      • Comparison of Performance Indicators of the Primary Health Care in Republic of Korea Army after Pilot Project on Improvement of Medical Battalion"s Function

        Aim(s): Soldiers have received primary health care from the medical company in Republic of Korea Army, and they are transferred to the medical battalion or military hospital if needed. However, the medical company has a restriction in providing timely care because of the lack of resources, which causes a decrease quality of primary health care. Therefore, we conducted a pilot project that the medical battalion conducted unctions of primary health care instead of the medical company. The purpose of the study was to compare performance indicators of the primary health care before and after the pilot projects. Method(s): This study evaluated differences in performance indicators of primary health care before and after the pilot projects. The indicators consisted of three categories: accessibility (travel time to medical unit, waiting time per patient, and the number of untreated among outpatient visits), system infrastructure (the number of X-ray inspections and pathological tests), and care coordination (the number of transferring to an advanced or private hospitals). The pilot project was conducted from September 2019 to April 2020, and the data were collected from January 2019 to March 2020. Result(s): The system infrastructure and care coordination were improved while accessibility was deteriorated. Travel time to medical unit, waiting time, and the number of untreated patients were increased. The number of X-ray inspections and pathological tests were increased by 23.0% and 20.2%, respectively. The number of transferring was increased by 29.9% as well. Conclusion(s): This study identifies that the pilot project on improvement of medical battalion"s function was effective in improving system infrastructure and care coordination. Meanwhile, this study may suggest that strengthening strategies for accessibility should be prioritized to expand the pilot projects.

      • Sleep Disturbance in Patients with Prostate Cancer: A scoping review

        Aim(s): Patients with prostate cancer have a high survival rates, but they report low quality of life (QOL) due to sleep difficulties. This scoping review aimed to explore and identify the current publication trend, main concepts, measurements, and interventions addressing sleep disturbance in patients with prostate cancer. Method(s): With the scoping review method of Arksey & O’Malley (2005), structured searches were conducted in six electronic databases using combination of the terms ‘prostate cancer’, ‘malignant neoplasm of prostate’, ‘sleep*’, ‘circadian rhythm’, and ‘insomnia’ for articles published from 2000 to 2021. Result(s): The search yielded 1173 references, 31 of which were included. The rate of publication has increased from 6.4% (2000~2005) to 51.6% (2016~2021). ISI (Insomnia Severity Index) was the most frequently used subjective measurement, subsequently followed by PSQI (Pittsburg Sleep Quality Index) and ESS (Epworth Sleepiness Scale). Objective measurements to measure sleep quality or sleep disturbance included Actigraphy and PSG (Polysomnography). Hormone therapy and radiation therapy, symptoms of hot-flashes and nocturia were identified as causal factors of sleep disturbance. Intervention strategies presented were CBT (Cognitive Behavioral Training), MBCT (Mindfulness-Based Cognitive Therapy), ACT (Acceptance and Commitment Therapy), and Qigong/tai chi programs. Conclusion(s): An increasing interest in the QOL of patients with prostate cancer was noted, and studies utilizing wearable devices and online surveys were also reviewed. Further research on interventions for prostate cancer patients undergoing hormone therapy is needed.

        육군 병사의 의무부대 의료서비스 경험, 재이용 의도 및 일차의료 질 평가

        Purpose: The purpose of this study was to identify the relationships among patient experience, revisit intention, and assessment of primary health care of military division medical facilities. Methods: This study was a cross-sectional survey, and investigated a total of 198 soldiers who visited medical facilities. Data were collected from August to December 2020 using the structured questionnaires. Results: The average score of patient experience was 86.40±11.83, revisit intention 5.98±1.15, and assessment of primary health care 84.63±12.88 out of 100 each. There was significantly positive correlation between patient experience and revisit intention (r=.66, p<.001), patient experience and assessment of primary health care (r=.65, p<.001), and revisit intention and assessment of primary health care (r=.68, p<.001). Conclusion: This study identified the importance of virtuous cycle among patient experience, revisit intention and assessment of primary health care. The result is expected to be used as a evidence for developing policy and strategies to improve quality of primary health care in the military.

        Purpose: The purpose of this study was to investigate the trends of research on postpartum anger experienced by women after childbirth, through a narrative review. Methods: Articles published from 2008 to 2018 were searched in seven electronic databases using combinations of the terms 'anger', 'angry', 'aggress*', and 'postpartum'. Results: Ultimately seven studies were selected and analyzed. Six of the quantitative studies and one qualitative study were included and postpartum anger or anger was found to be measured in all six quantitative studies. Postpartum anger was influenced by hostility during pregnancy and women’s dependence and self-criticism of women after childbirth. Attachment with parents in the past and affection between married couples in the present influenced the expression and management of anger. Conclusion: The results indicated that postpartum anger was influenced by internal and external factors such as the individual characteristics of women, past experiences, current marital relations and social support. However, there was no study focusing on postpartum anger and related factors in domestic context. Thus, we suggest future studys which clarify the concept of postpartum anger and its measurement so that awareness of postpartum anger can be improved and mediation could be developed.

        노년기 건강행동의 결정요인 : 앤더슨 행동 모델의 적용

        본 연구의 목적은 앤더슨 행동 모델을 기반으로 노년기의 건강행동에 어떠한 요인이 영향을 미치는지를 분석하기 위함이다. 국민노후보장패널 6차 부가조사 자료를 활용하여 65세 이상의 노인 4,229명의 건강행동관련 응답에 대한 위계적 로지스틱 회귀분석이 사용되었다. 연구 결과 첫째, 배우자가 있고, 사회적지지가 높으며, 기타질환이 없는 노인일수록 아침식사 실천이 높았다. 둘째, 남성이며, 종교가 있고, 사회적지지와 가족관계 만족도가 높을수록, 주관적 건강상태가 높고, 질환이 존재하지 않는 경우 적정수면 실천이 높게 나타났다. 셋째, 연령이 낮고, 종교가 있으며, 학력, 사회적지지, 주관적 건강상태가 높을수록, 4대 중증질환, 기타질환이 없을수록 규칙적 운동을 하는 것으로 나타났다. 넷째, 연령이 낮고, 배우자와 종교가 있으며, 학력, 월평균 소득, 사회적지지, 주관적 건강상태가 높을수록, 가족 외 관계만족도가 낮을수록, 질환이 존재할수록 정기 건강검진을 실천하는 것으로 나타났다. 이는 건강행동의 영향요인별로 접근을 달리하여야 함을 의미하며, 노년기의 건강수명 유지를 위해 지역사회의 보건복지교육 프로그램과 관련 정책마련의 필요성을 제안한다. The purpose of this study is to examine the factors of health behavior among the elderly by applying Andersen’s model. A sample of 4,229 from the sixth KReIS was used for the statistical analyses and hierarchical logistic regression was performed. The results, showed that first, the elderly who had spouses, higher levels of social support, and no other illnesses were likely to have breakfast. Second, the elderly who were male, had higher levels of social support and satisfaction with family relationships, better health condition, and no illness were likely to demonstrate adequate sleeping time. Third, the elderly who were younger, had religion, higher levels of education, social support, better health condition, and no severe or other illnesses were likely to exercise regularly. Lastly, the elderly who were younger, had spouses and religion, showed higher levels of education, income, social support, and better health condition were likely to practice regular health check-ups. The elderly who were less satisfied with family relationships and had illness tended to practice regular health check-ups. The results mean that policy and interventions should differ depending on the factors of health behaviors among the elderly and community programs and policies are necessary for health life extension.

        군 근무자의 비만 여부에 따른 대사증후군 발생 영향요인 : 2017년 건강검진자료 분석

        Purpose: The purpose of this study was to explore the factors affecting the metabolic syndrome depending on the obesity. Methods: This study was a secondary analysis and 144,399 military workers' health check-up data for 2017 was retrieved from the National Health Insurance Service. Results: 53.4% of military workers was metabolically healthy non-obese (MHNO); Metabolically healthy obese (MHO) 35.0%, Metabolically unhealthy obese (MUO) 10.1%, and 1.5% were Metabolically unhealthy non-obese (MUNO). High risk drinking and smoking rates were the highest in the MUO, and the MUNO had the lowest level of physical activity. In the obese group, sex, age, drinking, smoking, and high and moderate intensity exercise were significantly associated with increased odds of being metabolic syndrome. On the other hand, in the normal weight group, sex, age, drinking, smoking, and high and low intensity exercise were significantly associated. Conclusion: The affecting factors of the metabolic syndrome were different depending on the obesity. It is needed for nurses to consider tailored interventions based on the group classification. This study is expected to contribute to develop the health promotion program to prevent metabolic syndrome in the military.

        Psidium guajava L. leaf extract inhibits adipocyte differentiation and improves insulin sensitivity in 3T3-L1 cells

        BACKGROUND/OBJECTIVES: Psidium guajava L. (guava) leaves have been shown to exhibit hypoglycemic and antidiabetic effects in rodents. This study investigated the effects of guava leaf extract on adipogenesis, glucose uptake, and lipolysis of adipocytes to examine whether the antidiabetic properties are mediated through direct effects on adipocytes. MATERIALS/METHODS: 3T3-L1 cells were treated with 25, 50, 100 μg/mL of methanol extract from guava leaf extract (GLE) or 0.1% dimethyl sulfoxide as a control. Lipid accumulation was evaluated with Oil Red O Staining and AdipoRed assay. Immunoblotting was performed to measure the expression of adipogenic transcription factors, fatty acid synthase (FAS), and AMP-activated protein kinase (AMPK). Glucose uptake under basal or insulin-stimulated condition was measured using a glucose analog 2-[N-(7-nitrobenz-2-oxa-1,3-diazol-4-yl) amino]-2-deoxy-D-glucose. Lipolysis from fully differentiated adipocytes was measured by free fatty acids release into the culture medium in the presence or absence of epinephrine. RESULTS: Oil Red O staining and AdipoRed assay have shown that GLE treatment reduced lipid accumulation during adipocyte differentiation. Mitotic clonal expansion, an early essential event for adipocyte differentiation, was inhibited by GLE treatment. GLE inhibited the expression of transcription factors involved in adipocyte differentiation, such as peroxisome proliferator-activated receptor γ (PPARγ), CCAAT/enhancer-binding protein α (C/EBPα), and sterol regulatory element-binding protein-1c (SREBP-1c). FAS expression was also decreased while the phosphorylation of AMPK was increased by GLE treatment. In addition, GLE increased insulin-induced glucose uptake into adipocytes. In lipid-filled mature adipocytes, GLE enhanced epinephrine-induced lipolysis but reduced basal lipolysis dose-dependently. CONCLUSIONS: The results show that GLE inhibits adipogenesis and improves adipocyte function by reducing basal lipolysis and increasing insulin-stimulated glucose uptake in adipocytes, which can be partly associated with antidiabetic effects of guava leaves.

      • Comparison of Patients Satisfaction after Pilot Project on Improvement of Medical Battalion"s Function

        Aim(s): In the current Army health care delivery system, soldiers have received primary health care from the medical company. If the symptoms get worse, they are transferred to the medical battalion or military hospital. However, the medical company has a problem in providing timely care because of the lack of medical personnel and equipment, which worsens patient conditions and reduces patient satisfaction. Therefore, we conducted a pilot project that the medical company focuses on the role of evacuation and the medical battalion reinforces functions of primary health. The purpose of this study was to compare patient satisfaction in health care before and after the pilot project. Method(s): This study was a nonequivalent control group pretest-posttest design, recruiting 82 soldiers who experienced in medical services before and after pilot projects. The patient satisfaction was investigated as in 4 criteria: accessibility, system infrastructure, care coordination, and patient-centeredness. The pilot project was conducted from September 2019 to April 2020, and the data were from January 2019 to March 2020. Result(s): The results of independent t-test showed that the soldiers evaluated the system infrastructure sector was significantly increased (p=.003) while the accessibility was decreased (p=<0.001) after the pilot project compared to before the pilot project. The care coordination and the patient-centeredness were increased as well after the pilot project, but their figures were not statistically significant. Conclusion(s): This study identifies that the pilot project on improvement of medical battalion"s function has a positive impact on patient satisfaction. In addition, it suggests that strengthening strategies on accessibility should be prioritized in order to expand the pilot project.
