Long-term result of direct frontalis sling with fascia lata in congenital ptosis

Background: Congenital ptosis with poor levator function are very difficult to make good results of functional and cosmetic side. Surgical methods for the correction of congenital ptosis with poor levator function, including frontalis suspension or maximal levator resection, remain controversial. We evaluated the postoperative surgical and cosmetic long-term outcomes after frontalis sling operation with preserved fascia lata for congenital ptosis with poor levator function. Objective: To evaluate the long-term outcome of asymmetric ptosis with frontalis sling operation preserved fascia lata by modified direct tarsal fixation in congenital ptosis patients. Methods: A retrospective case series was performed. From January 2000 to December 2018, 53 patients (30 unilateral and 23 bilateral asymmetric ptosis) with 70 eyelids who underwent frontalis sling with preserved fascia lata were included. Six patients underwent levator resection at opposite eye. The surgical results were graded as excellent, good and poor. Results: The mean age at the time of surgery was 5.2±3.2 years (range, 1-43 years) with a mean follow-up time of 106.6±29.1 months (range, 60-196 months). Satisfactory results (excellent or good result) were obtained in 94.2% of the patients. Patients were divided into two groups based on levator function as follows: 0 to 2 mm (24 cases) and 2.5 to 4.0 mm (46 cases). Preoperative levator function, margin reflex distance-1 and levator dehiscence were not correlated with postoperative surgical outcomes. Complications were exposure keratopathy (8.6%), lidcrease asymmetry (7.1%), overcorrection (5.7%), entropion (5.7%), and eyelash ptosis (2.9%). Conclusion: Frontalis sling operation by preserved fascia lata with modified direct tarsal fixation is a simple but effective treatment for severe congenital ptosis with poor levator function. And it has good long-term results.
Using nasal septal cartilage-bone complex for anterior septal reconstruction in Asians

In Asian rhinoplasty, nasal tip projection and lengthening are often required because of the low nasal tip position and short nose. Anterior septal reconstruction (ASR) is performed for nasal tip projection or lengthening with severe caudal septal deviation. The septal cartilage should be sufficiently large for ASR and other grafting procedures; however, many Asians have a small septal cartilage. Here, we introduce a nasal tip-supporting technique using a septal cartilage-bone complex for Asians with a small septal cartilage and severe caudal septal deviation.
Treatment of acne fulminans with intense pulsed light: a case report

Acne fulminans is a severe form of acne that has a considerable psychosocial impact. Acne scarring is a potential complication of this condition. The treatment of acne fulminans includes conventional topical antibiotics, systemic antibiotics, hormonal therapy, isotretinoin, and light therapy. Intense pulsed light can have marked effects on acne fulminans. We aimed to describe the treatment of acne fulminans with intense pulsed light. This article is a case report together with a literature review to demonstrate how intense pulsed light can be used to treat acne fulminans. Pre-treatment and post-treatment clinical photographs are provided to show the effects of intense pulsed light therapy on acne fulminans. No comparisons were made with the other treatment modalities; nevertheless, this study provides an alternative treatment option for acne fulminans. Our case report revealed that intense pulse light using multiple filters at certain energy levels can effectively treat acne fulminans. Current evidence suggests that ablative CO2 and Er:YAG lasers provide the best curative effect on acne scars on all skin types. Potential complications with intense pulsed light include pain, burns, and post-inflammatory hyperpigmentation. Intense pulsed light is an effective treatment modality for acne fulminans. However, more cases of acne fulminans treated with intense pulsed light need to be documented in order to affirm intense pulsed light as one of the best options for treating this severe form of acne.
Clinical effectiveness of labia minora reduction surgery

Background: The demand for labia minora reduction operation has been increasing nowadays but there have been some questions about the safety and effectiveness of labioplasty surgery. Objective: The purpose of this study was to evaluate the effectiveness and satisfaction among patients who have undergone labia minora reduction surgery. Methods: Patients who had undergone labia minora reduction from January 2015 to December 2015 were reviewed. The procedure applied in this study was edge excision technique. Results: Eighty-four patients underwent labia minora reduction surgery. Of these, 41 patients (48.8%) underwent labia minora reduction surgery in combination with other procedures. All of the patients were satisfied with the outcome of the edge resection surgery. The main operative indications were regarding labial appearance and functional discomfort. Of these, one patient (1.2%) experienced the complication of postoperative bleeding, which was recovered without any additional treatment. There were no serious complications requiring reoperation. Conclusion: Labiaplasty using an edge excision technique is an effective and safe operation, has a low complication rate, and makes patients satisfied with the outcome of the labiaplasty.
Empty nose syndrome treated by autologous auricular cartilage turbinate augmentation: a case report

Empty nose syndrome (ENS), once viewed as a psychological issue, is now a recognized complication of turbinate surgery. It is most frequently associated with total turbinate excision. Its symptoms include chronic dryness of the nose and pharynx, paradoxical obstruction of the nose, feeling of suffocation, lack of airflow sensation, and even neuropathic pain. It may cause severe stress upon the patient resulting in mood instability issues. Surgical treatment to augment turbinate volume has been reported to be an effective treatment method. We would like to report a case of ENS treated by autologous auricular cartilage turbinate augmentation.
Periorbital changes in patients with Poly(Lactic Acid/Caprolactone) threads for brow augmentation

Background: Overlooked compensatory brow ptosis can lead to patients dissatisfaction and poor surgical results. Objective: To evaluate the quantitative changes in periorbital dimensions in 11 patients with mild to moderate brow ptosis after browlift with Poly(Lactic Acid/Caprolactone) (P[LA/CL]) sutures. Method: Retrospective analysis and data collected from reviewing clinical records, 11 patients, with mild to moderate brow ptosis, whom only has P(LA/CL) browlift, fulfilled the criteria and included in the analysis. All patients had 2-dimensional clinical photos and 3-dimensional clinical pictures in 5 standard views at baseline and follow-up. All periorbital measurements determined by the computer and collected for analysis. Results: There was a significant difference in all periorbital measurements at 4 to 6 months’ follow-up when compared with baseline (p<0.05). Moderate lifting refers to changes in brow height of at least 2 mm at follow-up. A higher proportion of patients with moderate brow lift were able to achieve moderate lifting compared with mild brow ptosis. Moderate lifting is observed in 30% to 60% medial brow and 45% to 60% lateral brow for patients with moderate brow ptosis compared with 25% in those with mild brow ptosis. Infection, significant bruising, or scarring did not occur. Transient side effects, such as bruising and dimpling, eventually resolved. Conclusion: Preoperative examination is vital to identify pre-existing compensatory brow ptosis. P(LA/CL) thread-lifting for brow augmentation is a simple and effective procedure which can be used as an adjunct to upper eyelid surgery to prevent secondary brow ptosis following eyelid surgery and in turn, improve patients’ satisfaction.
EMLA cream induced bilateral corneal de-epithelization

EMLA cream is widely used in dermatological procedures and is frequently applied prior to cosmetic procedures. Limited restriction of use among the personnel who handle the patients or customers. The exposure of EMLA cream to the cornea can have profound effects; it poses the risks of eye infections, reduced vision, discomfort, and pain. The aim of this study was to increase the awareness of the side effects of EMLA cream application near the eyes. EMLA, being an alkaline cream, causes corneal injury rapidly; hence, precautions should be taken during its use. EMLA cream is easily accessible and widely used in the cosmetic industry. Its use near the eye region requires extra caution. Patients experiencing difficulty in opening the eyes and discomfort after application of EMLA cream near the eyes should not be ignored. It is important to avoid EMLA cream entering into the eyes during its applications on the eyelids. De-epithelized cornea is more prone to infection, subsequently resulting in scarring and permanent reduction in vision.

Background: High-intensity focused electromagnetic waves (HIFEM) can achieve optimal body contours by training muscle mass, strength, and toning, as well as fat reduction. Objective: This study aimed to define the effect of HIFEM devices on muscle training and subcutaneous fat reduction. Methods: This is the first-time report of a clinical case series by using a modified protocol. The study was a single-centre evaluation in Hong Kong, China, involving 13 participants, of whom nine were female. Each subject underwent six treatments, 1-2 times a week for 3–6 weeks, and each treatment session lasted for 60 minutes. Body contours were recorded using 2D and 3D high-resolution images. Ultrasonography was performed for standardized areas in each participant. The above assessments were performed at the first and last treatment visits, and then at the 1- and 3-month follow-up visits. Data were quantified and compared. The main outcomes were percentage improvement, satisfaction levels, and adverse events. Results: A significant improvement in muscle thickness of the rectus abdominis with a corresponding decrease in subcutaneous fat thickness was demonstrated in the subjects. The mean decrease of subcutaneous fat was 0.31 cm (19.9%), 0.40 cm (25.6%), and 0.53 cm (34.0%) at the last visit, 1 month after the treatment, and 3 months after the treatment, respectively (p=0.017, p=0.014, and p<0.001, respectively). The rectus abdominis demonstrated a mean increase of 0.29 cm (27.8%), 0.28 cm (26.2%), and 0.28 cm (26.2%) at the last visit, 1 month after the treatment, and 3 months after the treatment, respectively (p=0.001, p=0.006, and p<0.001, respectively). No significant discomfort was reported by the subjects, and 100% of the subjects were satisfied with their results. Conclusion: HIFEM is an effective method for training the rectus abdominis and reducing fat in the abdomen, offering participant satisfaction, comfort, and sustainability.

Eyebrow compose of very unique characteristics hair. Only hair from lower nape of occipital scalp or very light pubic hair can match loosely as donor hair. Blonde hair or light color hair give better result. Black Asian hair is more difficult because of the contrast between skin and hair. Design eyebrow follow natural pattern, gender, direction within normal variation is very important to make it look natural. Donor hair can be done by 2 methods. Common one is H.H.H. FUE. FUE is done by extract each unit of hair follicle out from occipital scalp then transplant to eyebrow. Second is Strip harvesting by remove a piece of occipital scalp divide to follicular unit. Then transplant this unit to eyebrow. FUE produce no scar at the occipital scalp (Donor site). Strip harvesting produce a linear scar at the donor site (occipital scalp).

Background: As social media gets commonly used worldwide, there is an enormous amount of information available on YouTube. While cosmetic surgery-related content in English on YouTube has been studied priorly, those in Korean have not been explored. Objective: The purpose of this study is to analyze the status quo of aesthetic-related content, specifically on East Asian blepharoplasty, on YouTube in South Korea. Methods: The 535 returned videos for the search term ‘SSangkkeopul Susul’, the Korean word for East Asian blepharoplasty, were collected on July 9, 2020, and classified by types of content and uploader, view counts, and length of videos, then, analyze, using Excel (Microsoft). Results: Fifty five percent of collected videos was to provide surgery-related information, and 20.0% was to show patient experience. Also, 76.8% of the videos were uploaded by medical doctors or clinics, while 13.6% was uploaded by personal accounts and 5.4% by content creators. The most common type of content was surgery-related information uploaded by medical doctors and clinics, followed by patient experience posted by personal channels. The average length of all videos was 4 minutes 46 seconds. However, among the ten most-viewed videos, only one was uploaded by medical doctor and the others were uploaded by non-medical personnel. Conclusion: Medical doctors have a quantitatively huge influence on cosmetic surgery-related content on YouTube in Korea, which leads the information in those videos more reliable. However, due to the lack of clarity in relevant regulations and the absence of constant monitoring, potentially harmful or sensitive contents are still available on YouTube. Thus, preventative measures at the governmental level are needed.
