      • Poster Session:PS 1195 ; Cardiology : Incidence of Pulmonary Arterial Hypertension in Korean Patients with Connective Tissue Disease; Result of Two Sessions of Echocardiographic Screening

        Background: Pulmonary arterial hypertension (PAH) is a major cause of morbidity and mortality among patients with connective tissue disease (CTD). Although the incidence of PAH in CTD patients is higher than that of normal population, the true incidence of PAH in Korean patients with CTD is still unknown. Methods: We performed two sessions of echocardiographic screening in cooperation with rheumatologic department in a tertiary hospital. In each session, we enrolled up to 20 patients with CTD and performed echocardiographic study by an experienced examiner. The possibility of PAH is determined by the estimation of pulmonary arterial pressure by tricuspid regurgitation (TR) maximal velocity (TR Vmax >3. 0m/sec). After the screening, the patients with possible PAH were recommended to undergo cardiac catheterization to confi rm the diagnosis. Results: We screened total 30 patients with CTD (27 females, mean age 51±14 years old). Of them, 22 patients had systemic sclerosis (SSc), 6 had systemic lupus erythematosis (SLE) and 2 had Raynaud`s phenomenon. They showed normal left ventricular function and no evidence of signifi cant valvular heart disease. After echocardiographic screening, possible PAH was detected in 9 patients (30%). The incidence of possible PAH was 36% in SSc (8 of 22 patients) and 17 % in SLE (1 of 6 patients). We confi rmed PAH with cardiac catheterization in 4 of 9 patients and started to treat disease specifi c vasodilator therapy in them. Conclusions: We performed two sessions of echocardiographic screening of PAH with cooperation with rheumatology department. The incidence of possible PAH was 37% in SSc and 17% in SLE. To fi nd exact incidence of PAH in CTD patients, more large numbers of patients in many centers will be needed.

        The Role of Autoantibodies in Idiopathic Inflammatory Myopathies

        Idiopathic inflammatory myopathies (IIM) are a heterogeneous group of autoimmune muscle diseases with systemic involvement. Patients with IIM present with varying degrees of muscle disease, cutaneous manifestations, and internal organ involvement. The diagnosis and classification of IIM is based primarily on the classification system composed of clinical features, laboratory value and muscle biopsy. In addition, the identification and characterization of myositis-related autoantibodies can help diagnosis and classification. Recently, many studies have also demonstrated that the physician can define the clinical syndromes, establish treatment strategy and predict outcomes based on the patients’ myositis-specific autoantibodies (MSA) and myositis-associated antibodies (MAA) profiles. MSAs are found exclusively in IIMs and facilitate the identification of subsets of patients with relatively homogeneous clinical features. MAAs are frequently found in association with other MSA; however, they may also be detected in various connective diseases. (J Rheum Dis 2019;26:165-178)

      • Remote sensing of wave runup over breakwater slope in field using optical imagery

        <P>In this paper, wave run-up on the seaward slope of a breakwater in field was measured remotely using optical video imagery and investigated through comparisons with an empirical formula. In order to extract properties of wave run-up and over-topping on the breakwater slope from field videos, individual image frames were sequentially sampled at 3 Hz from a video burst first. Then, the sampled sequential image frames were rectified into real world coordinates. A cross-shore image timestack was generated by collecting an image pixel array, which was defined along the cross-shore transect on the seaward breakwater face of interest. Trajectory of the run-up on the seaward face evolving in time was extracted from the image timestack using a line detection method. The properties of the run-up estimated from field video bursts were combined to the measurements of incident waves, in order to derive an empirical relation between incident waves and run-up. The video-based relative wave run-up height (i.e. R-u,R-2%/ Hs) at a typical armored breakwater with tetrapods was found to be about 2.15. This value is about in the range of the general formula, which was derived from laboratory experiments in earlier studies.</P>

        Case Report : Clinical Implication of (18)F-FDG-PET in Diagnosing and Monitoring Disease Activity in a Case of Subclinical Stage of Giant Cell Arteritis

        Giant cell arteritis (GCA) is a systemic vasculitis which typically occurs in persons over 50 years old. GCA is closely related to polymyalgia rheumatica (PMR). A temporal artery biopsy is the gold standard test for the diagnosis of GCA. Recently, there is increasing evidence for the role of (18)F-fluorodeoxyglucose positron emission tomography ((18)F-FDG-PET) in diagnosis of vasculitis. Here, we report on a case of a 67-year-old Korean male who was diagnosed with atypical GCA in subclinical stage concomitant with PMR by (18)F-FDG-PET. After treatment, abnormal findings of (18)F-FDG-PET were improved. (J Rheum Dis 2015;22:382-386)

        Pharmacological Properties of CDBT in Hypoxia-induced Neuronal Cell Injury and Their Underlying Mechanisms

        Objectives: This study aimed to reveal the pharmacological properties of the newly prescribed herbal mixture, Chenmadansamgamibokhap-tang(CDBT), against hypoxia-induced neuronal cell injury (especially mouse hippocampal neuronal cell line, HT-22 cells) and their corresponding mechanisms. Methods: A cell-based in vitro experiment, in which a hypoxia condition induced neuronal cell death, was performed. Various concentrations of the CDBT were pre-treated to the HT-22 cells for 4 h before 18 h in the hypoxia chamber. The glial cell BV-2 cells were stimulated with $IFN{\gamma}$ and LSP to produce inflammatory cytokines and reactive oxygen species. When the neuronal HT-22 cells were treated with this culture solution, the drug efficacy against neuronal cell death was examined. Results: CDBT showed cytotoxicity in the normal condition of HT-22 cells at a dose of $125{\mu}g/mL$ and showed a protective effect against hypoxia-induced neuronal cell death at a dose of $31.3{\mu}g/mL$. CDBT prevented hypoxia-induced neuronal cell death in a dose-dependent manner in the HT-22 cells by regulating $HIF1{\alpha}$ and cell death signaling. CDBT prevented neuronal cell death signals and DNA fragmentation due to the hypoxia condition. CDBT significantly reduced cellular oxidation, cell death signals, and caspase-3 activities due to microglial cell activations. Moreover, CDBT significantly ameliorated LPS-induced BV-2 cell activation and evoked cellular oxidation through the recovery of redox homeostasis. Conclusions: CDBT cam be considered as a vital therapeutic agent against neuronal cell deaths. Further studies are required to reveal the other functions of CDBT in vivo or in the clinical field.

        해양플랫폼의 거동 모니터링 시스템 분석

        It is important to measure and analyze offshore platform motion data for the safety of offshore platforms that are installed in an extreme environmental conditions. Especially, in the Gulf of Mexico, because of the hurricane and loop current that has been strengthened due to changes in the ocean environment, a monitoring system is necessary to measure the offshore platform motion data in real time, as well as simultaneously analyze it for safe operation and accident prevention. In order to develop such a monitoring system, it is extremely critical to obtain accurate data. However, since data measured in real time is not suitable for analysis due to quality problems, the data must be processed. The problem is that the processed data differs depending on the processing methods being employed, and results in entirely different analysis results. Incorrect results can mislead the current state of the offshore platform causing missed maintenance periods or, in the worst case, major accidents. Therefore, verifying both the adequacy of the methods used to process the data and the accuracy of the results is most vital. In this paper, improving reliability of the monitoring system that can measure offshore platform motion data and analyze data in real time, the validity of the method used in measurement data processing and the accuracy of the processed data were verified.

        가미지황탕(加味地黃湯)이 고지혈증(高脂血症) 관련(關聯) 인자(因子)에 미치는 영향

        Objective : Gamijihwang-tang(GJT) has been used as a therapeutic agent for hyperlipidemia in oriental medicine for several years. This study was performed to investigate the effects of GJT on hyperlipidemia in rats using diverse biological methods. Method : Hyperlipidemia was induced by a hyper-lipidemic diet fed for 4 weeks. Total cholesterol, LDL-cholesterol, HDL-cholesterol, triglyceride, and glucose were measured in the serum after an oral administration of GJT. Lipid peroxidase, SOD, catalase, ACAT, and HMG-CoA were measured in liver after oral administration of GJT. Result: 1. GJT showed safety against cytotoxicity and toxicity in liver. 2. GJT significantly decreased rat's body and liver weight. 3. GJT significantly decreased serum total cholesterol and LDL-cholesterol, but increased serum HDL-cholesterol. 4. GJT significantly decreased serum triglyceride and glucose. 5. GJT significantly decreased lipid peroxidation and increased SOD and catalase in liver. 6. GJT significantly decreased ACAT and HMG-CoA reductase of cholesterol manifestation in liver. Conclusions : These results suggest that GJT might be effective in treatment and prevention of hyperlipidemia.

        Neuromyositis: A Rare Extramuscular Manifestation of Dermatomyositis

        Dermatomyositis (DM) and polymyositis (PM) are representative idiopathic inflammatory myopathies characterized by symmetric and progressive proximal muscle weakness. Especially, DM is identified by characteristic skin lesions and has many extramuscular manifestations including various cardiac abnormalities, interstitial lung disease, and malignancy. However, involvement of peripheral nervous system in DM/PM is very rare and less known. The term “Neuromyositis” was introduced by Senator in 1893 to describe the concomitant involvement of the peripheral nervous system in DM/PM. Since then, a very few cases of neuromyositis have been reported mainly in the United States and Europe. Therefore, the pathogenetic mechanism and disease progression are unclear. In recent years, a few more cases were reported in Asia, specifically, China and Japan; however, none in Korea. Here, we describe a case of DM-associated neuromyositis in a 42-year-old man in Korea and review previous publications through literature research. (J Rheum Dis 2019;26:211-218)

        과루인약침(瓜蔞仁藥鍼)이 Ovalbumin-induced Asthma Mouse Model에 미치는 영향(影響)

        Objectives : The aim of this study was to investigate the effect of Trichosanthis Semen Herbal-acupuncture(TS-HA) at Joksamni(ST36) on ovalbumin-induced asthma in mice. Methods : C57BL/6 mice were sensitized and challenged with ovalbumin(OVA) once a week for twelve weeks. The experimental group was treated with 1% concentrations of TS-HA at Joksamni(ST36) three times a week for the last eight weeks. Results : 1. The weight and total lung cells of the mice in the group treated with TS-HA decreased significantly compared with those of control group. 2. Total leukocytes and eosinophils in Bronchoalveoler-lavage fluid(BALF) of the mice in the group treated with TS-HA decreased significantly compared with those of control group. 3. Eosinophils in BALF of the mice in the group treated with TS-HA in photomicrographs decreased significantly compared with those of control group. 4. According to histological analysis of lung sections, adhension of collagen in TS-HA decreased significantly compared with that of control group. 5. The concentration of IgE, IL-4, IL-5 in BALF and IL-4, IL-5, IL-13 in serum of the mice in the group treated with TS-HA decreased significantly compared with that of control group. 6. The number of $Gr-^+/CD11b^+,\;CD11b^+,\;CD3e^-/CCR3^+,\;CD4^+,\;CD8^+\;CD3e^+/CD69^+,\;CD23^+/B220^+\;cells$ in the lungs of the mice in the group treated with TS-HA decreased significantly compared with those of control group. Conclusion : These results suggest that Trichosanthis Semen-herbal acupuncture at Joksamni(ST36) done on C57BL/6 mice is effective in part in relieving OVA-induced asthma in C57BL/6 mice.

        db/db mouse에서 인진(茵蔯), 울금(鬱金), 산사(山査) 복합추출물의 항고지혈 및 항산화 효과

        Objective: This study was performed to investigate the effect of IUS (Inulsan, an extract of Artemisiae capillaris (茵蔯), Curcumae longae (鬱金), and Crataegi fructus (山査)) on anti-hyperlipidemia, anti-oxidation, and anti-inflammation.Method: We administered water extracts of Artemisiae capillaris, Curcumae longae, and Crataegi fructus for three weeks to db/db mice (C57BL/Ks), animal models induced with type 2 diabetes mellitus. Mice were divided into three groups: normal (C57BL/6J mice group), control group (db/db mice without administration of IUS) and IUS group (db/db mice treated with IUS). Then we measured total cholesterol, LDL cholesterol, HDL cholesterol, and triglyceride in the serum after the oral administration of IUS.Results: 1. IUS did not show any cytotoxicity in RAW 264.7 cells. 2. IUS decreased AST, ALP, and creatinine levelsand did not show any liver or renal toxicity in the db/db mice. 3. IUS increased DPPH and ABTS radical scavenging activity and decreased ROS production in RAW 264.7 cells. 4. IUS significantly decreased IL-1β, IL-6, and TNF-α production in RAW 264.7 cells. 5. IUS increased HDL cholesterol and significantly decreased total cholesterol and triglyceride in db/db mice. 6. IUS significantly decreased the atherogenic index and cardiac risk factor. 7. In contrast with the control group, fat infiltration in the liver and aorta decreased in IUS treated mice. The cell nucleus was located in the central area in H&E staining of liver. And endomembranes also were more thinner than the control group in H&E staining of aorta.Conclusions: These results suggest that IUS might be effective in the prevention and treatment of dyslipidemia.
