        Permanent Pacemaker for Syncope after Heart Transplantation with Bicaval Technique

        <P>Sinus node dysfunction occurs occasionally after heart transplantation and may be caused by surgical trauma, ischemia to the sinus node, rejection, drug therapy, and increasing donor age. However, the timing and indication of permanent pacemaker insertion due to sinus node dysfunction following heart transplantation is contentious. Here, we report a case of a permanent pacemaker insertion for syncope due to sinus arrest after heart transplantation, even with a bicaval technique, which has been known to associate with few incidences of sinus node dysfunction.</P>

        <P>Since T cells express diverse sex steroid hormone receptors, they might be a good model to evaluate the effects of sex steroid hormones on immune modulation. Porcine testicular extract contains several sex steroid hormones and may be useful to study the effects of sex steroid hormones during T cell activation. We have examined the effects of the porcine testicular extract on T cell activation: proliferation and secretion of cytokines (IL-2 and IFN-관) by activated T cells were severely decreased after treatment with porcine testicular extract. The extract produced an immunosuppressive effect and inhibited the proliferation of activated T cells by blocking the cell cycle transition from the G(1) phase to S phase. These effects were mediated by a decrease in the expression of cyclin D1 and cyclin E and constitutive expression of p27(KIP1) after T cell activation.</P>

        Microsurgical treatment of distal middle cerebral artery aneurysm: A single-center review

        Objective: To review the characteristics of distal middle cerebral artery (MCA) aneurysm treated by microsurgery, the detailed surgical options, and the clinical result.Methods: We retrospectively reviewed cerebral aneurysm in the M2 and M3 segments of the MCA surgically treated between January 2015 and December 2022. The demographic data, aneurysm-related findings, type of surgical approach, surgical technique, and clinical outcomes of the enrolled patients were analyzed.Results: Sixteen distal MCA aneurysms were treated with microneurosurgery (incidence, 1.0%; female, 12; mean age, 58.1 years; ruptured, three). Twelve aneurysms were in the M2 segment (insular segment), two aneurysms at the M2–M3 junction, and two aneurysms in the M3 segment (opercular segment). Twelve aneurysms were saccular (average size, 4.9 mm; multiplicity, 50%; average aneurysms, 3.0; partially thrombosed, 1; sidewall aneurysm, 2). Three aneurysms were fusiform, of which two were ruptured. Of the ruptured aneurysms, one was a ruptured dissecting aneurysm. The trans-sylvian and trans-sulcal approaches were used in fourteen and two patients, respectively. Neck clipping, wrap clipping, and surgical trapping were performed in twelve, one, and one patient, respectively. Proximal occlusion was performed in one patient. Bypass technique was required in two patients (neck clipping and proximal occlusion). The modified Rankin Score was 6 in the two patients with ruptured aneurysms. The remaining patients did not show further neurological deterioration after microneurosurgery.Conclusions: Distal MCA aneurysms had a high incidence of being diagnosed with multiple other aneurysms and were relatively non-saccular.

      • Highly Enhanced Performance of Network Channel Polysilicon Thin-Film Transistors

        <P>This letter presents the electrical characteristics of newly proposed network-channel low-temperature polysilicon channel (LTPS) thin-film transistors (TFTs). Due to effective reduction of grain boundary traps and enhanced gate controllability, the network-channel TFTs show better subthreshold slope, lower threshold voltage, and higher ON-OFF current ratio, compared with conventional planar devices. The extracted grain boundary trap density and the interface trap density are significantly reduced in the network-channel devices. In addition, the network-channel devices show higher immunity to hot-carrier stressing, which are confirmed from the low-frequency noise characteristics with various stressing time. These results suggest that the network-channel devices are very promising for next-generation LTPS TFT applications.</P>

        Additional Drug Resistance in Patients with Multidrug-resistant Tuberculosis in Korea: a Multicenter Study from 2010 to 2019

        Background: Drug-resistance surveillance (DRS) data provide key information for building an effective treatment regimen in patients with multidrug-resistant tuberculosis (MDR-TB). This study was conducted to investigate the patterns and trends of additional drug resistance in MDR-TB patients in South Korea. Methods: Phenotypic drug susceptibility test (DST) results of MDR-TB patients collected from seven hospitals in South Korea from 2010 to 2019 were retrospectively analyzed. Results: In total, 633 patients with MDR-TB were included in the analysis. Of all patients, 361 (57.0%) were new patients. All patients had additional resistance to a median of three antiTB drugs. The resistance rates of any fluoroquinolone (FQ), linezolid, and cycloserine were 26.2%, 0.0%, and 6.3%, respectively. The proportions of new patients and resistance rates of most anti-TB drugs did not decrease during the study period. The number of additional resistant drugs was significantly higher in FQ-resistant MDR-TB than in FQ-susceptible MDR-TB (median of 9.0 vs. 2.0). Among 26 patients with results of minimum inhibitory concentrations for bedaquiline (BDQ) and delamanid (DLM), one (3.8%) and three (11.5%) patients were considered resistant to BDQ and DLM with interim critical concentrations, respectively. Based on the DST results, 72.4% and 24.8% of patients were eligible for the World Health Organization's longer and shorter MDR-TB treatment regimen, respectively. Conclusion: The proportions of new patients and rates of additional drug resistance in patients with MDR-TB were high and remain stable in South Korea. A nationwide analysis of DRS data is required to provide effective treatment for MDR-TB patients in South Korea.

        Pulmonology ; Intermediate risk of multidrug-resistant organisms in patients who admitted intensive care unit with healthcare-associated pneumonia

        Background/Aims: Healthcare-associated pneumonia (HCAP) was proposed as a new pneumonia category in 2005, and treatment recommendations include broad-spectrum antibiotics directed at multidrug-resistant (MDR) pathogens. However, this concept continues to be controversial, and microbiological data are lacking for HCAP patients in the intensive care unit (ICU). This study was conducted to determine the rate and type of antibiotic-resistant organisms and the clinical outcomes in patients with HCAP in the ICU, compared to patients with community-acquired pneumonia (CAP) or hospital-acquired pneumonia (HAP). Methods: We conducted a retrospective cohort analysis of patients with pneumonia (n = 195) who admitted to medical ICU in tertiary teaching hospital from March 2011 to February 2013. Clinical characteristics, microbiological distributions, treatment outcomes, and prognosis of HCAP (n = 74) were compared to those of CAP (n = 75) and HAP (n = 46). Results: MDR pathogens were significantly higher in HCAP patients (39.1%) than in CAP (13.5%) and lower than in HAP (79.3%, p < 0.001). The initial use of inappropriate antibiotic treatment occurred more frequently in the HCAP (32.6%) and HAP (51.7%) groups than in the CAP group (11.8%, p = 0.006). There were no differences in clinical outcomes. The significant prognostic factors were pneumonia severity and treatment response. Conclusions: MDR pathogens were isolated in HCAP patients requiring ICU admission at intermediate rates between those of CAP and HAP.

        국내 3D 영상 콘텐츠의 국제경쟁력 분석연구 : 영구아트무비의'디워' 분석을 중심으로

        과거 어느 때보다 빠르게 변화 발전하고 있는 고도화 산업사회 속에서 미디어 엔터테인먼트 산업과 영상 콘텐츠 산업은 보다 빠르게 변화하며 성장하고 있다. 2007년 순수 국내 컴퓨터 그래픽 기술력으로 만들어진 3D 영화 콘텐츠인'디워'는 그동안 해외 기술력의 도움을 받아 만들어진 영화들과 달리 100% 국내 컴퓨터 그래픽 기술을 통해 만들어진 첫 3D 영화다. 이 영화는 개봉 후 국내 누적 관객 수 850만이라는 예상외의 대흥행 성적을 기록했고 미국 시장에서도 박스오피스 10위권 진입에 하는 등 메이저 미국 시장에서 수입을 창출한 첫 영화라는데 그 의미를 부여할 수 있다. 본 연구는 국내 3D 영상콘텐츠를 전형적인 영화 분석방법인 스토리 구조와 스타일 표현분석이라는 개념으로 콘텐츠의 질적 완성도를 분석한다는 전제하에 스토리텔링 기획에 나타난 문제점 등의 구조적 분석과 CG등의 표현적 완성도의 기술력과 실제 표현적 특장점 등을 분석하여 연구함으로써 콘텐츠의 전체적인 완성도를 객관적으로 분석하고 아울러 문화적 요인과 트렌드 사회적 요인 등 마케팅적 측면에서의 분석을 통해 세계적인 3D 영상콘텐츠 강국으로 가기 위한 발전 방향에 대해 중점적으로 다루고자 한다. In today's high technology society which changed and developed more faster than any other period in past media entertainment industry has been grown up much faster than that. The'The war' the first feature film in Korea which was made by 100% domestic CG technology not like other CG film that made by foreign CG technology. Beyond expectation this movie has recorded 8 and half million number of audiences in Korea and ranked in 10th box office in U. S market. this is first movie in Korea that earned the profit in major american market. This study will be based on the analysis of its structure and style which is classical method in hollywood. This analysis will produce the result of the problems in storytelling and the structure as well as style CG technique and so on. This study also will cover the cultural and trend factor in terms of marketing for the better development 3D contents industry in Korea.

        뉴 테크놀로지를 통한 3D 콘텐츠 설치조형예술작품의 표현 연구 : 3D 프로젝션 매핑을 통한 리얼리티와 가상현실 동시구현에 대해서

        21세기 현대사회는 테크놀로지의 비약적 발전으로 예술적 지각능력의 향상과 매체를 활용하는 범위가 확장되고 대중의 지각방식을 바꿔놓게 되었고 이에 길들여진 대중은 새롭고 더욱 생생하며 무한한 정신적 예술의 산물을 요구하게 되었다. 이에 현대 대중예술의 아이콘격인'팝아트'는 기술적 가공 과정과 융합한 형태 기존 장르 구분의 속박에서 벗어난 혼합 매체의 사용이라는 큰 방향을 제시함으로써 테크놀로지와의 조화로운 발전과 함께 동시대 문화와 사회에 대한 통찰과 직관력 우리시대의 문화 전체를 반영하는 거울 같은 존재적 역할을 했다. 본 연구에서는 새로운 테크놀로지의 산물인 프로젝션 매핑(Projection mapping) (특정 지형 지물에 빔프로젝터를 쏘아 특정 화면을 연출하는 기법 )의 대중예술 디자인 작품으로의 적용을 통해 과거의 기존 작품들의 표현의 한계를 넘어선 새로운 표현의 가능성을 분석 하였다. 결론부에서는 3D 콘텐츠를 3차원적으로 투영하는 이 기술의 물리적 과정 및 현상의 특징에 기인하여 과거 전통 공연예술인 연극이 가지고 있는 퍼포먼스적인 성격과 현실공간의 관객과의 동 시공간적 공존이 주는 현장성과 긴밀함과 영화가 가지고 있는 가상현실에 대한 시공간적 표현의 무제한성 등의 장점을 동시에 가지게 돼 그 작품 표현에 있어 새로운 가능성이 무한하게 열리게 되어 상업적인 디스플레이는 물론 설치예술 및 환경 및 공공 디자인 등의 표현에도 새로운 패러다임을 제시할 수 있을 것을 논술하고 있다. Due to radical development of technology after 21th century the range of material and media in art has been extended and it changed the way of understanding of art. As this process goes faster people start to have demand that new art form and contents which can be expressed beyond limitation in past. The pop art called "The icon of modern popularity of fine arts" succeeded to make new way of expression mixed with product's manufacturing process which got out of restriction of its classic form. It contributed well balanced development between technology and art show artist how they should prepare in future in terms of adoption of technology today. This thesis will suggest the possibility (can be expressed beyond limitation in past) through adoption of the projection mapping the product of new technology in art as an new way of express. In the conclusion I have found that this new technology has both of good express way from traditional theater play and feature film which is reality and intimacy due to exist in same time and space from play and unlimited expressional freedom from movie. I believe this new technology will open up new possibility not only for fine art like installation but also public & environmental design as well.
