        National Health Insurance Data Analysis for the Second-Wave Development of Korean Medicine Clinical Practice Guidelines in South Korea

        Objectives: After the evidence-based Korean medicine clinical practice guidelines (KMCPGs) for 30 targeted diseases were developed in 2021, 34 diseases have been proposed for the second-wave development of the KM-CPGs. The purpose of this study was to investigate the development priorities of the candidate diseases into the second-wave development of KM-CPGs in south Korea. Methods: In this study, we analyzed the Health Insurance Review and Assessment Service National Patient Sample data from 2017 to 2018 to determine the demand and economic importance of the candidates for the second-wave development of KM-CPGs in real-world clinical settings in Korea. Results: The annual number of visits and patients, annual healthcare expenditure per patient, and healthcare expenditure per institution were analyzed. Musculoskeletal disorders, including sciatica and adhesive capsulitis of the shoulder, were the most important topics regarding the number of visits and patients and annual healthcare expenditure per institution. Specifically, sciatica (52.05% of the total number of visits, 48.34% of the total number of patients, and 42.12% of the total treatment expenditure per institution) showed overwhelmingly high proportions. However, cerebral palsy (36.03% of the total number of inpatient visits and 24.55% of the total number of inpatient patients) was a more important topic in inpatient clinical settings than musculoskeletal conditions or cancer, and healthcare expenditure per patient in this topic had the highest ranking. Furthermore, fractures were found to be highly important in inpatient clinical settings. No patients had influenza A virus infection or posttraumatic stress disorders who visited the KM medical institution of interest. Conclusion: This study highlights the gap between the real-world clinical setting and the research field in some topics. The results of this study can provide guidance for the second-wave development of KM-CPGs in the future.

        이기종 스마트워치의 라이프로그 수집을 위한 관계형 데이터베이스 모델

        Recently, the market for wearable devices has been growing, and many researches have been worked to collect lifelog using smart watches. However, it is difficult to collect lifelog on each device as various smart watch manufacturers such as Fitbit, Apple, Samsung, and Xiaomi serve on different platforms. Therefore, this paper proposes a system that collects lifelog generated from heterogeneous smart watches and a database model that can manage life logs integrally. The proposed model is based on a relational database and focuses on scalability for heterogeneous smart watches. This paper also builds a life log collector on each smart watch platform and builds a proposal database model. For evaluation, We define two types of database models which store lifelog and evaluate their extensibility. In addition, we show the superiority of the proposed database model by comparing each lifelog collector by smart watches.

        Supercritical extraction of decellularized extracellular matrix from porcine adipose tissue as regeneration therapeutics

        Background: Extracellular matrix (ECM) has been broadly applied and shown great promise in medical applications. ECM products should be used after decellularization and purification. Supercritical carbon dioxide treatment is of particular interest for purifying ECM due to its medical availability and rapid process speed. However, it is not fully researched for treatment of biomaterials for tissue engineering. Therefore, we investigated the optimal conditions of supercritical carbon dioxide processing at different extracting parameters in porcine adipose tissue. Objective: We aimed to identify the optimal supercritical extracting conditions to produce non-cytotoxic and sterile decellularized extracellular matrix (DE-ECM) for regeneration therapeutics. Methods: The three-day dual treatment including enzymatic decellularization and supercritical fluid extraction of pork adipose tissue was performed. Two protocols using different extracting parameters were applied to evaluate the influence of extracting pressure and temperature on the extraction yield, DNA concentration, and remaining collagen in product. Results: Yield rate increased when high temperature or pressure was applied and pre-enzyme treatment had higher yield rate percent than pre-supercritical processing. Nearly 90% DNA was removed from the pre-enzyme sample when extracted at 3.04×10 7 Pa and 30°C±5°C. The pre-enzyme process had efficient extracting ability at each temperature and pressure and the remaining collagen steadily decreased with increase in extracting pressure and temperature. At the lowest temperature (20°C±5°C) and pressure (1.01×10 7 Pa), remaining collagen was 75.74%±1.83%. Supercritical extraction technology can produce DE-ECM eliminating DNA content efficiently and the remaining proper collagen amount successfully. Conclusion: This study evaluated the feasibility of utilizing supercritical extraction technology in bio-materials and was proven to be is successful. Through controlling the extracting pressure and temperature, this technology has a potential for DE-ECM mass production, which can be useful as tissue regeneration therapeutics as well new drug delivery paradigm.

      • Altered Gut Microbiota Composition in <i>Rag1</i> -deficient Mice Contributes to Modulating Homeostasis of Hematopoietic Stem and Progenitor Cells

        <P>Hematopoietic stem and progenitor cells (HSPCs) can produce all kind of blood lineage cells, and gut microbiota that consists of various species of microbe affects development and maturation of the host immune system including gut lymphoid cells and tissues. However, the effect of altered gut microbiota composition on homeostasis of HSPCs remains unclear. Here we show that compositional change of gut microbiota affects homeostasis of HSPCs using <I>Rag1</I><SUP>-/-</SUP> mice which represent lymphopenic condition. The number and proportions of HSPCs in <I>Rag1</I><SUP>-/-</SUP> mice are lower compared to those of wild types. However, the number and proportions of HSPCs in <I>Rag1</I><SUP>-/-</SUP> mice are restored as the level of wild types through alteration of gut microbiota diversity via transferring feces from wild types. Gut microbiota composition of <I>Rag1</I><SUP>-/-</SUP> mice treated with feces from wild types shows larger proportions of family <I>Prevotellaceae</I> and <I>Helicobacterceae</I> whereas lower proportions of family <I>Lachnospiraceae</I> compared to unmanipulated <I>Rag1</I><SUP>-/-</SUP> mice. In conclusion, gut microbiota composition of lymphopenic <I>Rag1</I><SUP>-/-</SUP> mice is different to that of wild type, which may lead to altered homeostasis of HSPCs.</P>

        국립춘천박물관 소장 서대의 과학적 보존

        국립춘천박물관이 소장한 서대(犀帶)의 보존처리 과정과 제작기법에 대해 소개하고자 한다. 서대는 가죽으로 된 틀을 직물로 감싸고 물소뿔과 금속테로 만든 띠돈을 장식한 형태이다. 가죽간의 접착력이 약화되어 분리가 진행중이고 서각의 일부가 탈락되었으며 직물의 변·퇴색이 진행되고 있다. 또한 금속테 부분과 띠돈을 연결한 금속사가 심하게 부식되었다. 금속부분의 성분분석을 위해 이동형 X-선형광분석기를 이용하여 비파괴 분석을 실시하였고 전체를 해체하여 재질에 따른 보존처리를 실시하고 다시 재결합하는 방법으로 보존처리를 진행하였다. Here is an introduction of the conservation and making process of Seodae(Rank Belt with A Rhinoceros' Horn) of the Chuncheon National Museum. The seodae has a structure composed of wrapping fabric around a leather frame and decorated with ttidon(an ornament) made of a rhinoceros' horn and a metal frame. The adhesion between leather was weak and separation was taking place, parts of the horn were missing and the fabric was discoloring. Also the metal frame and the metal wire attaching ttidon were severly corroded. Nondestructive analysis was carried out by using a portable x-ray fluorescence spectroscopy to analyze the components of the metal parts. Conservation was carried out by dismantling parts and carrying out conservation process of each one according to material, then the parts were put back together.

        Enhancements in catalytic activity and duration of PdFe bimetallic catalysts and their use in direct formic acid fuel cells

        The palladium–iron based bimetallic catalysts (PdxFey/Cs) and facile synthetic method are introduced toenhance the catalytic activity and operational duration for direct formic acid fuel cells. To improve theproperties of PdxFey/C catalysts, such as degree of PdFe alloy and its crystallinity, heat treatment isconducted at 600 C. According to results, the Pd–Fe bond and Fe metal particle are formed after the heattreatment, while iron oxides and Pd particles are observed in the untreated samples. Electrochemicalevaluations for measuring the formic acid oxidation reaction rate demonstrates heat treated Pd3Fe1/C(HT-Pd3Fe1/C) is the best catalysts of six samples which are synthesized by using different Pd to Fe ratios(3:1, 1:1, 1:3) before and after heat treatment. This is because the HT-Pd3Fe1/C has high Pd–Fe alloyingand Pd contents. Through the heat treatment, the indirect formic acid oxidation reaction way is activatedand the resistance to CO poisoning is significantly improved. The maximum power density of directformic acid fuel cells using HT-Pd3Fe1/C whose open circuit voltage is 0.83 V is 137 mW cm 2, which is 1.6and 1.9 times higher than that of direct formic acid fuel cells using untreated catalyst and Pd/C.

      • DVD 홈시어터용 다채널 스피커의 고속시리얼 연결 구현

        멀티미디어 시스템이 발달하면서 오디오 시장 또한 날로 발전하며 커지고 있다. 스피커는 오디오 시스템에서 최종적으로 소리를 출력해 주는 기구로써 오디오 시스템 전체의 성능을 결정하는 매우 중요한 부분이다. 초기 오디오 시스템의 경우에는 단지 사람이 들어 음향을 확인 할 수 있는 것에 목적을 두었다. 따라서 한 개의 스피커만을 사용한다. 이 후 오디오 기술이 발전 하면서 두 개의 스피커를 사공하는 스테레오 시스템이 일반화 되었다. 오늘날의 오디오 기술은 더욱 발전하여 HTS(Home Theater System)가 일반화 되어 기존의 스테레오 시스템에서 6 개의 스피커를 사용하는 5.1 채널이 보편화 되고 있으며 더 나아가 7.1 채널 또한 활발히 연구되고 있다. 현재의 5.1 채널 HTS는 우퍼를 포함한 6 개의 스피커를 각 스피커 마다 서로 다른 연결선으로 오디오기기 본체에 연결한다. 따라서 배선이 복잡해지고 설치 과정에서 임피던스를 맞추는 등의 특별한 작업들이 병행되어야 한다. 본 논문에서는 DYD HTS애서 여러 개의 스피커를 하나의 연결선으로 연결하여 제어하는 방법을 연구하고 구현하였다. 시리얼 연결 방식을 사용하여 스피커들을 연결할 경우 다양한 방법으로 연결할 수 있다. 이때 모든 선로상의 신호는 동일한 디지털 데이터 이므로 배선에 연결하는 스피커의 숫자나 연결 순서의 제약은 아날로그 데이터의 경우와 비교하여 대폭 개선된다.
