The aim of this study was to compare a new type of reconstruction method (Noh's operation) with Roux-en-Y operation after subtotal gastrectomy. Noh's operation described herein includes a jejunal occlusion, an end-to-side gastrojejunostomy, a side-to-end jejunoduodenostomy, and a side-to-side jejunojejunostomy after subtotal gastrectomy. A series of 43 patients who had the new operation were compared with 47 patients with the Roux-en-Y procedure. The postgastrectomy syndromes, and the raucosal change of the remnant stomach and esophagus were evaluated after surgery. In the new operation, the Roux stasis syndrome occurred in 34.9% at 3 months, in 23.3% at 6 months, in 14.0% at 12 months, and in 11.6% at 24 months. In patients undergoing the Roux-en-Y operation, the syndrome occurred in 42.6% at 3 months, in 34.0% at 6 months, in 31.9% at 12 months, and in 29.8% at 24 months. This study shows that the new type of operation (Noh's operation) can be a good option for reconstruction after subtotal gastrectomy.

감마선 조사가 조미오징어의 저장 중 품질특성에 미치는 영향

This study was conducted to investigate the changes in the quality of gamma-irradiated seasoned cuttle during storage. Seasoned cuttle packed in PVC film (0.06 mm) was stored at 15±1℃ for 6 months after treatment with doses of 0 to 7 kGy. Microbial populations of seasoned cuttle were 1.6×10^5 CFU/g in total aerobic bacteria, 10⁴~10^5 CFU/g in yeasts & molds, and negative in coliforms, which were effectively reduced by 3 kGy or higher up to the undetectable level(〈20 CFU/g). The pH and moisture content of the samples were not changed with irradiation, but moisture was some decreased during storage. The instrumental color (especially Hunter b value), pigments (lipophilic & hydrophilic) and TBA value of the samples increased with storage time as well as irradiation dose more than 3 kGy. The influence of storage condition, however, were more significant. Irradiation did not induce any changes in volatile basic nitrogen (VBN) and trimethylamine (TMA) contents, thereby maintaining their contents lower than those of the non-irradiated samples during storage by reducing the microbial load.

A common form of relapse in adenocarcinoma of the stomach is intraperitoneal dissemination, in fact, among gastric adenocarcinoma patients who have undergone surgery intended to cure, approximately 50% of the patients develope initial recurrence in the peritoneal cavity regardless of the anatomic site of the primary tumor within the stomach. The efficacy of systemic postoperative chemotherapy to prevent peritoneal recurrence of gastrric adcnocarcinoma is not satisfactory. There is still a great need for improved therapeutic strategies on the disseminated microscopic disease and small miliary nodules remaining on the peritoneal surface or lymphatics after operation. The authers have made fluorouracil-polyglycolic acid composite disks(Fu-PGA disks) with fluorouracil and biodegradable polymer: polyglycolic acid for more effective intraperitoneal chemotherapy. We inserted the Fu-PGA disk(s) in the peritoneal cavity of rat and pharmacokinetic study was performed to measure fluorouracil concentration in the peritoneal fluid, plasma, liver, kidney and heart tissue at 24 hour, 72 hour and 168 hour after insertion of Fu-PGA disk(s). Myelosuppressive action of this composite also was determined following its administration. The data of this study suggested that Fu-PGA composite will be a new device releasing drugs in a controlled manner and having targetability to peritoneum, and this device will be improving the efficacy of intraperitoneal chemotherapy for gastric adenocarcinoma.

V₂O-B₂O₃-PbO 유리의 전자 상자성 공명연구

V₂O??-B₂O₃-PbO 유리의 VO??에 관한 전자 상자성 (EPR) 스펙트럼으 연구하였으며, vanadium이온의 스핀해밀토니안 상수들을 구하였다. 유리시료안의 V₂O??양을 증가시킴에 따라 g??과 g??값은 증가하였으나 A??과 A??값은 감소하였다. 이는 V₂O??수가 증가하면 동일 평면상의 vanadium과 oxygen배위간 결합의 공유성이 증가함을 의미한다. 유리내의 vanadium 4가이온은 정방형 일그러진 8면체를 한 vanadyl착물로 존재한다. Electron Paranmagnetic Resonance(EPR) SPECTRA OF (VO)?? in V₂O??-B₂O₃-PbO glasses have been studied and spin hamiltonian paramenters of vanadium ion have been calculated. It is found that the value of g??and g?? increases while the value of A?? and A?? decreases with an increase of V₂O?? content in the glass samples, showing that the covalency in the in-plane vanadium -oxygen ligand bonds increase with increasing the concentration of V₂O??. The tetravalent vanadium ions exist as vanadyl complexes with tetragonally distorted octahedral sites in the glasses.

위전절제시 단순공장간치술의 임상적 의의

Purpose: The aim of this study was to evaluate the short-term outcome of a jejunal interposition, by comparing it with a conventional Roux-en-Y gastrojejunostomy, after a total gastrectomy. Materials and Methods: For 28 patients (20 men and 8 women) with a gastric adenocarcinoma, who underwent an isoperistaltic simple jejunal interposition, weight, hemoglobin, hematocrit, serum protein and albumin, and cholesterol levels were checked before the operation and at 1 year and 2 years after the surgery. Also, endoscopy was performed to confirm reflux esophagitis. In this study, the data were collected between January 1993 and July 1999 at Chungnam National University Hospital, and the results were compared with those of the Roux-en-Y procedure. Results: The body weights at 1 year and 2 years after the surgery had returned to 86.0% and 87.6% of the recent original body weight in the jejunal interposition (Jl) group and to 90.8%, 87.0%, respectively in the Roux-en-Y (RY) group. The levels of hemoglobin (g/dl) were 13.3, 12.5, and 11.9 in the Jl group, and 13.8, 12.6, and 12.1 in the RY group at the time of the operation and at 1 year, and 2 years after the surgery, respectively. The total serum protein (g/dl) levels were 7.1, 7.2, and 7.5 in the Jl group and 7.1, 7.0, and 7.2 in the RY gropu at the time of the operation and at 1 year and 2 years after the surgery, respectively. The serum albumin (g/dl) levels were 4.2, 4.1, and 4.2 in the Jl group and 4.2, 4.2, and 4.2 in the RY group at the time of the operation and at 1 year, and 2 years after the surgery, respectively. The total serum cholesterol (mg/dl) levels were 186.5, 174, and 164 in the Jl group and 213.7, 171.1, and 141.0 in the RY group at the time of the operation and at 1 year and 2 years after the surgery, respectively. The endoscopic finding showed that reflux esophagitis occurred in 7.1% of the patients in the Jl group and in 3.5% in the RY group. Conclusion: We think that from the view point of quality of life, a jejunal interposition, as well as a Roux-en-Y procedure, is a useful reconstruction methods for a total gastrectomy. 디자인 교육의 사회적 요구의 도자공예 교육의 방법적 제언

최근의 디자인 교육은 대인 관계와 사회적 역량 및 전문분야의 업무처리 능력, 경제와 시장변화에 대한 신속한 대처, 현장에 바로 적용되는 인력 양성, 여성 인력의 증가, 디자인의 재교육 연수 등의 필요성을 강조하고 있으며 이에 부응하는 대학교육의 Curriculum이 짜여가고 있는 것을 알 수 있다. 디자인 교육에서 기획과 크리에티브의 응용사고 영역이 미국과 영국에서는 강조되고 있으며 이에 이들 나라가 세계 디자인을 주도하고 있음을 간과할 수 없다. 우리나라에는 연간 3만명에 달하는 디자인 관련학과 졸업생이 있음에도 전문 인력이 부족하다는 모순적 지적과 함께 초등, 중등, 고등 교육기관의 디자인 교육의 재검토가 요구되기도 하지만, 산업, 시각, 공예 디자인의 특성상의 분리와 보완적 상호관계의 디자인 교육방법론의 연구가 시급하다. 이에 도제적 공예교육을 버릴 수도 없는 특수성과 현실에서 산업적 디자인 교육을 받아 들여야 되는 공예학과 디자인 교육 방법의 적극성과 자율성 및 다양한 취업의 진로를 제언해 보고자 한다.
