        Computed Tomography Assessment of Intestinal Gas Volumes in Functional Gastrointestinal Disorders

        Background/Aims Many patients with functional gastrointestinal disorders (FGIDs) rank sensations of bloating and distension among their most debilitating symptoms. Previous studies that have examined intestinal gas volume (IGV) in patients with FGIDs have employed a variety of invasive and imaging techniques. These studies are limited by small numbers and have shown conflicting results. The aim of our study was to estimate, using CT of the abdomen and pelvis (CTAP), IGV in patients attending FGID clinic and to compare IGV in patients with and without FGID. Methods All CTAP (n = 312) performed on patients (n = 207) attending a specialized FGID clinic over 10-year period were included in this study. Patients were classified into one of 3 groups according to the established clinical grading system, as organic gastrointestinal disorder (OGID, ie, patients with an organic non-functional disorder, n = 84), FGID (n = 36) or organic and functional gastrointestinal disorder (OFGID, ie, patients with an organic and a functional disorder, n = 87). Two independent readers blinded to the diagnostic group calculated IGV using threshold based 3D region growing with OsiriX. Results Median IGVs for the FGID, OGID, and OFGID groups were 197.6, 220.6 and 155.0 mL, respectively. Stepwise linear regression revealed age at study, gender, and calculated body mass index to predict the log IGV with an r2 of 0.116, and P < 0.001. There was a significant positive correlation between age and IGV in OGID (Spearman’s = 0.253, P = 0.02) but this correlation was non-significant in the other groups. Conclusions Although bloating is a classic symptom in FGID patients, IGV may not be increased compared with OGID and OFGID patients. (J Neurogastroenterol Motil 2012,18:419-425)

      • Cardiorespiratory Fitness and Leisure Time Physical Activity are Low in Young Men with Elevated Symptoms of Attention Deficit Hyperactivity Disorder

        Objectives: The purpose of this study was to determine whether cardiorespiratory fitness and leisure time physical activity in men screening positive for ADHD were low compared to age- and sex-matched reference values. Methods: 30 college men screening for ADHD were recruited. Data were collected between October 2014 and April 2015. Maximal cycle exercise tests were performed to assess cardiorespiratory fitness and typical 7-day leisure time physical activity was self- reported. Results: Mean cardiorespiratory fitness was 0.94 SD lower than age- and sex-matched reference values. Cardiorespiratory fitness and ADHD symptoms were significantly negatively associated (r= -.39). Leisure time physical activity was 0.3 SD lower than a prior study of 411 college students. A linear regression showed a significant prediction of cardiorespiratory fitness from the combination of ADHD symptoms and leisure time physical activity. Conclusions: Men screening positive for ADHD was characterized by low cardiorespiratory fitness and leisure time physical activity.

        Variation in radial head fracture treatment recommendations in terrible triad injuries is not influenced by viewing two-dimensional computed tomography

        Background: We analyzed association between viewing two-dimensional computed tomography (2D CT) images in addition to radiographs with radial head treatment recommendations after accounting for patient and surgeon factors in a survey-based experiment. Methods: One hundred and fifty-four surgeons reviewed 15 patient scenarios with terrible triad fracture dislocations of the elbow. Surgeons were randomized to view either radiographs only or radiographs and 2D CT images. The scenarios randomized patient age, hand dominance, and occupation. For each scenario, surgeons were asked if they would recommend fixation or arthroplasty of the radial head. Multi-level logistic regression analysis identified variables associated with radial head treatment recommendations. Results: Reviewing 2D CT images in addition to radiographs had no statistical association with treatment recommendations. A higher likelihood of recommending prosthetic arthroplasty was associated with older patient age, patient occupation not requiring manual labor, surgeon practice location in the United States, practicing for five years or less, and the subspecialties “trauma” and “shoulder and elbow.” Conclusions: The results of this study suggest that in terrible triad injuries, the imaging appearance of radial head fractures has no measurable influence on treatment recommendations. Personal surgeon factors and patient demographic characteristics may have a larger role in surgical decision making.

        Short-Term Effects of Black Pepper (Piper nigrum) and Rosemary (Rosmarinus officinalis and Rosmarinus eriocalyx) on Sustained Attention and on Energy and Fatigue Mood States in Young Adults with Low Energy

        The purpose was to test whether a single dose of black pepper or rosemary produced short-term enhancements in sustained attention, motivation to perform cognitive tasks, or feelings of mental energy and fatigue. Outcomes were measured in 40 young adults with below average feelings of energy before and twice after they orally consumed capsules containing either black pepper (2.0 g), rosemary (1.7 g), or a placebo (3.1 g rice flour). Sustained attention was measured using a 16-min dual task, in which, single-digit numbers were presented every second on a screen and the participant performed both a primary task [detection of three successive, different odd digits] and a secondary task [detection of the number 6]. Feelings of energy and fatigue were measured using the vigor and fatigue subscales of the Profile of Mood States and visual analog scales (VAS). Analysis of variance showed nonsignificant condition (spice versus placebo) · time (T1, T2, & T3) effects for motivation, measured with a VAS, and the intensity of energy and fatigue feelings. Unadjusted effect sizes revealed that rosemary induced small, transient reductions in false alarm errors (d = 0.21) and mental fatigue (d = 0.40) at isolated time periods. Time-varying analysis of covariance, controlling for motivation to perform cognitive tasks, showed no significant effects on the primary or secondary task outcomes of correct responses (hits), errors (false alarms, misses), speed of response (reaction time), and signal detection sensitivity. It is concluded that black pepper and rosemary, consumed in a capsule form, in the doses used and while wearing a nose clip to block olfactory effects, do not induce consistent short-term improvements in sustained attention, motivation to perform cognitive tasks, or feelings of mental energy and fatigue in young adults with low energy.
