      • Enhanced Aerodynamic and Multi-body Dynamic Analysis in Active Trailing-Edge Flap

        본 논문은 헬리콥터의 진동 하중을 줄이기 위한 능동형 뒷전 플랩이 적용된 블레이드의 축소 모델인 SNUF(Seoul National University Flap)에 관한 것으로 현재 작동기와 플랩에 대한 벤치 테스트가 진행 중이다. 힌지 모멘트를 계산하기 위하여 플랩의 작동 각도는 ±4°, 진동수는 0-65 Hz 범위에서 CFD(Computational Fluid Dynamics) 해석을 수행하였다. 이를 통해서 플랩에 공력과 관성력이 작용할 때 힌지 메커니즘을 고려한 작동기의 하중을 예측하였다. 또한 실제 로터가 회전할 때 길이 방향으로 블레이드에 작용하는 힘을 고려할 수 있도록 작동기와 플랩에 하중을 모사하여 적용한 실험이 계획되어 있다. This paper considers a small-scaled blade prototype equipped with an active flap-driving mechanism, denominated SNUF (Seoul National University Flap), aimed at dampening the vibratory load that helicopters experience. Specifically, CFD (Computational Fluid Dynamics) calculations are implemented in order to predict the hinge moment acting on the flap during its actuation in the expected oscillation range (±4°) and throughout the frequency range, namely 0-65 Hz. Also, analyses of the link mechanism were conducted to forecast the force required from the actuator to overcome the inertial and aerodynamic loads and properly actuate the device as desired. Future works include a pulling test whose objective is to verify the spanwise strength of the whole flap in a rotating environment as well as to calculate the contribution of the actuator to the overall centrifugal force.

        Thin and superthin perforator flap elevation based on preoperative planning with ultrahigh-frequency ultrasound

        The ability to directly harvest thin and superthin perforator flaps without jeopardizing their vascularity depends on knowledge of the microsurgical vascular anatomy of each perforator within the subcutaneous tissue up to the dermis. In this paper, we report our experience with ultrahigh-frequency ultrasound (UHF-US) in the preoperative planning of thin and superthin flaps. Between May 2017 and September 2018, perforators of seven patients were preoperatively evaluated by both ultrasound (using an 18-MHz linear probe) and UHF-US (using 48- and 70-MHz linear probes). Thin flaps (two cases) and superthin flaps (five cases) were elevated for the reconstruction of head and neck oncologic defects and lower limb traumatic defects. The mean flap size was 6.5×15 cm (range, 5×8 to 7.5×23 cm). No complications occurred, and all flaps survived completely. In all cases, we found 100% agreement between the preoperative UHF-US results and the intraoperative findings. The final reconstructive outcomes were considered satisfactory by both the surgeon and the patients. In conclusion, UHF-US was found to be very useful in the preoperative planning of thin and superthin free flaps, as it allows precise anticipation of very superficial microvascular anatomy. UHF-US may represent the next frontier in thin, superthin, and pure skin perforator flap design.

      • AERODYNAMIC ANALYSIS FOR A TWO-DIMENSIONAL AIRFOIL WITH A DEFLECTED FLAP

        본 논문에서 플랩에 작용하는 힌지 모멘트 값을 예측하기 위해 NACA 0015 익형을 분석하였다. 복잡한 유동장이 포함된 전산유체해석(CFD)이 우선적으로 수행되었다. 이러한 정적 해석은 압전 작동기가 외부의 공력에 견디면서, 플랩 변위 목표 값을 획득하기 위한 플랩 움직임이 가능한지를 평가하기 위해 수행되었다. 몇 가지 받음각에 대한 해석이 수행되었으며, 각각의 경우에 대한 힌지 모멘트 결과가 도출되었다. 그 결과, APA 1000L 작동기가 현재 비행조건에서 플랩 구동에 가장 적합한 모델로 선정되었다. In this paper NACA 0015 airfoil is analyzed in order to estimate the value of the hinge moment acting on the deflected flap. Due to the complexity of the flow field involved, CFD analysis was conducted to have a first estimate. This static analysis is carried on in order to evaluate the capability of the piezo-stack actuator to withstand external forces and thus drive the flap movement according to the deflection desired. Several angles of attack are considered and, per each case, hinge moments were extracted. Thus, a new actuator capable of driving the flap under the present flight condition was chosen.

        Contralateral Breast Symmetrisation in Immediate Prosthetic Breast Reconstruction after Unilateral Nipple-Sparing Mastectomy: The Tailored Reduction/Augmentation Mammaplasty

        Background In the literature on nipple-sparing mastectomy (NSM) with one-stage immediate implant reconstruction, contralateral symmetrisation has drawn little attention, with many surgeons still performing standard cosmetic mammaplasty procedures. However, standard implant-based mammaplasty usually does not result in proper symmetry with the mastectomy side, especially regarding breast projection, overall shape, and volume distribution. Methods We retrospectively reviewed 19 consecutive patients undergoing unilateral NSM with immediate prosthetic reconstruction and contralateral simultaneous symmetrisation by using the tailored reduction/augmentation mammaplasty technique between June 2012 and August 2013. Results The average follow-up time was 13 months (range, 10-24 months). No major complications, such as infection, haematoma, and nipple-areola complex necrosis, were experienced. Conclusions Our experience suggests that simultaneous contralateral symmetrisation with tailored reduction/augmentation mammaplasty after unilateral immediate implant reconstruction after NSM facilitates durable and pleasant symmetric outcomes.

        Fat Graft in Prosthetic and Autologous Breast Reconstruction

        Nowadays autologous fat transplant represents an effective cell therapy, due to the fact that a fraction of the lipoaspirate consists of adipose-derived adult stem cells. In this study we retrospectively report the two-year experience of 41 patients treated with autologous fat transplant to repair different breast- related problems. A postoperatively 3 months interval follow-up was performed in association to imaging follow-up, using ultrasound as first step. According to the grading scale, we report the results assisted by the plastic surgeons and the patients. A total of 68 breasts were treated in 65 operations. 38 patients were treated unilaterally and 15 patients bilaterally. In the follow- up we evaluate the clinical outcome using a grading scale consisted of 5 crescent grades, from 1 to 5, based on the patient’s satisfaction and on the clinical evidence of the achievement of the desired target. A percentage of 70.7% are classified with a grade over 3. Among 12 patients, one developed small liponecrotic lesion. We confirm that placing small aliquots of fat in the recipient site strongly reduce the phenomenon of liponecrosis on sonography. Thus, our experience confirms that this procedure is an easy, safe, effective and reliable surgical procedure also in the breast.

        Remote digital monitoring during the retention phase of orthodontic treatment: A prospective feasibility study

        Objective: To evaluate if a remote digital monitoring system added at the end of orthodontic treatment could positively influence the retention phase by reducing the occurrence of misfit of removable appliances, number of emergency appointments (EA), and orthodontic relapse. Methods: Twenty-seven patients who completed active orthodontic treatment were divided into the study and control groups. In addition to the standard chairside follow-up appointments at month 1 (T1), month 3 (T2), month 6 (T3), the study group patients were monitored using Dental Monitoring® with monthly intra-oral scans. Occurrence of misfit of removable retainers, number of EAs, and intercanine width change were recorded for both groups. Differences in EAs and retainer fit were assessed using the chi-square test. Intra-group and inter-group differences in the intercanine width were assessed with Friedman test and Mann–Whitney U test, respectively (α = 0.05). Results: The study group showed a significantly lower occurrence of misfit of removable retainers (p = 0.027) compared to the control group. No significant inter- and intra-group difference was found in the EAs and intercanine width change at each time-point. Conclusions: Integrating remote monitoring systems, such as Dental Monitoring®, to the retention phase of the orthodontic treatment may lower the occurrence of misfit of removable retainers. However, a small sample size and a short observation period limit the strength of this evidence. These preliminary results tentatively suggest that remote monitoring technologies may be beneficial, especially during the COVID-19 pandemic, when the regularity of in-office visits might be disrupted.

        Prevalence and factors associated with non-use of health services in the Peruvian population with COVID-19 symptomatology: a secondary analysis of the 2020 National Household Survey

        OBJECTIVES: The objective of this study was to estimate the prevalence of non-use of health services (NUHS) and its associated factors in Peruvians with symptoms of coronavirus disease 2019 (COVID-19). METHODS: A secondary analysis of the 2020 National Household Survey (ENAHO) was carried out. Participants over 18 years of age with any COVID-19 symptom (fever, cough, sensation of shortness of breath) in the last 4 weeks who did not visit health services were defined as exhibiting NUHS. Adjusted prevalence ratios (aPRs) were estimated to determine the factors associated with NUHS. RESULTS: Data from 1,856 participants were analyzed; the prevalence of NUHS was 52.2% (95% confidence interval [CI], 48.0 to 56.5). Living in urban areas of the jungle (aPR, 1.61; 95% CI, 1.32 to 1.98; p<0.001) and rural areas of the jungle (aPR, 1.48; 95% CI, 1.15 to 1.90; p=0.002) was associated with a higher probability of NUHS than living in urban coastal areas. The factors associated with a lower probability of NUHS were being 50-59 years old (aPR, 0.72; 95% CI, 0.58 to 0.90) and 60 years and over (aPR, 0.74; 95% CI, 0.59 to 0.95), having a secondary educational level (aPR, 0.67; 95% CI, 0.48 to 0.93) or superior educational level (aPR, 0.67; 95% CI, 0.48 to 0.96), and having health insurance (aPR, 0.79; 95% CI, 0.68 to 0.92). CONCLUSIONS: More than half of the participants with COVID-19 symptoms did not use health services, and NUHS was associated with the geographic and socio-demographic characteristics of the population. The formulation of health strategies and programs is required to increase the use of health services by people with COVID-19 symptoms.

        A System for Optimizing Fertilizer Dosing in Innovative Smart Fertigation Pipelines: Modeling, Construction, Testing and Control

        Smart fertigation is a topic of great interest in the eff ort to optimize different activities involved in local and extensive agriculture for assisting crops, optimizing production by using wireless technologies, data-processing electronic boards and sensors network. With the advent of Agriculture 4.0, similar to Industry 4.0, Information Communication Technology (ICT), associated with mechatronics, is giving an added value to this technique allowing optimization of water, fertilizers, control of water flow in pipes and period of irrigation. This paper intends to illustrate findings related to an innovative low cost system for assisting crops and achieving an accurate farming by investigating on the design, construction, testing and control of dosing system for liquid and granular fertilizers. Four different dosage systems have been designed, realized and tested with different granular and liquid fertilizers; the analysis of an extensive experimental campaign allows to define the characteristic and the mathematical expressions for each analyzed fertilizer and for each dosage system. The accurate modeling allows to control with extreme precision the realized dosing systems after estimating the quantity of fertilizer which the crop needs by means of the smart fertigation system. The obtained results permit the optimization of the fertilizer dosage in terms of quantity, which at the same time translates into lower production costs, greater environmental sustainability and optimization of production in terms of quantity and quality.
