Evolution of ALPPS: The Simpler, Safer and Effective One---TELPP

Aims: The characteristic of associating liver partition and portal vein ligation for staged hepatectomy(ALPPS) carries high mortality and morbidity. There is room for improvement. We suggest Terminal Branches Portal Vein Embolization (TBPVE) as a way to compart the liver. As a result, only a single surgical operation is required.This method is termed Terminal branches portal vein Embolization Liver Partition Planned hepatectomy (TELPP). Methods: Patients with unresectable primary or metastatic liver tumor were performed with TELPP. The procedure of TELPP was that in addition to PVE, embolization agent was infused to the terminal branches of portal vein of S5,S8 or S4. CT scan was taken one or two weeks later, and standard liver volume(SLV), FLR and FLR/SLV are calculated. Two weeks later when the FLR and liver function is appropriate, open or laparoscopic hepatectomy is performed. Results: The study included 11patients including hepatocellular carcinoma: n =8, intrahepatic cholangiocarcinoma: n = 1, hilarcholangiocarcinoma: n =1, colorectal liver metastasis: n =1. After a waiting period of 14 days, the volume of theFLR had increased from 382mlto 578ml, representing a median volume increase of 51% (range =32.5%-86.7%). Of the 11patients with hepatectomy, right hemihepatectomy (n=2), extended right hemihepatectomy (n=5), right trisecmentectomy(2), extended left hemihepatectomy (n=1) and left trisecmentectomy(1). No patient died, and no serve perioperative morbidity occurred. Conclusions: ALPPS and all modifications need two-stage operations with a high morbidity and mortality rate. It seems that TELPP is very promising. It has the merit of ALPPS as extraordinarily rapid increasement of FLRvolume, yet the morbidity and mortality is much lower, owing to the fact that unlike ALPPS, there is no two liver raw surfaces left behind in the abdominal cavity to produce bile leak, as only single surgical operation is required

Objective : Epidural haematoma (EDH) most commonly occurs in the supratentorial area, particularly in the temporal region, of the brain. Posterior fossa epidural haematoma (PFEDH) is less frequently observed, accounting for only 1.2% to 12.9% of all EDH cases. Because of the non-specific symptoms and the potential for rapid and fatal deterioration in children, an early computed tomography (CT) scanning is necessary for all suspicious cases. The aim of the present study was to share the experience of 48 cases and review the literature concerning PFEDH. Methods : A retrospective analysis was conducted for 48 paediatric cases diagnosed with PFEDH and admitted to Yuying Children's Hospital of Wenzhou Medical University from January 2010 to August 2015. The clinical features and outcomes were analyzed and compared with previous literature. Results : Seventeen patients were surgically treated in this series and 31 patients received non-operative treatment. The outcomes were good in 46 patients, evaluated using the Glasgow outcome score (GOS), while mild disability was observed in one patient, and only one case showed severe disability. There were no cases of mortality in this series. Conclusion : Posterior fossa epidural haematoma is relatively rare compared with supratentorial epidural haematoma. Early and serial CT scans should be performed for all suspicious cases. The criteria for the surgical treatment of paediatric patients with PFEDH were concluded. The overall prognosis was excellent in paediatric patients.

In the present study, an anthraquinone derivative, questinol was successfully isolated from the broth extract of the marine-derived fungus Eurotium amstelodami for the first time. The structure of questinol was determined based on the analysis of the MS and NMR spectral data as well as comparison of those data with the published data. Moreover, the anti-inflammatory effect of questinol in lipopolysaccharide (LPS)-stimulated RAW 264.7 cells was investigated. The results showed that questinol did not exhibit cytotoxicity in LPS-stimulated RAW 264.7 cells up to 200 μM. Questinol could significantly inhibit NO and PGE2 production at indicated concentrations. Questinol was also found to inhibit the production of pro-inflammatory cytokines, including TNF-α, IL-1β, and IL-6. Furthermore, the western blot analysis showed that questinol suppressed the expression level of iNOS in a dose-dependent manner. However, questinol could slightly inhibit the expression of COX-2 at the concentration of 200 μM. Therefore, our study suggests that questinol might be selected as a promising agent for the prevention and therapy of inflammatory disease.

In the introgression of the distinguished powdery mildew resistance from Haynaldia villosa to wheat, a focus task is to develop applicable molecular markers for tracing alien genetic substances and breeding selection. In an attempt to breed wheat breeding lines combining powdery mildew resistance of wheat stock 010714 conferred by alien 6V chromosome from H. villosa with thermo- photo-sensitive genic male sterility (TGMS) from wheat line C050S, a molecular marker RM874 was identified in PCR amplification using RAPD primer S2018. Batched sequencing indicated that the band contained large amounts of equal-length homologues amplified from Ty3-gypsy retrotransposon-like repetitive regions. Based on comparative cloning and bioinformatic analysis, it was converted into a more reproducible sequence characterized amplified region (SCAR) marker SM142. In wheat genetic background, the marker is 6V-specific and linked to powdery mildew resistance. The linkage of the marker is further verified in F5 generation. The approach of transforming repetitive regions-derived RAPD band into a specific SCAR marker can be referred for SCAR marker development of other genes. GISH (Genome in situ hybridization) analysis of 010714 and F7 translocation lines indicated that the resistance gene is conferred by the short arm of 6V.
Lack of Association Between GSTM1 and GSTT1 Polymorphisms and Brain Tumour Risk

Objective: Glutathione S-transferases (GSTs) are important enzymes that are involved in detoxification of environmental carcinogens. Molecular epidemiological studies have been conducted to investigate the association between GSTM1 and GSTT1 homozygous deletion polymorphisms and brain tumours but results have been conflicting. The aim of this study was to clarify this problem using a meta-analysis. Methods: A total of 9 records were identified by searching the PubMed and Embase databases. Fixed- and random-effects models were performed to estimate the pooled odds ratios. Results: No significant association was found between the GSTM1 and GSTT1 homozygous deletion polymorphisms and risk of brain tumours, including glioma and meningioma. Similar negative results were also observed in both population-based and hospital-based studies. Conclusion: These findings indicate that the GSTM1 and GSTT1 polymorphisms may not be related to the development of brain tumours.

<P>Summary: Novel hyperbranched poly(amine-ester) (HPAE) cross-linked films were prepared by cross-linking the terminal hydroxyl groups of HPAE using glutaraldehyde (GA). Atom force microscope and scanning electron microscope revealed their smooth surfaces, dense and homogenous matrices. Property characterizations indicated that these cross-linked films had good hydrophilicity, relative low protein adsorption, and high tensile strength. Objective: The aim of this retrospective study is to analyze the clinical and pathological factors related to the prognosis of Chinese patients with stage Ib to IIb cervical cancer. Methods and Results: 13 clinical pathological factors in 255 patients with stage Ib to IIb cervical cancer undergoing radical hysterectomy and systematic lymphadenectomy were analyzed to screen for factors related to prognosis. The cumulative 5-year survival of the 255 patients was 75.7%. The result of the univariate analysis suggested that clinical stage, cell differentiation, depth of cervical stromal invasion, parametrial tissue involvement, and lymph node metastasis were prognostic factors for patients with stage Ib to IIb cervical cancer (P<0.05). Compared with cases with involvement of iliac nodes, obturator nodes, or inguinal lymph nodes, cases with metastasis to the common iliac lymph nodes had a poorer prognosis (P<0.05). Cases with involvement of four or more lymph nodes had a poorer prognosis than those with involvement of three or fewer lymph nodes (P<0.05). Using multivariate Cox proportional hazards model regression analysis, non-squamous histological type, poor differentiation, parametrial tissue involvement, and outer 1/3 stromal invasion were found to be independently related to patients poor prognosis (P<0.05). Conclusion: Non-squamous histological type, poor cell differentiation, parametrial tissue involvement, and outer 1/3 stromal invasion are the independent poor prognostic factors for patients with stage Ib to IIb cervical cancer.
Fault Detection of Induction Motor Based on ALO Optimized TKSVDD

Failure of asynchronous motor will cause motor short circuit accident, personal electric shock and other hazards, so it is very important to detect its abnormalities during its operation. In order to solve the problems of low detection accuracy and inaccurate detection results in asynchronous motor detection, a fault detection method of asynchronous motor based on ant lion optimizer optimizes three kernel support vector data description (ALO-TKSVDD) is proposed in this paper. Firstly, for the current signal of asynchronous motor, stochastic resonance is used to improve the signal-to-noise ratio; Secondly, ant lion optimizer (ALO) is used to optimize the three kernel support vector data description (TKSVDD) to detect abnormal data of the target signal; Finally, the accuracy and feasibility of ALO-TKSVDD are verifi ed. Comparative experiments show that the asynchronous motor anomaly detection method proposed in this paper has the highest accuracy and the lowest false detection rate.
