찰스 거킨의 이야기치료에 근거한 이혼위기 부부상담의 치료적 적용 - 기독교인 이혼위기 부부상담 사례를 중심으로 -

본 연구의 목적은 기독교인 이혼위기 부부를 대상으로 찰스 거킨의 이야기치료에 근거한 이혼위기 부부상담의 치료적 적용에 있다. 본 연구에 참여한 사례자는 중년의 기독교인 부부로서 남편의 퇴직후 부부 갈등이 심화되어 이혼위기에 처해 있었다. 상 담은 1주일에 1회씩 총 8회기로 진행하였으며 연구자는 거킨의 이야기해석학을 바탕 으로 4단계의 이야기치료를 적용하였다. 연구결과, 처음에 부부는 상대방에게 받았던 상처와 분노의 감정으로 인해 관계의 절망을 느끼고 있었는데, 1단계 상담을 통해 부 부는 자신들의 이혼위기를 유발한 핵심이야기를 발견하였고, 2단계 상담을 통하여 이 혼위기를 유발한 가장 중요한 동기와 이유를 발견하고 서로에게 이야기함으로써 상대 방의 마음을 깊이 이해하게 되었다. 3단계 상담을 통하여 부부는 하나님의 관점으로 자신들의 이혼위기 상황을 바라봄으로써 하나님앞에서 스스로의 영성을 되돌아보며 회개하는 마음을 갖게 되었고 이혼위기의 책임이 자신에게 있음을 고백하게 되었다. 4단계 상담을 통하여 예수그리스도의 십자가 사건을 깊이 묵상하면서 진정한 사랑의 의미를 새롭게 깨닫고 십자가의 사랑을 부부관계 사랑에 적용하게 되었다. 아울러 교회에서 신뢰할만한 목회자와 성도에게 도움을 요청함으로써 신앙공동체의 지속적인 지지를 받게 되었다. 상담과정을 통해 확인한 거킨의 이야기치료의 가장 큰 영향력은 부부가 만든 잘못된 이야기가 하나님이야기와 만났을 때 부부의 이야기가 재해석된다 는 점이다. 이혼위기에 있었던 부부는 처음에는 상대방에 대해서 상처받고 절망적인 감정에 치우친 이야기가 대부분이었다가 점차 하나님 안에서 자신들의 문제를 바라보 게 되었고, 결국 이전에 하나님과 함께 했던 지난날 긍정적 신앙의 경험들을 토대로 새로운 부부의 미래이야기를 품을 수 있게 되었다. 본 연구는 기독교인 부부의 이혼 위기에 대해 거킨의 이야기치료 방법론을 실제 사례에 적용하였다는 점에서 시사하는 바가 크다고 여겨진다. The purpose of this study is to apply therapeutic application of divorce crisis couple counseling based on Charles Guckin's narrative therapy to Christian divorce crisis couples. The participants in this study were middle-aged Christian couples, who were in danger of divorce due to intensifying marital conflicts after their husband's retirement. Counseling was conducted once a week for a total of eight sessions, and the researcher applied four stages of story therapy based on Guckin's story analysis. As a result of the study, the couple initially felt despair of the relationship due to the feelings of pain and anger received from the other patner, through the first stage of counseling, the couple discovered the key story that caused their divorce crisis, and through the second stage of counseling, they discovered the most important motives and reasons for causing the divorce crisis and talked to each other. Through the third stage of counseling, the couple looked at their divorce crisis from God's point of view, and they confessed that they were responsible for the divorce crisis by looking back on their spirituality in front of God. Through the fourth stage of counseling, the couple meditated deeply the Jesus Christ's Cross, realized the meaning of true love, and applied the love of the Cross to marital love. In addition, by asking for help from trusted pastors and saints in the church, it received continuous support from the religious community. The biggest influence of Guckin's story treatment confirmed through the counseling process is that the couple's story is reinterpreted when the wrong story created by the couple meets the story of God. Couple in crisis of divorce were hurt and desperate about the other person, but gradually looked at their problems in God, and eventually, they were able to embrace the future story of a new couple based on their past positive faith experiences with God. This study is considered to have great implications in that it applied Guckin's narrative therapy methodology to actual cases for the divorce crisis of Christian couples.

완화 의학에서의 평가도구

완화의학에서 환자의 평가는 치료의 의사결정과 그 결과에 이르기까지 중요한 역할을 한다. 평가도구의 사용 목적은 이를 환자 진료에 반영하여 개개인 환자와 가족들에 대한 돌봄의 질을 높이는 데 있다. 환자의 기능 상태와 통증 및 비 통증 증상을 평가하는 다양한 단일 영역 및 다차원 평가 척도들이 사용 가능하며, 삶의 질을 측정하는 다양한 도구들이 존재한다. 삶의 질이나 증상 측정에서 우월하다고 추천되는 단일 도구는 없는 실정이다. 도구마다 측정하는 시간의 틀이 다르고, 특성이 다르므로 사용 목적과 상황에 따라 적절한 도구를 선택하여야 한다. 여명 예측을 위해서는 예후 지수의 병용이 권고되는 추세이고, 웹을 기반으로 하는 예측 프로그램들도 등장하였다. 예후지수로는 최근 우리나라에서 다기관 연구를 통해 개발한 객관적 예후지수가 임상적인 여명 예측을 포함하지 않으면서 새로운 객관적인 예후 요인을 반영하여, 누구나 쉽게 사용이 가능하다. 완화의학 입문자의 경우 상대적으로 사용이 용이한 도구가 좋다. 따라서 기능의 평가에는 Eastern Cooperative Oncology Group 기능지수를 사용하고, 통증에서는 10점만점의 숫자 통증 등급(Numeric Rating Scale) 도구를 활용하면서 초기 통증 평가로는 간이 통증 조사지를 추천한다. 여러 가지 증상을 한꺼번에 측정하기 위해서는 숫자 등급으로 직접 물어보거나 혹은 M.D. Anderson 증상조사지(the Korean version of MD Anderson Symptom Inventory) 설문지를 사용하기 바란다. 삶의 질을 평가하는데에는 European Organization Research and Treatment Quality of Life Questionnaire Core 15 for Palliative Care, 예후 지수로는 객관적 예후 지수(Objective Prognostic Score)를 사용하기를 권한다. 향후 완화의학에서 평가도구의 발전방향은 국제 공동 연구의 활성화와 디지털 기기를 통한환자 보고의 전산화이고, 머지않은 앞날에 우리나라에도 이러한 경향이 도입될 것이다. The assessment of patient status in palliative medicine is essential for determining treatments and for clinical outcomes. The objective of assessment tools is to raise the quality of care for individual patients and their families. There are a number of tools available to assess pain, non-pain symptoms and quality of life. The tools are either uni-dimensional or multi-dimensional measures. Unfortunately, however, no single tool is recommended to be a superior to others in symptoms or quality of life assessment. Therefore, to select an appropriate assessment tool, one should consider the time frame and unique characteristics of tools depending on purpose and setting. The combination of prognostic index is highly recommended in prognostication, and web-based prognostic tools are available. Recently, a new objective prognostic score has been constructed through multicenter study in Korea. It does not include clinicalestimates of survival, but includes new objective prognostic factors, therefore, anyone can easily use it. For beginners in palliative medicine, relatively easy-to-use tools would be convenient. We recommend Eastern Cooperative Oncology Group performance status to assess functional status, numeric rating scale for pain assessment and the Korean version of brief pain inventory for initial pain assessment. Asking directly with numeric rating scale or the Korean version of MD Anderson Symptom Inventory would be desirable to assess various symptoms together. We think that European Organization Research and Treatment Quality of Life Questionnaire Core 15 for Palliative Care is good to assess the quality of life, while Objective Prognostic Score is convenient as prognostic index for beginners.

〈李生窺墻傳〉의 寓意性 考察

Isaenggyujangjeon(李生窺墻傳) is regarded as a work expressing allegorically the campaign of King Danjong(端宗)'s restoration. This is, also, a work adding two accidents centering around Honggeonjeok(紅巾賊)'s rebellion. It can be seen that this is composed of three stories, because another accident is added to this work in the form of an inserted poem(揷入詩). This paper is to examine the allegory of Isaenggyujangjeon centering around its backgrounds, characters, accidents, inserted poems, and so on. First, the backgrounds of this novel are Seonjuk-ri(善竹理), the Naktagyo(낙타교), Ulju(蔚州), and Bokju(復州). Seonjuk-ri, the Naktagyo, and Seonggyungwan(成均館) in Songdo(松島) are located in the same places as present, so it serves to raise the reality of historical accidents. Isaeng(李生)'s going down to Ulju, the family origin of Kim Siseop(金時習)'s mother Mrs. Jang(張氏) being from Ulju, and his mother's many relatives there - judging from these, Ulju is closely related to Kim Si -seop. Second, the characters of this novel are Isaeng, his parents, Choerang(崔?), and Sibit(시비). Of these Isaeng lives near the Naktagyo, and under his parents' order, he goes down to Ulju without a word to Choerang, and is also a passive and mean man who abandons his family and escapes alone in Honggeonjeok's rebellion. Kim Si-seop looks at Sayuksin(死六臣) tortured after the failure of King Danjong‘s restoration, but takes no measures to help them and wanders the country for the pain. It proves Isaeng to be Kim Si-seop. Choerang goes down to Bokju and is killed by thieves there. His parent before Honggeonjeok's rebellion is regarded as King Sejo(世祖) except whom King Danjong has no one to tum to, and his parents after the rebellion are able to be seen as faithful subjects as Sayuksin. Finally, Sibi serves as a foil to the atmosphere of the novel. Third, the happenings of the novel are Honggeonjeok's rebellion, and Choerang's and her parents' death. The former means that King Danjong is expelled from his throne and goes down to Yeongwol(寧越) owing to the failure of his restoration. The latter reveals allegorically the death of King Danjong's and his six royal subjects'(死六臣) by King Sejo's followers. And Isaeng's burying Choerang s parents at Ogwansan(五冠山) is compared with Kim Si-seop's burying five corpses at Noryangjin(鷺梁津). Fourth, inserted poems involve the fear caused by the failure of King Danjong's restoration and the advice of watching villainous subjects like a parrot. Six poems which Choerang gives to Isaeng show King Danjong's exile to Yeongwol by the failure of his restoration and their betrayal caused by the trial of his faithful subjects. They are written in "Yeongangcheopjangdo"(烟江疊壯圖) and "Yuhwanggomokdo"(幽皇古木圖). Poems compared to four seasons reveal their meanings clearly when each is connected with one another. A poem in the first picture scroll shows royal subjects arrested by King Sejo's followers owing to the failure of King Danjong's restoration, and a writer's helpless grief. A poem in the second picture scroll reveals Geumseongdaegun(錦城大君)'s plotting a rebellion, and he is symbolized as a virgin of the south. A poem in the third picture scroll presents Geumseongdaegun's death and the people's bitterness due to the exposure of his plotting a rebellion. Finally, a poem in the fourth picture scroll allegorises a story about King Danjong looking at crows in crowds in the night sky and dropping tears, foreseeing his death. Finally, weeping willow trees surrounding Choerang's house wall suggest Suyang(King Sejo)'s followers, and Choerang living there represents metaphorically King Danjong who demises his throne and is confined in Changdeokgung(昌德宮). In conclusion, Isaenggyujangjeon is able to be seen as a work expressing allegorically from the failure of King Danjong's restoration to his death.

빠르게 변화하는 고도의 정보사회에서 정보통신윤리 교육의 필요성과 중요성이 부각되고 있다. 그러나 현재 우리의 정보통신윤리 교육은 윤리적 사고력의 함양보다는 정보화의 역기능 문제에 대한 이해의 수준에 그치고 있으며, 대부분 일회성의 형식적, 단편적 지식전달 위주의 수업이 이루어지고 있다. 이에 본 연구는 교훈을 주입하는 정보통신윤리 교육에서 벗어나 방법 면에서 반성적 사고 전략을 활용하여 윤리적 사고력을 함양하고, 반성적 사고 증진을 위한 환경적 도구로 e-포트폴리오를 활용함으로써 학습자 주도의 학습 환경에서 지속적이고 연계성 있는 정보통신윤리 교육을 위한 수업 설계를 제안 하고자 한다.

예방접종. 요즘 예방접종에 대한 권유를 많이 받았을 것이다. 또한 때가 때이니 만큼 일찌감치 예방접종을 시행한 사람들도 있을지 모르겠다. 신종플루 덕분에(?) 예방접종에 대한 인식이 높아지면서 어린이에게만 해당된다고 생각했던 예방접종의 중요성이 이제는 성인들 사이에서도 통하고 있다. 이 시점에서 고대 구로병원 감염내과 정희진 교수의 자문을 받아 감염의 고위험군인 당뇨병환자에게 꼭 필요한 예방접종을 강조하고자 한다.

인간이 두 발로 서고 두 팔을 사용함으로써 만물의 영장이 되었지만, 그렇게 직립 보행한 덕분에 우리 몸의 허리, 즉 척추는 막대한 체중을 지탱해야 하는 운명에 놓여 과도한 역학적 부담을 평생 갖게 되었다. 실제로 우리 나라 인구의 80% 이상이 요통을 경험하며, 최근에는 20대의 젊은 사람들에게도 많이 발생하고 있다고 하니, 이제는 요통이 남녀노소 구분 없이 광범위하게 나타나 사회적인 문제가 아닐 수 없다. 그러나 다행히도 요통 환자의 90% 이상은 바른 자세유지, 바른 들기방법, 정상체중의 회복, 규칙적인 운동만으로도 요통의 예방과 치료가 가능하다고 한다. 이에 요통에 대해 알아보고 철저히 예방해 보도록 하자.
