        Sarcomatoid Carcinoma Arising from Mature Cystic Teratoma

        Malignant transformation of mature cystic teratoma (MCT) is rare. Sarcomatoid carcinoma is a neoplasm comprising malignant mesenchymal cells and a conventional carcinomatous area. Here, we report on a case of sarcomatoid carcinoma arising from an MCT in the left ovary of a 45-year-old female. A unilocular cyst consistent with MCT was observed; however, a nodule within the cyst was confirmed from the resected ovary. Microscopically, the nodule showed both squamous cell carcinoma and pleomorphic sarcomatous components admixing with each other. Lining epithelial cells at the periphery of the main tumor showed squamous metaplasia. When a sarcomatous component is observed in the ovary tumor, it is important to find a squamous cell component, either benign or malignant.

      • Development of Korean Pain Rating Scale

        본 논문은 한국인의 통증을 사정하기 위해 1983년부터 1986년까지 제 4 차의 연구와 추가연구에 의해 개발된 세 형태의 통증척도를 종합보고하는 것이다. 한국어로 된 통증척도는 90개의 어휘로 구성되며 이는 20개군으로 묶여지고, 이는 다시 3개영역으로 함축된다. 감각영역에 10개군, 정의 영역에 5개군이 포함되는 것은 MPQ와 같다. 그러나 한국어 통증척도에는 MPQ에서와 달리 평가영역의 어휘군이 없으며 나머지 5개군은 감각표현과 감정표현이 혼합되어 있으므로 기타군에 포함시켰다. MPQ는 78개의 어휘를 포함한데 비하여 한국어 통증척도는 90개의 어휘로 표현된다. 몇개의 어휘군의 명칭이 MPQ에서와 다르고 MPQDP 없는 화학적 통증군이 포함되어 있다. 본 연구에서 1~7점의 통증 강도척도와 1~7점의 불쾌감척도로 구성된 구술적 평정척도가 Gracely등의 척도를 근거로 개발되었다. 뿐만 아니라 통증의 강도를 20cm 선상에 나타내는 도표평정척도도 개발되었다. 앞으로 이상의 세 종류의 도구의 타당도 검증을 위한 연구, 급성통증과 만성통증을 구분하는 민감성을 검증아흔 연구 및 임상활용의 유용성을 검증하는 연구등의 계속적인 연구가 요구된다. Through 4 consecutive studies, three different types of Korean Pain Scales have been develop. These are Korean Pain Rating Scale (KPRS), Verbal Rating Scale (VRS), and Graphic Rating Scale (GRS). The KPRS contains 90 pain terms with 20 subclasses within 3 dimensions. The structure was almost same as the MPQ in that 10 subclasses are in the sensory dimension and 5 subclasses in the affective dimension. However, the rest of 5 subclasses are included in the miscellaneous dimension. In these miscellaneous subclasses, sensory and affective terms were mixed. The first subclass, temporal pain in the MPQ was divided into two subclasses. Many other subclasses were renamed and included more terms on the basis of the terms included. A new subclass, chemical pain was included in the sensory dimension. The Korean Version of Verbal Rating Scale composed of 7-point intensity scale and 7-point unpleasntness scale was developed from a scale of Gracely and others A graphic Rating Scale with 3 words indicating levels of pain under a 20 cm line was also developed. Even though a few validation studies by other researchers have been conducted, more thorough studies are needed for distinction of acute and chronic pain and for development of a simple form to use in the clinical setting.

        Dioscorea Extract (DA-9801) Modulates Markers of Peripheral Neuropathy in Type 2 Diabetic db/db Mice

        The purpose of this study was to investigate the therapeutic effects of DA-9801, an optimized extract of Dioscorea species, on diabetic peripheral neuropathy in a type 2 diabetic animal model. In this study, db/db mice were treated with DA-9801 (30 and 100 mg/kg, daily, p.o.) for 12 weeks. DA-9801 reduced the blood glucose levels and increased the withdrawal latencies in hot plate tests. Moreover, it prevented nerve damage based on increased nerve conduction velocity and ultrastructural changes. Decrease of nerve growth factor (NGF) may have a detrimental effect on diabetic neuropathy. We previously reported NGF regulatory properties of the Dioscorea genus. In this study, DA-9801 induced NGF production in rat primary astrocytes. In addition, it increased NGF levels in the sciatic nerve and the plasma of type 2 diabetic animals. DA-9801 also increased neurite outgrowth and mRNA expression of Tieg1/Klf10, an NGF target gene, in PC12 cells. These results demonstrated the attenuation of diabetic peripheral neuropathy by oral treatment with DA-9801 via NGF regulation. DA-9801 is currently being evaluated in a phase II clinical study.

      • Heyneanol A induces apoptosis via cytochrome c release and caspase activation in human leukemic U937 cells

        Heyneanol A, a tetramer of resveratrol, is isolated from the roots of Vitis amurensis by cytotoxicity based fractionation. In this study, the mechanism of apoptosis by heyneanol A was evaluated in human leukemic U937 cells. Heyneanol A (IC_(50) = 6.6 μM at 24 h) exhibited stronger cytotoxic effect than resveratrol (IC_(50) = 100 μM at 24 h) by 15-fold an human leukemic U937 cells by XTT assay. Apoptotic bodies were observed in U937 cells treated with 6 μM of heyneanol A by TUNEL assay. Heyneanol A effectively increased the Portion of sub-G₁ DNA content in a time- and concentration-dependent manner by flow cytometric analysis. Heyneanol A also induced cytochrome c release from mitochondria into the cytosol and subsequent caspase activation involving caspase 9 and 3 to cleave PARP. However, it did not affect the expressions of Bax and Bcl-2 by western blotting. It was confirmed that the activation of caspase 8, 9 and 3 and the cleavage of PARP by heyneanol A were completely blocked by adding Z-VAD-FMK, a caspase inhibitor. These findings suggest that heyneanol A has anti-tumor activity, which may be mediated by apoptosis caused by cytochrome c release and caspase activation in human leukemic U937 cells.

        In vivo anti-cancer activity of Korean Angelica gigas and its major pyranocoumarin decursin.

        <P>We have reported that a 10-herbal traditional formula containing Korean Angelica gigas Nakai (AGN) exerts potent anti-cancer efficacy and identified decursin and decursinol angelate (DA) from AGN as novel anti-androgens. Here, we determined whether AGN would exert in vivo anti-cancer activity and whether decursin or DA could account for its efficacy. The AGN ethanol extract was tested against the growth of mouse Lewis lung cancer (LLC) allograft in syngenic mice or human PC-3 and DU145 prostate cancer xenograft in immunodeficient mice. The pharmacokinetics of decursin and DA were determined. The AGN extract significantly inhibited LLC allograft growth (30 mg/kg) and PC-3 and DU145 xenograft growth (100 mg/kg) without affecting the body weight of the host mice. Biomarker analyses revealed decreased cell proliferation (Ki67, PCNA), decreased angiogenesis (VEGF, microvessel density) and increased apoptosis (TUNEL, cPARP) in treated tumors. Decursin and DA injected intraperitoneally were rapidly hydrolyzed to decursinol. Decursinol and decursin at 50 mg/kg inhibited LLC allograft growth to the same extent, comparable to 30 mg AGN/kg. Therefore the AGN extract possessed significant in vivo anti-cancer activity, but decursin and DA only contributed moderately to that activity, most likely through decursinol.</P>

        <P>Because evasion of tumor suppression is a critical step in cancer development, cancer cells have developed a variety of mechanisms to circumvent the influence of tumor suppressive pathways. Thus, genes that negatively regulate tumor suppressors could be considered novel types of oncogenes such as Bmi-1 repressing p16Ink4a and inhibiting p53 and were found to be frequently up-regulated in a variety of cancers. p38 mitogen-activated protein kinase (MAPK), which reportedly plays a crucial role as a tumor suppressor, is activated in number of lung adenocarcinomas, which is seemingly at odds with its role as a tumor suppressor.</P>

        <P>Ka-mi-kae-kyuk-tang (KMKKT) is an Oriental herbal medicinal cocktail. Our collaborative team has shown that it has potent anti-angiogenic, anti-cancer and anti-metastatic activities in vivo without observable side effects. We have documented evidence for KMKKT to alleviate drug-induced hematotoxicity in vivo. In the present study, we investigated the mechanistic and signaling events through which KMKKT enhances hematopoiesis, using hematopoietic stem cells (HSCs) isolated from the bone marrow of 8-12 week-old C57BL/6 mice. Our results show that KMKKT significantly increased the expression of the hematopoietic cytokines interleukin (IL)-3, stem cell factor (SCF), granulocyte-macrophage-colony stimulating factor (GM-CSF), thrombopoietin (TPO) and erythropoietin (EPO) at the level of mRNA and secretion in HSCs. KMKKT also increased the expression of c-Kit, a cytokine receptor expressed in HSCs. In addition, KMKKT enhanced phosphorylation of Janus kinase 2 (JAK2) and signal transducer and activator of transcription 5 (STAT5), and increased the binding activity of STAT5 to gamma interferon activated sites (GAS) that mediate JAK2 downstream signaling. Furthermore, we found that KMKKT significantly enhanced the growth rate of colony-forming unit granulocyte erythrocyte monocyte macrophages (CFU-GEMM) and burst forming unit erythroid (BFU-E) of mouse HSCs (mHSCs) stimulated by IL-3/EPO. Overall, our results demonstrated that KMKKT alleviated drug-induced side effects through enhanced hematopoiesis, at least in part through cytokine-mediated JAK2/STAT5 signaling.</P>

      • Construct Validity of Korean Pain Rating Scale Using Multitrait-Multimethod Matrix Procedure

        한국인의 통증을 사정하기 위한 한국어 통증척도(KPRS)가 일련의 연구결과로 개발되었다. 본 연구에서는 개발된 이 척도의 수렴타당도와 판별타당도를 조사하기 위해 113명의 임상적 통증을 가진 환자를 대상으로 다속성 다방법행렬 접근법이 이용되었다. 통증을 측정하기 위한 척도로 한국어 통증척도와 시각적상사척도 (Visual Analogue Scale)가 사용되었고, 교회에 대한 태도라는 다른 속성을 측정하기 위해서는 Thurstone의 척도와 시각적 상사척도가 사용되었다. 상관관계 행렬을 이용한 결과, 한국어 통증척도는 같은 통증을 측정하는 다른 종류의 척도에 대해서는 수렴타당도를 보였고, 다른 속성을 측정하는 척도에 대해서는 판별타당도를 보였다. 또한 반분 신뢰도 검사결과 높은 신뢰도 계수를 나타내었다. 이러한 연구결과로 한국어 통증척도는 임상에서 뿐 아니라 연구를 위한 목적으로도 사용될 수 있다는 것을 시사하고 있다. The Korean Pain Rating Scale (KPRS) has been developed through a series of studies to assess pain. In this study, the KPRS was administered to 113 patients with clinical pain to investigate convergent and discriminant validity of the scale using the multitrait-multimethod matrix procedure. In addition to the KPRS, subjects were requested to report their pain using the Visual Analogue Scale (VAS), and their attitude toward the church using the Thurstone scale and the VAS. Using the intercorrelational matrix, it was found that the KPRS showed convergent validity across the different scales and divergent validity across the different concepts. A split-half reliability test showed a high reliability coefficient. These findings indicate that the KPRS can be utilized for pain assessment of Korean poeple in clinical as well as in research settings.
