        Evaluation of YouTube Videos Regarding Clean Intermittent Catheterization Application

        Purpose: The aim of this study was to assess the clean intermittent catheterization (CIC) training and education videos on YouTube. Methods: This study was conducted in English language in descriptive type to evaluate the content, reliability, and quality of Internet videos related to CIC training. The search was performed by using term “clean intermittent catheterization” and “intermittent self catheterization” on YouTube in August 2019. The content of the selected videos was analyzed by 2 independent experts in the field. The DISCERN questionnaire was used to analyze the reliability of the video and the global quality score (GQS) was used for the quality of the video. Results: When the contents of the 64 videos included in the study were examined, it was found that 41 of them contained useful information and 23 of them contained misleading information. DISCERN scores, video coverage scores, and GQS of videos with useful information were found to be statistically higher than those with misleading information (P<0.001). When the sources of the videos were examined, it was seen that 78.05% of the videos containing useful information were published by medical advertisement/for-profit companies (P<0.001). The reliability scores, coverage scores, and GQS of the videos from medical advertisement/for-profit companies were significantly higher than the other sources (P<0.001). Conclusions: In this study, it was seen that the majority of YouTube CIC training videos examined within the scope of the research were published by medical advertisement/for-profit companies and most of these videos contain useful information.

        The Effect of Video-Assisted Clean Intermittent Catheterization Training on Patients’ Practical Skills and Self-Confidence

        Purpose: This research was carried out in order to examine the effect of clean intermittent catheterization (CIC) training with a video developed by the researchers on patients’ ability to practice CIC and self-confidence. Methods: The population of the study consisted of patients who had just started performing CIC in the urology polyclinic of a city hospital in Istanbul. The sample consisted of a total of 80 patients, 40 of whom were in the experimental group and 40 in the control group. The experimental group patients were given CIC training with a training video that was downloaded to the mobile phone of the patient, a family member, or caregiver. The patients’ practice skills were evaluated by 2 independent observers. The DISCERN Inquiry Form and the Global Quality Score, the Patient Information Form, the CIC Skill Checklist and the Self-Confidence Scale in Clean Intermittent Self-Catheterization were used to collect data. Results: In the experimental group, consisting of patients who received video-assisted training, the mean scores for the CIC Skill Checklist and the Self-Confidence Scale in Clean Intermittent Self-Catheterization were statistically significantly higher than in the control group (P<0.001), the experience of feeling pain during catheterization was less than in the control group, and the patients in the experimental group experienced statistically significantly fewer complications such as urinary tract infections, urgency, urinary incontinence, hematuria and urethral stricture (P<0.05). Conclusions: Video-assisted CIC training had a positive effect on patients’ practical skills and self-confidence.

        Repair of a postappendectomy massive ventral hernia using tissue expanders

        Reconstruction of large abdominal wall defects is a challenging problem. Various reconstructive techniques have been described in the surgical literature each with its advantages and disadvantages. In this report we describe our experience in treating a patient with large abdominal wall defect by staged abdominal wall reconstruction utilizing prosthetic mesh in conjunction with tissue expanders. A 41-year-old male presented with abdominal pain. Exploratory laparotomy showed perforated appendicitis with intraabdominal abscess of 1,500 mL. Postoperatively, he developed intraperitoneal sepsis. To prevent abdominal compartment syndrome, he was reoperated and left with “pen abdomen” After several open abdomen lavages, his abdominal wall defect was allowed to granulate. After epithelization of the defect, the abdominal wall was reconstructed using prosthetic mesh and tissue expanders. The tissue expansion process was well tolerated. We suggest that the use of tissue expanders provides reliable and well-vascularized soft-tissue coverage in abdominal wall reconstruction.

        Effects of hyperbaric oxygen and iloprost on intestinal ischemia-reperfusion induced acute lung injury

        Purpose: To research the effects of iloprost (IL) and hyperbaric oxygen (HBO) combination treatment on lung injury and on tumor necrosis factor alpha (TNF-α), myeloperoxidase (MPO), malondialdehyde (MDA), and soluble intercellular adhesion molecule-1 (sICAM-1) levels after tissue or organ ischemia-reperfusion, and on ischemia-reperfusion induced lung neutrophil sequestration. Methods: Forty white New Zealand rabbits were assigned randomly into 5 groups: HBO, IL, HBO+IL, control, and sham groups. TNF-α values were checked before ischemia, in the 1st hour of ischemia and in the 1st and 4th hours of reperfusion, also at the end of reperfusion period, plasma and tissue MPO values, MDA values, and sICAM-1 levels were detected. After sacrifice, the degree of lung injury was determined by histopathological examination. Results: Compared to the control group all therapy groups showed a drastically meaningful reduction in TNF-α increase in 1, 2, and 4 hours. Plasma and lung MDA, MPO, and sICAM-1 levels were significantly lower in IL, HBO, HBO+IL, and sham groups compared with the control group. IL and/or HBO suppressed MDA and MPO increase in the lung tissue and in plasma. Additionally, histopathological score was significantly lower in HBO, IL, HBO+IL, and sham groups than that of the control group. Conclusion: Both HBO and IL therapy have a beneficial effect by causing a meaningful reduction in TNF-α production, MPO, MDA, sICAM-1 levels and pulmonary neutrophil sequestration; which play a role, especially, in ischemia reperfusion induced lung damage.

        The effect of atropine in preventing catheter-related pain and discomfort in patients undergoing transurethral resection due to bladder tumor; prospective randomized, controlled study

        Background: Catheter-related bladder discomfort (CRBD) has been observed in many patients undergoing a urethral catheterization. CRBD may be so severe that the patients require additional analgesics. Muscarinic receptors are involved in the mechanism of CRBD. The aim of this study is to determine the effects of the anti-muscarinic properties of atropine, which is frequently used in current practice on CRBD, by comparing it with sugammadex which has no antimuscarinic effects.Methods: Sixty patients selected for transurethral resection due to bladder tumors were randomized into 2 groups: an atropine group and a sugammadex group, with no antimuscarinic effect. The patients were given rocuronium (0.6 mg/kg) as a neu-romuscular-blocker. In addition to the frequency and severity of CRBD postopera-tively at 0, 1, 6, 12, and 24 hours, postoperative numeric rating scale (NRS) scores, and postoperative nausea and vomiting were examined.Results: The incidence of CRBD was significantly lower in the atropine group in all postoperative measurements. The score was found to be significantly lower in the atropine group when NRS measurements were performed at all time periods (P < 0.01). There was no difference between the groups in terms of nausea and vomiting (P > 0.05).Conclusions: Atropine is a cheap, easy-to-access, safe-to-use drug for reducing CRBD symptoms, without any observed adverse effects. Since it not only reduces CRBD symptoms but also has a positive effect on postoperative pain, it can be used safely to increase patient comfort in patients receiving general anesthesia and a urinary catheter.

        Ballistic impact response of Kevlar Composites with filled epoxy matrix

        Impact resistance and weight are important features for ballistic materials. Kevlar fibres are the most widely reinforcement for military and civil systems due to its excellent impact resistance and high strength-to-weight ratio. Kevlar fibres or spectra fiber composites are used for designing personal body armour to avoid perforation. In this study, the ballistic impact behaviour of Kevlar/filled epoxy matrix is investigated. Three different fillers, nanoclay, nanocalcite and nanocarbon, were used in order to increase the ballistic impact performance of Kevlar-epoxy composite at lower weight. The filler, nanoclay and nanocalcite, content employed was 1 wt.% and 2 of the epoxy resin–hardener mixture while the nanocarbon were dispersed into the epoxy system in a 0.5%, 1% and 2% ratio in weight relating to the epoxy matrix. Specimens were produced by a hand lay-up process. The results obtained from ballistic impact experiments were discussed in terms of damage and perforation. The experimental tests revealed a number of damage mechanisms for composite laminated plates. In the ballistic impact test, it was observed whether the target was perforated completely penetrated at the back or not. The presence of small amounts of nanoclay and nanocalcite dispersed into the epoxy system improved the impact properties of the Kevlar/epoxy composites. The laminates manufactured with epoxy resin filled by 1 wt.% of nanoclay and 2 wt% nanocalcite showed the best performance in terms of ballistic performance. The addition of nanocarbon reduced ballistic performance of Kevlar-epoxy composites when compared the results obtained for laminates with 0% nanoparticles concentration.

        The Effect of Running Water Sound Listened to Patients During Urodynamics on Anxiety and Urodynamic Parameters

        Purpose: This study was conducted to determine the effect of listening to the sound of running water during urodynamics on the patient’s anxiety and parameters in the pressure-flow study. Methods: The population of the study, which was planned in the nonrandomized experimental study design, consisted of patients who will undergo urodynamics in the Urology Department of a city hospital in Istanbul between September 2022 and January 2023, and the sample consisted of 60 patients, 30 of which were in the experimental group and 30 in the control group. During the pressure-flow study, the patients in the experimental group listened to the sound of running water from a smartphone, while the patients in the control group did not undergo any intervention during urodynamics. The level of anxiety in both groups before, during and after urodynamics was evaluated with the visual analogue scale. During the pressure-flow study, it was evaluated whether the patients emptied on command, and the maximum flow rate (Qmax) and the detrusor pressure at the maximum flow rate (PdetQmax) were measured. Bladder outlet obstruction index (PdetQmax–2Qmax) and bladder contractility index (Pdetqmax+5Qmax) were calculated using these values. Results: During the pressure-flow study, in the experimental group patients who listened to the sound of running water from a smartphone; anxiety level mean scores during and after urodynamics were found to be statistically significantly lower than the control group patients (P<0.001). The mean bladder contractility index score in the experimental group patients was statistically significantly higher than the control group patients (P<0.001), and the cases of urinating with a catheter during the pressure-flow study were statistically significantly higher than the control group patients (P<0.001). Conclusions: Listening to the sound of running water during urodynamics had a positive effect on reducing anxiety in patients and micturating during pressure-flow study.

        Evaluation of biogeneric design techniques with CEREC CAD/CAM system

        PURPOSE. The aim of this study was to evaluate occlusal contacts generated by 3 different biogeneric design modes (individual (BI), copy (BC), reference (BR)) of CEREC software and to assess the designs subjectively. MATERIALS AND METHODS. Ten pairs of maxillary and mandibular casts were obtained from full dentate individuals. Gypsum cast contacts were quantified with articulating paper and digital impressions were taken. Then, all ceramic crown preparation was performed on the left first molar teeth and digital impressions of prepared teeth were made. BI, BC, and BR crowns were designed. Occlusal images of designs including occlusal contacts were superimposed on the gypsum cast images and corresponding contacts were determined. Three designs were evaluated by the students. RESULTS. The results of the study revealed that there was significant difference among the number of contacts of gypsum cast and digital models (P<.05). The comparison of the percentage of virtual contacts of three crown designs which were identical to the contacts of original gypsum cast revealed that BI and BR designs showed significantly higher percentages of identical contacts compared with BC design (P<.05). Subjective assessment revealed that students generally found BI designs and BR designs natural regarding naturalness of fissure morphology and cusp shape and cusp tip position. For general occlusal morphology, student groups generally found BI design "too strong" or "perfect", BC design "too weak", and BR design "perfect". CONCLUSION. On a prepared tooth, three different biogeneric design modes of a CAD/CAM software reveals different crown designs regarding occlusal contacts and morphology.

        Eco-Friendly, Thermal Resistant Cotton Fabric Enhanced with Nano Zinc Borate Powder

        The easy burning of cotton fabrics, which have the widest usage area among natural fibers, is a serious problem. Thus, treatment with an environmental friend flame retardant material is essential for cotton fabrics. Zinc borates are multifunctionalfire retardants. They can act as smoke suppressor, flame retardant and spark suppressor. Since they are non-toxicand environmental friend materials, they are ideal flame retardants for fabric treatment. In this study, nano-sized zinc borate(ZnO·3B2O3·7H2O) was synthesized and cotton fabric was treated with zinc borate powder with a novel method. Zinc boratesare slightly soluble in water. They were dissolved with aid of ammonia as zinc ammonia borate complexes and wererecrystalised on the cotton fabric fibers through the evaporation of ammonia. The burning resistance of treated cotton fabricwas investigated. The hardness, hydrophobicity, smoke suppression and burning resistance of untreated and treated cottonfabrics were compared. Since zinc borates have smoke suppression feature, a more slight smoke was observed and a higherrate of carbonization occurred during combustion test of treated cotton fabric compared to the untreated cotton fabric. Inaddition, the burning rate was reduced from 2.26 cm/s to 1.26 cm/s.
