Tissue adhesives have gained popularity for quicker and painless closure of lacerations. The use of tissue glue is currently popular for the closure of superficial lacerations, especially in children. Histoacryl $Blue^{(R)}$(2-N-butylcyanoacrylate) is a topical wound closure that precludes the need for foreign bodies to close wounds. The purpose of this study was to compare the applications of Histoacryl $Blue^{(R)}$(HAB) and conventional suture, regarding cosmetic outcome. To compare the short term and long term results of various repair methods, we designed the prospective, randomized, blind study. Patients with laceration undergoing repair were randomly allocated to conventional suture, subcutaneous suture plus HAB, and HAB only groups. The exclusion criterions were large wound that require large tension for repair or avulsion wound. An independent, blinded observer assessed cosmetic result at 7-10 days after repair and 3-9 months postoperatively. Physician's satisfaction with wound appearance was recorded on 100 mm Visual Analogue Scale(VAS)(0=worst, 100=best). The difference in VAS score between conventional suture method and subcutaneous suture plus HAB methods were not significant. Tissue glue being easy to use with no complications and still resulting in equivalent cosmetic outcomes has several benefits. Especially in the case of children, the wound closure with Histoacryl $Blue^{(R)}$ could be a good alternative for repair of laceration in emergency room.
Fabrication of TiO<sub>2</sub> Nanowires Using Vapor-Liquid-Solid Process for the Osseointegration

In order to improve osseointegration for biomedical implants, it is crucial to understand the interactions between nanostructured surfaces and cells. In this study, $TiO_2$ nanowires were prepared via Vapor-Liquid-Solid (VLS) process with Sn as a metal catalyst in the tube furnace. Nanowires were grown with $N_2$ heat treatment with their size controlled by the agglomeration of Sn layers in various thicknesses. MC3T3-E1 (pre-osteoblast) were cultured on the $TiO_2$ nanowires for a week. 진료데이터는 진료를 보면서 축척된 데이터로서 다양한 병명들에 대한 의사들의 진료행위를 추적해볼 수 있는 유용한 정보가 될 수 있으며, 진료에 재활용함으로써 환자들에 대한 진료행위를 표준화하는데 사용될 수 있다. 본 연구에서는 다양한 상황에서 환자를 진료한 근거자료인 진료데이터를 이용하여 병원에서 활용 가능한 임상진료데이터를 추출하기 위한 방법에 대한 연구를 진행하였다.

The world's first face transplantation was performed in France, in 2005. Since then, 21 cases of face transplantation have been performed. Face transplantation is one of the most prominent part of composite tissue allotransplantation (CTA) along with hand transplantation. Since these fields are not deal with life-saving organs, there are many arguments about immunosuppression therapy. Recent paradigm of face transplantation shows that surgical ranges are expanded from partial face transplantation to full face transplantation. Most immunosuppression protocols are triple therapy, which consists of tacrolimus (FK-506), mycophenolate mofetil and prednisolone. Anatomical researches, immunosuppression, and immunotolerance take great parts in the researches of CTA. The medical fields directly related to face transplantation are microsurgery, immunology, and transplantation. Nowadays, each field is performed widely. Therefore people, even medical teams think face transplantation could be easily realized, sooner or later. But there are lots of things that should be prepared for not only practice and immunosuppression therapy but also for the cooperation with relevant fields. That's the reason why only 21 cases of face transplantation have been done, while more than 70 cases of hand transplantation have been done in the past years. Especially in Korea, brain death patients are not enough even for organ transplantation and furthermore there are some troubles in taking part in the society of transplantation. Face transplantation has lots of problems concerning variable medical fields, administration, society, ethics, and laws. Therefore, for the realization of face transplantation in Korea, not only medical skills but also political powers are needed.

This study was conducted to evaluate the quality in medical records by analyzing its completeness through setting up the level of record on the patient`s past history and through examining the actual medial records. Targeting the information on the patient`s past history in interns` records, residents` records and nurses` records toward 403 inpatients who were admitted first in 2004 at an university hospital due to stomach cancer. We analyzed whether the charts were recorded or not, recording level, the satisfaction with the expectant level of the records in the hospital targeted for a research and the level of agreement. The results were as follows; first, as for the rate of recording those each items, they were high in the chief complaint & present illness and the past illness history. Depending on the group of recorders, the recording rate showed big difference by items. Second, as a result of measuring the level after dividing the recording level of items for the patient`s past history from Level 1 to Level 4 by each item, the admission history, the past illness history, and the family history were about Level 3, and the smoking history, the medication history, the chief complaint & present illness, the drinking history and allergy were about Level 2. In the admission department, it was excellent in the interns` records for the medical department. Third, as a result of its satisfactory level by comparing the expectant level of a record and the actual record by item in information on the patient`s past history, which was expected by the medical-record committee members of the hospital targeted for a study. And forth, we analyzed the level of agreement with Kappa score in the level of ``Yes`` or ``None`` related to the corresponding matter in Level 1, in terms of information on the past history in the intern`s record, the resident`s record, and the nurse`s record. The level of agreement in the resident`s record & the nurse`s record, and in the intern`s record & the resident`s record was from excellent to a little good. There were differences in the level of completeness and in reliability for the information on the past history by the recorder group or by the admission department. The encounter process that was performed by the admission department or the recorder group, indicated the result that was directly reflected on the quality of medical records, thus it was required further study about the medical record documentation process and quality of care. The items that showed the high recording rate quantitatively were rather low, consequently we`d should develop the tool for the qualitative inspection and evaluate the medical records further. And the items were needed to be detailed in the record level were rather low, and hence there needed to be a documentation guideline and education by the clinical departments.

Silicone tube를 이용한 누소관 재건술의 성적

Traumatic laceration near the medial canthal area invariably results in debilitating epiphora due to transection of lacrimal canaliculi. Many treatment methods have been developed, but they often deem failure because of the difficlty in isolation of proximal segment of lacerated canaliculi and the development of scar adhesion. The silicaone tube is the most widely used material in the canalicular reconstrucion. Compared to nylon, silk, plastic and polyethylene, the silicone is relatively inert and flexible, therefore, its use is infrequently associated with corneal irritation, iatrogenic injury of canaliculi, and erosion of punctum. The authors present the clinical experience of canalicular reconstruction using silicone tubes and Quickert- Dryden method : 25 cases of canalicular laceration were treated with the silicone tubes and the pigtail probe, and 27 cases with Quickert-Dryden method. The overall result was gratifying.
