      • S-352 A case of breast metastasis from small cell lung cancer in male patient with gynecomastia

        Introduction:?Although primary breast cancer is one of the most common malignancies in adult females, breast metastases from extramammary primary malignancies are rare accounting for approximately 0.4-1.3%. The primary sites are usually melanoma, hematologic malignancies, and lung cancer. Only a few cases of breast metastases from lung cancer in males have been reported. Gynecomastia is also an infrequent manifestation of lung cancer. To our knowledge, this is the first report of breast metastasis from small cell lung cancer in male patient with gynecomastia and treatment with palliative radiotherapy.?Case: A 56-year-old male, 37 pack-years smoker, presented with cough for 2 months. A chest computed tomography (CT) scan showed lung mass in the right upper lobe. Bronchoscopic biopsy specimens revealed small cell carcinoma. Immunohistochemical stains were positive for CD56, synaptophysin and transcription factor-1 (TTF-1), and negative for chromogranin. A positron emission tomography-CT scan and a brain magnetic resonance imaging study found no other metastatic lesion. He was diagnosed with limited-stage small cell lung cancer and received 2 cycles of chemotherapy (cisplatin and etoposide) plus concurrent radiotherapy, followed by 4 cycles of chemotherapy and prophylactic cranial irradiation. He was asymptomatic for next 2 years with no evidence of recurrence. In a routine follow-up visit, he complained of bilateral gynecomastia. On physical examination, a well movable round nodule was palpable at right subareolar area. A mammography and a chest CT scan showed 10 cm sized nodule in the right breast. An ultrasound-guided core needle biopsy was performed. The needle biopsy specimens revealed small cell carcinoma. Immunohistochemical stains were positive for CD56, synaptophysin and TTF-1, and negative for chromogranin. The histologic features were consistent with those of the previous biopsy specimens of his lung cancer. He refused to receive another chemotherapy and was treated with palliative radiotherapy to the right breast. A total dose of 3000 cGy was delivered in 10 fractions. His right breast nodule had improved until his death from progressive disease 10 months after breast irradiation.

        In vitro anti-oxidant activity of the leaves of Dillenia indica

        The methanol extract of Dillenia indica was tested for antioxidant activity as determined by free radical scavenging of DPPH radical scavenging assay, reducing power, total antioxidant capacity measured by phosphomolybdenum method, total phenolic content and total flavonoids content determination assays. The extract showed significant activities in all antioxidant assays compared to the standard antioxidant in a dose dependent manner. In DPPH radical scavenging assay the IC50 value of the extract was found to be 100.53 μg/ml while ascorbic acid has the IC50 value 58.92 μg/ml. Dillenia indica extract showed strong reducing power and total antioxidant capacity. Moreover, methanol extracts also possess high amount of phenolics and flavovonoids and expressed as gallic acid and rutin equivalent respectively. The remarkable activities exhibited in reactive oxygen species scavenging may attributed to the high amount of hydrophilic phenolics present in Dillenia indica.         A successful management after preterm delivery in a patient with severe sepsis during third-trimester pregnancy

        A 33-year-old woman visited the emergency department presenting with fever and dyspnea. She was pregnant with gestational age of 31 weeks and 6 days. She had dysuria for 7 days, and fever and dyspnea for 1 day. The vital signs were as follows: blood pressure 110/70 mmHg, heart rate 118 beats/minute, respiratory rate 28/minute, body temperature 38.7℃, and oxygen saturation by pulse oximetry 84% during inhalation of 5 liters of oxygen by nasal prongs. Crackles were heard over both lung fields. There were no signs of uterine contractions. Chest X-ray and chest computed tomography scan showed multiple consolidations and air bronchograms in both lungs. According to urinalysis, there was pyuria and microscopic hematuria. She was diagnosed with community-acquired pneumonia and urinary tract infection (UTI) that progressed to severe sepsis and acute respiratory failure. We found extended-spectrum beta-lactamase producing Escherichia coli in the blood culture and methicillin-resistant Staphylococcus aureus in the sputum culture. The patient was transferred to the intensive care unit with administration of antibiotics and supplementation of high-flow oxygen. On hospital day 2, hypoxemia was aggravated. She underwent endotracheal intubation and mechanical ventilation. After 3 hours, fetal distress was suspected. Under 100% fraction of inspired oxygen, her oxygen partial pressure was 87 mmHg in the arterial blood. She developed acute kidney injury and thrombocytopenia. We diagnosed her with multi-organ failure due to severe sepsis. After an emergent cesarean section, pneumonia, UTI, and other organ failures gradually recovered. The patient and baby were discharged soon thereafter.

        Enhanced sunlight photocatalytic activity of silver nanoparticles decorated on reduced graphene oxide sheet

        A facile and straightforward method has been developed to synthesize silver nanoparticles decorated on reduced graphene oxide (RGO) nanosheets through hydrothermal reaction. The composite was characterized by XRD, UV-Visible spectroscopy, SEM and TEM techniques. In this synthesized RGO-Ag nanocomposite, the Ag nanoparticles size ranges 30-50 nm. Moreover, the RGO-Ag composites exhibited excellent photocatalytic activity towards the degradation of methylene blue (MB) in presence of sunlight. This photocatalytic reaction is completed within 20 min and the rate of reaction depends on the amount of RGO present in the nanocomposites.

        <P>Nitrogen-doped/undoped thermally reduced graphene oxide (N-rGO) decorated with CoMn<SUB>2</SUB>O<SUB>4</SUB> (CMO) nanoparticles were synthesized using a simple one-step hydrothermal method. The activity and stability of this hybrid catalyst were evaluated by preparing air electrodes with both primary and rechargeable zinc–air batteries that consume ambient air. Further, we investigated the relationship between the physical properties and the electrochemical results for hybrid electrodes at various cycles using X-ray diffraction, scanning electron microscopy, galvanodynamic charge–discharging and electrochemical impedance spectroscopy.         Optimization and characterization of extracellular cellulase produced by Bacillus pumilus MGB05 isolated from midgut of muga silkworm (Antheraea assamensis Helfer)

        Mature larvae of Antheraea assamensis were collected from different locations of Assam to isolate the cellulolytic gut microflora. Altogether sixty cellulase degrading bacteria were isolated on agar plates containing microcrystalline cellulose as the sole carbon source. Among them, ten isolates showed hydrolyzing zone on agar plates containing carboxy methyl cellulose (CMC) after staining with Congo-red. Isolate MGB05 exhibited the highest CMCase activity (0.262 U/mL) at 72 h of incubation under submerged condition. FPase and β-glucosidase activity were 0.012 U/mL and 3.71 U/mL respectively. It showed maximum FPase (0.022 U/mL) activity on the 3rd day of incubation in the media containing wheat bran as a carbon source. β-glucosidase production was also found to be highest with wheat bran (20.03 U/mL) at 48 h of incubation. The optimum pH and temperature of FPase activity of MGB05 were found at 6.0 and 50 °C respectively while for β-glucosidase activity, it was maximum at pH 6.0 under 50 °C. In addition, metal ion Mg ++ and Ca ++ enhanced FPase activity up to 110.92% (0.026 U/mL) and 105.31% (0.025 U/mL) respectively. In-vitro antimicrobial bioassay of the most potent cellulolytic bacteria (MGB05) also showed high antimicrobial activity against Escherichia coli (2.9 cm) and Pseudomonas aeruginosa (3.0 cm). The isolate MGB05 has been identified based on 16S rDNA homology as Bacillus pumilus MGB05 with accession KP298708.2. Results encompass the prospective beneficial role of gutmicroflora on digestion and disease resistance, which might be a potential probiotic component to enhance silk productivity.
