        약물관련 골괴사(MRONJ) 치료권고안, 무엇이 달라졌나?

        Antiresorptives, such as bisphosphonate, denosumab, are widely prescribed drugs for the treatment of osteoporosis, osteopenia, malignant bone metastases, multiple myeloma and Paget`s disease. They have therapeutic efficacy for controlling bone pain and reducing skeletal related events including the pathologic fractures. However, it was reported that the development of osteonecrosis of the jaw(ONJ) could be related to antiresorptive treatments. It was first defined as Bisphosphonaterelated osteonecrosis of the jaw in 2007 American Association of Oral and Maxillofacial Surgeons(AAOMS) position paper. This was changed to Medication-related osteonecrosis of the jaw(MRONJ) in the AAOMS position paper 2014. This position paper was updated recently in May 2022 and contains revisions to disease definition and management strategies at “AAOMS position paper on MRONJ-2022 update”. The important changes in clinical implications are as follows; 1) In the definition, immune modulators (such as Romosozumab) were included with potential medications related with osteonecrosis of the jaw (ONJ), 2) Operative therapy was included for all stages (1, 2 and 3) in treatment strategies, 3) Formerly mentioned drug holiday before surgical procedure to prevent MRONJ was not identified in 2022 AAOMS position paper. Most of the statements in AAOMS 2022 position paper were based on strict literature reviews and changes were made if there is little evidence to suggest the specific diagnosis or therapeutic options. Understanding the changes compared to the previous position papers is important in selecting adequate treatment strategies for MRONJ.

      • 측사위경두개방사선사진(Oblique Transcranial Radiograph)을 이용한 하악전돌증환자의 하악지 시상골절단술후 하악과두위치 변화분석

        하악지 시상골절단술을 시행한 25명의 하악전돌증환자에 있어 술직전, 술직후, 술후 6개월이상의 장기관찰기간에 촬영된 측사위경두 개방사선사진 및 측모두부계측방사선사진을 이용하여 과두위치변화와 이에따른 과두위치변화에 따른 술후 안정성의 관계를 파악하고자 Paired t-test 및 다중회귀분석을 통하여 계측치의 각 항목별 변량과 악관절 간극을 변수로 하여 통계적 관찰을 시행한 결과 다음과 같은 결론을 얻었다. . 측사위경두개방사선사진상에서, 술직후 후방관절극이 증가하며 장기관찰시 다시 감소하는 경향이 있었으나 전반적으로 술전과 장기관찰시의 계측지를 비교한 결과, 관절극의 통계적으로 유의할만한 변화가 없었다. (p>0.05) . 전체 관절극중 0.2mm 미만의 변화를 나타낸 경우가 30%, 0.2mm 이상 1.0mm 미만의 변화를 나타낸 경우가 60.7%, 1.0mm 미만의 변화를 나타낸 경우가 60.7%, 1.0mm 이상의 변화가 나타난 경우가 9.3%로 나타나 하악과두가 술전위치로 비교적 재현성있게 위치되었음을 알수 있었다. . 수술에 의한 하악골의 후퇴량이 술후재발에 영향을 미치지 않음이 관찰되었다. (p>0.05) . 수술에 의한 하악골 후퇴량이 술직후 하악과두의 위치변화에 영향을 미치지 않았으며 (p>0.05) 수술에 따른 하악과두의 위치변화가 술직후-장기관찰간의 재발성향에 영향을 주지 않았다. (p>0.05) 이는 수술에 따른 과두의 변위정도가 계측오차 내에 있을정도로 미약하거나, 미세하게 변위된 과두도 술전위치로 재위치되는 경향이 있어 수술후 안정성에 영향을 줄 정도의 것은 아닌것으로 사료된다. This study was intended to evaluate condyle position and the relationship of condyle position change and post surgical relapse following the sagittal split ramus osteotomy for mandible setback in 25 patients by paired t-test and multiple regression analysis. We used oblique transcranial and cephalometric radiographs taken before operation, immediated after operation, and at least 6 months post operatively. 1. In oblique transcranial view, posterior joint space was decreased immediate after operation and increased 6 months after operation. To compare the measurement before and 6 months after operation, there was no statistically significant change in over all joint spaces(p>0.05). 2. The joint spaces changed under the 0.2mm were 30%, 0.2mm to 1.0mm were 60.7%, above 1.0mm were 9.3%. This result reveals that condyle position was relatively reproduced to pre-operative state. 3. Statistically, the amount of mandible set back didn't influence the post operative relapse(p>0.05) 4. Statistically, the amount of mandible set back didn't influence the condylar displacement(p>0.05), and the amount of joint space change didn't influence the post operative relapse.(p>0.05) The changes in joint space is in the standard tracing error or within the adaptive capacity of the individual, it was too small to influence the stability of surgery.

        하악전돌증 환자에 대한 양악 수술의 안정성

        The factors related to relapse in 20 skeletal class III patients who performed two-jaw surgery with Le Fort I maxillary osteotomy and bilateral sagittal split ramus osteotomy was investigated. All patients were fixed with miniplate on the maxilla and three screws at each mandible. Cephalograms taken at preoperative, immediate postoperative and 8 months postoperative after surgery were traced and digitized. 1.The horizontal and vertical relapse of maxilla and mandibular chin points was within 1mm postoperatively. Compare to the preceding report concerning the mandibular set-back surgery only group, this reveals two-jaw surgery for mandibular prognathism using rigid fixation is more stable. 2.Although there was no significant relapse tendancy was observed at chin points, the screw tip land-marks moves anterio-superiorly and each side of the screws moved as a one unit. The screw tip points moved similar direction to the masticatory force and this movements might be influenced by the muscular tension to the distal segment of the mandible. 3.According to the regression analysis, the amount of horizontal and vertical movement of mandibular set-back influenced the mandibular relapse. However, direction and amount of maxillary surgical movement did not inf1uenced the maxillary and mandibular relapse.

        사례 분석을 통한 국내 장수명 주택 활성화 방안에 관한 연구 - 저해요인 분석 중심으로 -

        본 연구의 목적은 해외사례 분석을 통하여 국내 장수명 주택의 활성화를 위한 방안을 모색하고자 한다. 문헌 및 사 례 분석, 전문가의 인터뷰를 통하여 활성화 저해요인을 도출하였고, AHP기법을 이용하여 주요 저해요인을 선정하 였다. 선정된 주요 저해요인을 중심으로 활성화 방안을 제시하였다. 주요 저해요인은 장수명 주택의 보급을 위한 정부의 지원 및 홍보 부족(0.099), 조세 및 금융상의 지원대책 미흡(0.091), 주택을 주거보단 투자를 위한 재화수단으로 여기는 인식(0.087), 신기술 개발 및 도입을 위한 경제적 지원 미흡(0.083) 등의 순으로 도출되었으며, 이는 향후 국내 장수명 주택 활성화를 위한 기초자료가 될 것으로 사 료된다. The purpose of this study is to seek plans for encouraging domestic long-life housing by analyzing foreign cases. Reference and case analyses, as well as expert interviews were performed to deduct impediments, and AHP method was used for selecting major impediments. Based on selected major impediments, activation plans were suggested. Major impediments were deducted in order of; lack of governmental support and PR activities for long-life housing supply (0.099), insufficient supporting measures for taxation and finance (0.091), Consideration of housing as means of money for investment than dwelling (0.087), and inadequate economical support for new technology development and introduction (0.083). These are considered to be base data for encouraging domestic long-life housing in the future.

        Photoshop을 이용한 하악 후퇴 수술후의 연조직 안모 예측

        Various computer software has been applied to predict soft tissue profile after orthognathic surgery. In this study, the Photoshop program was introduced to simulate the surgical result. The purpose of the study was to evaluate reliability of the program in prediction of profile changes as a result of mandibular set-back surgery. Based on predicted hard tissue changes, soft tissue changes were recorded with pre- and post-operative lateral cephalogram of 26 patients with skeletal Class Ⅲ deformity. All patients underwent bilateral sagittal split osteotomy. The mean mandibular set-back was 6.2㎜ (Pog) and soft tissue moved 6.3㎜, 100% in Pog' point post-operatively. Difference between the between real horizontal soft tissue changes and predicted changes has not been detected statistically. However, the result revealed vertical soft tissue profile prediction was unsatisfactory. This prediction method can be used as additional informative tool for the patient explanation.

        900MHz RFID 표준 프로토콜(ISO/IEC 18000-6 type B)에서의 충돌방지 성능 개선

        본 논문에서는 900 MHz RFID 표준 프로토콜 중 하나인 ISO/IEC 18000-6 type B 프로토콜의 충돌방지 알고리즘의 성능을 분석하고, 이에 대한 성능 향상 방법을 제안한다. 제안된 방법은 ISO/IEC 18000-6 type B 프로토콜의 충돌방지 알고리즘에서 명령 전송 횟수 및 전송 데이터의 양을 줄임으로써 태그 인식 속도를 개선한다. 시뮬레이션 결과를 통해 제안된 방법이 리더 인식 영역 내의 태그 개수와 관계없이 태그 인식 속도를 기존 방법에 비해 21.7% 향상시킬 수 있음을 보인다. This paper proposes a novel anti-collision method for the ISO/IEC 18000-6 type B protocol which is the standard protocol of the 900 MHz RFID system. We improve the anti-collision performance by reducing the transmission number of commands and the length of bits required for multi-tag identification in the ISO/IEC 18000-6 type B protocol. Simulation results show that the proposed method improves the multi-tag identification time by 21.7% over the conventional method, irrespective of number of tags.

        Antioxidative Effect of CLA Diet and Endurance Training in Liver and Skeletal Muscles of Rat

        The purpose of this study is to investigate the effects of conjugated linoleic acid (CLA) supplementation and endurance exercise on the oxidative/anti-oxidative status in rat liver and skeletal muscles. Sprague-Dawley male rats were randomly divided into HS (high-fat diet sedentary group, n = 8), CS group (CLA supplemented sedentary group, n = 8), and CE group (CLA supplemented exercise group, n = 8). For CLA supplementation, 1.0% CLA was substituted for dietary fat. For endurance exercise, the rats swam for 60 min a day, 5 days a week for 8 weeks. The MDA content, Cu, Zn-SOD and Mn-SOD expression in the soleus muscle (SOM) of the CE group improved significantly compared to the HS (p < 0.01) and CS groups (p < 0.05). Moreover, Mn-SOD expression in both the SOM and extensor digitorum longus muscle (EDL) of the CS were enhanced significantly compared to the HS (p < 0.05). From these results, it was suggested that CLA supplementation under the endurance exercise condition may improve the oxidative status by decreasing the MDA content via potential scavenging of Cu,Zn-SOD, and Mn-SOD protein in red muscle, respectively. Therefore, our study demonstrated long-term endurance exercise with CLA supplementation plays a crucial role for maintenance of antioxidative properties in the skeletal muscle of rat.
