        아시아 12개국의 응급의학 실태에 대한 연구 조사

        Background : To assess the current level of development of emergency medicine (EM) systems in Asia. Method : Survey of EM professionals from 12 Asian countries during a 90-day period from August to November 1998. 12 EM professionals from 12 Asian countries completed the survey. All participants were physicians. 7 participants (58%) gave presentations at an international EM conference during the study period. Respondents completed a 103 question questionnaire about the status of EM specialty, academic, patient care, information and management systems and the factors influencing the future of EM in their countries. Results : 92% of respondents stated that their countries have hospital-based emergency departments (ED). More than 80% of respondents reported that their countries have EMS systems and ED systems for trauma care and patient transfer. More than 70% stated that their countries have national EM organizations, EM research, national EMS activation phone numbers, ED systems for pediatric emergency care, emergency physician (EP) training in ACLS and ATLS and peer review. More than 60% reported official recognition of EM as an independent specialty status, ED triage systems and systems for customer service. More than 50% reported EM residency training programs, EM journals and EP ability to perform rapid sequence intubation (RSI). 50% reported EP ability to perform thrombolysis for acute MI and 33% reported EP ultrasonography. 92% felt that a lack of funding posed a moderate or great obstacle to the future development of EM in their countries. Conclusions : Many essential systems of EM now exist throughout Asia. In the systems of administration and emergency medical information in many countries, there are some parts to be developed further.

      • Real Life Data of Daclatasvir and Asunaprevir Combination Therapy for Korean Patients Infected with HCV G1b without NS5A Resistance: Retrospective Multicenter Study

        Aims: Daclatasvir (DCV) and asunaprevir (ASV) combination therapy has shown a high viral efficacy for chronic genotype 1 (G1) hepatitis C virus (HCV) infected patients. However, the real life efficacy and safety of the therapy for Korean patients with HCV G1 infection are unknown. This study was aimed to investigate the efficacy and safety of DCV and ASV combination therapy in Korean patients infected with HCV G1b without NS5A resistance. Methods: A total of 145 chronic HCV genotype 1b patients treated with DCV+ASV were enrolled and analyzed in a multicenter retrospective cohort. Results: The study population was female dominant (54.5%) with 52.2 years. Thirty seven percent (56/145) had cirrhosis and 40% (58/145) had previous history of HCV treatment at baseline. HCV RNA and ALT levels were 5.82±0.70 log10IU/mL and 64.6±85.1 IU/L, respectively. Of the total 145 Patients, 142 Patients (98%) were completed DCV and ASV combination therapy for 24 weeks. Only 3 patients had early discontinuation therapy (viral breakthrough; 2 and financial problem; 1). Rapid viral response rates at week 4 were observed in 125 among 132 patients (94.7%). The viral response rates at the end of treatment were 96.6% (140/145). Sustained viral response (SVR) rates at post treatment week 12 were 95.1% (136/143). There were no serious adverse events. The most common adverse events were headache and fatigue. Aminotransferase increases of greater than five times the ULN were observed in 5 patients. Conclusions: DCV and ASU combination therapy provided high SVR rates and was well tolerated in Korean patients with HCV G1b infection without NS5A resistance.

        동기침법을 이용한 악관절 장애 환자의 임상적 관찰

        Objective : The purpose of This study is to analyze influence of functional and clinical improvement effect on temporomandibular disorders of Dong-Qi acupuncture treatment that often be used to temporomandibular disorder patients. Methods : We investigated to focus temporomandibular disorders in Jaseng oriental medicine hospital from September 8, 2000 to March 22, 2001. We compared to before and after treatment situation using temporomandibular disease & facial ache dolor pain and malignant function disorders & activation inhibition analysis from one cured Dong-Qi acupuncture treatment of outpatients-40 examples. Results : These results appeared that Sex distinction occurrence ratio was much more female than male of 1: 7. Age distinction occupied the most ratio in 20-30 generation. Jobs was mainly ranked students and companion. In cases treated before visiting our hospital, was mainly ranked our hospital first dignosis. the distribution interval of solider was mainly followed above 3 years. the distribution interval of treatment was mainly gone within I month and above 3 month, frequency of treatment was examined below 5 times, into 6 to 10 times, more than 11 times, respectively. Everything showed a very valuable effect except chewing and dentalgia in decrease of ache dolor pain and linkage pain related to mal-function. There are meaningful effect in Every part on Mal-function improvement. Every contents showed a very excellent effect except activation such as motivation ingestion and recreation in influence of activation. Conclusion : The above results showed a very valuable treatment effect of temporomandibular disorders on Dong-Qi acupuncture treatment. Therefore we are considered the necessity of continuous study and observation applying to other disease as well as temporomandibular disorders.

        Efficacy and safety of gemigliptin, a dipeptidyl peptidase‐4 inhibitor, in patients with type 2 diabetes mellitus inadequately controlled with combination treatment of metformin and sulphonylurea: a 24‐week, multicentre, randomized, double&

        <P><B>Aims</B></P><P>To assess the efficacy and safety of gemigliptin, a dipeptidyl peptidase‐4 inhibitor, added to metformin and sulphonylurea in patients with type 2 diabetes (T2DM).</P><P><B>Materials and methods</B></P><P>We conducted a randomized, double‐blind, placebo‐controlled trial in 219 Korean patients inadequately controlled with metformin and glimepiride. Participants were randomized to gemigliptin 50 mg once daily or placebo added to metformin and glimepiride. The primary endpoint was change in glycated haemoglobin (HbA1c) level from baseline to week 24.</P><P><B>Results</B></P><P>The baseline HbA1c was 8.2% in both groups. The addition of gemigliptin to metformin and glimepiride significantly reduced HbA1c levels at week 24 compared with placebo (between‐group difference in adjusted mean change −0.87%, 95% confidence interval [CI] −1.09% to −0.64%). Fasting plasma glucose level was also significantly reduced with gemigliptin (−0.93 mmol/L, 95% CI −1.50 to −0.35 mmol/L), and a higher proportion of participants achieved an HbA1c level of <7% (39.3% vs 5.5%; <I>P</I><.001) in the gemigliptin group than in the placebo group. Total cholesterol and LDL cholesterol were modestly but significantly reduced in the gemigliptin group compared with the placebo group (−0.21 mmol/L, 95% CI −0.38 to −0.03 mmol/L for total cholesterol, −0.18 mmol/L, 95% CI −0.34 to −0.01 mmol/L for LDL cholesterol). The incidence of hypoglycaemia was 9.4% in the gemigliptin group and 2.7% in the placebo group.</P><P><B>Conclusions</B></P><P>Gemigliptin significantly improved glycaemic control in patients with T2DM inadequately controlled with metformin and sulphonylurea. The incidence of hypoglycaemia was higher with gemigliptin than with placebo, which highlights the importance of optimal dose adjustment for sulphonylurea.</P>

      • Recent Epidemiologic, Clinical Implications of Lung and Ocular Toxocariasis: A Retrospective Study at One Tertiary Hospital

        Purpose Toxocariasis is known to be one of the common causes of peripheral blood eosinophilia and eosinophilic infiltration of various organs. In this study, the prevalence, demographic characteristics of lung and ocular toxocariasis were investigated, and clinical implications were evaluated. Methods From July 1, 2017 to May 8, 2019, a retrospective data collection was conducted on 604 people who had a parasite antibody test out of 138,059 subjects who performed blood tests at a single tertiary hospital. Results Of the 604 people who underwent parasite antibody tests, 540 (89.4%) people were tested for the Toxocara antibody, 245 (45.4%) subjects showed positive for the Toxocara antibody. Among the Toxocara antibody-positive subjects, 49 (20.0%) patients were diagnosed with lung toxocariasis, 44 (89.8%) were male, and 39 (79.6%) urban residency. Among the 49 patients with lung toxocariasis, 44 of them had medical records for raw food ingestion and 43 out of 44 cases had raw food ingestion history. They had raw cow liver, meat, and fish ingestion history. Nineteen patients were diagnosed with ocular toxocariasis, 17 (89.5%) were male, and 15 (83.3%) lived in urban. Raw food ingestion was found in 13 (92.9%) out of 14 cases with medical records. There was no difference between the lung and ocular toxocariasis groups in age, sex, residency, smoking and alcohol history, and underlying diseases. In this study, 33 (67.3%) of 49 lung toxocariasis patients were referred to an ophthalmologist, and 3 (9.1%) of them were diagnosed with ocular toxocariasis. Conclusions It has long been known that lung toxocariasis is a self-limiting disease, whereas ocular toxocariasis, which can be a fatal effect on vision. Therefore, it is recommended that patients who have lung toxocariasis should undergo an ophthalmological examination.

      • Effects of Ginsenosides, Active Ingredients of <i>Panax ginseng</i>, on Development, Growth, and Life Span of <i>Caenorhabditis elegans</i>

        <P>The backbone structure of ginsenosides, active ingredients of <I>Panax ginseng</I>, is similar with that of sterol, especially cholesterol. <I>Caenorhabditis elegans</I> (<I>C. elegans</I>) is one of free living nematodes and is well-established animal model for biochemical and genetic studies. <I>C. elegans</I> cannot synthesize <I>de novo</I> cholesterol, although cholesterol is essential requirement for its growth and development. In the present study, we investigated the effects of ginseng total saponins (GTS) on the average brood size, growth, development, worm size, and life span of <I>C. elegans</I> in cholesterol-deprived and -fed medium. Cholesterol deprivation caused damages on normal growth, reproduction, and life span of worms throughout F1 to F3 generations. GTS supplement to cholesterol-deprived medium restored the growth, reproduction, and life span of worms as much as cholesterol alone-fed medium. GTS co-supplement to cholesterol-fed medium not only promoted worm reproduction but also induced bigger worms and faster growth than cholesterol-fed medium. In study to identify which ginsenosides are responsible for life span restoring effects of GTS, we found that ginsenoside Rc supplement not only restored life span of worms grown in cholesterol-deprived medium but also prolonged life span of worms grown in cholesterol-fed medium. Worms grown in medium supplemented with ginsenoside Rb<SUB>1</SUB> or Rc to cholesterol-deprived medium exhibited strong filipin staining, in which filipin forms tight and specific complexes with 3β-hydroxy sterols. These results show a possibility that ginsenosides could be utilized by <I>C. elegans</I> as a sterol substitute and further indicate that ginsenoside Rc is the component of <I>Panax ginseng</I> that prolongs the life span of <I>C. elegans</I>.</P>

      • 3D-engineered personalized airway stent (“custom GINA stent”): introduction and evaluation in a pig model of tracheal stenosis

        Background: Bronchoscopic treatments of airway stenosis have been evolving. We formerly reported the new silicone airway stent (GINA stent). GINA stent has anti-migration designs, smooth ends for minimalizing granulation tissue, and radio-opaqueness. We herein report “custom GINA stent”, which is a 3D-engineered personalized airway silicone stent made with GINA stent as a backbone. Objectives: To introduce the production process of custom GINA stent, and to know its feasibility by evaluating short term (3 weeks) performance in a pig model of tracheal stenosis. Method: Two female farm pigs (12 weeks old, 40-45kg, 16-20mm of tracheal size) were used. Baseline CT scan was done for stent production, then tracheal stenosis was induced by electrocautery. Using 3D-engineering technology, the mold of stent was fabricated, and the stent was made by injection molding method. When the stent was manufactured, it was inserted through a rigid bronchoscope under general anesthesia. Short term (3 weeks) stent performance was evaluated in terms of migration, granulation (at both ends), and mucostasis. Results: In the first pig, it took 16 days to manufacture the custom GINA stent. In the second pig, the production time was reduced to 7 days. For short term (3 weeks) performance of custom GINA stent, no migration, granulation (at both ends), or mucostasis was occurred in all 2 pigs. Conclusion: In the present study, we developed a personalized airway stent (custom GINA stent) using 3D-engineering within 7 days. In terms of stent performance, it showed perfect results for a short period (3 weeks) in a pig model. Our results should also be demonstrated in clinical trials in humans.

        Benign pulmonary tumors are rare and represent only less than 1% of pulmonary tumors, of these, hamartomas are most common. Among them, Endobronchially located hamartomas are rare and account for only for 1.4% of all pulmonary hamartomas. The improving techniques of therapeutic bronchoscopy have been replacing conventional surgery for resecting endobronchial benign tumor, although there are some limitations for performing, such as completely obstructing large-sized tumor. We report here a case of giant endobronchial hamartoma inducing total atelectasis of a lung, which was successfully treated with electrosurgical snare via fi exible bronchoscopy. A 50-yearold male, presented with dyspnea, cough and sputum for 3 weeks. Chest radiograph revealed total atelectasis of the left lung. Chest computed tomography (CT) scan of the patient showed a mass lesion completely obliterating left main bronchus. An initial bronchoscopy revealed a large mass totally obstructing left main bronchus, and bronchoscopic biopsy was confi rmed as endobronchially located hamartoma. Three sequential therapeutic (fi exible) bronchoscopy were performed under the conscious sedation. An elongated tight mass was split and removed by piecemeal with electrosurgical snare-cutting method, and we discovered that the root of the mass was from the superior lingular segment. We removed the mass until the lingular bronchial passage was completely open. After endoscopic treatment, the left lung total atelectasis was improved little by little through several weeks. 3 months later, chest CT revealed that the left lung except lingula was completely recovered from atelectasis. One year after treatment, He is doing well without any complications. fiexible bronchoscopy with electrosurgical snare under conscious sedation could be a useful option for completely obstructing large-sized benign endobronchial tumors without signifi cant risk.

      • The Effect of Cryoablation in a Pig Model with Induced Benign Tracheal Stenosis

        Background There have been reports that cryotherapy-based treatment is effective for benign central airway stenosis. However, the pathophysiologic mechanism of this modality is not well known. Objectives The purpose of this study was to investigate whether cryoablation has the effect of preventing stenosis recurrence in a pig model of induced benign tracheal stenosis, and to explore the mechanism behind it. Method After inducing airway stenosis in 6 pigs, 3 pigs treated with balloon dilatation, and other 3 treated with balloon dilatation and cryoablation. They were then observed for 3 weeks and underwent post-mortem tissue analysis. Results: Pigs treated with cryoablation showed fewer sudden deaths (1/3 vs 2/3) and less progression of stenosis (on day 14: 80-84% vs 97%). In addition, they had less fibrosis, granulation tissue formation, and epithelial denudation in tissue analysis. Conclusion Cryotherapy (cryoablation) showed to be effective in preventing the progression of stenosis, and its mechanism is thought to be through inhibition of fibrosis, granulation tissue formation, and epithelial denudation.
