' needs. Especially development of second stage compression system has contributed much to the improvement of refrigeration equipment efficiency. Also development of the system of hot gas defrost by using inverter has contributed a lot to the stability of defrosting operation. We have developed and releasee! a freezer system for a large-scaled low temperature warehouse which was integrated those above technologies since March, 2007.

Background: In order to manage the patient exposure dose in X-ray diagnosis, it is preferred to evaluate the entrance skin dose; although there are some evaluations about entrance skin dose, a small number of report has been published for direct measurement of patient. We think that a small-type optically stimulated luminescence (OSL) dosimeter, named nanoDot, can achieve a direct measurement. For evaluations, the corrections of angular and energy dependences play an important role. In this study, we aimed to evaluate the angular and the energy dependences of nanoDot. Materials and Methods: We used commercially available X-ray diagnostic equipment. For angular dependence measurement, a relative response of every 15 degrees of nanoDot was measured in 40-140 kV X-ray. And for energy dependence measurement, mono-energetic characteristic X-rays were generated using several materials by irradiating the diagnostic X-rays, and the nanoDot was irradiated by the characteristic X-rays. We evaluated the measured response in an energy range of 8.1-75.5 keV. In addition, we performed Monte-Carlo simulation to compare experimental results. Results and Discussion: The experimental results were in good agreement with those of Monte-Carlo simulation. The angular dependence of nanoDot was almost steady with the response of 0 degrees except for 90 and 270 degrees. Furthermore, we found that difference of the response of nanoDot, where the nanoDot was irradiated from the randomly set directions, was estimated to be at most 5%. On the other hand, the response of nanoDot varies with the energy of incident X-rays; slightly increased to 20 keV and gradually decreased to 80 keV. These results are valuable to perform the precise evaluation of entrance skin dose with nanoDot in X-ray diagnosis. Conclusion: The influence of angular dependence and energy dependence in X-ray diagnosis is not so large, and the nanoDot OSL dosimeter is considered to be suitable dosimeter for direct measurement of entrance surface dose of patient.

[OBJECTIVES] To understand the condition of athletes engaged in daily sports activities, it is important to investigate the pain that many such athletes can experience as well as the pain that can be an initial symptom of injury. Although handball is a contact sport associated with frequent injuries, the actual nature of pain symptoms that develop has not been sufficiently studied, and the relationship between pain occurrence and athletes’ daily changing physical load remains unclear. This study aimed to clarify the nature of pain symptoms in handball players and examine the relationship between pain occurrence and physical load. [METHODS] This study involved a 12-month daily assessment of pain symptoms and physical load involving 11 university female handball players. Pain was examined in terms of body region and a pain severity score using a pain questionnaire, and physical load on handball was assessed based on playing hours, types of matches and training recorded by a video camera. [RESULTS] The total number of pains was 1698, and the pain incidence rate was 288.1 pains per 1000 player hours. In terms of body regions, the ankle (18.3%) was the most common, followed by the lower back (13.3%), foot (12.8%), Achilles tendon (9.2%), and thigh (8.9%). With regard to the relationship between pain and physical load, significant moderate or weak correlations were observed between handball (rs=0.657), training (rs=0.626), and on-court training (rs=0.591) and overall pain occurrences. In terms of body regions, the ankle and thigh, significant moderate or weak correlations were observed with respect to all categories in on-court training. [CONCLUSIONS] We found that pain occur frequently and athletes continue to compete in games despite experiencing pain. In addition, pain occurrence was related to physical load, indicating that the type of physical load depends on the body region.
