Lipoblastoma of the Neck : A case Report and Reminding of Diagnostic Clues

Lipoblastoma is a rare benign tumor that occurs in infancy or early childhood. We experience a case of lipoblastoma in the anterior neck of 22-months-old boy. Computerized tomographic images revealed a low enhanced mass pushing the adjacent tissues. Fine needle aspiration cytology suggested a lipogenic tumor. A well-circumscribed mass was excised. Histopathologic examination showed mainly mature adipocytes and some immature fat cells including lipoblasts with the myxoid stroma and numerous fine capillaries complexly interconnected. Diagnosing this tumor was difficult because we had to be reminded of some important things that are often ignored.

<P><B>Background—</B></P><P>The aim of this study is to explore the differential effect of 3-dimensional color Doppler echocardiography for the quantification of mitral regurgitation (MR). Two-dimensional color Doppler echocardiography–based MR quantification has well-documented limitations.</P><P><B>Methods and Results—</B></P><P>We consecutively enrolled 221 patients with MR. Adequate image quality was obtained by 2D- and 3D-color Doppler echocardiography in 211 (95.5%) patients. The quantitative differences between the MR volumes obtained by 2D- and 3D-proximal isovelocity surface area (PISA) were analyzed in various MR subgroups. In the validation cohort (n=52), MR volume obtained by 3D-PISA showed a better agreement with phase-contrast cardiac MRI than 2D-PISA (<I>r</I>=0.97 versus 0.84). In all 211 patients, 2D-PISA underestimated the MR volume when compared with 3D-PISA (52.4±19.6 versus 59.5±25.6 mL; <I>P</I>=0.005). A total of 33.3% with severe MR based on 3D-PISA were incorrectly assessed by 2D-PISA as having nonsevere MR. In the subgroup analysis, the MR severity (odds ratio, 6.96; 95% confidence interval, 3.04–15.94; <I>P</I><0.001) and having an asymmetrical orifice (odds ratio, 11.48; 95% confidence interval, 3.72–35.4; <I>P</I><0.001), and an eccentric jet (odds ratio, 3.82; 95% confidence interval, 1.27–11.48; <I>P</I>=0.017) were predictors of significant difference in MR volume (>15 mL) between 2D- and 3D-PISA methods.</P><P><B>Conclusions—</B></P><P>Quantification of MR by 3D-PISA method is clinically feasible and more accurate than the current 2D-PISA method. MR quantification by 2D-PISA significantly underestimated MR volume with severe, eccentric MR with an asymmetrical orifice. This article demonstrates that 3D-color Doppler echocardiography could be used as a valuable tool to confirm treatment strategy in patients with significant MR.</P>
${\alpha}$-Glucosidase Inhibitiors from Seed Extract of Paeonia lactiflora

Alpha-glucosidase inhibitors are oral antihyperglycemic agents that act on alpha-glucosidase competitively, delaying intestinal carbohydrate absorption and lessening postprandial increases in glucose levels. In the search for new classes of ${\alpha}$-glucosidase inhibitors extracted from natural resources, 420 different plant extracts were evaluated for their inhibitory effect on ${\alpha}$-glucosidase, in vitro. Amongst the tested extracts, the seed extract of Paeonia lactiflora (Paeoniaceae) demonstrated a significant degree of inhibition on the enzyme, according to dose. Extensive bioassay-guided fractionation of the extract resulted in the isolation of six materials, luteolin (1), resveratrol (2), trans-${\varepsilon}$-viniferin (3), gnetin H (4), suffruticosol A (5) and suffruticosol B (6), that are active agents for ${\alpha}$-glucosidase inhibition. Resveratrol (2) is a well-known naturally occurring hydroxystilbene, particularly abundant in wine and some related stilbene oligomers. Materials (3-6) exhibited relatively strong inhibition of ${\alpha}$-glucosidase and their $IC_{50}$ values were calculated as: 0.92mM (2), 3.15mM (3), 1.15 mM (4), 2.53mM (5) and 3.15mM (6). In contrast, the ($IC_{50}$) value of the reference drug, acarbose, was estimated as 8.28mM in vitro.

A variety of benzylidenethiazole analogs have been demonstrated to inhibit 5-lipoxygenase (5-LOX). Here we report the anti-atherogenic potential of 5-(4-hydroxy-2,3,5-trimethylbenzylidene) thiazolidin-2,4-dione (HMB-TZD), a benzylidenethiazole analog, and its potential mechanism of action in LDL receptor-deficient ($Ldlr^{-/-}$) mice. HMB-TZD Treatment reduced leukotriene $B_4$ ($LTB_4$) production significantly in RAW264.7 macrophages and SVEC4-10 endothelial cells. Macrophages or endothelial cells pre-incubated with HMB-TZD for 2 h and then stimulated with lipopolysaccharide or tumor necrosis factor-alpha (TNF-${\alpha}$) displayed reduced cytokine production. Also, HMB-TZD reduced cell migration and adhesion in accordance with decreased proinflammatory molecule production $in$ $vitro$ and $ex$ $vivo$. HMB-TZD treatment of 8-week-old male $Ldlr^{-/-}$ mice resulted in significantly reduced atherosclerotic lesions without a change to plasma lipid profiles. Moreover, aortic expression of pro-atherogenic molecules involved in the recruitment of monocytes to the aortic wall, including TNF-${\alpha}$, MCP-1, and VCAM-1, was downregulated. HMB-TZD also reduced macrophage infiltration into atherosclerotic lesions. In conclusion, HMB-TZD ameliorates atherosclerotic lesion formation possibly by reducing the expression of proinflammatory molecules and monocyte/macrophage recruitment to the lesion. These results suggest that HMB-TZD, and benzylidenethiazole analogs in general, may have therapeutic potential as treatments for atherosclerosis.
10-Gb/s direct modulation of polymer-based tunable external cavity lasers.

<P>We demonstrate a directly-modulated 10-Gb/s tunable external cavity laser (ECL) fabricated by using a polymer Bragg reflector and a high-speed superluminescent diode (SLD). The tuning range and output power of this ECL are measured to be >11 nm and 2.6 mW (@ 100 mA), respectively. We directly modulate this laser at 10 Gb/s and transmit the modulated signal over 20 km of standard single-mode fiber. The power penalty is measured to be <2.8 dB at the bit-error rate (BER) of 10(-10).</P>
Hydrogen Peroxide- and Nitric Oxide-mediated Disease Control of Bacterial Wilt in Tomato Plants

Reactive oxygen species (ROS) generation in tomato plants by Ralstonia solanacearum infection and the role of hydrogen peroxide ($H_2O_2$) and nitric oxide in tomato bacterial wilt control were demonstrated. During disease development of tomato bacterial wilt, accumulation of superoxide anion ($O_2{^-}$) and $H_2O_2$ was observed and lipid peroxidation also occurred in the tomato leaf tissues. High doses of $H_2O_2$ and sodium nitroprusside (SNP) nitric oxide donor showed phytotoxicity to detached tomato leaves 1 day after petiole feeding showing reduced fresh weight. Both $H_2O_2$ and SNP have in vitro antibacterial activities against R. solanacearum in a dose-dependent manner, as well as plant protection in detached tomato leaves against bacterial wilt by $10^6$ and $10^7$ cfu/ml of R. solanacearum. $H_2O_2$- and SNP-mediated protection was also evaluated in pots using soil-drench treatment with the bacterial inoculation, and relative 'area under the disease progressive curve (AUDPC)' was calculated to compare disease protection by $H_2O_2$ and/or SNP with untreated control. Neither $H_2O_2$ nor SNP protect the tomato seedlings from the bacterial wilt, but $H_2O_2$ + SNP mixture significantly decreased disease severity with reduced relative AUDPC. These results suggest that $H_2O_2$ and SNP could be used together to control bacterial wilt in tomato plants as bactericidal agents.

<P>Modification of the electronic structure and lattice contraction of Pt alloy nanocatalysts through control over their morphology and composition has been a crucial issue for improving their electrocatalytic oxygen reduction reaction (ORR) activity. In the present work, we synthesized PtCo alloy nanocubes with controlled compositions (Pt<SUB><I>x</I></SUB>Co NCs, <I>x</I> = 2, 3, 5, 7, and 9) by regulating the ratio of surfactants and the amount of Co precursor to elucidate the effect of the composition of nanocatalysts on their ORR activity. Pt<SUB><I>x</I></SUB>Co NCs had a Pt-skin structure after electrochemical treatment. The electrocatalysis experiments revealed a strong correlation between ORR activity and Co composition. Pt<SUB>3</SUB>Co NCs exhibited the best ORR performance among the various Pt<SUB><I>x</I></SUB>Co NCs. From density functional theory calculations, a typical volcano-type relationship was established between ORR activity and oxygen binding energy (<I>E</I><SUB>OB</SUB>) on NC surfaces, which showed that Pt<SUB>3</SUB>Co NCs had the optimal <I>E</I><SUB>OB</SUB> to achieve the maximum ORR activity. Background: Sugammadex is a specific antagonist of aminosteroidal neuromuscular blocking agents with 1:1 binding to guest molecules. Sugammadex can also bind to other drugs having a steroid component in its chemical structure. In this in vivo experiment, we investigated the differences in the recovery of rocuronium-induced neuromuscular blockade using sugammadex pre-exposed with two different concentrations of hydrocortisone.Methods: The sciatic nerves and tibialis anterior muscles of 30 adult Sprague–Dawley rats were prepared for the experiment. The sciatic nerves were stimulated using a train-of-four (TOF) pattern with indirect supramaximal stimulation at 20 s intervals. After 15 min of stabilization, a 250 μg loading dose and 125 μg booster doses of rocuronium were serially administered until > 95% depression of the first twitch tension of TOF stimulation (T1) was confirmed. The study drugs were prepared by mixing sugamadex with the same volume of three different stock solutions (0.9% normal saline, 10 mg/ml hydrocortisone, and 100 mg/ml hydrocortisone). The recovery of rats from neuromuscular blockade was monitored by assessing T1 and the TOF ratio (TOFR) simultaneously until T1 was recovered to > 95% and TOFR to > 0.9.Results: In the group injected with sugammadex premixed with a high concentration of hydrocortisone, statistically significant intergroup differences were observed in the recovery progression of T1 and TOFR (P < 0.050).Conclusions: When sugammadex was pre-exposed to a high dose of hydrocortisone only, recovery from neuromuscular blockade was delayed. Delayed recovery from neuromuscular blockade is not always plausible when sugammadex is pre-exposed to steroidal drugs.
