        Stevens-Johnson 증후군 이후에 발생한 폐색성 세기관지염 1례

        Bronchiolitis Obliterans(BO) is one of the rare inflammatory lung disease that primarily affects bronchi and bronchioles, followed by partial or complete obstruction. In children, the etiology of BO has been under consideration, but infections and toxic inhalation, connective tissue diseases, lung transplantation, congestive pulmonary edema, etc., may be causes of BO. Even though steroid therapy was reported as an effective treatment, there are few data regarding the epidemiology, pathophysiology, long term prognosis and therapy. Here we describe a 5-year-old boy who presented with intractable wheezing associated with Stevens-Johnson syndrome after Varicella-Zoster virus infection and was diagnosed as BO through chest high-resolution computed tomography. 폐색성 세기관지염은 기관지와 세기관지가 부분적 혹은 완전히 섬유조직으로 막히는 드문 질환이다. 저자들은 Varicella- zoster 바이러스 감염에 의한 Stevens-Johnson 증후군 치료 이후에 나타난 호흡곤란을 주소로 내원한 5세 남아에서 임상 양상과 흉부 고해상 단층촬영 소견으로 폐색성 세기관지염을 진단하였기에 문헌 고찰과 함께 보고하는 바이다.

        전기 경련 요법에서 etomidate, propofol과 thiopental sodium의 효용성 비교

        Background:Electroconvulsive therapy (ECT) is effective for major psychosis and affective disorder. The ideal anesthetics for ECT provide rapid induction and recovery, and they attenuate the adverse effects of ECT. We compared the effects of etomidate, propofol and thiopental sodium during ECT. Methods:Nine patients were enrolled in this double blinded cross over design study. The ECT was done 3 times per week for two weeks. We monitored the patients with electrocardiography, pulse oximetry, the bispectral index score and the blood pressure. We recorded the data at the time of arrival to the therapy room, just before ECT and at 1, 2, 3, 4, 5, 7 and 10 minutes after ECT. Hypnosis was induced with 3 mg/kg of thiopental sodium (group T), 1.5 mg/kg of propofol (group P) or 0.15 mg/kg of etomidate (group E). The ECT was done after administering 1 mg/kg of succinylcholine. The duration of seizure were measured after ECT. Results:There were significantly different durations of motor seizure among the three groups. The duration of EEG seizure in group E was longer than that of group P and group T. The blood pressure and the heart rate of group P were significantly lower than that of the other groups (P < 0.05). Conclusions:Compared to thiopental sodium and etomidate, propofol was not associated with clinically significant changes in the duration of seizure and hemodynamic stability. It has a good Etomidate is effective for patients for achieving a short duration of seizure after ECT is applied. (Anesth Pain Med 2011; 6: 32∼36)

        Laparoscopic appendectomy under spinal anesthesia with dexmedetomidine infusion

        Background: Laparoscopic appendectomy (LA) is rarely performed under regional anesthesia because of pneumoperitoneum-related problems. We expected that dexmedetomidine would compensate for the problems arising from spinal anesthesia alone. Thus, we performed a feasibility study of spinal anesthesia with intravenous dexmedetomidine infusion. Methods: Twenty-six patients undergoing LA received spinal anesthesia with intravenous dexmedetomidine infusion. During surgery, the patient’s pain or discomfort was controlled by supplemental fentanyl or ketamine injection, and all adverse effects were evaluated. Results: No patient required conversion to general anesthesia, and all operations were completed laparoscopically without conversion to open surgery. Seventeen (65.4%) patients required supplemental injection of fentanyl or ketamine. Bradycardia occurred in seven (26.9%) patients. Conclusions: Spinal anesthesia with dexmedetomidine infusion may be feasible for LA. However, additional analgesia, sedation, and careful attention to the potential development of bradycardia are needed for a successful anesthetic outcome.

        토픽모델링 기반의 국방기술 동향분석 방안 : 장갑전투차량에의 적용

        국방 분야 무기체계 연구개발 계획을 수립하기에 앞서 사전 기획단계에서 관련 기술의 수준 과 발전 동향을 분석하는 단계가 필수적이다. 그러나 ‘폐쇄성’, ‘보안성’이라는 국방분야의 특성 상 공개된 정보를 활용한 기술정보의 공유·분석·현업 재활용이 쉽지 않은 것이 현실이다. 이에 본 연구는 텍스트마이닝과 토픽모델링을 기반으로 국방기술의 동향 분석 체계를 수립하는 것을 목표로 한다. 텍스트마이닝과 토픽모델링을 통해 산재해있는 국방 빅데이터의 체계적·정형적 분 석이 가능하다. 본 연구에서는 최근 인공지능, 로봇의 발전과 함께 미래 무인체계 개발의 첫 걸 음으로서 국방 연구기관에서 활발히 연구 중인 장갑전투차량을 대상으로 사례분석을 실시하였 다. 본 연구를 통해 국방 기술의 변화 추세와 미래 개발 이슈를 도출하고, 향후 국방과학기술의 연구개발 기획에 활용되기를 기대한다. Establishing an R&D plan for the weapon system in the defense sector requires a preliminary step to analyze the current level and development status of the related technologies. However, the closed and confidential characteristics of the defense sector make it difficult to share, analyze, and utilize technology information available to researchers. In light of the above, this study seeks to establish a systematic analysis system for defense technologies based on the text mining and the topic modeling technologies. Text mining and topic modeling enable a systemic and programmed analysis of the big data scattered throughout the defense sector. For this study, we carried out a case analysis on the Armoured Fighting Vehicle development actively pursued by a defense research institute as the first step in the development of future unmanned systems along with artificial intelligence and robot development.         최근 10년간 강원도내 소아의 혈중 칼슘농도의 변화

        목적 : 최근 우리 나라의 음식물에는 칼슘을 보강한 제품이 널리 유통되고 있고, 사람들의 칼슘섭취에 대한 인식도 달라지고 있다. 최근 본원에서는 혈뇨를 주소로 내원하여 검사를 시행한 환아에게서 고칼슘뇨증 소견에 합당한 경우가 늘어나 이에 본원을 방문한 소아를 대상으로 최근 10년간의 혈중 칼슘치와 그에 관계된 혈중 ALP, 크레아티닌, 나트륨, 칼륨, 혈중 중탄산염과 요중 pH와의 상관관계 및 변화 양상을 비교 분석해 보았다. 대상 및 방법 : 1991년, 1992년과 2000년, 2001년 본원에서 편도선 및 아데노이드 제거수술과 고환 고정술 및 탈장 봉합술을 시행한 환아를 대상으로, 혈중 칼슘, ALP, 크레아티닌, 나트륨, 칼륨, 중탄산염과 요중 pH 값을 조사하였다. 연도군 별로는 1991년, 1992년에 355명의 환아와 2000년, 2001년에 488명의 환아를 대상으로 하여 각 연령군 간에 t-test를 통해 분석하였다. 결과 : 2001년도군에서는 칼슘, ALP, 크레아티닌, 나트륨, 칼륨, 소변 pH의 값이 각각 $9.91{\pm}0.50\;mg/dL,\;248.58{\pm}94.98\;U/L,\;0.61{\pm}0.14\;mg/dL,\;138.64{\pm}2.22\;mM/L,\;4.35{\pm}0.40\;mM/L,\;6.18{\pm}0.86$으로 1991년도군의 각 각의 값인 $9.13{\pm}0.68;mg/dL,\;198.26{\pm}79.34\;U/L,\;0.433{\pm}0.18\;mg/dL,\;137.86{\pm}2.67\;mM/L,\;4.22{\pm}0.36\;mM/L,\;5.83{\pm}0.95$에 비해 모두 P-value 0.05이하의 유의한 증가를 보였고, 혈중 중탄산염은 2001년도군에서는 $23.64{\pm}2.57\;mM/L$로 1991년도군의 $24.60{\pm}2.23\;mM/L$에 비해 P-value 0.000으로 통계적으로 유의한 감소를 보였다. 각 연령별, 성별 비교에서도 같은 양상을 보인다. 결론 : 최근 10년간의 강원도 소아의 혈중 칼슘치는 의미 있게 증가하였다. 이는 칼슘의 경구 섭취량의 증가에 기인한 것이 큰 영향을 미친다고 사료되며 이에 대한 재인식과 교육이 필요하다고 생각한다. Purpose : Nowadays, drinks, foods and snacks have frequently been intensified with calcium and the insights into the importance of calcium-intake in general has developed in Korea. In this decade, we found the numbers of children who was visited to our hospital for evaluation of hematuria defined with hypercalciuria were increased. So we tried to compare the mean levels of serum calcium, alkaline phosphate, sodium, potassium, chloride, BUN, creatinine, bicarbonate and urinary pH who visited our hospital in 1991, 1992 with in 2000, 2001. Materials and methods : Between January 1991 to December 1992, and between January 2000 to December 2001, each 366 children and 488 children, aged 1 month to 15 years, who presented in our hospital for tonsilectomy and adenoidectomy or for inguinal herniorrhaphy were enrolled in the study, The children in the study were checked the level of serum calcium, alkaline phosphate, sodium, potassium, chloride, BUN, creatinine, bicarbonate and urinary pH with the machine which was corrected the similar levels of practical chemical levels in serum. We compared each mean levels in 1991s' group with in 2001s' group totally and separately through the age and sex. We used t-test to analysis data. Results : The levels of serum calcium, alkaline phosphate, creatinine, sodium, potassium, and urinary pH of 2001s' group were significantly higher than the levels of 1991s' group(P<0.05). The each level was $9.91{\pm}0.50\;mg/dL,\;248.58{\pm}94.98\;U/L,\;0.61{\pm}0.14\;mg/dL,\;138.64{\pm}2.22\;mM/L,\;4.35{\pm}0.40\;mM/L,\;6.18{\pm}0.86$ in 2001s' and $9.13{\pm}0.68;mg/dL,\;198.26{\pm}79.34\;U/L,\;0.433{\pm}0.18\;mg/dL,\;137.86{\pm}2.67\;mM/L,\;4.22{\pm}0.36\;mM/L,\;5.83{\pm}0.95$ in 1991s'. And the levels of serum bicarbonate, $23.64{\pm}2.57\;mM/L$ in 2001s' was significantly lower than the 1991s', $24.60{\pm}2.23\;mM/L$(P<0.05). The similar results were detected each age and sex group. Conclusion : The levels of serum calcium increase in this decade. The results will be used as a basic data for the national health plan in the years to come.

        의사결정나무를 활용한 방산육성지원 수혜기업 결정요인 분석

        . This study attempted to analyze the factors that influence the participation of beneficiary companies in the government’s defense industry promotion support project. To this end, experimental data were analyzed by constructing a prediction model consisting of highly important variables in beneficiary company decisions among various company information using the decision tree model, one of the data mining techniques. In addition, various rules were derived to determine the beneficiary companies of the government’s support project using the analysis results expressed as decision trees. Three policy measures were presented based on the important rules that repeatedly appear in different predictive models to increase the effect of the government’s industrial development. Using the analysis methods presented in this study and the determinants of the beneficiary companies of the government support project will help create a sustainable future defense industry growth environment.

        In the DC electric railway system, the effective use of regenerative break energy is an important issue. Since regenerative break energy causes voltage rise or drop in the system, it should be also solved effectively. To solve the problems, applying electric double layer capacitor (EDLC) or voltage source converter (VSC) to the DC electric railway system has been studying. In this paper, the coordination of EDLC and VSC is proposed to solve the problem effectively with its coordinated control algorithm. The proposed method is tested to show its feasibility using Matlab/Simulink.

        Joubert 증후군 1례

        저자들은 18개월 된 여아에서 주기성 빈호흡과 무호흡, 정신 발달 지체, 안구 운동 이상의 소견을 보여 실시한 뇌 자기공명 영상에서 소뇌 충부의 완전 결손을 보여 Joubert 증후군으로 진단된 1례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Joubert syndrome is a rare autosomal recessive condition characterized by dysgenesis of the cerebellar vermis, hypotonia, developmental delay, ataxia, abnomial eye movements, and a respiratory pattern of alternating tachypnea and apnea. Brain MRI shows the molar tooth sign resulting from dysplasia of the isthmic segment of the brain stem, superior cerebellar peduncles, and vermis. We experienced a case of Joubert syndrome, who was a 18 month-old female with episodic hyperpnea, developmental delay, abnormal eye movements and complete agenesis of the cerebellar vermis. We present this case with a brief review of literature.

        불활성 압축가스를 이용한 미세물분무 소화시스템의 유류화재 소화특성

        Water mist fire suppression system is environmental system and needs a flange pump to jet water. In this research, high pressure Nitrogen cylinder is used as a pressurizing source instead of flange pump, and also we tried to find the possibility of using compressed Nitrogen as a fire suppression agent. As a result, it was possible to design water mist fire suppression system with Nitrogen cylinder and suppress oil fire effectively. With DK1.58 nozzle, the optimum Nitrogen pressure was 80bar and the pressure was stable during water mist spray. However, jet of Nitrogen was not effective fire suppression agent when it was dually used with water mist because water mist has blown away, and it is efficient way to use compressed Nitrogen as a pressurizing source only
