국소맛사지 특수운동요법 및 신체교정요법이 만성요통환자에게 미치는 효과

국소맛사지, 특수운동요법 및 신체교정요법이 만성요통환자에게 미치는 효과. 운동과학, 제8권 제2호, 231-244, 1998, 비만형인 40∼50대 여성중 6개월 이상 통증을 호소하는 16명을 대상으로 맛사지요법, 운동요법 및 신체교정요법 등 3가지 상이한 치료법을 적용했을 때 골밀도분석(골밀도, 골무기질함유량), 기초체력분석(유연성, 근력, 평형성, 근지구력, 민첩성), 요천추전만 각도와 요천추측만 각도 분석을 함으로써 가장 안전하면서도 효율적인 요통치료법을 관련환자 및 정형외과적인 의료인 등에게 중요한 기초자료를 제공하기 위한 연구결과는 다음과 같다. 1. 골밀도 및 골무기질 함량은 정상군과 요통환자군과의 비교에서 유의한 차이가 없었다. 2. Sit & reach, Trunk extension, Back strength는 시술기간별에서는 유의한 차이가 나타났다. 3. Balance (Left)는 시술방법과 기간간의 상호작용에서 유의한 차이가 나타났으며, 시술기간별에서도 유의한 차이가 나타났다. 4. Balance (Right)는 시술기간별, 방법, 기간과 방법간의 상호작용에서 유의한 차이가 나타나지 않았다. 5. Sit-up에서는 시술기간별에서는 유의한 차이가 나타났다. 6. Side step은 시술방법과 기간간의 상호작용에서 유의한 차이가 나타났으며, 시술기간별에서도 유의한 차이가 나타났다. 7. 요천추전만 및 요천추측만에 있어서는 시술기간별에서만 유의한 차이가 나타났다. 이상의 연구 결과를 비추어 볼 때 비만인 만성요통환자들에게 국소맛사지, 특수운동요법, 신체교정요법이 유용한 치료요법으로 사용될 수 있는 것으로 나타났으며, 물리치료사나 정형외과적 의료인, 기타 환자들에게 매우 활용가치가 높다고 할 수 있다. The effect to chronic low back pain patients for massage, special exercise program and chiropractic. Exercise Science, 8(2): 231-244, 1999. The purpose of this study was to analyze bone mineral density (B.M.D, B.M.C), physical fitness (flexibility, strength, balance, endurance, agility), lumbosacral lordosis angle and scoliosis angle, when the chronic low back pain patients for obese women of ages 40-50 olds adapted three different remedy of the massage, special exercise program. Thus, the important basic data for the efficient remedy method of the chronic low back pain presents to the orthopedic doctor, physical therapist, and et al.. Subjects were 16 women to feel the chronic low back pain over 6 months. The results obtained were as follows 1. Bone mineral density 1) There was no significant difference for B.M.D and B.M.C between control and low back pain group. 2 Physical fitness: 1) There was a significant difference for sit & reach, trunk extension, back strength among the three different treatment periods. 2) There were significant differences on main effect of treatment period and the interaction of method±treatment period for balance(lett). 3) There was no significant differences on main effect of treatment period and method, and the interaction of method±treatment period for balance(right). 4) There was a significant difference for sit-ups among the three different treatment periods. 5) There were significant differences on main effect of treatment period and the interaction of method±treatment period for side step. 3. X-ray 1) There was a significant difference for lumbosacral lordosis and lumbosacral scoliosis angle among the three different treatment periods. As Use above results, the massage, special exercise program, and chiropractic for chronic low back pain patients of obesity person can use one of the remedy method. Thus, the important basic data for the efficient remedy method of the chronic low back pain can be considered to present the orthopedic doctor, Physical therapist, and et al.

콘텐츠산업 진흥법 제36조의 비밀유지의무규정에 대한 문제점과 검토 - 콘텐츠분쟁조정위원회 사례를 중심으로 -

Mediation is a means of ADR(Alternative Dispute Resolution). And Confidentiality issue or Privacy protection is big merit of the characteristics of ADR. In Korea, Content Industry Promotion Act provided that “No person who conducts or has conducted affairs related to the mediation of a dispute of the Committee shall divulge confidential information he/she has become aware of in the course of performing his/her duty to any third person or appropriate it for any purpose other than the official purpose: Provided, That this shall not apply where expressly provided for otherwise in any other Acts.”(Article 36 Confidentiality) By the way The Content Dispute Resolution Committee(The CDRC) has provided Facilitation and Mediation process recently. But for the well conducting of confidentiality preservation, the CDRC must notify mutual confidentiality preservation to the parties from Facilitation process. And The CDRC should announce that those who are concerned in mediation process has no recording, no picture and videotape–taking in Mediation place. Also the category of persons whom have Confidentiality preservation obligation is so ambiguous that we need to enlarge the obligation to the 3rd party(mediator) and extra experts. And we should consider new article which provides “Unless otherwise agreed by the parties, the mediator and the parties shall not introduce as evidence or in any manner whatsoever in any judicial or arbitration proceeding”. There are about 50 or more Administrative Mediation Organizations In Korea but there is no criteria on mediation material keeping term owing to insufficient study on confidentiality preservation. The CDRC’s confidentiality preservation has legal exceptions. They are ACT on Testimony, Appraisal, etc. before the National Assembly, ACT on The Inspection and Investigation of State Administration and ACT on The Consumer Protection in Electronic Commerce, etc. For the energetic activities of Korea mediation organizations, it is important not only to raise the ability of mediators but also to provide enough physicalㆍlegal system environments for the protection of privacy and business confidentiality. 조정은 소송 외 대안적 분쟁해결(ADR : Alternative Dispute Resolution)의 한 방안이며, ADR 다양한 장점들 중 당사자의 비밀(Confidentiality) 또는 사생활을 보호한다는 것은 매우 큰 장점 중의 하나이다. 콘텐츠산업 진흥법에서도 콘텐츠분쟁조정위원회의 운영에 있어서 조정위원회의 분쟁조정 업무에 종사하는 자 또는 종사하였던 자에게 비밀유지 의무를 준수하게 하고 있는데, 다른 법률에 특별한 규정이 있는 경우를 제외하고는 그 직무상 알게 된 비밀을 타인에게 누설하거나 직무상 목적 외의 목적으로 사용하여서는 아니 된다고 규정하고 있다. 그런데 분쟁조정위원회에서 알선, 조정을 진행하면서 비밀유지 의무를 보다 더 잘 준수하기 위해서는 알선단계부터 당사자들에게 상호 비밀유지에 대한 고지를 할 필요가 있으며 조정회의에 있어서는 녹음, 촬영 등이 금지됨을 고지할 필요가 있다. 또한 비밀유지 준수자의 범위도 기존에 너무 모호하게 되어 있었는데 이를 조정위원 내지 전문위원까지 확대할 필요가 있으며 조정에서 제출한 기록이나 진술은 소송에서 원용하지 못하게 하는 법령의 신규제정도 필요하다. 우리나라는 50개 이상의 행정형 조정기구를 갖고 있지만, 실제 비밀유지에 대한 연구가 상대적으로 부족해서 조정자료 보관기간 등에 대해 일정한 표준이 없는 실정이다. 조정위원회의 비밀유지가 법률적으로 예외가 될 수 있는 부분은 현재 국정감사 등과 관련한 법률과 전자상거래법상에 따른 한계가 있다. 국내 많은 조정기구들이 원활하게 운영되기 위해 조정위원 자질의 역량강화도 중요한 부분이지만, 당사자의 프라이버시와 영업상 기밀을 보호할 수 있도록 충분한 물리적ㆍ제도적 뒷받침을 하는 것이 중요하다.

Purpose: Various kinds of local flap or free flap have been used for coverage for soft tissue defects with bone exposure over the ankle and dorsum of foot. Adipofascial flaps, nourished by vascular plexuses of the subcutaneous tissue and deep fascia originating from the local perforators of the major vessels, appear particularly to be indicated for the reconstruction of these areas. Our experience with this flap on the dorsum of foot and ankle has also been quite encouraging.Methods: The design of the flap is determined by the size and the location of the defect. The base of the flap is chosen depending on the availability of the soft tissue around the defect. The ratio of the area of the flap to the area of the base wound be more reliable to predict the survival of the turnover flap by the conventional length-to-width ratio. Nineteen patients with defect over the dorsum of the foot and ankle were resurfaced with adipofascial turn-over flaps and skin graft.Results: The average age of the patients was 38.2 years(3-81 years). The flap size was from 2×3cm to 8×5cm. The average follow-up time was 6 months. All flaps survived completely except one case who suffered distal necrosis of the flap. The additional skin graft was required for partial skin loss in the five cases. Other functional impairment was not noted. Conclusion: Dissection of the local adipofascial turnover flap is quite easy, quick, requires less time and sacrifice of surrounding muscle itself, and maintains major arteries. In most cases, donor-site morbidity is minimal with an acceptable scar, and both functional and esthetical results were satisfactory. Therefore, Adipofascial flap could be an option for the difficult wounds around the foot and ankle.

이차성 구순열 비변형 환자에서 비변형과 비폐쇄와의 상관관계

Patients with secondary cleft lip nose deformity usually complain not only aesthetic problems but also functional problems such as nasal obstruction. However, there have been few studies on nasal airway problem in secondary cleft lip nose deformity patients and no reports for the correlation between degree of the deformity and nasal airway obstruction. The authors selected 34 patients with secondary cleft lip nose deformity and measured the degree of morphological deformity by photo analysis and the degree of nasal airway obstruction by VAS score and acoustic rhinometry. Almost all patients complained of more obstructive symptom on the affected side. Functional anatomic structure and volume of the nasal airway examined by acoustic rhinometry showed that narrowest point was located above the nostril and pathological obstruction was more significant in patient group in both side and the area of the narrowest point and the total nasal volume was smaller in patient group. The result that alar contour difference has correlation with total nasal volume but not with objective symptom shows that there may be other factors affecting the nasal obstruction. Therefore the authors conclude that not only the correction of the external morphologic deformity but also the consideration of nasal airway obstruction is essential when performing operation on the secondary cleft lip nose deformity patients.
