목적: 복강 내 세포검사는 몇몇 연구에서 복강 내 전이를 예측할 수 있는 유용한 방법으로 알려져 있으며, 7판 AJCC staging에서는 복강 세포검사가 양성으로 나오면 다른 인자를 배제하고 4기로 구분한다. 이번 논문에서는 위암 환자에서의 복강 내 암세포의 존재가 가지는 임상적 그리고 예후적 가치에 대해 논하겠다. 대상 및 방법: 위절제술을 시행한 125명의 위암환자에서 복강내 세포 검사를 시행하였으며, 이 환자군을 복강 내 세포 검사 양성과 음성의 두 군으로 나누었다. 결과: 125명의 환자 중 16명(12.8%)이 양성으로 나왔으며, 이 군에서 T, N 그리고 M stage에서 높은 결과를 나타내었으며, 또한 이 군에서 미분화암의 빈도가 높은 것으로 나타났다. 세포 내 검사 양성과 음성, 두 군의 전체 생존율의 비교에서는 통계적으로 유의한 차이를 나타내지는 못하였으나(p =0.106), 질병 외 생존율을 비교해보면 복강 내 세포 검사 음성군에서 질병 외 생존율이 짧은 것으로 나타났다(p <0.001). 복강 내 세포 검사 양성군 중에서 전이가 없는 군과 세포 검사와 상관 없이 전이가 있는 군을 비교해 보면, 차이가 통계적으로 유의하지 않은 것을 볼 수 있다(p =0.076). 결론: 복강 내 세포검사는 암의 진행뿐만 아니라 수술 후 암의 재발을 예측할 수 있는 마커로 사용될 수 있으나, 이 마커가 생존과 재발을 나타낼 수 있는 독립적인 영향력이 있는지는 추후의 연구가 필요하다. Purpose: Peritoneal cytology has been reported to be a useful tool for the detection of peritoneal dissemination in several studies, and in the 7th edition of the AJCC staging, positive peritoneal cytology is classified as metastatic disease. This study evaluated the presence of free cancer cells through the peritoneal cytology in gastric cancer patients and analyzed their clinical and prognostic values. Materials and Methods: Total 125 gastric cancer patients who underwent gastrectomy with intraoperative peritoneal cytology were enrolled in this study, and they were categorized into two groups; cytology negative (Cy-) and cytology positive (Cy+). Results: 16 of total 125 patients (12.8%) were Cy+, and they had more advanced T, N, and M stages, as well as histologically undifferentiated carcinomas. The difference of the overall survival rates between Cy- and Cy+ group was not significant (p =0.106), but the disease free survival rates of patients with Cy+ were significantly shorter than those with Cy- (p <0.001). The survival curves of patients with positive cytology but no gross metastasis (Cy+/M0) showed no significant difference, compared to those with gross metastasis (M1) (p =0.076). Conclusions: Peritoneal lavage cytology may be a useful marker of tumor progression and a strong predictor of recurrence in gastric cancer patients. However, it should be questioned whether this novel marker is an independent prognosticator in survival and recurrence.
Untranslatable Coat Protein RNA - Mediated Potato Virus Y Resistance in Tobacco Plants

Total viral RNA was purified from potato virus Y-necrotic strain (PVY-VN) which was isolated from Nicotiana tabacum var. Burley 21. Coat protein (CP) cDNA fragment which has starting and termination codons, was synthesized by RT-PCR. PVY CP cDNA was manipulated to untranslatable form by megaprimer PCR method and confirmed by DNA sequencing. The untranslatable CP cDNA was inserted to plant expression vector (pPEV-6) and was transformed to Nicotiana tabacum cv. NC82 by Agrobacterium-mediated transformation. The untranslatable PVY CP cDNA transformed plants were regenerated on MS media containing 100 ppm kanamycin. Highly resistant plants to the PVY were selected based on symptom development after mechanical inoculation in a greenhouse. By the genomic PCR and Southern blot hybridization, it was confirmed that the untranslatable CP cDNA fragments were integrated into genome of transformed tobacco plants which showed high resistance to PVY-VN with one to three copies of the transgene.

Objective: To investigate changes in the median nerve, retinaculum, and carpal tunnel on ultrasound after successful endoscopic carpal tunnel release (ECTR). Materials and Methods: This prospective study involved 37 wrists in 35 patients (5 male, 30 female; mean age ± standard deviation [SD], 56.9 ± 6.7 years) with primary carpal tunnel syndrome (CTS). An in-house developed scoring system (0–3) was used to gauge the clinical improvement after ECTR. Ultrasound was performed before ECTR, and at 1, 3, and 12 months post-ECTR. Changes in the median nerve, flexor retinaculum, and carpal tunnel morphology on ultrasound after ECTR were analyzed. Ultrasound parameters for different clinical improvement groups were compared. Results: All patients improved clinically after ECTR. The average clinical improvement score ± SD at 12 months post-ECTR was 2.2 ± 0.7. The median nerve cross-sectional area proximal and distal to the tunnel decreased at all time intervals post- ECTR but remained swollen compared to normal values. Serial changes in the median nerve caliber and retinacular bowing after ECTR were more pronounced at the tunnel outlet than at the tunnel inlet. The flexor retinaculum had reformed in 25 (68%) of 37 wrists after 12 months. Conclusion: Postoperative changes in median nerve and retinaculum parameters were most pronounced at the tunnel outlet. Even in patients with clinical improvement after ECTR, nearly all ultrasound parameters remain abnormal at one year post- ECTR. These ultrasound parameters should not necessarily be relied upon to diagnose persistent CTS after ECTR.
