The results of the investigation of literature were summerized as follows ; 1. Information of voice, the pharynx, the larynx, the epiglottis, the uvula and the hyoid bone were concerned. 2. In disorders of voice and speech, Lung channel, Stomach channel, Spleen channel, Heart channel, Liver channel, Kidney channel, Im channel (任脈), and Chung channel (衝脈) were concerned. 3. The disorders of voice and speech were showed as follows ; aphonia, ashasia, seong-shi (?嘶), seom-eo(?語) kwang-eo (狂語), jeong-seong (鄭?), dok-eo (獨語) and chak-eo (錯語). 4. The cause of Aphonia and Aphasia were freauently as follows ; abnormal rising of Liver energy (肝邪暴逆), excessive heart fire (心火太過), deficiency of heart-blood (心血太虛), apoplexy of heart spleen (心脾俱中風), consumption of lung fluid caused by heat evil (肺津被灼), deficiency of lung energy (肺氣虛寒) and dificiency of kidney energy (腎虛). 5. The cause of seom-eo, kwang-eo, Jeong-seong were as follows ; the heart of stomach (胃中熱), the heat evil attach the blood chamber (血人血室) and the consumption of healthy energy (精氣奪). 6. In disorders of voice and speech, flaceid tong with aphasia (舌?) and aphasia due to throat disease (喉?) were divided.

<P>Obesity has become a major health threat in developed countries. However, current medications for obesity are limited because of their adverse effects. Interest in natural products for the treatment of obesity is thus rapidly growing. Korean Medicine (KM) is characterized by the wide use of herbal formulas. However, the combination rule of herbal formulas in KM lacks experimental evidence. According to <I>Shennong's Classic of Materia Medica</I>, the earliest book of herbal medicine, <I>Veratrum nigrum</I> (VN) has antagonistic features against <I>Panax ginseng</I> (PG), and the PG-VN pair is strictly forbidden. In this study, we have shown the effects of PG, VN, and their combination on obesity in high-fat (HF) diet-induced obese mice and in 3T3-L1 cells. PG, VN, and PG-VN combination significantly reduced weight gain and the fat pad weight in HF diet-induced obese mice. They also significantly decreased lipid accumulation and the expressions of two major adipogenesis factors, PPAR<I><I>γ</I></I> and C/EBP<I><I>α</I></I>, in 3T3-L1 cells. In addition, the PG-VN combination had synergistic effects compared with the mixture of extracts of PG and VN on inhibition of PPAR<I><I>γ</I></I> and C/EBP<I><I>α</I></I> expressions at lower doses. These results indicate a new potential anti-obese pharmacotherapy and also provide scientific evidence supporting the usage of herbal combinations instead of mixtures in KM.</P>
Characteristic Analysis of Claw-Pole Machine Using Improved Equivalent Magnetic Circuit

<P>Magnetic field analysis of claw-pole machines using improved equivalent magnetic circuit (EMC) is presented in this paper. On the basis of 3-D geometry, improved EMC, which consists of magneto motive-force (MMF) source and permeance considering main and leakage flux distribution, is composed and field analysis is performed. Nonlinear characteristics of magnetic core are considered for precise analysis results. Three-dimensional finite element analysis (FEA) is performed to verify calculated results and flux density in rotor and stator areas are compared. In addition, calculated and experimental back-EMF versus MMF of field and rotor speed is compared. From the verification with calculated and experimental results, it is proved that improved EMC compared with 3-D FEA provided reasonable results in initial design of claw-pole machine having asymmetric structure.</P>

<P>The aim of the present study was to describe a method of gastric lymphatic basin dissection for sentinel node biopsy using natural orifice transluminal endoscopic surgery with laparoscopic assistance (hybrid NOTES) in a porcine model. Lymph node dissection was performed in three healthy female domestic farm pigs (each around 40 kg) between October, 2007, and December, 2007. The pigs were administered a general anesthetic and laparoscopy-guided transvaginal colpotomy was performed. A two-channel endoscope was then inserted through the incision into the peritoneal cavity via the transvaginal route. An endoscope was inserted simultaneously into the mouth and indocyanine green solution was injected into the submucosal layer of the gastric wall at four sites. Dyed omentum and lymphatics were dissected using a laparoscopic dissector and the grasping forceps of a transvaginal endoscope. Lymphatics and omentum (mean 13.3 cm, range 8-20 cm) were removed transvaginally. The mean number of detected and resected sentinel nodes was 2.6 (range 1-4, diameter 2∼12 mm). Sentinel lymphatic basin dissection was performed successfully and without intraoperative complications in all three cases. Hybrid NOTES is technically feasible, and this procedure may represent an alternative to laparoscopic sentinel lymph node dissection of the stomach.</P>

<P> According to the advent of ubiquitous world, the requirement for the mobile information devices with high display quality and compact device size is surprisingly increased. Among the diverse candidates for this mobile display system for mobile devices, Liquid Crystal on Silicon (LCoS) is the most competitive device due to the high aperture ratio and simple fabrication process. The inorganic liquid crystal (LC) alignment layers are widely used for LCoS devices because of the thermal and photochemical stability. In this work, the reactive sputtering was selected for the preparation method of inorganic LC alignment layers. The nitrogen (N2) gas had the effect on the deposition process of SiO2 and the surface morphology of SiO2 thin layers were affected by the N2 mixing ratio of sputtering gas. In addition, the LC alignment properties on SiO2 thin layer were also closely related with the N2 mixing ratio and other sputtering conditions. In the case of high RF power of reactive sputtering, the N2 mixing ratio has little effect on the LC alignment on SiO2 thin layers. However, in the case of low RF power of reactive sputtering, the LC alignment properties were enfeebled by increasing the N2 mixing ratio. This result might be attributed to the change of the surface morphology of inorganic SiO2 thin layers.</P>
Novel role of Pin1 induction in type II collagen-mediated rheumatoid arthritis.

<P>Rheumatoid arthritis (RA) is an autoimmune disease characterized by chronic inflammation in joints and subsequent destruction of cartilage and bone. Inflammatory mediators such as PGs and proinflammatory cytokines contribute to RA progress. Pin1, a peptidyl prolyl isomerase, plays important pathophysiological roles in several diseases, including cancer and neurodegeneration. We found that both Pin1 and cyclooxygenase-2 (COX-2) were highly expressed in ankle tissues of type II collagen-induced RA mice. HTB-94 cells overexpressing Pin1 and primary cultured human chondrocytes showed increased basal expression of proinflammatory proteins (COX-2, inducible NO synthase, TNF-alpha, and IL-1beta). Site-directed mutagenesis revealed that Pin1-mediated transcriptional activation of COX-2 was coordinately regulated by NF-kappaB, CREB, and C/EBP. Gel shift, reporter gene, and Western blot analyses confirmed that NF-kappaB, CREB, and C/EBP were consistently activated in chondrocytes overexpressing Pin1. Treatment of RA mice with juglone, a chemical inhibitor of Pin1, significantly reduced RA progress and COX-2 expression in the ankle tissues. Moreover, juglone dose dependently decreased the basal COX-2 expression in primary cultured chondrocytes from RA patients. These results demonstrate that Pin1 induction during RA progress stimulates proinflammatory protein expression by activating NF-kappaB, CREB, and C/EBP, and suggest that Pin1 is a potential therapeutic target of RA.</P>

Role of Surgical Salvage for Regional Recurrence in Laryngeal Cancer

OBJECTIVES:: The aims of this study were to analyze the pattern of regional recurrence in laryngeal cancer, evaluate the role of surgical salvage, and identify factors affecting salvage outcome. METHODS:: Retrospective analysis was conducted on medical records from a 16-year period. Of 463 patients diagnosed with laryngeal cancer, 25 patients with regional recurrence managed with salvage neck dissection were identified and subject to study. Isolated local recurrences and all distant metastases were excluded. RESULTS:: All patients were male with a median age of 61 years. The overall rate of regional recurrence was 5.4%. Median time to regional recurrence was 13 months. Isolated regional recurrence occurred in 76% of cases, whereas locoregional recurrence occurred in 24%. A 5-year survival rate for patients undergoing neck dissection as salvage management was 61.2%. Patients with recurrence in the contralateral neck were definitely associated with poor prognosis. Although standard statistical significance was not met, trends for poorer salvage result were identified in patients with a history of local recurrence before regional recurrence, recurrence in a previously dissected neck, and recurred node size of 3 cm or above. CONCLUSIONS:: Our study shows that salvage neck dissection for regional recurrence in laryngeal cancer is an acceptable approach. Surgical eradication of disease should be warranted whenever possible. Prudent planning of management is mandatory in the presence of history of local recurrence before regional recurrence, previously dissected neck, large size of recurrent node, and contralateral neck recurrence.
