To evaluate the rehabilitative care needs of the patients with spinal cord injury in this regional area, the complications which were seen in the department of rehabilitation medicine in Chungnam National University Hospital during the 1st year since the opening of the isolated rehabilitation medicine ward in September 1989 were analyzed. The study population was consisted of 25 men and 2 women. Nine cases (33.3%) were in the 20-29 age group and 11 cases (40.8%) were injured by traffic accident, the most common cause of traumatic spinal cord injury in this study. Sixteen cases(59.2%) were paraplegics and eleven cases(40.8%) were quardriplegics. The duration from the onset of injury to admission to the department of rehabilitation medicine ranged from 21 days to 14 years with mean of 18 months. The complications seen in this study were as follows. 1. Urinary tract infections, most common complication, were found in 14 cases (51.6%) followed by decubitus ulcers (44.4%), edema of leg (22.2%), spasticity (18.5%). 2. The most common organism of urinary tract infection was E.coli(43.0%). 3. Among the 22 cases which underwent IVP and VCUG, 2 cases showed hydronephrosis in IVP and 4 cases showed vesicoureteral reflux in VCUG. 4 The most common site of pressure sores was sacrum(53.3%). 5. The mean duration of pressure sore healing with non-surgical treatment was 18 days in Grade II and 42 days in Grade III. With the above results, it is concluded that most frequent complications of patients with spinal cord injury were urinary tract infection and pressure sores, with most of them being occurred before the admission to department of rehabilitation medicine and being treated completely during the stay in the department of rehabilitatiom medicine, and that the comprehensive rehabilitation treatment for the prevention of complications is necessary from the onset of injury.

The purpose of this study was to evaluate the effectiveness of hydraulic management in patients with frozen shoulder which were not improved with previous conservative treatment. The study population was consisted of 4 men and 6 women, 6 cases were affected in the dominant side and 4 cases in the non-dominant side and the duration of frozen shoulder was ranged from 3 to 18 months, with mean duration of 8.8 months. Associated conditions were trauma or fractures in 5 cases and cervical radiculopathy in 2 cases. The results were as follows : 1. The pre-treatment shoulder ROM was 77° of flexion, 53° of abduction, 21° of external rotation, and 39° of internal rotation, and more severely limited in external rotation and abduction than in flexion and internal rotation. 2. The post-treatment ROM was 132° of flexion, 118° of abduction, 64° of external rotation, and 68° of internal rotation. 3. With the hydraulic management, increased ROM were 55° of flexion, 65° of abduction, 43° of external rotation and 29° of internal rotation. 4. The performance of ADL's and subjective symptoms such as pain, stiffness, sleep disturbance showed moderate to marked improvement in 8 cases. 5. No complications were observed with the hydraulic management. With the above results, hydraulic distension was thought to be the effective method for improvement of subjective symptoms and increment of ROM in patients with frozen shoulder.
급성 근골격질환에 있어서 피록시캄 근육주사제의 임상적 연구 : An Open, Non-Comparative Trial

The authors have conducted a clinical study to evaluate the efficacy and toleration with newly developed piroxicam injectable (I. M.) to thirty-one cases of the patients of musculoskeletal disorders during five months from Oct. 1986 to Feb. 1987. Treatment for the patients was initiated with two amples of piroxicam 20mg I. M. daily for first two days and followed administration one ample of 20mg piroxicam daily for three to seven days. Results were as follows: 1. Out of 31 cases, male were 16 (50.5%), female 15(49.5%), and predominant age group was 20-29 years old 11(35.4%). 2. Major indications were sprained ankle and sprained PIP joint disorders 11(35.7%), 12(38.7 %), respectively. 3. Overall efficacy, more than good was 83.8% by patients and 96.8% by physician. 4. Overall tolerance, more than good was 90.3% by patients and 90.3% by physician (excluded four cases). 5. Side effects were seen in 4 patients such as I case of constipation, 2 cases of abdominal pain and 1 case of vomiting. 6. Conclusion: The piroxicam I. M. was very effective and well tolerated for the treatment of musculoskeletal disorders and side effects were few and generally mild.

Objectives: The aim of this study was to evaluate the associations of frailty with perceived neighborhood walkability and environmental pollution among community-dwelling older adults in rural areas. Methods: The participants were 808 community-dwelling men and women aged 65 years and older in 2 rural towns. Comprehensive information, including demographics, socioeconomic status, grip strength, polypharmacy, perceived neighborhood environment (specifically, walkability and environmental pollution), and frailty, was collected from participants using face-to-face interviews conducted between June and August 2018. Perceived neighborhood walkability was measured using 20 items that were selected and revised from the Neighborhood Environment Walkability Scale, the Neighborhood Walkability Checklist from the National Heart Foundation of Australia, and the Physical Activity Neighborhood Environment Survey. The Kaigo-Yobo Checklist was used to assess participants' frailty. Results: The overall prevalence of frailty in this community-dwelling population was 35.5%. Sex, age, cohabitation status, educational attainment, employment status, grip strength, and polypharmacy were significantly associated with frailty. In the logistic regression analysis, frailty was associated with low perceived neighborhood walkability (adjusted odds ratio [aOR], 0.881; 95% confidence interval [CI], 0.833 to 0.932; p<0.001) and severe perceived neighborhood environmental pollution (aOR, 1.052; 95% CI, 1.017 to 1.087; p=0.003) after adjusting for sex, age, cohabitation status, educational attainment, employment status, monthly income, grip strength, and polypharmacy. Conclusions: More studies are warranted to establish causal relationships between walkability and environmental pollution and frailty.

V-set and immunoglobulin domain containing 1 (VSIG1) is a newly discovered member of the junctional adhesion molecule (JAM) family; it is encoded by a gene located on human chromosome X and preferentially expressed in a variety of cancers in humans. Little is known about its physiological function. To determine the role(s) of VSIG1 in mammalian spermatogenesis, we first generated a specific antibody against mouse VSIG1 and examined the presence and localization of the protein in tissues. RT-RCR and Western blot analysis of the mouse tissues indicated that VSIG1 was specifically expressed in the testis. Furthermore, the results of our trypsinization and biotinylation assays strongly support the assumption that VSIG1 is localized on the testicular germ cell surface. In order to determine whether VSIG1 is capable of participation in homotypic interactions, we performed a GST-pull down assay by using recombinant GST-fusion and Histagging proteins. The pull-down assay revealed that each GST-fusion Ig-like domain shows homotypic binding. We further show that mVSIG1 can adhere to the Sertoli cells through its first Ig-like domain. To identify the protein that interacted with cytoplasmic domain, we next performed co-immunoprecipitation analysis. This analysis showed that ZO-1, which is the central structural protein of the tight junction, is the binding partner of the cytoplasmic domain of mouse VSIG1. Our findings suggest that mouse VSIG1 interacts with Sertoli cells by heterophilic adhesion via its first Ig-like domain. In addition, its cytoplasmic domain is critical for binding to ZO-1.

Members of the Vps (Vacuolar protein sorting) protein family involved in the formation of the retromer complex have been discovered in a variety of species such as yeast, mouse, and human. A mammalian retromer complex is composed of Vps26, Vps29, and Vps35 proteins and plays and important role in cation-independent mannose-6-phosphate receptor retrieval from the endosome to the trans-Golgi network. In this study, we have identified the full-length sequences of the retromer components of Vps26, Vps29, and Vps35 in micro pigs. The cDNA sequences of these retromer components have been determined and the result showed there is 99% homology among the component counterparts from mouse, micro pigs, and humans. In addition, the retromer complexes formed with hetero-components were found in the brain of micro pigs. Based on above results, we suggest mammalian Vps components are well conserved in micro pigs.

<P>In chiral magnetic materials, numerous intriguing phenomena such as built-in chiral magnetic domain walls (DWs) and skyrmions are generated by the Dzyaloshinskii-Moriya interaction (DMI). The DMI also results in asymmetric DW speeds under an in-plane magnetic field, which provides a useful scheme to measure the strength of the DMI. However, recent findings of additional asymmetries such as chiral damping have inhibited the unambiguous determination of the DMI strength, and the underlying mechanism of overall asymmetries comes under debate. Here, we experimentally investigate the nature of the additional asymmetry by extracting the DMI-induced symmetric contribution from the DW speed. Our results reveal that the additional asymmetry has a truly antisymmetric nature with the typical behavior governed by the DW chirality. In addition, the antisymmetric contribution alters the DW speed by a factor of 100, dominating the overall variation in DW speed. Thus, experimental inaccuracies can be largely removed by calibration with such antisymmetric contributions, enabling the standard DMI measurement scheme.</P>

KR-33028 (N-[4-cyano-benzo[b]thiophene-2-carbonyl]guanidine) is a new cardioprotective agent for preventing ischemia-reperfusion injury. This study was performed to identify the metabolic pathway of KR-33028 in human liver microsomes and to compare its metabolism with that of cryopreserved human hepatocytes. Human liver microsomal incubation of KR-33028 in the presence of NADPH and UDPGA resulted in the formation of four metabolites, M1, M2, M3, and M4. M1 and M2 were identified as 5-hydroxy-KR-33028 and 7-hydroxy-KR-33028, respectively, on the basis of LC/MS/MS analysis with the synthesized authentic standard. M3 and M4 were suggested to be dihydroxy-KR-33028 and hydroxy-KR-33028-glucuronide, respectively. Metabolism of KR-33028 in cryopreserved human hepatocytes resulted in the formation of M1, M2, and M4. These data show a good correlation between major metabolites formed in human liver microsomes and cryopreserved human hepatocytes. In addition, KR33028 was found to inhibit moderately the metabolism of CYP1A2 substrates. Based on the results obtained metabolic pathway of KR-33028 is proposed.
