Sarcopenia affects conservative treatment of osteoporotic vertebral fracture

Objectives: Sarcopenia and osteoporosis affects activities of daily living and quality of elderly people. However, little is known about its impact on elderly locomotor diseases, such as osteoporotic vertebral fracture (OVF). There is no report investigating the influence of both sarcopenia and osteoporosis on outcomes of OVF. This study aimed to evaluate the clinical outcomes of OVF in elderly patients from sarcopenic perspectives. Methods: This prospective study was conducted with 396 patients, aged 65 years or more, hospitalized for the treatment of OVF (mean age, 81.9 ± 7.1 years; 111 males, 285 females). The primary outcome was the Japanese Orthopaedic Association (JOA) score for lumbar disease (at first visit, hospital discharge, and 1 year after treatment) and Barthel index (at the same time and before hospitalization). The second outcome was living place after discharge. Susceptibility to sarcopenia and osteoporosis were evaluated and clinical results of conservative treatment were compared. Results: Sarcopenia significantly affected Barthel index at first visit and discharge. Sarcopenia patients had significantly higher rate for discharge to nursing home and living in nursing home after 1 year than patients without sarcopenia. Osteoporosis significantly affected the JOA score at the first visit and the Barthel index before hospitalization, at the first visit, discharge, and after 1 year. Osteoporosis did not affect the living place at discharge and after 1 year. Conclusions: Sarcopenia and osteoporosis affected outcomes of conservative treatment for OVF; moreover, sarcopenia affected the living place of OVF patients at discharge and after 1 year

Background/Aims: It is sometimes difficult to distinguish between malignant and benign biliary strictures using imaging studiesalone, and pathological diagnosis is necessary. The aim of this study was to determine the usefulness of endoscopic transpapillary tissuesampling and factors predictive of diagnostic accuracy. Methods: From April 2008 to December 2014, 136 patients underwent endoscopic transpapillary tissue sampling for malignantbiliary strictures. The cytological and histological findings were reported as negative, suspicious, or positive. Suspicious and positivefindings were defined as pathologically positive. Results: The sensitivity was 65.0% for forceps biopsy, 49.5% for brush cytology, 46.2% for bile aspiration cytology, and 21.9% forendoscopic nasobiliary drainage cytology. The combination of these procedures improved the sensitivity (72.8%). Endoscopictranspapillary tissue sampling was more sensitive for lesions of biliary origin (91.4%) than for extrabiliary lesions (66.3%). In surgicalcases, the sensitivity for tumors with an infiltrative growth pattern (53.3%) was significantly lower than for a tumor with an expandingor intermediate growth pattern (87.5%). Conclusions: Combining procedures can improve diagnostic accuracy. It may be possible to predict the sensitivity of endoscopictranspapillary tissue sampling by evaluating the etiology and tumor growth pattern using preoperative imaging studies.
Muscle Thickness and Echo Intensity of the Abdominal and Lower Extremity Muscles in Stroke Survivors

Background and Purpose This study compared the muscle thickness (MT) and echo intensity (EI) of the abdominal, thigh, and lower leg muscles between the paretic and nonparetic sides in chronic stroke survivors. Methods Thirty-two stroke survivors living in the community participated in this study. The MT and EI, which are indicators of muscle mass and intramuscular fat or connective tissue, were assessed in the rectus abdominis, external oblique, internal oblique, transversus abdominis, rectus femoris, vastus intermedius, vastus lateralis, vastus medialis, tibialis anterior, gastrocnemius, and soleus via transverse ultrasound imaging. In addition, a possible indicator of physical activity—the frequency of going out per week—was evaluated. Results All quadriceps muscles and the tibialis anterior were significantly thinner and the EI values of the vastus intermedius, vastus lateralis, vastus medialis, and soleus were significantly higher in the paretic limb than the nonparetic limb. The MT and EI values of abdominal muscles did not differ significantly between the two sides. The MT values of the paretic rectus femoris, vastus lateralis, and vastus medialis were significantly associated with the frequency of going out after adjusting confounding factors. The MT of the nonparetic vastus lateralis was significantly associated with latency from stroke onset after adjusting confounding factors. Conclusions Our results indicate that quantitative and qualitative changes on the paretic side in stroke survivors were the most robust in the thigh muscles, whereas such changes might not occur in the abdominal muscles
Decolonial Possibilities of Transnationalism in Contemporary Zainichi Korean Art

This study discusses the works created by three contemporary zainichiKorean artists—Oh Haji, Kum Soni, and Chong Ri Ae—after the 2000s. These artists have addressed the issue of zainichi Korean identity and thehistory of zainichi Koreans. This study also deals with two exhibitionmaking projects led by young zainichi Korean and Japanese artists in the2010s: Zainichi Genzai Bijutsu and Totsuzen Me no Mae ga Hirakete. Theseexhibitions shed light on the presence of zainichi Korean art on the globalart scene. Through these case studies, this article examines the differentways in which these artists embody the notion of transnationalism intheir works and projects. By way of conclusion, this article points outthree ways that the ethos of transnationalism articulated in the practicesof zainichi Korean artists could contribute to the process of decolonizationin the context of postcolonial Japan: first, it creates a new expressionof ethnic identity by emphasizing the notion of hybridity in the placeof homogeneity; second, it challenges the nationalized narratives ofmodern history by highlighting a form of transnational historicity; third,it promotes multiethnic dialogue on postcolonial issues by forgingalternative platforms.
