Fatigue and related factors after liver transplantation

Backgrounds/Aims: Fatigue is common in chronic hepatitis and end-stage liver disease. However, little is known about fatigue after liver transplantation (LT). We therefore evaluated the prevalence, severity, and related factors of fatigue after LT. Methods: We retrospectively reviewed adult recipients who responded to our survey at outpatient clinics between April and May 2013. Fatigue and its severity were assessed using a questionnaire with the Fatigue Severity Scale (FSS). We defined fatigue as FSS of 4.0 or more and severe fatigue as FSS of 5.1 or more. The related factors including hepatocellular carcinoma and complications were analyzed. Results: A total of 93 patients were included in this study. The mean age was 54.9 (19-76) years and two-thirds were men (67.7%). Living donor LT was 77.4%. Hepatitis B related liver disease was the main underlying disease (77.4%), with hepatocellular carcinoma accompanied in 33.3%. The mean follow-up period was 66.8±43.2 (2-171) months. The mean FFS was 2.83±1.48 (1.0-6.7) overall and 5.10±0.82 (4.0-6.7) in the fatigue group. Of the 93 adult patients, fatigue was presented in 20 patients (21.5%). Among these, 9 patients (45.0%) showed severe fatigue. Even though post-LT complications tended to be greater in the fatigue group (50.0% vs. 30.1% in the non-fatigue group, p=0.098), there were no significant related factors of fatigue after LT, including hepatocellular carcinoma and major complication. Conclusions: Fatigue is present in a considerable portion of recipients after LT, and almost half of them have severe fatigue. Further efforts are needed to decrease fatigue in LT recipients.
Aspergillus kawacchii 코지를 이용한 홍게(Chionoecetes japonicus) 어간장의 제조 및 품질변화

This study investigated changes in the quality of fermented red snow crab Chionoecetes japonicus saucewith or without Aspergillus kawachii koji and added salt. Samples were divided into four groups depending on whether koji was added and the amount of salt: RC15, 15% added salt, no koji; RC20, 20% added salt, no koji; RK15, 15% salt plus 10% koji; and RK20, 20% salt plus 10% koji. The samples were fermented at 20± ℃ for 4 months. During the fermentation period, the moisture contents of the four types of sauce decreased while the crude ash and protein contents increased. The pH of the RK groups decreased and was lower than in the RC groups. The acidity of the RK groups increased and was higher than in the RC groups. Both the total nitrogen (TN) and amino nitrogen (AN) levels increased continuously and were higher in the RK groups than in the RC groups. The volatile basic nitrogen (VBN) content increased rapidly and was higher in the RC groups than in the RK groups. The color did not differ significantly among the four groups. The viable cell counts in the four groups increased and no coliforms were detected. The total free amino acid and glutamic acid contents were highest in the RK15 group and the main amino acids in RK15 were aspartic acid, glutamic acid, alanine, leucine, phenylalanine, and lysine. Overall acceptance was significantly higher for the RK groups than the RC groups and RK15 ranked highest among the four sauces. These results suggest that Aspergillus kawachii koji is beneficial for processing fish sauce made using red snow crab.

<P><B>Abstract</B></P> <P>20 Hz pellet injection system was installed for improving a fueling system of KSTAR (Korea Superconducting Tokamak Advanced Research) in 2016. Several things of hardware such as a liquid helium transfer line, an extruder nozzle and an extension part of pellet guide tube are changed for 2017 campaign. For plasma experiments, the pellet injection system uses the smallest pellet (1.5 mm of length, 2 mm of diameter). The pellet velocity can be variable from 150 m/s to over 200 m/s by controlling of propellant gas pressure. The clear increase of plasma density observed during KSTAR experiments is also presented in this paper.</P> <P><B>Highlights</B></P> <P> <UL> <LI> The smallest pellet (1.5 mm of length, 2 mm of diameter) injection is successful. </LI> <LI> Slower pellet injection less than 150 m/s is achieved. </LI> <LI> Pellet trajectory is closer to plasma by straight extension part. </LI> </UL> </P>

This study investigated the effects of freezing storage temperature and thawing time on the separation of leg meat of the red snow crab Chionoecetes japonicus. Crabs were stored at -20, -30, -40, or -50°C for 2 days and thawed for either 5, 10, 20, 30, or 40 seconds. While thawing, there were no significant differences in pH or acidity among the experimental groups, while the volatile basic nitrogen content increased continuously. The redness of samples stored at -20°C was higher than that of the other groups. The overall acceptance of samples stored at -20°C was also the best. These results demonstrate that no-heating methods may be useful for separating red snow crab leg meat.

In this paper, performance simulator is developed for fuel cell hybrid electric vehicle(FCHEV) using ultra-capacitor. Dynamic models of powertrain components such as fuel cell stack, ultra-capacitor, traction motor, DC/DC converter, etc. are obtained by modular modeling using MATLAB SIMULINK. Using the simulator, hydrogen fuel economy and performance of FCHEV are evaluated for different energy storage device(Battery and Ultra-capacitor). Simulations are carried out for the power assist algorithm. It is found from the simulation results that using ultra-capacitor provides better performance especially in the braking energy recuperation and assist power than using battery.
연료전지 하이브리드 자동차의 ESU 용량 최적화에 대한 연구

In this paper, optimization of energy storage unit(ESU) sizing is investigated for a fuel cell hybrid electric vehicle(FCHEV) using genetic algorithm. The ESU optimization algorithm is proposed based on genetic algorithm, and performance simulator is developed for FCHEV. Using the simulator, hydrogen fuel economy and acceleration performance of FCHEV are evaluated. It is found from the simulation results that the optimized ESU provides improved hydrogen fuel economy and acceleration performance owing to the high power density characteristics of the ultra-capacitor in transient mode and high energy density characteristics of the battery in steady state, which enables the fuel cell stack operation in a relatively high efficiency region.

Backgrounds/Aims: Acute appendicitis is one of the most common emergent disease in the general population requiring surgical treatment. However, only a few cases of appendicitis after liver transplantation (LT) were reported. We described experiences of acute appendicitis after LT in single center. Methods: From March 1988 to July 2019, we reviewed retrospectively all the patients who diagnosed with acute appendicitis after LT at the Seoul National University Hospital. We described and analyzed clinical outcomes of appendectomy after LT. Results: A total of 12 patients out of 2,237 LT patients underwent appendectomy due to acute appendicitis. The mean age was 48±12 years. Nine patients (75%) underwent deceased donor liver transplantation (DDLT) and three patients (25%) underwent living donor liver transplantation (LDLT). The mean days from transplantation to the onset of acute appendicitis was 739 (range 57-4,496) days. Every patients underwent appropriate appendectomy within 1 days after visiting hospital. Leukocytosis was seen in three patients (25%). Seven patients (58%) underwent laparoscopic appendectomy and five patients (42%) underwent open appendectomy. The total operative time was 47.5 (range 25–135) minutes. The median postoperative hospitalization was 4 (range 2-11) days. There was no postoperative complications documented. There was no significant difference of clinical outcomes between laparoscopic appendectomy group and open appendectomy group. Conclusions: Early surgical management achieved satisfactory postoperative results without graft dysfunction after LT. Laparoscopic appendectomy also could be applicable with safe and feasible outcomes even in post-LT recipients.

Recently, as the frequency of natural disasters has increased, causing enormous human and material damage, the importance of managing disasters is emphasized. In particular, as the concept of Build Back Better becomes meaningful, it has appeared that considering it during Post-disaster Reconstruction is a necessity. Therefore, in this study, the relative importance evaluation and correlation analysis were performed for each influencing factor through the derivation of the influencing factors that should be considered during the reconstruction approach stage of reconstruction after disasters, and a survey of overseas disaster-related experts. As a result of the analysis, 'Adaptability to the environment changes in the future was derived as the most significant influencing factor. It seems that the results of this study can be used as basic data when carrying out the reconstruction project.
