        자궁경부 세포진에서의 미확정 편평세포, 미확정 선세포 및 양성 내막세포 : 조직학적 상관성과 임상적 의의 Histologic Correlations and Clinical Significance

        Objective: The purpose of this study is to evaluate the histologic correlations and the clinical significance among patients with atypical squamous cells of undetermined significance (ASCUS), atypical glandular cells of undetermined significance(AGUS) and benign endometrial cells identified on cervical Pap smear screening. Materials & Methods: The computerized files of the Department of Pathology at Samsung Cheil Hospital were searched from 1991 to 1997 to evaluate the annual statistics of cytologic diagnoses including normal/benign, ASCUS, AGUS, low-grade squamous intraepithelial lesion (LSIL), high-grade squamous intraepithelial lesion(HSIL) and cancer classified by the Bethesda System(TBS). Cytohistologic correlations on follow-up were separately analysed in ASCUS(190 cases), AGUS(268 cases) and benign endometrial cells(169 cases), respectively. Additionally, post-menopausal squamous atypia(83 cases) were also included in this study. TBS terminology was used in both cytologic and histologic diagnoses. Results: During 7-year period (1991-1997), 447,049 cervicovaginal smears were evaluated. The median rate of abnormal cytology was 4.4%, with 2.1% of ASCUS, 2.06% of squamous intraepithelial lesion(SIL), and 0.08% of AGUS. The median ratio of ASCUS versus SIL was 1.24. Specimen adequacy was evaluated on 47,525 cases, of which categories of “satisfactory for evaluation but limited by” and “unsatisfactory for evaluation” were 28.3% and 0.03%, res-pectively. Follow-up of 190 patients with ASCUS cytology showed 30%(57 cases) with SIL on biopsy; 18%(35cases) with LSIL, 11%(21cases) with HSIL, and 1%(1case) with microinvasive squamous cell carcinoma. On histologic examination, 77%(37/48cases) with ASCUS favoring SIL revealed SIL in contrast to 14%(20/142cases) with ASCUS favoring reactive change, which is statistically significant.(Chi-Square test, P <0.0001). Of 83 cases of post-menopausal squamous atypia(PSA), smears with LSIL showed 34.9%(15/43cases) with LSIL on biopsy. 268 patients with AGUS smears had 25%(67cases) with clinically significant cervical or endometrial lesions on histologic examinations. Among 17.9%(48cases) with cervical lesions, squamous abnormalities were 10.5%(28cases); including 1.5%(4cases) with LSIL and 9.0%(24cases) with HSIL. Glandular lesions in cervix were 7.5%(20cases); 3.0%(8cases) of glandular atypia or dysplasia, 1.9%(5cases of adenocarcinoma in situ, 1.1%(3cases) of microinvasive adenocarcinom and 1.5%(4cases) of adenocarcinoma. Of 7.1%(19cases) of endometrial lesions, 2.2%(6cases) was endometrial hy-perplasia, 4.1%(11cases) endometrial carcinoma, 0.4%(1case) MMMT and 0.4%(1case) metastatic adenocarcinoma from stomach were verified. The pathologies of 169 cases with benign endometrial cells shed in cervicovaginal smears were confirmed to be endometrial polyp(8.3%), endometrial hyperplasia(4.1%) and endometrial carcinoma(5.9%). Conclusion: The results of this study indicates that clinicians should communicate with pathologists for proper management of abnormal cytology. Further evaluation and decision of management should be made based on input from pathologists as well as on clinical setting and professional guidelines.$quot;

      • 한국인 심부전증 환자 심근에서의 인형 거대 세포 바이러스 감염

        Objectives: In order to evaluated the prevalence and the site of infection of cytomegalovirus in terminally failing heart, cytomegaloviral DNA was detected in the explanted hearts of transplantation recipients. Methods: DNA extractions were performed from explanted failing hearts(N=22) and normal hearts(N=5) and polymerase cain reactions(PCRs) were done for detection of late gene sequence coding pp150 glycoprotein. The products were confirmed by electrophoresis on the 1% agarose gel. n order to improve the detectability of cytomegaloviral genome, nested PCRs were executed with the primers designed for the original 607 bp products. In situ PCRs also were done with the samples which were confirmed as positive for CMV viral genome by nested PCRs. Results: All patients had IgG anti-cytomegalovirus antibody and did not have IgM anti-cytomegalovirus antibody. Cytomegaloviral genomes in myocardium were detected by polymerase chain reaction. The 607bp products by PCRs were found in both explanted failing heart(3 cases/22. 13.5%) and normal hearts( 1 case/5, 20.0%). In nested PCRs, 186bp products were found in both failing hearts(LV 4/22, LA 3/2, RV 4/22, RA 0/17) and normal hearts(LV 2/5, LA 1/4, RV 2/5, RA 2/5). These was no significant change in positivity of cytomegaloviral DNA genome between failing and normal hearts. Total positivity of cytomegaloviral genome in explanted hearts was 44.4% according to the nested PCR results. The positivity of cytomegaloviral DNA by n situ PCR was 33.3%(4/12), and the site of positive reaction was the nuclei of the myocardial cells. Conclusion: Cytomegalovirus was rarely observed in explanted hearts of terminal heart failure and nested PCR could enhance the sensitivity of cytomegaloviral genome detection. The result of the in situ PCR showed the site of the cytomegaloviral infection was nuclei of the myocardial cells. Cytomegalovirus, however, might have no direct causal relationship in the development of terminal heart failure.

        자궁경부암 세포주들에서 리포좀을 이용한 종양억제 유전자 [pRcCMVp53/liposome]의 직접 이입법에 의한 세포 성장 억제 효과

        목적: 자궁경부암 유전자 치료를 위하여 자궁경부암 세포주들에 리포좀을 이용한 종양억제 유전자[wild-type p53 gene]의 직접 이입을 통하여 세포성장 억제효과를 보고자 하였다. 방법: 자궁암세포주들 [CaSki, SiHa, HeLa, HeLaS3, C33A, HT3]을 12 well에 104, 96 well에 5×103의 수로 triplicate를 형성한 뒤 대조군, pRcCMVp53, lipofectin, pRcCMV/lipofectin, pRcCMVp53/lipofetin이 함유되있는 것 을 각기 12 well에 20 ㎕씩 96 well에는 10 ㎕씩을 투여한 다음 7일간 배양하였다. 세포수 측정, ELISA [neutral red] assay, MTT등을 이용하여 세포성장과 cell viability를 측정하고 이를 one-way ANOVA F-test, multiple comparison Tukey method에 의해 날짜별 서로 군의 차이를 비교 하였다. 결과: p53 유전자에 의한 자궁경부암 세포주의 성장억제를 확인하기 위하여 세포 수 측정, ELISA assay, MTT assay를 시행하였으며 각각의 assay에 대한 측정 마지막날인 6일째를 기준으로 정상대조군 세포와 비교한 결과 세 포수 측정에서는 CaSki[89%], SiHa[59.2%], HeLa[86%], HeLaS3[78%], C33A[91%] 그리고 HT3[74%] 성장억제를 나타냈다. 또한 Neutral red를 이용한 세포 viability assay 에서는 CaSki[70%], SiHa[93%], HeLa[73%], HeLaS3[50%], C33A[67%] 그리고 HT3[52%]에서 는 52%의 세포감소를 보여주었다. MTT assay를 통한 각각의 자궁경부암 세포주의 세포성장억제 결과는 CaSki[38%], SiHa[72%], HeLa[53%], HeLaS3[28%], C33A[33%]그리고 HT3[53%]였다. 결론: 자궁경부암 세포주에 직접 이입법을 통한 유전자 전달 후 세포성장 억제는 매우 유익하게 감소되었으며 이와같은 결과는 향후 유전자 직접 이입법에 의한 자궁경부암 환자에 있어서 유전자 치료를 실시할 수 있는 새로운 장으 로 생각이 되어진다. Purpose: We investigated cell growth inhibitory effect of the wild type p53 gene into the cervical cancer cells via the recombinant p53 plasmid, pRcCMVp53 with lipofectin. Method: Inhibition of the growth of cervical cells as determined by a cell count assay. The cells were inoculated at density of 104 cells/well in each 12 well plate, 24 hrs before infection. At each indicated point, cells in three wells on each well plate were trypsinized and counted. The mean cell counts of triplicated well after transfection at day 1~6 was checked. Inhibition of the growth of cervical cells were checked by ELISA assay, MTT assay. The cells inoculated at densities of 5×103/well in each 96 plate 24 hrs before infection. At each indicated point cells of three wells were transfected for 6 days, harvested and counted by liquid scintillation counter the mean cpm per triplicate wells were plated for ELISA and MTT assay. One-way ANOVA F-test and multiple comparison Tukey method was used for statistical analysis. Result: Inhibition of the growth of cervical cells in cell count showed CaSki[89%], SiHa[59.2%], HeLa[86%], HeLaS3[78%], C33A[91%] and HT3[74%]. ELISA assay showed CaSki[70%], SiHa[53%], HeLa[73%], HeLaS3[50%], C33A[67%] and HT3[73%]. MTT assay showed HeLa[33%], HeLaS3[28%], SiHa[75%], CaSki[38%], C33A[33%] and HT3[53%]. Conclusion: These results indicate that transfection of cervical cancer cells with the wild type p53 gene via pRcCMVp53 with lipofectin is a potential novel approach to the gene therapy of cervical cancer.

        자궁경부암 세포주들에서 Transforming Growth Factor-β1의 세포성장 및 c-myc암유전자 발현 억제효과

        Transforming growth factor-β1(TGF-β1) is known to be a potent growth inhibitor for many cell types, including most epithelial cells. In skin keratinocytes, TGF-β1 has been shown to inhibit growth and to rapidly reduce c-myc expression. However, the molecular mechanism of TGF-β1 action on cell growth of cervical carcinoma has not yet been elucidated. We thus assessed the effect of TGF-β1 on the growth of cervical carcinoma cell lines. Two cervical squamous carcinoma cell lines were incubated with varying concentration of TGF-β 1, and growth inhibition was evaluated with tetrazolium-based colorimetric assay. After culturing in concentrations of 0.1~10 ng/ml in both cell lines. Northern blot analysis revealed c-myc mRNA expression was suppressed by 10 ng/ml of TGF-β 1 following 6-hour of treatment in both cell lines. These results suggest that TGF-β1 inhibits the growth of cervical carcinoma cells by suppressing c-myc oncogene expression.

        Two Cases of Leptomenigeal : Case Report

        외상성 두개골 선상골절후 발생될 수 있는 leptomeningeal cyst는 그 발생이 희소하고 90%이상이 3세이하의 어린애에서 발생된다고 한다. Leptomeningeal cyst의 발생기전은 외상후 선상골절이 있는 부위로 경막이 함몰되면 이 부위로 뇌척수액의 박동에 의해 두개골이 있는 부위로 경막이 함몰되면 이 부위로 뇌척수액의 박동에 의해 두개골의 미란이 생겨서 cyst가 커진다고 알려져 있다. 저자들은 드물게 17세 남아와 5세 여자에 발생된 leptomeningeal cyst 2례를 수술치험 하였기에 보고하고 두통과 두개골 결손등은 경막성형 및 두개골 성형술로 치료할 수 있었으나 5세 여아에서 보여 주었던 전간증 및 편부전마비는 호전되지 않았다.
