The broad-specificity amino acid racemase (Bsar) from Pseudomonas putida catalyzes the racemization of various amino acids, offering a flexible and feasible platform to develop a new non-antibiotic selectable marker system for plant transformation. In the present study, we demonstrated that a Bsar variant, Bsar-R174K, that is useful as a selectable marker gene in Arabidopsis and rice that were susceptible to l-lysine and D-alanine. The introduction of wild-type Bsar, Bsar-R174K or Bsar-R174A into E. coli lysine or asparagine auxotrophs was able to rescue the growth of these microorganisms in minimal media supplemented with selectable amino acid enantiomers. The transformation of Arabidopsis with Bsar or Bsar variants based on d-alanine selection revealed that Bsar-R174K had the greatest efficiency (2.40%), superior to kanamycin selectionbased transformation (1.10%). Whereas, l-lysine-based selection exhibited lower efficiency for Bsar-R174K (0.17%). The progenies of selected Bsar-R174K transgenic Arabidopsis revealed normal growth properties. In addition, Bsar-R174K transgenic rice was obtained on l-lysine medium with an efficiency of 0.9%, and the progenies of the transgenic rice revealed morphologically normal phenotypes comparable with their wild-type counterparts. This study presents the first report of broad range amino acid racemase Bsar-R174K as a non-antibiotic selectable marker system applied in transgenic plants.

<P>An Arabidopsis thaliana mutant defective in chloroplast protein import was isolated and the mutant locus, cia5, identified by map-based cloning. CIA5 is a 21-kD integral membrane protein in the chloroplast inner envelope membrane with four predicted transmembrane domains, similar to another potential chloroplast inner membrane protein-conducting channel, At Tic20, and the mitochondrial inner membrane counterparts Tim17, Tim22, and Tim23. cia5 null mutants were albino and accumulated unprocessed precursor proteins. cia5 mutant chloroplasts were normal in targeting and binding of precursors to the chloroplast surface but were defective in protein translocation across the inner envelope membrane. Expression levels of CIA5 were comparable to those of major translocon components, such as At Tic110 and At Toc75, except during germination, at which stage At Tic20 was expressed at its highest level. A double mutant of cia5 At tic20-I had the same phenotype as the At tic20-I single mutant, suggesting that CIA5 and At Tic20 function similarly in chloroplast biogenesis, with At Tic20 functioning earlier in development. We renamed CIA5 as Arabidopsis Tic21 (At Tic21) and propose that it functions as part of the inner membrane protein-conducting channel and may be more important for later stages of leaf development.</P>

We conducted a three-stage genetic study to identify susceptibility loci for type 2 diabetes (T2D) in east Asian populations. We followed our stage 1 meta-analysis of eight T2D genome-wide association studies (6,952 cases with T2D and 11,865 controls) with a stage 2 in silico replication analysis (5,843 cases and 4,574 controls) and a stage 3 de novo replication analysis (12,284 cases and 13,172 controls). The combined analysis identified eight new T2D loci reaching genome-wide significance, which mapped in or near GLIS3, PEPD, FITM2-R3HDML-HNF4A, KCNK16, MAEA, GCC1-PAX4, PSMD6 and ZFAND3. GLIS3, which is involved in pancreatic beta cell development and insulin gene expression, is known for its association with fasting glucose levels. The evidence of an association with T2D for PEPD and HNF4A has been shown in previous studies. KCNK16 may regulate glucose-dependent insulin secretion in the pancreas. These findings, derived from an east Asian population, provide new perspectives on the etiology of T2D.

<P><B>Abstract</B></P> <P><B>Background and objectives</B></P> <P>Axillary lymph node (ALN) status is a key indicator in assessing and determining the treatment strategy for patients with newly diagnosed breast cancer. Previous studies suggest that sonographic features of a primary tumor have the potential to predict ALN status in the preoperative staging of breast cancer. In this study, a computer-aided prediction (CAP) model as well as the tumor features for ALN metastasis in breast cancers were developed using breast ultrasound (US) images.</P> <P><B>Methods</B></P> <P>A total of 249 malignant tumors were acquired from 247 female patients (ages 20–84 years; mean 55 ± 11 years) to test the differences between the non-metastatic (130) and metastatic (119) groups based on various features. After applying semi-automatic tumor segmentation, 69 quantitative features were extracted. The features included morphology and texture of tumors inside a ROI of breast US image. By the backward feature selection and linear logistic regression, the prediction model was constructed and established to estimate the likelihood of ALN metastasis for each sample collected.</P> <P><B>Results</B></P> <P>In the experiments, the texture features showed higher performance for predicting ALN metastasis compared to morphology (<I>Az</I>, 0.730 vs 0.667). The difference, however, was not statistically significant (<I>p</I>-values > 0.05). Combining the textural and morphological features, the accuracy, sensitivity, specificity, and <I>Az</I> value achieved 75.1% (187/249), 79.0% (94/119), 71.5% (93/130), and 0.757, respectively.</P> <P><B>Conclusions</B></P> <P>The proposed CAP model, which combines textural and morphological features of primary tumor, may be a useful method to determine the ALN status in patients with breast cancer.</P> <P><B>Highlights </B></P> <P> <UL> <LI> A computer-aided prediction (CAP) system contained automatic tumor segmentation and feature quantification on breast ultrasound was developed to evaluate the axillary lymph node (ALN) status in the preoperative staging with breast cancer. </LI> <LI> The proposed CAP model based on morphological and textural features of primary tumor would provide a promising diagnostic suggestion for recognizing the breast cancers as a case of ALN metastasis. </LI> <LI> In view of there has been a decrease in the patients presented with palpable ALN at diagnosis of breast cancer, a non-invasive examination like our CAP model may help physicians in the choice of the optimal axillary approach. </LI> </UL> </P>

Background: The aim of our study was to assess the practical utility of the palliative prognostic index (PPI) as a prognostic tool used by nurse specialists in a hospice consultation setting in Taiwan. Methods: In total, 623 terminal cancer patients under hospice consultation care from one medical center in northern Taiwan were enrolled between January 1 and June 30, 2011. PPI was assessed by a nurse specialist at first hospice consultation and patients categorized into groups by prognosis (good, intermediate, poor). Patient survival was analyzed retrospectively to determine significance of between-group differences. Results: By PPI sum score, 37.2% of patients were in the good prognosis group, 18% in the intermediate prognosis group and 44.8% in the poor prognosis group. The death rates were 56%, 81.2% and 89.6% and median survivals were 76, 18 and 7 days, respectively. The hazard ratio was 0.19 (95% confidence interval [CI] 0.10-0.24, p<0.001) for the poor versus good prognosis group and 0.54 (95% CI 0.43-0.69, p<0.001) for the poor versus intermediate prognosis group. The sensitivity and specificity for the poor prognosis group was 66% and 71%; the positive predictive value and negative predictive value were 81% and 52%, respectively, to predict patient death within 21 days (area under the curve of the receiver operating characteristic was 0.68). Conclusions: Assessment by PPI can accurately predict survival of terminal cancer patients receiving hospice consultation care. PPI is a simple tool and can be administered by nurse members of hospice consultation teams.
Polypoid Lesions of the Gallbladder: A Long-Term Follow-Up of 1204 Patients

Aims: Polypoid lesions of the gallbladder (PLG) are common, and most of them are benign. Few lesions are found malignant, but cannot be distinguished preoperatively by using common image modalities. Therefore, we compared characteristics of benign and malignant PLGs. Methods: We enrolled 1204 consecutive patients diagnosed with PLG at Taipei Veterans General Hospital during January 2004 to December 2013. Patients underwent either surgery or regular follow-up with various imaging modalities for at least 24 months. The mean follow-up duration was 72 ± 32 months. Results: Amoung our 1204 patients, 194 underwent surgical treatment and 1010 underwent regular follow-up. In addition, 73% patients were asymptomatic. The mean PLG size was 6.9 ± 7.7 (range, 0.8-129) mm; the PLGs of 337 patients (28%) grew during their follow-up periods. The majority of PLGs (90.4%) were single lesions, and 10.5% of patients had associated gallstones. The PLGs of 20.1% of surgical patients were malignant. Malignant PLGs were found in 32.4% of patients ≥50 years old and in 4.7% of those <50 years old (p < 0.001). Right quadrant abdominal pain, epigastric pain, and body weight loss were the three most common symptoms associated with malignancy. Malignant PLGs were significantly larger than benign lesions (means: 27.5 ± 18.4 mm vs. 12.3 ± 12.3 mm respectively, p < 0.001). Notably, the size of 5% of malignant PLGs was 3-5 mm, and that of 8% was 5-10 mm. The negative predictive value for gallbladder malignancy was 92.8% based on a size ≥10 mm and 100% based on a size ≥3 mm. Conclusions: Our study reassesses the PLG size that warrants more aggressive intervention. Cholecystectomy remains mandatory for PLGs >10 mm, but should also be considered a definitive diagnostic and treatment modality for PLGs with diameters of 3-10 mm.

This paper presents our new intelligent interactive robot, which is constructed to eagerly provide multi-functional services in an office environment. In order to endow a full interactive capability of our robots for realizing so-called human-robot interaction (HRI), we propose sensor fusion based human detection and tracking system and human pose estimation to deal with a number of situations which may take place in the office environment. Not only by these perceptions, human interact with the robot also by some natural way, such as touching the interface screen and talking with the robot through microphone. Finally, the effectiveness of the proposed work is tested and validated by some of experiments.
