      • 정상보행시 체중심의 수직 가속도 특성

        In this study, vertical acceleration of center of mass was observed along normal gait phases in 9 healthy male volunteers (aged 25.7±2.18). The developed wireless accelerometric device was attached on the intervertebral space between L3 and L4 using a semi-elastic waist belt. A three-dimensional motion analysis system, synchronized with the accelerometry, was used for detecting gait phases. There was no significant correlation between the body weight and the acceleration. The first peak curve covered loading response phase. The second downward peak point was matched accurately with the opposite toe-off. In mid-stance and terminal stance, the acceleration curve highly resembled the vertical ground reaction force curve. There was no significant difference in timing between the final upward peak point and the initial contact. Therefore, the developed accelerometry system would be helpful in determining determine temporal gait patterns in patients with gait disorders.

        하악골에 발생된 Low grade 골육종:증례보고

        Osteosarcoma is a primary malignant tumor arising most often in the long bone, but rare in jaw bone. Especially osteogenic sarcoma of the mandible is a rare tumor, comprising less than 0.5% of all head and neck tumors. Osteosarcoma occurs cheifly in young persons, and presenting very survival rates. Histopathologically this tumor can be classified three types, osteoblastic, fibroblastic, chondroblastic and classified from low grade to high grade by anaplasia or mitosis of the tumor cells. Sometimes, the exact diagnosis of osteosarcoma is difficult especially in low grade osteosarcoma, even if clinical or radiographical findings suggest to osteosarcoma. So suspcious to malignant bone tumor in clinical or radiological findings, biopsy should be taked from deep portion and multiple area, and sharing the patient history and radiographs with pathologist will assist in the development of the diagnosis. We report a case of low grade osteosarcoma on the mandible initially difficulty in accurate diagnosis. The patient visited our hospital for routine dental treatment but radiographic findings displayed ill-defined radiolucency with osteoid formation on the mandible. Final diagnosis was difficulty in initial biopsy but subsequent biopsy taked from deep portion presented infiltrative growth and mitosis of the tumor cell in some area with small osteoid bone formation, so we could reach final diagnosis as low grade sarcoma. We treated this patient with bloc resection of the mandible and immediate reconstruction with iliac corticocancellous block.

        방사선 치료후 시간경과에 따른 염색체이상 빈도의 변화

        임파구의 염색체이상 빈도로부터 피폭자의 흡수선량을 구하는 방법은 사고로 인해 급성피폭을 받는 경우 유용하게 사용될 수 있다. 그러나 방사선 피폭 후 시간이 경과함에 따라 불안정 염색체이상을 가진 임파구는 감소하게 된다. 이에 방사선 치료후 시간 경과에 따른 불안정 염색체이상 빈도의 변화를 규명하고자 한다. 전골반에 50.4 Gy의 방사선 치료를 받은 총 20명의 자궁경부암 또는 자궁내막암 환자를 대상으로 41개의 검체를 얻었다. 채혈의 시기는 방사선 치료후 1일 3주, 6주, 12주, 24주, 52주, 104주, 156주, 208주, 520주로 하였다. 이들 말초혈액의 임파구에 대해 전혈미세배양을 실시한 후 임파구의 불안정 염색체이상을 관찰하여 Ydr, Qdr, Qdra를 얻었다. Ydr 값은 방사선 치료가 끝난 직후부터 3주까지 plateau를 보였고 이후 감소하는 경향이었다. Ydr의 평균값은 치료후 3주에 0.29에서 급격히 감소하여 2년후 0.05로 감소하였으며 이후 5년까지 서서히 감소하였다. 회귀분석을 실시한바 Ydr = 0.259×exp(-0.0429T)+0.0560×exp(-0.00106T)(time in weeks)로 나타났다. Qdr값은 치료 직후부터 24주까지 1.51 전후로 거의 변화가 없었으며 이후 감소하여 52주 이후에는 1.17 전후로 거의 일정하였다. Qdra 값은 치료 직후부터 12주까지 1.10 전후이며 이후 감소하여 52주 이후에는 0.81 전후였다. 피폭 후 시간경과에 따른 Ydr 값의 감소는 두 component exponential 모델을 잘 맞고 이 식을 이용하여 생물학적 선량측정이 가능하다. Qdr 값 및 Qdra 값은 피폭후 시간경과가 짧은 경우 피폭선량을 추정하는 지료로 사용할 수 있다. It is good method to use frequency of chromosome aberration in lymphocytes for a biological dosimetry in cases of accidental exposure to radiation. But in cases of past edposure, biological dosimetry is limited because the friquency of aberration decreases by time after exposure. To provide a basic data for estimationof past radiation exposure, the changing pattern of frequency of unstable chromosome aberration by time interbal after exposure was studied. Observation was made on peripheral lymphocytes of 41 blood samples from 20 patients treated for uterine cervical carcinoma and endometrial caricinoma. The patients received 50.4 Gy radiation to whole pelvis. Elapsed times after the completion of radiation therapy were 1 day, 3, 6, 9, 12, 24, 52, 104, 156, 208, 260 and 520 weeks. All the blood sample were microcultured. The Ydr, Qdr and Qdra were calculated form frequency of unstable aberration. Ydr did not decrease for 3 weeks after radiation therapy, and thereafter, decreased very rapidly and reached 0.05 at two years after radiation therapy and decreased very slowly until 5 years after radiation therapy. Relationship between unstable chromosome aberration and time interval after radiation therapy was described as Ydr = 0.2590.259×exp(-0.0429T)+0.0560×exp(-0.00106T)(time in weeks) Qdr remained constant at 1.51 until 24 weeks after radiation therapy and then decreased to 1.17 at 52 weeks. Therafter, it did not change. Qdra remained constant at 1.10 for 12 weeks after radiation therapy and decreased to 0.81 at 52 weeks. Thereafter, it remanined constant. Two superimposed exponential Ydr disappearance rate wuggests that it is possible to calculate the past exposure dose. When the elapsed time after exposure is short, Qdr and Qdra are useful papameters for biological dosimetry for past radiation exposure.

        수종의 불소방출 수복재의 탈회억제 효과 및 불소침투에 관한 연구

        자이오머(Giomer)는 레진강화형글라스아이오노머에 견줄 만큼 불소방출량이 많다고 하나, 우식예방 효과에 관한 연구들이 많지 않아 임상적용에 대한 근거가 부족한 실정이다. 본 연구는 자이오머(BF 군)를 포함하여 레진강화형글라스아이오노머(FF 군), 콤포머(DA 군)와 대조군인 콤포짓(FZ 군)의 우식억제 효과와 주변 치질로의 불소침투 양상을 비교하기 위하여, 공초점레이저주사현미경과 전자탐침미세분석기를 이용하여 각 수복재의 우식예방 효과를 비교하였으며, 다음과 같은 결과를 얻었다. 1. 수복물 주위 이차 인공우식에서 법랑질 외측병변의 깊이는 FZ 군에서 가장 깊었으며, BF 군이 가장 작게 나타났다(p<0.05). 2. 수복물과 법랑질 계면에 나타난 탈회억제층의 두께를 관찰한 결과 FZ 군이 가장 작게 나타났으며, FF, DA, BF 간에는 유의성 있는 차이를 보이지 않았다(p<0.05). 3. 법랑질에 침투된 불소농도는 BF, FF, DA의 순으로 높게 나타났다. 4. 상아질에 침투된 불소농도는 DA, BF, FF의 순으로 높게 나타났으며, 이는 상아질-수복물의 접착 계면에 영향 받는 것으로 보인다. The purpose of this study was to evaluate the effect of light cured fluoride-releasing materials on the inhibition of demineralization. In addition, the pattern of fluoride uptake of adjacent tooth structure was analyzed with EPMA. Eighty intact premolars were restored with Filtek Z250(control group, composite), Fuji Filling LC(RMGI), Dyract AP (compomer) and Beautifil II(giomer). Restored teeth were stored in distilled water for 30 days. Thensixty teeth(n=15) were exposed to demineralizing solution(pH 4.3). Demineralized teeth were bisected and polished. The specimens were observed with confocal laser scanning microscope. The depth of outer lesion and the thickness of inhibition zone were measured. Remained twenty teeth(n=5) were bisected for fluoride uptake analysis. The fluoride analysis were taken at enamel-restoration interface and dentin-restoration interface by electron probe micro-analyzer. The results are as follows: 1. The depth of outer lesion of Fuji Filling LC, Dyract AP, Beautifil II was shallower than that of Filtek Z250 at the margin of restoration(p<0.05). 2. The thickness of caries inhibition zone of Fuji Filling LC, Dyract AP, Beautifil II was greater than that of Filtek Z250 at the margin of restoration(p<0.05). 3. Fuji Filling LC, Dyract AP, Beautifil II groups showed the greater fluoride uptake into enamel and dentine around restoration than Filtek Z250 group. 4. In dentin the difference of fluoride concentration were greater than in enamel, and Dyract AP showed the greatest fluoride concentration in dentin.

        This study was conducted to use for the Community Health Care Plan through analyzing the specific cause of death in Gangneung. The study subject were death in Gangneung that happened from January 1, to December 31 in 2000, and that were enrolled as the citizen of Gangneung. The causes of death were classified according to the 56 selected list for ranking of causes of death in Korean National Statistical Office in order to compare the data of nation. We also estimated Years of Potential Life Lost in order to evaluate the burden of disease in Gangneung.

      • 강릉시 의료기관 수요공급 분석

        This study was conducted to use for the Community Health Care Plan through the demand-supply analysis of medical institution in Gangneung. The study data were obtained from the Korean National Health Insurance Corporation and the government of Gangneung. The major findings is follows; First, the hospital beds in Gangneung was 793 beds, but supply was 2,187 beds. Second, the demand of clinic in rural area was 43, and supply was 21. But demand of clinic in urban area was 115, and supply was 144. Third, the pubic medical institution in rural covered 4.7% of outpatient demand, and that in urban covered 0.9% of outpatient demand.

      • 웨이브렛 계수 Zero Crossings를 이용한 영상의 잡음제거 및 압축

        윤곽선이나 에지같은 영상 데이터의 중요한 성분들은 웨이브렛 계수의 zero crossings를 통해 검출할 수 있다. 이런 방법은 영상의 패턴 인식에 관한 연구에서 sharp variation points를 검출하는데 사용되었다. 웨이브렛 변환과 zero-crossings를 거쳐 국부 극치의 위치를 검출하는데 이 방법을 적용하여서 영상의 잡음제거에 사용하도록 개선하였다. 더욱이 영상 데이터에서 전체 영상의 픽셀과 정보를 저장하는 대신, 대부분의 중요한 성분을 효과적으로 몇몇 국부 극치값으로 압축하였다. 그 결과 새로 개선된 기법은 패턴 인식 분야에서만 아니라 높은 압축률의 잡음 제거에도 사용될 수 있다. The important image data components such as contours and edges could be detected by zero crossings of wavelet coefficient. This method has been used for the studies on pattern recognition of images to detect sharp variation points. We have applied it to detection of the position of local maxima through zero-crossings and wavelet transform, so that it is improved to be used for noise-eliminating of image. In addition it could compress the image data into a few of local maxima which held the most of important component efficiently without saving All image pixels and information. As the result, the newly improved method could be implemented not merely for the field of pattern recognition but for that of noise removal at the higher compression ratio.

        하악 영구 견치의 의원성 매복에 대한 증례 보고

        상악 영구 견치의 매복에 비해 하악 영구 견치의 매복은 매우 드물게 일어난다. 치아가 매복된 경우 장애물 존재 여부와 매복 위치와 방향,맹출 가능한 공간의 유무,치근의 형성 단계 등의 요소에 따라 매복 치아의 발거 혹은 장애물의 제거 후 주기적 관찰,맹출이 지연된 치아의 외과적 노출,교정적 견인 등의 방법으로 치료하게 된다. 본 증례는 하악골의 골절 부위에 시행된 골간 강선 결찰과 미니 플레이트에 의해 하악 좌측 견치가 매복되어 있었던 경우로 CT 검사 결과 하악 정중부위 플레이트의 스크류가 맹출을 방해하고 있는 것으로 판단되어 전선 마취 하에 플레이트를 제거하였다. 이후 자연적인 맹출을 기대하였으나,맹출력이 부족하여 교정적 견인으로 양호한 결과를 얻었기에 보고하는 바이다. When a delayed eruption of tooth is diagnosed, the causative factor should be detected before an establishment of treatment plan, if possible. Although a panoramic radiograph is enough to evaluate the position of tooth and the stage of tooth development, a 3-D Dental CT would be a powerful tool to reveal a spatial relationships between objects. The reported case showed a delayed eruption of lower left permanent canine and a mini-plate with screws adjacent to the impacted canine. Although the screws adjacent to the root of impacted tooth showed a close proximity, it was not presumed that these screws would interfere the eruption of the tooth. The impacted canine did not show any spontaneous eruption during observation. After the mini-plate and screws were removed, an orthodontic traction using elastic power chain was performed. The position of mini-plates and screws should be carefully designed to avoid damaging the tooth follicles or tooth roots in the jaw. Also the screws should be removed before an orthodontic traction to prevent damaging the root surface of impacted tooth.
