      • 통풍성 관절염의 임상적 고찰

        통풍은 Purine 대사의 이상으로 발생하는 질환으로, 고뇨산혈증인 사람의 전부가 통풍으로 발현되지 않고 증상의 출현 양상이 다양하여 진단과 치료에 주의가 필요하며, 조기에 적절한 조치를 한다면 충분히 조절이 가능한 질환이다. 본대학 내과학교실에서는 통풍으로 치료한 32명의 환자에서 임상양상, 병력과 검사소견을 분석하여 다음과 같음 결과를 얻었다. 1. 32명 모두 남자이며, 최초 발병 시기는 24세에서 72세로 평균 43.8±11.9세이었으며, 30대에서 40대까지가 19명(59.4%)으로 대부분을 차지하였다. 내원 당시 나이는 27세에서 75세까지로 평균 52.3±10.4세였으며, 내원시 까지 평균 유병기간은 8.5±6.8년으로 나타났다. 2. 동반 질환으로는 고지혈증 12례, 신장질환 10례, 고혈압 12례, 비만 8례, 당뇨 2례 등이었다. 3. 이환된 관절은 단관절 침범이 19례(59.4%), 다관절 침범이 13례 이었으며, 최초 이한된 관절은 족무지 중족골지골 관절로 19례(59.4%)로 가장 많았고, 통풍 결절은 20례(62.5%)에서 관찰되었으며, 유병기간이 10년 이상된 12례중 11례 (91.7%)에서 결절이 관찰되었다. 4. 평균 혈중 요산치는 9.17±1.75 ㎎/dl이었으며, 8.0 ㎎/dl에서 9.9 ㎎/dl 사이가 19명으로 전체의 59.4%를 차지하였다. Objective: The gout is a heterogeneous group of diseases resulting from tissue deposition of monosodium urate or uric acid crystals from extracellular fluids supersaturated with respect to this end product of human purine metabolism. The clinical manifestations are such as hyperuricemia, gouty arthritis, gouty nephropathy, uric acid nephrolithiasis. We analyze of clinical manifestations and associated factors in gout. Method: We have reviewed the medical records, radiologic findings and clinical results of thirty-two patients admitted at our department from April 1996 to July 1997. Result: 1) All patients were male. The mean age at initial attack was 43.8 years old, ranging from 24 to 72 years old. 2) The mean level of serum uric acid was 9.17mg/dl on admission. 3) The first metatarsopharyngeal joint was involved in 19 cases (59.4%). Tophus was observed in 20 cases (62.5%). 4) Hyperuricemia was associated with hypertension, obesity, nephrolithiasis and hyperlipidemia.

      • 약국에서 처방된 상기도 감염 치료제에 포함된 항균제

        배경 : 상기도 감염은 국내에서 발생하는 가장 흔한 감염증이어서 이를 치료하기 위해 투여되는 약제는 부작용, 경제적 손실, 항균제 내성 유발에 큰 영향을 미칠 수 있다. 또한 국내 실정 상 약국에서 항균제나 부신피질호르몬을 포함해서 모든 약에 대해 처방이 가능하여, 감기약의 내용이 선진국과는 다를 가능성이 있으나 이에 대한 국내 조사가 적었다. 약국에서 처방되는 감기약에 항균제가 얼마나 들어 있는가를 조사하고 지역별 빈도 차이가 있는가를 알기 위해 이번 연구를 시행하였다. 재료 및 방벙 : 1996년 11월부터 1997년 2월까지 서울, 춘천, 천안의 개원 약국을 대상으로 의과대학 학생과 간호대 학생들이 직접 방문하여 조사하였다. 비염(발열이 동반되지 않은 1일간의 콧물)과 세균감염성 상기도 증상(기침과 발열)을 호소하여 감기약을 처방 받았고 이를 전문 약사가 동정하고 다시 전화로 확인하였다. 결과 : 서울 60개(감기 증상으로 28개 약국, 세균감염 상기도 증상으로 32개 약국), 천안 30개, 춘천 35개 약국을 조사했다. 처방에 포함된 약제의 평균 수는 평균 3.2개(SD 1.88)였고, 지방 도시의 처방 약재 수가 서울보다 많았다(p<0.001). 24개(19.2%) 처방에서 항균제가 포함되었고 지역 차이가 커서 춘천(31.4%)과 천안(33.3%)에서 서울(5.0%)보다 높았다(p<0.001). 약제 수가 많을수록 항균제가 포함될 가능성이 높았다(p=0.001). 항균제 종류는 amoxicillin(16), tetracycline/doxycycline(5), chloramphenicol(2), erythromycin(1)이었다. 부신피질호르몬은 8개 처방(6.4%)에서 확인하였다. 사용한 해열제는 mefenamic acid(8), acetaminophen(8), ibuprofen(6), piroxacim(3), 아스피린(2)의 순이었다. 결론 : 국내 감기약 처방에는 항균제가 높은 빈도로 들어 있고 치명적 부작용의 발생이 가능한 chloramphenicol이 포함되었다. 항균제, 부신피질호르몬, 독성이 강한 비스테로이드성 항염증제의 사용을 줄이기 위한 대책이 필요하다. Background : Upper respiratory infections (URIs) are the most common infection in developed countries and are a major cause of prescribing medications, including antibiotics Its magnitude result in frequent. sometimes serious, side effects. considerable economic burden and antimicrobial resistance In Korea, pharmacists can prescribe without any restriction even with antibiotics and glucocorticoids, and therefore, prescriptions for URI will be different from those of other developed countries The objective of this study is to investigate the rate of antibiotic prescription by pharmacists and differences in prescribing patterns by different areas. Methods : From November 1996 to February 1997, me were prescribed medications for relief of two simulating illnesses, common cold (rhinorrhea of one day's duration without fever) or possible pneumonia (fever and cough) by visiting drug stores in thee cities(Seoul, Chonan, Chunchun) Contents of prescriptions were identified by a pharmacist in a University Hospital, which were confirmed by telephone to the drugstores. Results : One hundred twenty-five drugstores m three cities were surveyed. The mean number of medication was 32(SD 1.88), and was significantly different between Seoul and the other two cities ($<0 001) Twenty-four (19.2%) prescription contained antimicrobial agents and there was also significant geographic difference; 33.3% in Chonan, 31.4% in Chunchun and 5.0% in Seoul (p<0.001). Another factor predicting the presence of antibiotics was the number of mediations, i.e., antibiotics were not included in 1-2 medications prescribed, but 50.0% included antibiotics in 7-9 medications prescribed(p=0.001). List of antibiotics was amoxicillin (16), tetracycline/doxycycline (5), chloramphenicol (2), and erythromycin (1). Glucocorticoids(prednisolon, betamethasone) were contained in 8 prescriptions and various antipyretics were included. Conclusion : Antimicrobial agents were contained in the prescriptions for URI at, high rates with occasional chloramphenicol. Considering the benign self-limited nature of URL, measures to reduce the prescription of antibiotics, glucocorticoids and relatively toxic nonsteroidal anti-inflammatory drugs are needed.

        AI합금의 고속가공에서 치수오차와 표면정도 추이 고찰

        High speed machining aims to raise the productivity and efficiency by making more precise and higher value-added products than any other machining method by means of the high speediness of spindle and feed drive system. The purpose of this study is to investigate the effects of the run-out of endmill on the dimension precision of workpiece and to obtain the fundamental data on high speed machining, which is available by machining the side of Al-alloy with solid carbide endmills in high speed machining center and by measuring dimensions and surface roughness. From the results of experimentations followings are obtained ; if spindle speed is ultrahigh in conditions that radial depth of cut and feed per tooth are very small, highly precise and accurate products are to be made efficiently with high feed rate, and so we can raise productivity.

        원통가공시 가공형상 모델링의 특성연구

        In this paper, we have discussed on the characteristics and modeling of machined geometry which was established for the case of round shape machining, also the effects of externally machined profile are analyzed and its modeling realia-biliy was verified by the experiments of roundness testing, especially in lathe operation. In this study, we established a harmonic geometric model with the parameter harmonic function. In general, we can calculate the theoretical roundness profile with an arbitrary multilobe parameter. But in real experiments, only 2-5 lobe profile was frequently measured. the most frequently measured ones are 3 and 5 lobe profile in experiments. With these results, we can predict that these results may be applied to round shape machining such as turning, drilling, boring, ball screw and cylindrical grinding operation in bearing and shaft making operation with the same method. In this study, simulation and experimental work were performed to show the profile behaviors. we can apply these new modeling methods in real process for the predic-tion of part profile behaviors machined such as in round shape machining operation.

      • 가네트를 활용한 고강도 콘크리트의 황산염저항성 특성 연구

        The purpose of this study is about to characteristic of sulfate resist performance following precipitation age of high strength concrete using the garnet minute power. Experiment was compared to the weight change and compressive strength to follow that unit water content 160kg/m³, W/B 30%, 35% and fine aggregate ratio(S/A) 40%, 42%, 44%, admixture(garnet minute power. fly ash) 0%, 10%, 20% and precipitation age. We got the conclusion of the downside through the experiment. Garnet concrete was appeared that was superior to sulfate resist performance by the increasing of garnet. Case of the W/B 30%. garnet concrete did not get up the diminution of weight to follow that small mixing ratio of S/A and expressed diminution of weight the increasing in ratio of S/A. Consequently. If we should make concrete as condition of optimum mixture. we will be expected the effect of sulfate resist performance.

      • 고장식염수가 토끼 빈창자 점막 산호성 과립세포의 미세구조에 미치는 영향

        This experiment was performed to study the influence of hypertonic saline on the production of the secretory granules by the cellular organelles of the jejunal Paneth cells of the rabbit fed on the 3 % hypertonic saline. The jejunal mucosae of the experimental rabbits were removed partially in the upper one third of the jejunum and fixed in 2.0% glutaraldehyde for 2 hours, followed by 1% osmium tetroxide fixative for 2 hours. Then they were embedded in epon mixture, cut with ultramicrotome and done uranyl acetate-lead citrate double stain, and observed with JEM100 CX Ⅱ electron microscope. Observed results were as follows: 1. The number of secretory granules of a higher electron density was gradually increased as a whole in the course of time after feeding on the hypertonic saline. 2. After feeding on the hypertonic saline, secretory granules of various sizes with a higher electron density mostly occupied the supranuclear region of Paneth cells, but a few secretory granules of various sizes with low electron density were observed. 3. After feeding on the hypertonic saline, the Goigi complex, the mitochondria and the rough surfaced endoplasmic reticula were gradually figured, especially the cisternae of the rough surfaced endoplasmic reticula displayed polyhedral shapes. 4. Considering the above findings, following the feeding on the hypertonic saline, the function of the jejunal Paneth cells was influenced and secretory granules of various sizes with a higher electron density were more abundant as compared with those of a lower electron density. From the above results, it was considered that the Golgi complex, the mitochondria and especially the rough surfaced endoplasmic reticula were activated to maintain homeostasis. As a result of that, the production and concentration of secretory granules were prominently observed.

      • 개 소뇌겉질과 후각망울에서 Parvalbumin과 Calbindin D-28k 양성신경세포에 관한 연구

        Background and Objectives : This study was carried out to investigate the morphology, size and distribution of calcium-binding proteins parvalbumin (PV-IR) and calbindin D-28k (CB-IR) immunoreactivity in the mongrel dog cerebellar cortex and olfactory bulb. Materials and Methods : Monoclonal antibodies and the avidin-biotin immunoperoxidase method was used In this experiment. The results obtained by immunohistochemical observation were as follows : 1. Calcium binding protein PV-IR and CB-IR containing neurons were mainly found In the Purkinje cell layer of the cerebellar cortex of the dog. 2. PV-IR and CB-IR cells were round, oval, spindle or polygonal In shape and were 55μm-70μm In dimater of the cerebellar cortex of the dog. 3. PV-IR and CB-IR were mainly located in the external plexiform layer, and a few scattered in the glomerular layer, mitral cell layer, and granule cell layer in the order of thier number of existence in the olfactory bulb of the dog. 4. Three neuronal types were observed in the glomerular layer and external plexiform layer border. Horizontal cells and vertical cells of Cajal were also observed after both PV-IR and CB-IR labeling of the olfactory bulb of the dog. 5. Distinct groups of PV-IR and CB-IR, differing In size, shape, dendritic branching pattern, and staining intensity, were distinguished in the all layers of oldfactory bulb of the dog. 6. The PV-IR and CB-IR cell somata were round, oval, spindle or polygonal in shape, and were unipolar, bipolar or multipolar in type of the oldfactory bulb of the dog. 7. The diameters of the somata of the PV-IR and CB-IR neurons were 20-40μm, respectively. And dendrites of the these neurons were densely arrayed in arborization of the oldfactory bulb of the dog. Conclusion : The number of PV-IR in cerebellar cortex and oldfactory bulb was more abundant than that of the CB-IR cells.
