Experimental Cyclic Behavior of Precast Hybrid Beam-Column Connections with Welded Components

Post-earthquake observations revealed that seismic performance of beam-column connections in precast concrete structures affect the overall response extensively. Seismic design of precast reinforced concrete structures requires improved beamcolumn connections to transfer reversed load effects between structural elements. In Turkey, hybrid beam-column connections with welded components have been applied extensively in precast concrete industry for decades. Beam bottom longitudinal rebars are welded to beam end plates while top longitudinal rebars are placed to designated gaps in joint panels before casting of topping concrete in this type of connections. The paper presents the major findings of an experimental test programme including one monolithic and five precast hybrid half scale specimens representing interior beam-column connections of a moment frame of high ductility level. The required welding area between beam bottom longitudinal rebars and beam-end plates were calculated based on welding coefficients considered as a test parameter. It is observed that the maximum strain developed in the beam bottom flexural reinforcement plays an important role in the overall behavior of the connections. Two additional specimens which include unbonded lengths on the longitudinal rebars to reduce that strain demands were also tested. Strength, stiffness and energy dissipation characteristics of test specimens were investigated with respect to test variables. Seismic performances of test specimens were evaluated by obtaining damage indices.

Background/Aims: Ustekinumab is effective in active Crohn’s disease. In a retrospective study, we assessed the clinical outcome in nonresponders to anti-tumor necrosis factor therapy, and/or conventional therapy and/or the α4β7-integrin inhibitor vedolizumab. As approval study populations do not always reflect the average “real world” patient cohort, we assessed weather patients who would not have qualified for approval studies show similar outcomes. Methods: Forty-one patients with mild to severe active Crohn’s disease were treated with ustekinumab (intravenous 6 mg per kg/body weight) followed by subcutaneous ustekinumab (90 mg) at week 8. Depending on the clinical response maintenance therapy was chosen every 8 or 12 weeks. Clinical response was defined by Crohn’s Disease Activity Index (CDAI) decline, decline of stool frequency or clinical improvement. Inclusion criteria for approval studies were assessed. Results: The 58.5% (24/41) showed clinical response to ustekinumab. The 58.3% of this group (14/24) achieved clinical remission. Clinical response correlated significantly with drop of stool frequency and improvement of CDAI score. The 39 out of 41 patients had no side effects and we observed no serious infections. About a third of our patients would not have met ustekinumab approval study criteria. However, patients who did not meet study criteria showed clinical improvement numerically in the same range compared to patients who would have qualified for approval studies. Conclusions: Ustekinumab is effective, safe and well tolerated in a highly therapy refractory patient cohort. Even though a reasonable number of patients did not meet ustekinumab approval study criteria, approval study results seem to be representative to the overall patient cohort.

Environmental biosensors and related techniques for monitoring organochlorines, endocrine disrupting chemicals and cyanobacterial toxins are described. The practical requirements for an ideal environmental biosensor are analyzed. Specific case studies for environmental applications are reported for triazines, chlorinated phenols, PCBs, microcystins, and endocrine disrupting chemicals. A new promising approach is reported for microcystins and alkylphenols that utilize electrooptical detection.
Lack of Prognostic Significance of C-erbB-2 Expression in Low- and High- grade Astrocytomas

Background: Astrocytic tumors, the most common primary glial tumors of the central nervous system, are classified from low to high grade according to the degree of anaplasia and presence of necrosis. Despite advances in therapeutic management of high grade astrocytic tumors, prognosis remains poor. In the present study, the frequency and prognostic significance of c-erb-B2 in astrocytic tumors was investigated. Materials and Methods: Records of 72 patients with low- and high-grade astrocytic tumors were evaluated. The expression of C-erbB-2 was determined immunohistochemically and intensity was recorded as 0 to 3+. Tumors with weak staining (1+) or no staining (0) were considered Her-2 negative, while tumors with moderate (2+) and strong (3+) staining were considered Her-2 positive. Results: Of the 72 patients, 41 (56.9%) had glioblastoma (GBM), 10 (13.9%) had diffuse astrocytoma, 15 (20.8%) had anaplastic astrocytoma, 6 (8.3%) had pilocytic astrocytoma. C-erbB-2 overexpression was detected in the tumor specimens of 17 patients (23.6%). Six (8.3%) tumors, all GBMs, exhibited strong staining, 2 (2.7%) specimens, both GBMs, exhibited moderate staining, and 9 specimens, 5 of them GBMs (12.5%), exhibited weak staining. No staining was observed in diffuse astrocytoma and pilocytic astrocytoma specimens. Median overall survival of patients with C-erbB-2 negative and C-erbB-2 positive tumors were 30 months (95%CI: 22.5-37.4 months) and 16.9 months (95%CI: 4.3-29.5 months), respectively (p=0.244). Conclusions: Although there was no difference in survival, C-erbB-2 overexpression was observed only in the GBM subtype.

Using panel data for the years 2006–2015, this study empirically investigates the effect of rule of law as an institution on Bangladesh’s exports to 28 European Union countries (EU28) based on gravity model analysis. Two-step econometric results suggest that institutional rule of law is strongly associated with Bangladesh’s exports, and institutional quality provides evidence of this significant effect. Furthermore, exports of Bangladesh are driven by economic size, market size, and the real exchange rate as well as rule of law. These results are suggestive of an important joint role for both trade and institutions in the long run.

Purpose Classification of focal skeleton/bone marrow uptake (BMU) can be challenging. The aim is to investigate whether anartificial intelligence–based method (AI), which highlights suspicious focal BMU, increases interobserver agreement amonga group of physicians from different hospitals classifying Hodgkin’s lymphoma (HL) patients staged with [18F]FDG PET/CT. Methods Forty-eight patients staged with [18F]FDG PET/CT at Sahlgenska University Hospital between 2017 and 2018 werereviewed twice, 6 months apart, regarding focal BMU. During the second time review, the 10 physicians also had access toAI-based advice regarding focal BMU. Results Each physician’s classifications were pairwise compared with the classifications made by all the other physicians,resulting in 45 unique pairs of comparisons both without and with AI advice. The agreement between the physicians increasedsignificantly when AI advice was available, which was measured as an increase in mean Kappa values from 0.51 (range0.25–0.80) without AI advice to 0.61 (range 0.19–0.94) with AI advice (p = 0.005). The majority of the physicians agreedwith the AI-based method in 40 (83%) of the 48 cases. Conclusion An AI-based method significantly increases interobserver agreement among physicians working at differenthospitals by highlighting suspicious focal BMU in HL patients staged with [18F]FDG PET/CT.

Lethargy in newborns usually indicates central nervous system dysfunction, and many conditions such as cerebrovascular events, infections, and metabolic diseases should be considered in the differential diagnosis. Nonketotic hyperglycinemia is an autosomal recessive error of glycine metabolism, characterized by myoclonic jerks, hypotonia, hiccups, apnea, and progressive lethargy that may progress to encephalopathy or even death. Cerebral sinovenous thrombosis is a rare condition with various clinical presentations such as seizures, cerebral edema, lethargy, and encephalopathy. Here, we report the case of a newborn infant who presented with progressive lethargy. An initial diagnosis of cerebral venous sinus thrombosis was followed by confirmation of the presence of nonketotic hyperglycinemia.
