Repair of tetralogy of Fallot (TOF) has shown excellent outcomes. However it leaves varying degrees of residual hemodynamic impairment, with severe pulmonary stenosis (PS) and free pulmonary regurgitation (PR) at both ends of the spectrum. Since the 1980s, studies evaluating late outcomes after TOF repair revealed the adverse impacts of residual chronic PR on RV volume and function; thus, a turnaround of operational strategies has occurred from aggressive RV outflow tract (RVOT) reconstruction for complete relief of RVOT obstruction to conservative RVOT reconstruction for limiting PR. This transformation has raised the question of how much residual PS after conservative RVOT reconstruction is acceptable. Besides, as pulmonary valve replacement (PVR) increases in patients with RV deterioration from residual PR, there is concern regarding when it should be performed. Regarding residual PS, several studies revealed that PS in addition to PR was associated with less PR and a small RV volume. This suggests that PS combined with PR makes RV diastolic property to protect against dilatation through RV hypertrophy and supports conservative RVOT enlargement despite residual PS. Also, several studies have revealed the pre-PVR threshold of RV parameters for the normalization of RV volume and function after PVR, and based on these results, the indications for PVR have been revised. Although there is no established strategy, better understanding of RV mechanics, development of new surgical and interventional techniques, and evidence for the effect of PVR on RV reverse remodeling and its late outcome will aid us to optimize the management of TOF.
Tissue Distribution and Binding Proteins of Radionuclides in Bivalve, Gomphina melanaegis

민들조개에 의해 흡수된 방사성 동위원소의 조직내 분포와 생화학적 거동을 알아 보기 위해 민들조개를 65Zn, 54Mn, 137Cs용액에 7일간 노출시킨 후 조직을 잘라내서 방사능을 측정하고 간과 콩팥을 균질화시켜서 Sephadex G-75 유리관을 통한 젤투석 실험을 하였다. 방사성 동위원소의 조직내 분포는 방사성 동위원소에 따라 서로 다른 조직내 축적경향을 나타냈으며 특히 다른 조직에 비해 간과 콩팥에 아주 높게 농축되었다. 방사능이 농축된 간과 콩팥에서 방사성 동위원소와 결합하고 있는 단백질을 조사하기 위해서 간과 콩팥을 균질화시킨 상등액을 Sephadex G-75 column chromatography 로 분리한 결과 65Zn, 54Mn, 137Cs가 서로 다른 유출 패턴을 나타냈고 또한 65Zn과 54Mn은 간과 콩팥에서 서로 다르게 유출되었다. 본 실험의 결과 방사성 동위원소의 종류와 조직에 따라서 결합하고 있는 단백질의 종류가 서로 다른 것을 알 수 있었다. Radioisotope tracer experiments on the distribution and the binding of radionuclides to proteins in bivalve were carried out in order to gain further information on biochemical behavior of radionuclides in marine bivalve, Gomphina melanaegis. The radioactivities (cpm/g) of 65Zn and 54Mn after 7 days exposure were highly concentrated in liver and kidney in comparison to soft parts. The gel filtration profile of 65Zn in liver and kidney showed three elution peaks, while 54Mn showed two peaks in liver and three peaks in kidney. On the gel filtration of 137Cs in liver and kidney, most of 137Cs were eluted on one peak. Thus, it was considered that each radionuclide was bound to different proteins in liver and kidney of bivalve.
Brain meningioma in a patient with systemic lupus erythematosus

Brain meningioma, the most common benign brain tumor, has been reported to account for 13-26% of all intracranial tumors, with a crude incidence rate of 2.3 per 100,000 persons for all types of meningiomas. The prevalence of neuropsychiatric lupus erythematosus is 15-91% and its clinical manifestations are diverse: from mild cognitive dysfunction to serious neurological or psychiatric symptoms. Here, we report the first Korean patient with brain meningioma and systemic lupus erythematosus who had undergone surgical tumor resection.

As a new humanized monoclonal antibody against the interleukin-6 receptor, tocilizumab is currently used for the treatment of rheumatoid arthritis (RA) patients. Tocilizumab was reported to provoke drug-related liver toxicity, although there have been no reports on significant liver toxicity from tocilizumab in Korean patients with RA to date. Here, we describe the first case of tocilizumab-related liver toxicity in a patient with complicated RA, accompanied with macrophage activation syndrome, who had received tacrolimus and prednisolone and in whom both conventional disease modifying anti-rheumatic drugs, including methotrexate, leflunomide and sulfasalazine or tumor necrotizing factor-α blockades, were contraindicated due to drug eruption and a history of lung cancer.

The objective of this study was to both isolate and identify non-mutans streptococci organisms (nonMSO) from dental plaques recovered on mitis-salivarius sucrose bacitracin agar (MSB) plates. The dental plaque samples, which had been collected from 63 human subjects, were diluted and plated on MSB. The bacteria growing on the MSB plates were then identified with biochemical tests, as well as with 16S rDNA cloning and sequencing techniques. Our data indicated that bacteria from 30 subjects had been recovered on the MSB plates. Among the 21 typical colonies selected from the 30 subjects, 12 colonies, derived from 10 subjects, were identified as non-MSO. These 12 colonies were determined to be Streptococcus anginosus (8 colonies), S. sanguinis (1 colony), and Pantoea agglomerans (3 colonies). These results strongly suggest that a new selective medium will be required for the reliable isolation of mutans streptococci.
