        The effects of estradiol-17β or progesterone on the contractility of neurotransmitters on pig oviductal isthmic smooth muscle were investigated. The motility of the isolated smooth muscle was recorded by using physiological recording system. The results were summarized as follows : 1. The contractility induced by histamine or prostaglandin F2α was depressed by the pretreatment of estradiol-17β but was not by the preteatment of progesterone in follicular or luteal stage. 2. The contractility induced by acetylcholine was depressed un follicular stage but excited in luteal stage by the pretreatment of estradiol-17β or progesterone. 3. The contractility induced by norepinephrine was depressed by the pretreatment of progesterone in follicular stage but was excited by the pretreatment estradiol-17β in luteal stage.

        승모판 협착증 환자에서 운동부하 심장풀스캔을 이용한 좌심실기능의 평가

        N/A We performed exercise radionuclide ventriculography to evaluate left ventricular (LV) function in 35 patients with pure mitral stenosis (MS). There were 6 males and 29 females ranging from 21 to 63 years of age (mean 37±10 years). We also studied 8 healthy men as control group (mean age 27±5 years. Each patients was evaluated at rest and during maximal exercise on an isokinetic bicycle ergometer. Peak filling rate (PFR), peak ejection rate (PER), ejection fracion (EF), end-diastolic volume(EDV), end-systolic volume (ESV), stroke volume (SV), and cardiac output (CO) were determined by the radionuclide technique. The results were summarized as follows: 1) LV systolic dysfunction and reduced PFR were noted in patients with MS. 2) EDV as well as SV decreased with exercise in patients with MS. 3) No significant increase in EF during exercise compared to rest value was observed because it was caused by reduced EDV and SV during exercise in patients with MS. 4) CO increased with exercise was significantly lower than normal in patients with MS. 5) Patients with MS were divided into two groups according to whether EF increased more than 5% druing exercise compared to resting state or decreased. Significant difference were found between these two groups. Patients with a fall in EF with exercise were older, had larger left atrial size, and had lesser decreased in ESV during exercise.

      • Macrophage HIF-2α Ameliorates Adipose Tissue Inflammation and Insulin Resistance in Obesity

        <P>In obesity, adipose tissue macrophages (ATMs) play a key role in mediating proinflammatory responses in the adipose tissue, which are associated with obesity-related metabolic complications. Recently, adipose tissue hypoxia has been implicated in the regulation of ATMs in obesity. However, the role of hypoxia-inducible factor (HIF)-2 alpha, one of the major transcription factors induced by hypoxia, has not been fully elucidated in ATMs. In this study, we demonstrate that elevation of macrophage HIF-2 alpha would attenuate adipose tissue inflammation and improve insulin resistance in obesity. In macrophages, overexpression of HIF-2 alpha decreased nitric oxide production and suppressed expression of proinflammatory cytokines through induction of arginase 1. HIF-2 alpha-overexpressing macrophages alleviated proinflammatory responses and improved insulin resistance in adipocytes. In contrast, knockdown of macrophage HIF-2 alpha augmented palmitate-induced proinflammatory gene expression in adipocytes. Furthermore, compared with wild-type mice, Hif-2 alpha heterozygous-null mice aggravated insulin resistance and adipose tissue inflammation with more M1-like ATMs upon high-fat diet (HFD). More-over, glucose intolerance in HFD-fed Hif-2 alpha heterozygousnull mice was relieved by macrophage depletion with clodronate treatment, implying that increase of proinflammatory ATMs is responsible for insulin resistance by haplodeficiency of Hif-2 alpha upon HFD. Taken together, these data suggest that macrophage HIF-2 alpha would counteract the proinflammatory responses to relieve obesity-induced insulin resistance in adipose tissue.</P>

        급성 약물중독에 합병된 일과성 혹은 지속성 고암모니아혈증의 특성

        Purpose: In patients with altered mentality caused by drugs or unknown causes, ammonia is checked to facilitate differential diagnosis or diagnose hepatic coma. This helps early prevention and treatment of brain damage due to hyperammonemia. This study was conducted to evaluate clinical characteristics of intoxicated adult patients with hyperammonemia. Methods: We evaluated 95 patients with hyperammonemia among intoxicated patients above the age of 15 who visited our ED from January 2013 to December 2015. We analyzed the demographic characteristics and type of poisoning substance, reason for ingestion, toxicological characteristics such as elapsed time from ingestion to hospital visit, lab, clinical progression and complications. Data were evaluated using the student`s t test or Mann-Whitney U test for continuous variables, and Chi-square test and Fisher`s exact test for frequency analysis of categorical variables. Results: When compared to healthy individuals, patients with hyperammonemia showed statistical significance on their SOFA score (p=0.016) and poison severity score (p<0.001). Additionally, patients with hyperammonemia showed significantly different initial serum AST level (p=0.012) and maximum serum AST level during the hospital stay (p=0.026) when compared to healthy individuals. Moreover, individuals with sustained hyperammonemia compared to transient hyperammonemia showed clinically significant SOFA scores (p<0.001), poison severity scores (p=0.007), mortality rates in the ICU (p=0.021), as well as different duration of hospital stay (p=0.037), serum creatinine level (p=0.002), erythrocyte sedimentation rate (p=0.025), and serum myoglobin (p=0.015). Conclusion: Most poisoning-induced hyperammonemia cases were transient and recovered without special treatment. Therefore, hyperammonemia is almost non-specific among poisoning patients.

        해저케이블용 피기백 클램프의 기계적 성능 연구

        지속적으로 증가하는 세계적인 전력수요로 인해 고전압 해저케이블 수요도 증가하고 있다. 해저케이블은 고가의 제작 비용과 높은 단가의 포설 비용 등이 해결되어야 할 부분 중 하나이다. 해양 환경에 노출된 해저케이블은 파랑 및 해류 등의 외력들에 의해 케이블 손상이나 사고 시에는 높은 유지 보수 비용 등의 문제가 발생할 수 있다. 따라서 해저케이블은 다양한 종류의 보호 기자재를 통해 해저 환경의 불확실성에 대비하고 있다. 본 연구에서는 대표적인 보호 기자재인 피기백 클램프(PBC)와 클램프에 사용되는 스트랩(strap)을 대상으로 해석적 방법을 통해 휨 성능을 살펴보았다. 피기백 클램프의 구조적 성능 검토를 위해 유연 보호 튜브의 최대 굽힘 모멘트 기준치인 15 kN·m 값을 토대로 휨 성능을 분석하였다, 그 결과, 피기백 클램프 및 strap의 굽힘 모멘트에 대한 휨 성능이 확보되었음을 확인할 수 있었다. Due to the continuously increasing global demand for electricity, the demand for high-voltage submarine cables is also increasing. One of the issues that need to be addressed for submarine cables is the high production cost and expensive laying costs. Submarine cables exposed to the marine environment encounter external forces such as wave and current, leading to issues such as cable damage due to external factors or high maintenance costs in the event of an accident. Therefore, we are preparing for the uncertainty of the submarine environment through many protective materials and protective equipment. In this study, we examined the bending performance of piggyback clamps (PBC) and strap, which are representative protective equipment, in response to the submarine environment through analytical methods. To examine the structural performance of PBC, the bending performance were assessed under the maximum bending moment criterion of 15 kN·m for the flexible protection tube. As a result, it was confirmed that the structural performance regarding the bending moment of both PBC and straps was ensured.

        퍼블릭 스피치 실천공동체(community of practice) 토스트마스터즈 인터내셔널 참여자들의 학습경험에 관한 연구

        본 연구는 우리사회의 증가하는 퍼블릭 스피킹에 대한 수요와 함께 ‘퍼블릭 스피킹’을 공통의 관심사로 모인 성인학습자들이 그 분야의 전문성을 배양하고자 여러 가지 형태의 퍼블릭 스피치를 실천하고 있는 실천공동체 ‘토스트마스터즈 인터내셔널(Toastmasters International: 이하 TI)’ 참여자들의 학습 경험을 분석하고 이를 심층적으로 이해하고자 하였다. 이를 위해 베이직 질적 연구방법론(basic qualitative study)을 선택하였다. 연구결과로 182개의 개념으로부터 45개의 하위범주와 19개의 상위범주가 도출되었다. 19개의 상위범주는 Wenger(2007)의 ‘실천공동체학습의 사회이론’의 네 가지 영역인 실천, 공동체, 의미, 정체성에 따라 분석하였다. (1) 실천: 주변참여에서 중심참여로, (2) 공동체: 단순히 영어와 퍼블릭 스피치만을 연습하는 곳이 아니라 삶을 배우는 곳, (3) 의미: 긍정적인 만남과 더 큰 무대를 통한 자신감 획득, 평생학습과 자아실현의 장, (4) 정체성: 내면의 소리, 리더로 당당하게 변화된 나로 나타났다. 이를 토대로 내린 결론은 다음과 같다. 첫째, 연구 참여자들은 클럽활동에 있어서 ‘주변참여’에 머무르지 않고 ‘중심참여’로 나아감으로써 리더가 되었다. 둘째, ‘중심참여’에 이르기 위하여 자신과의 싸움과 무대에의 최대한 노출이 필수적이었다. 셋째, 긍정적인 분위기가 클럽활동의 지속요인이었다. 넷째, 퍼블릭 스피치와 영어구사 역량향상을 위해 가입하였으나 이와 더불어 인생을 배우는 곳이었다. 다섯째, 평생학습과 자아실현의 장이었다. 여섯째, 세계시민성 습득의 기회를 제공하는 곳이었다. The purpose of this study is to analyze and understand participants’ experiences in the Community of Practice (CoP), Toastmasters International (TI), in which, adult learners who are interested in public speaking get together to engage in different types of public speech to become more professional public speakers. It is analyzed through basic qualitative study. They were analyzed using comparative analysis and resulted in 45 subcategories and 19 upper categories from 182 concepts. The 19 upper categories were analyzed based on four factors from the ‘Social Learning Theory of CoP’ which are; practice, community, meaning, and identity. (1) practice: peripheral participation to main participation, (2) community: a place to learn about life and not to just practice English and public speech, (3) meaning: positive encounters with people, a place for life learning, realizing true self, and gaining confidence through big events, (4) identity: becoming a confident leader by listening to one’s inner voice. This study concluded with the following: First, participants could evolve as a leader by moving forward to main participation. Second, they did not succumb to a battle with themselves to become main members of the group. They did not stop trying to expose themselves to new environments. Third, the “positive energy” of TI was what brought more members to the group and it led them to be ‘main participants.’ Fourth, for participants, TI was a place for learning life through communication. Fifth, TI was a place for life-long adult learning and self realization. Sixth, TI has a value as a global civil learning organization.
