It is Important to know the normal size, shape, and signal intensity of anterior cruciate ligament in the evaluation of knee injury Many studies have reported findings of tear or rupture as well as normal anatomy of these structure, especially with MR images. To our knowledge, there are few studies about normal structures of anterior cruciate ligament in MR images. The purpose of this study is to compare normal finding with tear of anterior cruciate ligament on MRI. Twenty-three cases with rupture of anterior cruciate ligament were evaluated on MRI, measuring the length, thickness, width of anterior cruciate ligament, and angles of anterior and posterior cruciate ligaments. Mean length, thickness, and width of anterior cruciate ligament in patients with rupture were 37.7±4.8mm, 9.0±1.2mm, 8.0±0.8mm, and mean angles of anterior cruciate ligament, and posterior cruciate ligament were 48.1±9.6˚, 118.8±13.1˚. Curvature ratio of posterior cruciate ligament was 0.25±0.09. The results showed significant differences between normal persons and patients with complete or partial rupture, and the mean angle and curvature ratio of posterior cruciate ligament which are known to related with indirect sign in tear of anterior cruciate ligament showed significant differences between ours and that of western countries. These results will be useful in the evaluation of patient wish rupture of anterior cruciate ligament, especially on MRI.

With the progress of industry, the much more demand of paint around us grows rapidly. However, the whole quantity of raw materials used to make powder coatings is presently imported to fill the domestic needs. Moreover, it is well known that the waste powder coatings from the domestic industries are mostly incinerated. The method of destructing the waste powder coatings by fire causes VOC to be generated, and this results in some serious environmental problems such as air pollution and so on. Therefore, the effective and proper measure must be devised to reuse the waste powder coatings for the purpose of eliminating waste of resources and avoiding the outflow of foreign currency. This study aims to make the plan to recycle the waste powder coatings, and its conclusion is drawn as follows: First, it can be seen that the waste powder coatings with a weak electrostatic power restore gradually to the original state of electrostatic power through the process of fusion and retreatment. Second, it can be estimated that those coatings, with the use of the cyclon, can be removed by dividing a powder of particles less than 10㎛. Third, it can be ascertained that the possibility of reusing the waste powder coatings is proved when it is divided into particles by cyclon and a revolving screen after the process of fusion, rolling, crushing and pulverization.

Purpose : Intraoperative suprachoroidal hemorrhage is one of the most serious complications. We report a case of suprachoroidal hemorrhage during phacoemulsification surgery, was managed properly. Methods : Suprachoroidal hemorrhage occurred during phacoemulsificaiton in other hospital. The patient was referred to our department immediately after closing the surgical wound. On initial examination, iris and vitreous were prolapsed through surgical wound. Ciliary body was closely place to anterior chamber, and retina was not visible. Results : Prolapased ocular tissue were reduced by emergency operation. We controlled the patient’s intraocular pressure and inflammation. Ultrasonography was repeatedly performed to check the degree of liquefaction of suprachoroidal hemorrhage. After 9 days of suprachoroidal hemorrhage onset, vitrectomy and drainage through sclera was performed. Postoperative state was stable with successful removal of suprachoroidal hemorrhage. After 2 weeks of secondary operation, retinal detachment has occurred on nasal retinal area, so we performed scleral buckling and endolaser photocoagulation. After 6 months, retina was attached well and intraocular pressure was in normal range. Conclusion : In suprachoroidal hemorrhage, drainage of suprachoroidal hemorrhage after liquefaction of thrombi is effective. After successful drainage, postoperative complications may accompany such as retinal detachment or elevation of intraocular pressure. 목적 : 술중 발생하는 맥락막상 출혈은 매우 심각한 합병증이다. 저자는 수정체 유화술 중에 발생한 맥락막상 출혈을 적절히 대처한 증례를 보고하는 바이다. 대상과 방법 : 수정체 유화술 중 발생한 맥락막상강 출혈환자가 수술창을 봉합 후 즉시 본원으로 전원되었다. 초진시 홍채와 유리체가 수술창을 통해 탈출되어 있었으며, 전방을 통해 모양체가 확인되었으나, 망막은 보이지 않았다. 탈출된 안구내 조직을 응급수술을 통해 복원 후 환자의 안압과 염증을 조절하였다. 초음파를 반복적으로 실시하여 맥락막상 출혈의 액화 정도를 확인하였으며, 출혈 9일 후 유리체 절제술과 공막을 통한 배액술을 시행하였다. 결과 : 맥락막상 출혈이 성공적으로 제거되었으며 술 후 상태는 안정적이었다. 수술 2주 후 비측 망막 박리가 발생하여 공막돌륭술과 안구내 삽관 레이저술을 시행하였다. 6개월 후 망막은 잘 붙어있었으며, 안압은 정상 범위를 유지하였다. 결론 : 맥락막상 출혈의 경우 출혈의 배액 시기를 결정하는 것이 어렵지만, 규칙적인 초음파를 통해 배액술의 시기를 결정할 수 있다. 또한 맥락막상강 출혈의 성공적인 배액 이후에도 망막박리 및 안압상승과 같은 합병증의 동반에 대해 유념해야한다.

Features of the forearm bone are common and results from severe trauma, accompained with many associated injuris. Authors analysed 49cases who had received operative treatment at department of orthopedic surgery from Feb. 1989 to Feb.1994. The most common cause of injury was a traffic accident and average age was 29.6 years, the ratio of male to female was 28 to 16cases. We treated these fractures surgically in most cases and obtained satisfactory result. Injured patients were admitted and treated by open reduction and internal fixation using plates(36cases) and IM nailing(8 cases) The average time of bone union in the closed fracture was 15.8 weeks in the radius. The complications were delayed union in 10 cases non-union in 2 cases and nerve injury in 9 cases. We caused obtain good results by accurate understanding to the fracture complexity before operation.

The arthroscopic surgery is an increasingly accepted procedure for the treatment of symptomatic arthritic knee. Authors reviewed the operative results of arthroscopic surgery in 35 knee joints of 27 patients from 1993 to 1997. 1. The average age of patients was 64 years old and the primary diagnosis was degenerative osteoarthritis in all patients. 2. Preoperative symptomes and signs were pain(98%), tenderness(85%), swelling(85%), decreased range of motion(71%) and effusion(55%). 3. The operative procedures were irrigation and synovectomy(20%), abrasion arthroplasty(57%), partial menisectomy and abrasion arthroplasty(23%). 4. The symptoms and signs were improved postoperatively: according to Nine point scale, the good results were recorded at postoperative 6 months and follow-up 2 years in nearly all patients. 5. The complications were minimal. Authors concluded that arthroscopic surgery is considered as a reliable method in the treatment of degenerative osteoarthritis of the knee but not as a curable method, needing a long term observation.

노년의 대퇴골 원위부 골절 시 동시에 시행한 전슬관절 성형술 및 금속판 내고정술 : 8예 보고 Report of Eight Cases

Supracondylar fracture of the femur can be treated by a wide variety of methods. Although satisfactory results have been achieved with either open and closed treatment, less certain results are obtained for the elderly who have pre-existing osteoarthritis and osteoporosis. Particularly, elderly patients who have preexisting osteoarthritis are at increased risk of posttraumatic arthritis, stiff knee, persistent pain. Primary total knee arthroplasty(TKA) might obviate many of these potential problems and help to return the high-risk patient to an ambulatory functional status immediately. There are few reports about this study. The purpose of this study is to evaluate the results treated by simultaneous internal fixation and TKA for the supracondylar fracture in the elderly who have osteoarthritis of the knee joint. From 1994 to 1998, eight primary TKAs using posterior cruciate sparing PFC knee(Press Fit Condylar, Johnson & Johnson, Raynham, U.S.A) were performed after internal fixation for the supracondylar fracture of the femur with Judet plate or condylar buttress plate by conventional means. Patella was not resurfaced in all knees. According to the M ller classification of the supracondylar fracture of the femur 5 fractures were type A, 2 in type B, 1 in type C. Five patients were female, 3 patients were male. The ages ranged from 66 to 81, with an average of 74 years. Patients were followed for at least 1 year(average 3 years). The mean interval between the injury and operation was 2 weeks(range, 1 week to 4 weeks). Active knee joint exercise was encouraged in a hinge brace under the postoperative pain control. Partial weight bearing was permitted at the postoperative 4 to 6 weeks using crutches. Full weight bearing was allowed after union of the fracture. Fractures were united in all patients and the average duration of bony union was 16 weeks. The average amount of flexion and average flexion contracture for the all patients at the most recent follow-up were 96 degrees (range, 45 to 120 degrees) and 7 degrees (range, zero to 20 degrees), respectively. Using the knee rating score advocated by the American Knee Society, the average postoperative score was 87 for pain and 80 for function. We concluded that simultaneous internal fixation and TKA for supracondylar fracture of the femur in elderly patients who have advanced knee osteoarthritis may provide satisfactory pain relief and function with acceptable morbidity.

Background : Most strains of methicillin-resistant Staphylococcus aureus (MRSA) now exhibit high-level resistance to various antibiotics, such as β -lactam antibiotics, aminoglycosides, macrolides, tetracyclines and quinolones. Recent reports describing the therapeutic failure of vancomycin for MRSA infections have arisen considerable concerns regarding the emergence of MRSA strains, which will require new therapeutic agents. Arbekacin, an aminoglycoside antibiotic, has antibacterial activity against both gram-positive and gram-negative bacteria and is stable in the presence of aminoglycoside inactivating enzymes produced by S. aureus. In this study, we compared the antibacterial activity of arbekacin with those of vancomycin, gentamicin, and amikacin against Staphylococcus aureus (S. aureus) and coagulase-negative staphylococci (CNS). Methods : For a collection of 549 S. aureus and 251 CNS isolates from three Catholic University Hospitals in Korea, minimum inhibitory concentrations (MICs) of arbekacin, vancomycin, amikacin and gentamicin were determined by agar dilution method using Mueller-Hinton agar according to NCCLS (National Committee for Clinical Laboratory Standards, USA)criteria. Results : Among 549 S. aureus isolates, 278 isolates were MRSA and 271 isolates were methicil sensitive S. aureus (MSSA). MIC50 & MIC90 of arbekacin against 549 S. aureus were 0.5 & 1 ㎍/mL, and MIC50 & MIC90 of vancomycin were 1 & 1 ㎍/mL. MIC of arbekacin against 549 S. aureus isolates ranges from 0.03 to 4 ㎍/mL, and MIC of vancomycin against 549 S. aureus ranges from 0.25 to 2 ㎍/mL. MIC90 of amikacin against 549 S. aureus was 32㎍/mL, and that of gentamicin was 128 ㎍/mL. MICs of amikacin and gentamicin were variable, ranging from 0.125 to 256, and otherwise arbekacin and vancomycin revealed relatively narrow range of MICs. MIC90 of arbekacin against 278 MRSA isolates & 271 MSSA were 1 & 0.5 ㎍/mL, and those of vancomycin against MRSA & MSSA were 1 & 1 ㎍/mL. MIC90 of amikacin against 278 MRSA & 271 MSSA isolates were 32 & 4 ㎍/mL, and that of gentamicin against MRSA & MSSA isolates were 128 & 32 ㎍/mL respectively. Among 251 CNS isolates, 122 isolates were MRCNS and 129 were MSCNS. MICSO & MIC90 of arbekacin against 251 CNS isolates were 0.25 & 2 ㎍/mL, and those of vancomycin were 1 & 2 ㎍/mL. MIC of arbekacin against 251 CNS isolates ranges from 0.015 to 32 ㎍/mL, and that of vancomycin isolates ranges from 0.25 to 2 ㎍/mL, MIC90 of arbekacin against 122 MRCNS & 129 MSCNS isolates were 2&0.3 ㎍/ML, and those of vancomycin were 2&s ㎍/ML. MIC90 of amikacin against 251 CNS isolates was 32 ㎍/ML, and that of gentamicin was 128 ㎍/ML for CNS. MIC90 of amikacin against 122 MRCNS & 129 MSCNS isolates were 128 & 8㎍/mL, and those of gentamicin ere 256 & 32 ㎍/mL. Conclusion : Considering above results, arbekacin can be useful agent against most strains of MRSA and MRCMS, which exhibit high-level resistance to amikacin and gentamicin.
