An alpheid shrimp Betaeus gelasinifer Nomura and Komai, 2000 new to Korean waters is described and illustrated based on two specimens collected from Changseun and Ulleungdo Island, Korea. This is the first record of B. gelasinifer outside Japanese waters. B. gelasinifer is readily separated from B. granulimanus Yokoya, 1927, the only member of Betaeus from Korea by the morphological differences in the first pereopods and the diaeresis of uropod. Brief note on the armature of cutting edge of the chela of the left first pereopod in Korean female specimen of B. gelasinifer is provided.
청소년의 지진방재 의식조사에 관한 연구 : 고등학생을 중심으로

본 연구는 지진발생에 따른 국내·외 피해 및 학교시설의 피해사례를 분석하였다. 또한 많은 시간을 학교에서 생활하고 있는 고등학생을 대상으로 지진방재 의식조사를 실시하였다. 그 결과 대부분의 학생들이 지진에 대한 위험성은 알고 있으나, 대피방법 및 대피요령에 대한 세부적인 사항의 정보는 미숙하였다. 이러한 의식조사결과를 바탕으로 향후 교육시설의 지진방재대책을 마련하기위한 기초자료를 제시하였다.
Hip Fracture Management during the COVID-19 Pandemic in South Korea

Background: The purpose of this study was to introduce a screening system for coronavirus disease 2019 (COVID-19), to evaluate the overall orthopedic management in hip fracture patients during the COVID-19 pandemic in South Korea, and to compare the surgical results in hip fracture patients during the COVID-19 pandemic with those of the previous year. Methods: Hip fracture patients who visited emergency rooms were screened at the screening clinics before admission. The medical management was carried out with the medical staff wearing surgical masks, meticulous hand hygiene observed, and a minimum distance of 2 m between patients maintained. The demographics, operative parameters, and surgical results of patients treated during the pandemic were compared with those from the previous year. Results: From January 2020 to July 21, 2020, 119 patients with hip fractures (33 men and 86 women) were admitted to our institution for surgical treatment. Five patients showed symptoms of pneumonia, but no patient was positive for COVID-19. The mortality rate during the study period was 4.2%, and none of the patients died due to COVID-19. The interval between admission and surgery and the length of hospital stay were significantly shorter (p = 0.008, p = 0.002) and the proportion of spinal anesthesia was greater in hip fracture patients during the COVID-19 pandemic compared to those from the previous year (p = 0.011). Conclusions: The COVID-19 screening system for hip fracture patients has proven to be effective in preventing intrahospital spread of the disease. Hip fracture surgery performed during the COVID-19 pandemic has shown comparable results without any COVID-19 infection and COVID-19-related mortality.

<P>Endoplasmic reticulum (ER) stress transducers, such as old astrocyte specifically induced substance (OASIS) and activating transcription factor 6 (ATF6), which are induced by bone morphogenetic protein 2 (BMP2), regulate bone formation and osteoblast differentiation. Here, we examined the role of cAMP response element-binding protein H(CREBH), a member of the same family of ER membrane-bound basic leucine zipper (bZIP) transcription factors as OASIS and ATF6, in osteoblast differentiation and bone formation. Proinflammatory cytokine TNF alpha increased CREBH expression by up-regulating the nuclear factor-kappa B (NF-kappa B) signaling pathway in osteoblasts, increased the level of N-terminal fragment of CREBH in the nucleus, and inhibited BMP2 induction of osteoblast specific gene expression. Overexpression of CREBH suppressed BMP2-induced up-regulation of the osteogenic markers runt-related transcription factor 2 (Runx2), alkaline phosphatase (ALP), and osteocalcin (OC) in MC3T3-E1 cells and primary osteoblasts, as well as BMP2-induced ALP activity and OC protein production. In contrast, knockdown of CREBH attenuated the inhibitory effect of TNF alpha on BMP2-induced osteoblast differentiation. Mechanistic studies revealed that CREBH increased the expression of Smad ubiquitination regulatory factor 1 (Smurf1), leading to ubiquitin-dependent degradation of Smad1, whereas knock-down of CREBH inhibited TNF alpha-mediated degradation of Smad1 by Smurf1. Consistent with these in vitro findings, administration of Ad-CREBH inhibited BMP2-induced ectopic and orthotopic bone formation in vivo. Taken together, these results suggest that CREBH is a novel negative regulator of osteoblast differentiation and bone formation.</P>

목적: 대퇴 골두 연골하 스트레스 골절(subchondral stress fracture of the femoral head)은 드문 질환으로서 최근에 주로 부전 골절(insufficiency fracture)의 형태로 발생한 예들에 대해 보고되고 있다. 본 연구에서는 부전 골절 및 피로 골절(fatigue fracture) 형태로 발생한 스트레스 골절 환자의 임상적, 방사선학적 특정을 평가하였다. 대상 및 방법: 1998년 1월부터 2003년 4월 사이에 대퇴 골두 연골하 스트레스 골절로 진단 및 치료 받은 8명 10예를 대상으로, 이들의 임상적 방사선학적 소견에 대해 평균 33개월 추시 결과를 평가하였다. 결과: 피로 골절의 양상으로는 6명의 젊고 건강한 군인에게서 입대 후 5개월 이내에 발생하였고, 부전 골절의 양상으로는 1명의 노인과 1명의 골형성 부전증 환자에게 발생하였다. 1명을 제외한 모든 환자에서 대퇴 골두 무혈성 괴사의 위험 요소 및 뚜렷한 선행 외상의 과거력은 있지 않았다. 최초의 단순 방사선 사진상 6명 7예에서 이상 소견이 관찰되었다. 이 중 4예에서 대퇴 골두의 함몰 소견이 관찰되었다. 골 주사 검사상 대퇴 골두에 증가된 흡수 음영 소견이 관찰되었다. 자기 공명 촬영에서 대퇴 골두 연골하 이상 신호 선(MR crescent sign)과 전반적인 골수의 부종 소견이 관찰되었다. 결론: 대퇴 골두 연골하 스트레스 골절은 부전 골절 및 피로 골절의 양상으로 모두 나타날 수 있으며, 의심되는 환자의 경우 골 주사 검사 등을 통하여 조기 진단함으로써 대퇴 골두의 함몰을 방지할 수 있을 것이다. Purpose: Subchondral stress fracture of the femoral head is a rare condition that usually occurs as an insufficiency fracture in people with poor bone quality. This study evaluated the clinical and radiographic aspects of the subchondral stress fracture of the femoral head that occurred in the form of an insufficiency or a fatigue fracture. Materials and Methods: Between January 1998 and April 2003, 10 cases of the subchondral stress fracture of the femoral head in 8 patients were treated. The characteristics of this condition were determined by assessing the clinical course and findings of the radiographs, bone scintigram and MR images. Results: A fatigue fracture developed in 6 young healthy conscripts within 5 months after recruitment. An insufficiency fracture developed in one senile patient and in one known osteogenesis imperfecta patient. All patients except for one did not have any predisposing factors for osteonecrosis and antecedent trauma. On the initial radiographs, femoral head collapse was observed in 4 hips. Bone scintigraphs showed increased radionuclide uptake in the femoral head. The MR images demonstrated an subchondral abnormal signal intensity line (MR crescent sign) and a bone marrow edema pattern. Conclusion: A subchondral stress fracture of the femoral head can occur as a fatigue fracture as well as an insufficiency fracture. Bone scintigraphy is a valuable tool for screening patients suspected of having a subchondral stress fracture.
