Purpose: The purpose of this case report is to describe accompanying ocular findings, especially optic neuropathy, in a patient with MELAS syndrome. Methods: A 10-year-old male patient who had lactic acidosis and neurological symptoms (loss of consciousness, vomiting, epilepsy, decreased vision, and constricted visual field), underwent a brain magnetic resonance imaging (MRI) scan and a molecular genetic analysis. Results: A diagnosis of cerebral infarction due to occlusion in the right posterior cerebral artery was made and confirmed by MRI scan. The diagnosis of MELAS syndrome was confirmed by performing molecular genetic analysis demonstrating the transformation of the mitochondrial tRNAleu(UUR) gene (MTTL1) A to G(3243). According to the ocular examinations, the patient??s left eye showed decreased vision, hemianopsia, relative afferent pupillary defects, mild swelling of optic nerve, and decreased amplitude on visual evoked potential exam. Conclusions: We encountered a case of MELAS syndrome involving optic neuropathy, which can (although rarely) occur with this syndrome. Therefore, in patients with an optic neuropathy of uncertain etiology, clinicians should consider performing comprehensive ocular exams and molecular genetic exams to rule out the possibility of genetic diseases.

Purpose: This clinical report describes the characteristics and ophthalmic treatment of a patient with anhidrotic ectodermal dysplasia showing bilateral upper eyelid entropion, conjunctival mass and dry eye syndrome. Methods: A 12-year-old boy presented to our clinic complaining of a white mass on the lower tarsal conjunctiva, foreign body sensation, and an itching in his right eye of one-year duration. He had developmental defects in his hair, teeth, nails and sweat glands. He had dry skin, a flat upper-lip, heat intolerance, a saddle-backed nose, and hypohidrosis. Ophthalmic examination showed blepharitis, a conjunctival mass, and bilateral upper eyelid entropion. Results: Upper eyelid entropion surgery was performed. An excisional biopsy of the conjunctival mass showed nonspecific acute and chronic inflammation. Conclusions: We concluded that the conjunctival mass developed as a result of chronic inflammation caused by chronic conjunctivitis and dry eye syndrome. Regular lid-hygiene with swab and lubrication is useful to minimize recurrent ocular inflammation in anhidrotic ectodermal dysplasia.

Purpose: To evaluate the diagnostic ability of the modified ISNT rule (disc rim thickness of the smaller of inferior and superior > the larger of nasal and temporal) for normal and glaucomatous eyes compared to the classic ISNT rule (disc rim thickness of inferior > superior > nasal > temporal). Methods: Color stereo optic disc photographs of 113 normal subjects and 108 open angle glaucoma patients with early and moderate stage were morphometrically evaluated. The classic ISNT rule and the modified ISNT rule were assessed by masked evaluation of disc photographs at the 3, 6, 9 and 12 o’clock positions. Results: Among normal subjects, 58 of 113 eyes (51.3%) were normal and in open angle glaucoma patients, 104 of 108 eyes (96.3%) were abnormal with the classic ISNT rule. Among normal subjects, 98 of 113 eyes (94.2%) were normal and in open angle glaucoma patients, 102 of 108 eyes (94.4%) were abnormal with the modified ISNT rule. The modified ISNT rule was more accurate than the classic ISNT rule in terms of Cohen’s Kappa analysis used for discriminating between normal and glaucomatous eyes. Conclusions: The modified ISNT rule is useful for differentiating between normal and glaucomatous optic nerves and easily applied in clinical practice. J Korean Ophthalmol Soc 2014;55(1):93-101
유리체강내 트리암시놀론 주입술 후 예방적 안압하강제의 효과

Purpose: The purpose of this study was to investigate the prophylactic effect of brimonidine 0.15% on intraocular pressure (IOP) elevation and the risk factors for its elevation after intravitreal triamcinolone acetonide injection (IVTA). Methods: A prospective, randomized clinical trial was conducted on 67 eyes of 64 patients undergoing IVTA. Eyes were randomly divided into two groups, those which had used brimonidine 0.15% (40 eyes) and those which had not used it (27 eyes). IOP was measured preoperatively, at one week, and monthly until six months post-injection in each group. Results: The mean post‐injection IOP at one week was 11.93±3.36 mmHg for the group that had used brimonidine and 13.58±3.25 mmHg for the group that had not used it. The difference between the two groups was statistically significant at one week (p=0.049), but others were not statistically significant. An elevation in the IOP of more than 22 mmHg was seen in 8 eyes (20%) in the group using brimonidine and in 7 eyes (25.9%) in the group not using brimonidine. There was no difference in the incidence of IOP elevation between the two groups. Conclusions: Prophylactic use of brimonidine 0.15% will prevent sudden IOP elevation and will, therefore, prevent damage to the retina and optic nerve. However, in the long term, there is no prophylactic effect of brimonidine 0.15% on IOP elevation because there was no difference in the incidence of IOP elevation of more than 22 mmHg between the two groups.
선형회귀와 국부적인 RBFN에 의한 점진적인 모델의 설계

본 논문은 선형회귀(LR: Linear Regression)와 국부적인 방사기저함수 네트워크(RBFN: Radial Basis Function Networks)를 결합한 점진적인 모델(incremental model)의 설계와 관련되어진다. 전형적인 RBFN에 의한 모델링과는 달리, 제안된 방법의 근본적인 원리는 두 단계에 의해 고려되어진다. 첫째, 전체 모델의 설계과정에서 전역적인 모델로써 선형회귀에 의해 데이터의 선형부분을 구축한다. 다음으로, 모델링 오차는 오차가 존재하는 국부적인 공간에서 RBFN에 의해 보상되어진다. 여기서, 오차의 분포로부터 RBFN을 설계하기 위해 컨텍스트 기반 퍼지 클러스터링(CFC: Context-based Fuzzy Clustering)를 통해 정보입자의 형태로 구축되어진다. 실험은 자동차 mpg 연료소비량 예측과 부동산 가격예측문제를 통해 제안된 방법의 우수성을 증명한다.
증례 : 혈액종양 ; 소장 폐쇄로 표현된 다발골수종의 골수 외 재발 1예

다발골수종에서의 형질세포종은 일반적으로 질병이 진행된 경우에 발생하며 나쁜 예후와 연관된다. 저자들은 다발골수종으로 항암 화학 치료를 받고 완전 관해 상태에 도달한 후 소장의 폐쇄를 일으킨 형질세포종으로 재발된 매우 드문 증례를 경험하였다. 내원 25개월 전에 다발골수종으로 진단받고 볼테조밉을 포함한 항암 화학 치료를 받은 후 완전 관해에 도달한 75세 남자가 복통이 발생하였다. 복부에서는 금속음이 청진되었고 단순 복부 방사선 촬영에서 계단식 공기음영이 관찰되었다. 복부 전산화 단층촬영에서 소장의 내강을 막고 있어 기계적 장폐쇄를 일으키고 있는 종괴가 관찰되었다. 이 종괴는 수술적 절제를 통한 조직 검사에서 형질세포종으로 확인되었고 이후 탈리도마이드를 포함한 항암화학 치료를 시행했으나 환자는 6개월 후에 질병의 진행으로 사망하였다. 이 증례로 미루어 보아 다발골수종으로 치료후 추적 관찰하는 동안에 드문 형태의 골수 외 형질세포종의 발생에 대한 주의가 필요할 것이다. Plasmacytoma in patients with multiple myeloma usually develops in the advanced stage of the disease. We report herein an atypical case of extramedullary relapse of multiple myeloma that presented as mechanical obstruction of the small bowel in a patient who had achieved complete remission after chemotherapy. A 75-year-old man was diagnosed with multiple myeloma 25 months previously and treated with a bortezomib-containing chemotherapy regimen. He presented for evaluation of abdominal pain. A circumferential mass resulting in mechanical ileus was observed by abdominal computed tomography. Biopsy after surgical resection confirmed the diagnosis of plasmacytoma. The patient was subsequently treated with thalidomide-containing chemotherapy, but he died of disease progression after 6 months. We suggest careful observation of unusual relapses of multiple myeloma in patients who have achieved complete remission after antimyeloma therapy. (Korean J Med 2015;88:330-334)

This paper describes the design of ensemble deep models based on time-scale transformation from electrocardiogram (ECG) signals for emotion recognition. As the number of senior citizens living alone increases, emotion robots that can interact with them and other emotion robots are becoming increasingly important. Existing emotion robots usually recognized emotion through images of the user’s facial expressions or voice signals. However, there are many situations where the user’s emotions cannot read under various environments. Therefore, research on recognizing the user’s emotions through ECG signals among different biomedical signals is actively being conducted. The proposed method converts ECG signals into various types of two-dimensional time-scale representations. We then designed a four-stream deep learning model by applying it to an ensemble form and transfer learning. Finally, an experiment was conducted using the ASCERTAIN sentiment database. This database contains data recorded by 58 people with 9 different emotions. Among these emotions, we used six representatives (surprise, happiness, anger, disgust, fear, and sadness). The experimental results revealed that the presented ensemble deep models showed good performance in comparison with each single deep model and the original model without transformation.
