Keratosis punctata palmaris et plantaris with unusual skin manifestation

Keratosis punctata palmaris et plantaris (KPPP) is a rare skin condition characterized by punctiform hyperkeratotic papules mainly on the palms or soles. It is known as a genodermatosis with an autosomal-dominant pattern of inheritance. KPPP is known as a punctiform subtype of hereditary palmoplantar keratoderma . A 20-year-old male visited our institution due to hyperkeratotic papules on both feet. The physical examination revealed multiple skin-colored to semi-translucent hyperkeratotic punctiform seed-like papules on the medial surfaces of both feet. A histopathological examination showed compact columns of massive, square-shaped hyperkeratosis and increased granular layer. Broad, cup-shaped depression of the underlying malpighian layer of the epidermis is noticed. No specific findings were seen in the dermis (Fig.1c,d). Based on both the clinical and histopathologic features, a diagnosis of KPPP was made. KPPP is a rare skin condition typically affecting the palm and sole. In this report, we show that KPPP may involve other parts of the feet where any type of the palmoplantar keratoderma is seldom develop even without KPPP on the palm or sole.
Venous malformation as a subcutaneous lobulated nodule without surface change

Among vascular malformations, venous malformation is relatively common, usually present with single bluish large soft nodule, found in head and neck in 50% of the cases. A 44-year-old male visited the clinic with 6-years history of palpable mass on chest. The mass had been there for several years. The patient did not have any family history of similar skin lesion. He was otherwise hearthy. On the physical examination, about 3 cm sized irregular shaped rigid nodule was palpated, with no surface change in color or shape. On ultrasonographic examination of the affected site demonstrated well localized heteroechoic subcutaneous mass with minimal increased vascularity. We performed excisional biopsy of the whole mass, which revealed 3 cm sized dark blue colored grape-like multi-lobulated tumor. A histopathological examination showed multiple thin walled dilated venous channels, compatible with venous malformation. Vascular malformation presenting as a subcutaneous well localized mass is rather atypical, one can be confused with common tumors, such as epidermal cyst, lipoma, and so on.
Characteristic Study of Fuel Injection into a Supersonic Flow in a Shock Tunnel

Flow features of helium and ethylene transverse jets exposed in a supersonic flow have been examined experimentally. Flow visualization images were obtained in a shock tunnel using a Schlieren technique. Both the helium and the ethylene experimental data showed that jets weekly penetrate deep into the crossflow just downstream of the injection port when the velocity shear is strong at high Reynolds number. At sufficient distance downstream from the injection port, however, the jets seem to recovered their momentum and the deviations from the existing data were found to be small.

<P>Chronic spontaneous urticaria (CSU) is a complex idiopathic disease of the skin with various cellular infiltrations. Although mast cells are key effector cells in the pathogenesis of CSU, CD4+ T helper 2 cells also have particular roles in the development and maintenance of CSU. Periostin is known as a downstream molecule of interleukin (IL)-4 and IL-13, key cytokines of type 2 immune responses. In this study, we examined periostin and IL-13 levels in the sera of patients with CSU (n=84) and healthy normal controls (NCs, n=43). Periostin levels were significantly lower in the CSU group than in NCs (71.4±21.8 vs 85.1±22.4 ng/mL, <I>P</I>=0.04). Periostin levels were also lower in the severe CSU group than those in mild CSU (59.7±18.0 vs 73.4±22.0 ng/mL, <I>P</I>=0.04). However, IL-13 levels were significantly higher in patients with CSU than in NCs (508.5±51.2 vs 200.7±13.3 pg/mL, <I>P</I>=0.001). In conclusion, periostin and IL-13 may be independently related to the pathogenesis of CSU.</P>
Mach 4 Scramjet Testing in a Shock Tunnel

The shock tunnel is known to be uniquely capable of simulating flow-field environments that can replicate supersonic/hypersonic flight conditions. As a consequence of the strong interest in hypersonic aerospace planes and research in Republic of Korea, attention has been given on this type of facility and the intermediate-sized shock tunnel has lately been built at KAIST. This paper discusses the performance of our tunnel and some of the recent progress made on the Mach 4 scramjet testing.

Background: Despite the autologous serum skin test (ASST) and autologous plasma skin test (APST) is widely used test accessing whether a patient with chronic spontaneous urticaria (CSU) has autoreactivity or not, the clinician often encounter difficulty making correlation between the test result and clinical implications. Objective: This study was aimed to find any clinical and laboratory findings related to the ASST and APST response. Agreement and correlation between the two tests was also analyzed. Methods: A retrospective study was conducted on 300 CSU patients who underwent ASST, APST. The subjects were divided into four groups according to the skin test result. Also, the degree of serum and plasma response was recorded. Results: Both ASST and APST positive group had shorter duration of the disease, higher incidence of at least one episode of angioedema than negative group. There were no significant differences in the positivity for autoantibodies including antinuclear, ds-DNA, and thyroid-related between the two groups. The predicted positive rate of ASST and APST according to age showed bimodal peak and decreasing pattern according to disease duration. Predicted positivity of both tests declined with increase in total immunoglobuline E (IgE) level. In the correlation study, the two tests showed high correlation coefficients. Conclusion: ASST and APST positivity may be related to disease duration and severity of CSU. The two tests showed a generally consistent result. Autoreactivity may be gradually lost as disease continues. We suggest the autoreactivity in CSU could arise independently from IgE mediated immune process. (Ann Dermatol 32(4) 280∼288, 2020)

Patients with subcutaneous panniculitis-like T-cell lymphoma (SPTL) present with typical nodular lesions or plaques which have usually persisted for at least several months to years. Furthermore, systemic symptoms including fever are often accompanied by SPTL. Thus, at early stage, SPTL may be confused with other benign panniculitis such as erythema nodosum associated with infectious conditions. A 67-year-old man presented with a four-day history of mild fever, and multiple erythematous round plaques or swelling on the both shins and left cheek. Based on the positive results for influenza A antigen, he was diagnosed as influenza infection with erythema nodosum. However, despite talniflumate and oseltamivir therapy for five days, the fever persisted and new lesions were developed. In addition, further work-ups disclosed an increased Widal O antigen titer (1:1280), and thus he was diagnosed as typhoid fever with erythema nodosum. However, the fever and cutaneous lesions were not improved by ciprofloxacin and naproxen. Finally, a skin biopsy revealed a dense atypical lymphocytic infiltration involving the subcutaneous fat layer in a lobular and septal pattern. The cells were positive for CD3, CD8, and granzyme B, and negative for CD4, CD20, CD56, CD30, and Epstein-Barr virus. Based on the skin biopsy findings, he was diagnosed as SPTL, and the fever and skin lesions were improved by administration of high dose prednisolone.
Hirsutism associated with administration of cetuximab

The administration of epidermal growth factor receptor inhibitors has been associated with various hair changes including brittle scalp hair, scarring and nonscarring alopecia, trichomegaly of eyelashes, and hirsutism. To our best knowledge, however, there has been no report published concerning hirsutism associated with epidermal growth factor receptor inhibitors in Korean literature. We here describe a case of hirsutism associated with administration of an epidermal growth factor receptor inhibitor. A 71-year-old woman presented with an one-month history of an excess of hairs on the upper lip. About 2 years ago, she was diagnosed as sigmoid colon cancer, and underwent an anterior resection followed by twelve sessions of combination chemotherapy. Six months ago, however, a follow-up computed tomography scan revealed multiple metastatic lesions in the lung. She thus started a therapy with cetuximab, irinotecan, 5-fluorouracil, and leucovorin, and developed an excessive terminal hair growth on the upper lip after seventh cycle of the therapy. Physical examination revealed numerous thick terminal hairs on the nostril, columella, and upper and lower lip. The patient was diagnosed as hirsutism associated with administration of cetuximab, but no treatment was given according to the patient’s wish.
