      • A case of tinea incognito masquerading as photosensitivity

        Tinea incognito refers to dermatophyte infection with an atypical clinical presentation that have been modified by the previous treatment with steroid. Since tinea incognito has various manifestation rather than typical clinical presentations, the diagnosis of tinea incognito can be delayed or difficult. A 61-year-old man presented with ill-defined erythematous patches with telangiectasia which persisted on his face and neck for several months. The patient was first diagnosed as photosensitivity, and used topical steroid for 3 months. However, the lesion had not been improved. Therefore, potassium hydroxide (KOH) examination was performed on his peripheral neck lesion, and the result was positive. He also had onychomycosis for several years. Trichophyton rubrum was isolated with fungus culture and polymerase chain reaction (PCR). On the skin biopsy, hyphae were observed in the keratin layer. The patient was thus diagnosed with tinea incognito, and was prescribed oral and topical terbinafine. The patient was followed-up every month, and the lesion completely cured 2 months later. Herein, we report a case of tinea incognito mimicking photosensitivity in sun exposed area.

        The New Criteria of Dissolved Gas Analysis for Oil-Filled Transformers Using a Cumulative Distribution Function

        This paper presents new criteria for DGA (Dissolved Gases Analysis) using CDF (Cumulative Distribution Function) obtained from the data from the diagnosis of transformers operated in KEPCO over a period of 16 years. Because of differences in operating environments, construction type, oil volume, and other factors, the interpretative criteria of DGA at KEPCO differs from other standards such as IEC-60599, or Rogers and Doemenburg. To suggest the most appropriate criteria, the DGA data from transformers under normal conditions as well as from developing fault transformers were collected. Using these data, this study suggests the limitative gas level of transformers under normal operating conditions and verifies the suitability of the criteria. Because the application of this new criterion to transformers at KEPCO increases the detectable ratio of incipient faults and reduces unnecessary follow-up sampling and analysis, the new criteria yields a more reliable prediction of transformer condition.

      • A case of elephantiasis nostras verrucosa on great toe

        Elephantiasis nostras verrucosa is an uncommon disorder that is characterized by verrucous changes with papillomatosis and hyperkeratosis. The affected skin acquires a mossy or cobblestone-like appearance and discrete fibrous papules or plaques may develop. Recurrent cutaneous infections, tumor obstructions, previous surgeries, radiations, and obesity can lead to chronic lymphatic obstruction and edema. A 63-year-old man presented with non-pitting edema and hyperkeratotic warty plaque in left great toe for 1 month. The patient had been diagnosed with diabetes for 30 years and had a history of diabetic foot and recurrent localized infection on the left great toe for 5 years. He denied history of other trauma, radiation therapy, and travel outside of Korea before. Staphylococcus aureus was identified on the bacterial culture performed with discharge, and fungal culture was negative. The histopathological examination of the biopsy sample revealed hyperkeratosis, papillomatosis, and pseudoepitheliomatous hyperplasia of the epidermis. Diffuse fibrosis and dilated lymphatic channels are observed in the dermis. Periodic acid-Schiff, and acid fast bacilli stains were negative for fungal and mycobacterial organisms. Based on the clinical and histopathological findings, the patient was diagnosed with elephantiasis nostras verrucosa. The patient was treated with topical salicylic acid, and showed little improvement after 3 month.

      • Linear facial lichen sclerosus et atrophicus mimicking scleroderma ‘en Coup de Sabre’

        Localized scleroderma is a inflammatory disease which ultimately leads to a scar-like sclerosis. When linear scleroderma occurs on the forehead, it is referred to as scleroderma ‘en coup de sabre’, given the resemblance of the skin lesions to the stroke of a sabre. We now report an Korean patient who were clinically suspected of ‘en coup de sabre’, but the histologic results confirmed the diagnosis of lichen sclerosus et atrophicus (LSA). A 52-year-old man presented with a several months history of asymptomatic atrophied patch on forehead. He was otherwise healthy, and did not have a family history of autoimmune diseases. There was no past history of trauma. Physical examination revealed linear vertically distributed porcelain white atrophied patch on forehead. The genital, perianal and mucosal areas were normal. A skin biopsy from the lesion showed hyperkeratosis, thinning of the epidermis, loss of the rete ridges, focal basal cell vacuolization, hyalinized collagen of the papillary dermis along with a moderate lymphocytic infiltrate. Based on the clinical and histological findings, this case was diagnosed as linear LSA of the forehead. After the diagnosis was made, the patient was prescribed with topical diflucortolone valerate 0.3% and partial improvement was noted at the 4-months follow up.

      • Porokeratosis at the site of previous moxibustion: a case report

        Porokeratosis is characterized by annular keratotic rims. The histopathological finding that characterizes this condition is the cornoid lamella, which is a column of parakeratotic cells. It has been suggested that external stimuli, including immunosuppressive therapies, ultraviolet light, and trauma may serve as etiopathogenetic contributors. We report a case of porokeratosis occurring at the site of previous moxibustion. A 59-year-old man visited our clinic with an annular plaque on the right buttock. The plaque showed an atrophic center with a slightly elevated keratotic border. The patient revealed a history of undergoing moxibustion of the lower back and buttocks, 8 years prior to presentation. Unfortunately, he experienced a burn at the site of moxibustion, which was located on the right buttock, and developed a brownish plaque at the site of the burn. The patient was healthy and denied a family history of similar skin lesions at his initial visit. His histopathological examination revealed hyperkeratosis, acanthosis, and cornoid lamella in the epidermis without a granular layer beneath it. Based on the clinical and histopathological findings, the patient was diagnosed with porokeratosis and was treated with 3 cycles of 0.05% ingenol mebutate gel. The lesion decreased in size following 2 weeks of treatment. The residual lesion was treated with carbon dioxide laser, which led to the disappearance of the keratotic annular plaque.

        Two Species of the Genus Grapholita (Lepidoptera: Tortricidae: Oletherutinae: Grapholitini) New to Korea

        The genus Grapholita Treitschke, belonging to the tribe Grapholitini, comprises over 150 species worldwide with the majority occurring in the Holarctic region. However, in other regions, only few species are reported owing to a lack of collection and taxonomic study. The genus includes important agricultural pests worldwide that feed on several plants in the families Fabaceae and Rosaceae. In Korea, a comprehensive taxonomic study is required as only eight species have been reported to date. In this study, two species of Grapholita Treitschke, G. latericia Komai, 1999 and G. pallifrontana (Lienig & Zeller, 1846) are newly recorded from Korea. We provide redescribed and illustrated adult and available genitalia. Additionally, all available information, including collecting localities, distribution, host plants and biological characteristics is presented.

        Syringomas are relatively common and benign appendageal tumors derived from the intraepidermal ducts of eccrine glands. They occur predominantly in women, presenting at puberty or later in life. Syringomas typically appear as soft, skin-colored to yellow-brown papules on the eyelid, but rarely can arise on the skin in other areas such as scalp, forehead, cheeks, trunk, extremities, genitalia, and buttocks. Although eruptive syringomas may also involve the umbilicus, it is very rare for the lesions to be only limited to this region. Herein, we report a rare and interesting case of a patient with umbilical syringomas. A 25-year-old man presented with numerous quiescent, localized papules on his umbilicus, with a history of onset four years ago. The patient was otherwise asymptomatic and healthy. There was no family history of similar skin lesions. On physical examination, we observed multiple 1-2 mm sized, firm, light-brown colored papules on the umbilicu. The histopathological examination of the biopsy sample revealed multiple ductal and small cystic structures, dispersed within a fibrous connective tissue stroma in the upper dermis. The ductal and cystic spaces were lined by 1 to 2 rows of flattened epithelial cells. Some of the cysts with their small comma-like tails, resembled tadpoles. The patient was diagnosed with localized umbilical syringomas, however he refused further management.

      • A clinicohistopathological analysis of cutaneous fibrous histiocytomas of the finger

        Background: Cutaneous fibrous histiocytoma is a common, benign skin tumor predominantly occurring on the extremities or trunk. However, cutaneous fibrous histiocytoma on the finger is uncommon. Objectives: This study was undertaken to examine the clinicohistopathologic features of cutaneous fibrous histiocytoma of the finger. Methods: This is a retrospective study of 12 cutaneous fibrous histiocytomas located on fingers in a tertiary hospital in Korea. All case slides were retrieved from saved files for diagnostic verification. Results: Cutaneous fibrous histiocytomas of the finger affected 9 to 48 year-olds in a 1.4:1 male to female ratio. Picker’s nodule or wart was the most common clinical diagnosis. In only 2 out of the 12 cases was the pre-biopsy diagnosis of cutaneous fibrous histiocytoma ventured. Fibrocollagenous type was the most common histologic type. Majority of cases were mitotically inactive, exhibiting only 0-1 mitoses per 10 high power field and there was no recurrence. Tumor cells were uniformly CD34 negative. Conclusion: Because cutaneous fibrous histiocytoma can resemble malignancies including dermatofibrosarcoma protuberans, a lack of familiarity with the occurrence of cutaneous fibrous histiocytoma of the finger may lead to more aggressive treatment. Dermatologists should include cutaneous fibrous histiocytoma in their differential diagnosis of circumscribed nodules on the fingers to ensure proper management.

      • Safety and Efficacy of Besifovir in Treatment-Naive Chronic Hepatitis B Virus Infection: A Randomized, Double-Blind, Double Dummy, Phase 3 Study

        Aims: Besifovir is an acyclic nucleotide phosphonate known to be effective in hepatitis B virus (HBV) DNA suppression for both treatment- naïve and lamivudine-resistant chronic HBV infection in preliminary studies. We assessed the safety and efficacy of besifovir comparing with tenofovir in treatment-naïve chronic hepatitis B patients. Methods: A total of 187 patients were randomly received besifovir dipivoxil 150mg or tenofovir disoproxil fumarate 300mg. Eligible subjects were patients with chronic HBV infection. We measured the proportion of patients who had HBV DNA less than 69 IU/mL at week 48 as the primary efficacy endpoint. Key secondary endpoints were histological response (i.e., a reduction in the Knodell necroinflammation score of 2 or more points without worsening fibrosis), serum HBV DNA reduction, and liver function tests. Also, bone mineral density (BMD) and renal parameters were evaluated. Results: The proportion of patients who achieved primary endpoint of HBV DNA (< 69 IU/mL) at week 48 were 85.33% and 88.75% among those who received besifovir and tenofovir, respectively. Besifovir was shown to be non-inferior to tenofovir (lower limit of 95% CI for the treatment difference =-0.14). Histological improvement of 29 patients who underwent liver biopsy was evaluated, and we found that significantly more patients treated with besifovir had improved histological response than those treated with tenofovir (77.78% vs. 36.36%, p=0.0482). There was no difference in intrahepatic cccDNA reduction between the two groups (p=0.35). None of the patients had resistant to mutations or increase in serum creatinine >0.5mg/dL from baseline. Patients who received besifovir had smaller decrease in BMD during 48 weeks than that of tenofovir (besifovir -0.02±0.44, tenofovir -0.10±0.86, p=0.0248). There was no adverse drug reaction leading the patients to withdrawal. Conclusions: This phase 3 study demonstrated that besifovir had comparable efficacy and safety profile to tenofovir in the treatment of treatment-naïve chronic hepatitis B patients. Besifovir showed better profile than tenofovir in both histological response and bone loss. An open-label extension study is ongoing with besifovir to investigate long-term efficacy and safety.
