        Due to overuse of commercial network systems such as bluetooth and WI-FI, the problems of frequency interruption and line-crossing may arise. For this reason, wireless communication frequency ISM 2.4㎓, a recently adapted concept in Korea which is employeed by RF-CBTC system, is predicted not to have guarantee for outstanding and continuous performance. Therefore in this study, considering these problems, 5㎓ capacity wireless Lans with international standard 802.11a/b/g applied were installed in the underground urban transit areas and it was proved that these lans exceeded the performance level of 2.4㎓ with international standard 802.11b which is being introduced in the nation. In addition, it was verified through carrying out an application test that the communication condition was stable in a running train with high speed in the tunnel.

        Eucheuma cottonii 추출물에 의한 인체 골육종암 Saos-2 세포의 자가사멸 유도

        본 연구에서는 인간 골육종암세포주인 Saos-2 세포를 이용하여 Eucheuma cottonii 추출물(Extract of Eucheuma cottonii, EE)의 항암 활성 및 분자적 작용기전을 분석하였다. 먼저 EE가 세포증식에 미치는 영향을 WST-1<SUP>®</SUP> assay를 통해 확인한 결과 EE는 인간 위암세포 AGS, 인간 간암세포 SK-Hep 1, 인간 뇌교모세포종 U87MG, 인간 정상 신장세포 HEK-293의 생존율에는 영향을 미치지 않고 Saos-2 세포주의 생존율만을 농도의존적으로 감소시킴을 확인 하였다. 또한 처리 농도가 증가함에 따라 Saos-2 세포의 외형적 변화가 나타남을 도립현미경을 통해 확인할 수 있었다. 그리고 DAPI staining을 통해 apoptosis classical hall marker라고 할 수 있는 DNA fragmentation이 EE 처리 농도 의존적으로 나타남을 관찰할 수 있었다. 이를 토대로 EE가 Saos-2 세포에서 세포 내의 어떠한 기작을 통해 apoptosis를 유도하는지 Western blot analysis를 통해 확인한 결과, Fas-Associated Death Domain(FADD)에 의한 caspase cascade signal pathway 발현이 증가하였고, apoptosis의 key protein 이라고 할 수 있는 cleaved caspase-3와 하위 인자인 cleaved PARP가 증가 함을 확인할 수 있었다. 이와 관련된 분자적 기전 분석을 위한 immunofluorescence staining과 Flow cytometry analysis를 추가로 수행한 결과, EE 처리시 caspase cascade signal pathway의 시발점인 FAS와 cleaved caspase-3의 발현증가가 실제 세포 내에서 일어남을 관찰 할 수 있었으며 apoptosis 유발군인 sub G1기가 증가 함을 알 수 있었다. 이상의 결과를 통해 EE는 인간 골육종암세포에서 FADD에 의한 caspase cascade signal pathway 발현증가가 유도되어 apoptosis를 유발시킨다는 것을 증명하였으며 새로운 골육종암 치료제로서의 개발 가능성과 기전연구를 위한 중요한 기초자료가 될 수 있음을 시사한다. Osteosarcoma (OS) is the most common and malignant bone tumors. Although many types of resection surgery and experimental agents were developed, median survival and clinical prognosis are poorly investigated. Recently, several researches have reported that Eucheuma cottonii has potent as protective effects of coal dust-induced lung damage via inhibition of malondialdehyde (MDA) and oxidative stress in bronchoalveolar lavage fluids (BALF). However, anti-cancer effects and specific molecular mechanism of extract from Eucheuma cottonii (EE) has not been clearly studied yet. This study evaluated that anti-cancer potential of EE in human osteosarcoma Saos-2 cells. EE indicated cytotoxicity on Saos-2 cells in a dose-dependent manner. Morphological degradation and nucleic condensation were also observed under the EE treatment. However, it did not significantly affect on non-cancerous kidney HEK-293 cells under the same concentration which is shown cytotoxicity on Saos-2 cells. The phosphorylation of Fas-Associated Death Domain (FADD) and expression of cleaved caspase-8, -7 and -3 were upregulated in a dose-dependent manner. In immunofluorescence staining, expression level of Fas and cleaved PARP were upregulated by EE treatment. Furthermore, treatment of EE induces upregulation of sub G1 phase by flow cytometry analysis. The results demonstrated that EE has a therapeutic potential against osteosarcoma via FADD mediated caspase cascade apoptosis signal pathway.

      • 대칭키 기반 키 관리 시스템의 구현상의 문제점 및 해결방안

        네트워크 보안 시스템을 설계하기 위해서는 가장 기본적으로 네트워크의 연결 및 분배 역할을 하는 Route, Gateway 등의 노드(Node)들 사이의 트래픽을 암호함으로써 각 노드간 전송되는 정보들을 보호하게끔 시스템을 구성하는 것이다. 이러한 네트웨크 보안 시스템에서는 많은 양의 트래픽을 암호화 하기 위해 대칭키 암호 기법을 사용하고 있으며, 이러한 대칭키 암호 기법에 사용되는 비밀키를 각 노드들에게 직접 수동적으로 주입을 하거나 또는 공개키 방식을 이용하여 자동적으로 비밀키를 자체적으로 생성하여 공유하는 방식을 사용한다. 본 논문에서는 일반적인 대칭키 기반 키 관리 시스템의 수동적인 키 분배 방식을 자동화함으로써 보안 시스템의 효율성과 확장성을 제공할 수 있는 자동화된 대칭키 기반의 키 관리 시스템에서 키 분배 기능의 온라인화로 인해 발생가능한 문제점들을 파악하고, 이에 대한 해결방안을 제시한다.

        A Multi-Center, Phase II Clinical Trial of PadexolTM (Paclitaxel) and Cisplatin for Patients Suffering with Advanced Gastric Cancer

        Purpose: We conducted a multi-center, phase II trial to evaluate the efficacy and safety of using Padexol (a paclitaxel formulation) combined with cisplatin for thepatients suffering with advanced gastric adenocarcinoma. Materials and Methods: 39 patients (median age: 60 years; males: 90%) who were diagnosed with advanced gastric cancer were enrolled from 5 hospitals. Padexol175 mg/m2 was administered as a 3-hr infusion, and this was followed by cisplatin 75 mg/m2 as an intravenous infusion on day 1, once every 3 weeks.Results: Out of these 39 patients, 34 patients were assessable for treatment efficacy and 39 patients were assessable for the toxicity. Objective responses occurred in 13 patients (33%); 1 patient (3%) had a complete response and 12 patients (31%) had partial responses. 6 patients (15%) achieved a stable disease state. The median duration of response was 7.1 months, and the median time to progression and the overall survival were 4.8 months and 6.7 months, respectively. The major treatment-related adverse events were hematologic toxicity, including WHO grade 3 or 4 neutropenia in 13 patients (33%). However, febrile neutropenia occurred in only 1 patient and the non-hematologic toxicity was usually mild. Conclusion: The combination of Padexol and cisplatin was found to be active and it seems to be a relatively well-tolerated regimen for the treatment of advancedgastric cancer.         Outcomes of Wedge Wing in the Lag Screw for Unstable Intertrochanteric Fractures in Elderly Patients

        Purpose: To evaluate clinical-functional and radiologic outcomes of elderly patients with an unstable intertrochanteric femur fracture treated with a wedge wing in the lag screw. Materials and Methods: Forty-eight patients treated with the Dyna Locking Trochanteric nail (DLT nail) to resolve an unstable intertrochanteric femur fracture were reviewed retrospectively. Based on AO/OTA classification, Fracture 31-A2 (34 cases) and 31-A3 (14 cases) were included in the analysis. We measured the femoral neck-shaft angle, tip-apex distance (TAD), Cleveland index, sliding distance of the lag screw, and time to the fracture union. The Harris Hip Score and Paker and Palmer’s mobility score for clinical evaluation were used. Results: The mean follow-up period was 21.4 months (range, 12-34 months). The postoperative state of reduction was good in 28 cases and acceptable in 20 cases. The mean TAD was 20.5 mm. The position of the lag screw was center-center in 30 cases and center-inferior in 18 cases. The mean sliding distance of the lag screw was 3.4 mm at the last follow-up. The mean union time was 4.5 months. Two cases had complications which included a cut-out (1 case) and non-union (1 case). The mean Harris Hip Score was 86.5±8.3 (range, 76-90). Walking ability in 34 of the cases (70.8%) at last follow-up was similar to that prior to fracture. Conclusion: Functional and radiological outcomes are satisfactory using the DLT nail in the treatment of elderly patients with unstable intertrochanteric fractures; however, wedge wing in the lag screw does not prevent implant-related complications.

        Usefulness of Myocardial Longitudinal Strain in Prediction of Heart Failure in Patients with Successfully Reperfused Anterior Wall ST-segment Elevation Myocardial Infarction

        Background and Objectives: Acute myocardial infarction-related heart failure (HF) is associated with poor outcome. This study was designed to investigate the usefulness of global longitudinal strain (GLS), global circumferential strain (GCS) and mean longitudinal strain of left anterior descending artery territory (LSant) measured by 2-dimensional speckle tracking echocardiography (2D STE) in prediction of acute anterior wall ST-segment elevation myocardial infarction (ant-STEMI)-related HF. Methods: A total of 171 patients with ant-STEMI who underwent successful primary coronary intervention and had available 2D STE data were enrolled. Patients were divided into 3 groups: in-hospital HF, post-discharge HF, and no-HF groups. Results: In-hospital and post-discharge HF developed in 39 (22.8%) and 13 (7.6%) of patients, respectively and 113 patients (69.6%) remained without HF. Multivariate analysis showed that GLS was the only factor significantly associated with the development of in-hospital HF. For post-discharge HF, LSant was the only independent predictor. Other echocardiographic or laboratory parameters did not show independent association with the development of ant-STEMI-related HF. Conclusions: GLS is a powerful echocardiographic parameter related to development of in-hospital HF and LSant was significantly associated with post-discharge HF in patients with successfully reperfused ant-STEMI.

        증례 : 혈액종양 ; 제1형 신경섬유종증 환자에서 발생한 위장관 기질종양 1예

        신경섬유종증이 있는 환자에서 위장관 기질종양은 일반인보다 잘 발생하며 이때 KIT 유전자 변이를 나타내는 경우는 드물다. 저자들은 내원 30년 전 제1형 신경섬유종증을 진단받은 환자가 7일간의 심와부 통증으로 내원하여 시행한 복부 전산화단층촬영에서 십이지장부위의 위장관 기질종양을 확인하였다. 수술 후 KIT 유전자 변이 음성인 위장관 기질종양을 진단하였으나 재발의 위험이 큰 고위험군으로 수술 후 보조요법으로 imatinib을 사용하였고 이후 4년간 재발 소견 없이 추적관찰 중인 증례를 경험하여 문헌고찰과 함께 보고하는 바이다. Neurofibromatosis type 1 (NF1) is a genetic disease characterized by neoplastic and non-neoplastic disorders involving tissues of neuroectodermal and mesenchymal origin. NF1 is caused by mutations in the NF1 gene, which is found on chromosome 17q11.2. Patients with NF1 are at increased risk of developing soft tissue sarcomas that arise within the stromal compartment of the gastrointestinal tract, termed gastrointestinal stromal tumors (GISTs). GISTs associated with neurofibromatosis differ from sporadic GISTs, particularly with respect to their lower response rate to imatinib. We recently experienced a case involving a 45-year-old man with NF1 who was admitted to the hospital with epigastric pain and vomiting. Abdominal computed tomography revealed a duodenal GIST with pancreatic invasion. He had a base substitution mutation involving replacement of 2041 cytosine with thymine. He was treated successfully with a surgical operation and adjuvant imatinib therapy. (Korean J Med 2013;85:535-539
