Endovascular Closure Resolves Trimethylaminuria Caused by Congenital Portosystemic Shunts

This study aimed to report three new cases of an association between two rare conditions, congenital portosystemic shunts (CPSS) and trimethylaminuria (TMAU), and the efficacy of endovascular closure of the CPSS for resolving TMAU. Between November 2014 and April 2017, 15 patients with CPSS were enrolled in this prospective study to assess the efficacy of percutaneous endovascular shunt closure. Three patients presented with clinical symptoms of TMAU that were confirmed by urine analysis of trimethylamine (TMA) and TMA n-oxide. One year after endovascular closure of the congenital portosystemic shunt, the same parameters were evaluated were obtained and the values were compared to the pretreatment values. The results indicated the disappearance of clinical symptoms of TMAU and normalization of the urine test parameters in two patients and no changes in one patient, who developed new portosystemic communications.

Statistical Analysis of a Subjective QoE Assessment for VVoIP Applications

A successful deployment of multimedia applications over wireless environments entails improving the quality of service (QoS), not only from a technical point of view, but also considering the quality of experience (QoE) from the final user′s perception. Although objective QoE measure models avoid the difficulties of subjective surveys, subjective QoE assessments are essential to understand the way users evaluate the QoS. In this work, we study the effect of a wide range of parameters on the QoE of VVoIP applications in a real wireless scenario. Through a complete statistical analysis of users′ ratings, we identify the following facts. Although the use of VVoIP in wireless networks does not yet represent an advantage for users, there are great expectations for all applications under study, and with greater popularity comes higher expectations. It is easier for respondents to identify good behavior than poor behavior. Whereas the respondents′ frequency of Internet use does not impact on the scores, respondents′ gender does. Finally, the most determining parameters of quality from a user′s perspective were instability, video quality, voice distortion, usefulness, and graphical interface.

Experimental design for optimization of olive mill wastewater final purification with Dowex Marathon C and Amberlite IRA-67 ion exchange resins

The final purification of two-phase olive mill wastewater (OMW-2) by ion exchange (IE) was addressed. This pollutant effluent was previously subjected to Pseudo-Fenton secondary treatment. Consequently,OMW-2 presented high concentrations of chloride, sodium, phenols and iron, which hinder its discharge. Experimental design was used to optimize the IE process. Around 88% removal efficiencies of thesepollutants were achieved when initial pH, temperature and flow rate were 5.1, 26.8 8C and 12.1 L h 1,respectively. Finally, 28 L treated water could be accomplished in continuous mode under optimalconditions, complying with standards for reuse in the olive oil production process.

Cerebral palsy is one of the most serious forms of disorders of the psychophysical development of children, which manifests itself in disturbances of motor functions, which are often combined with speech disorders, other complications of the formation of higher mental functions, and often with a decrease in intelligence. The article will discuss the speech disorder in children with cerebral palsy. Emphasis is placed on some important aspects, which should bear in mind, investigating the problem of specifics of speech development of children with cerebral palsy. In particular at the heart of speech disorders in the cerebral palsy is not only damage to certain structures of the brain, but also the later formation or underdevelopment of those parts of the cerebral cortex, which are of major importance in linguistic and mental activity. This is an ontogenetically young region of the cerebral cortex, which is most rapidly developing after birth (premotor, frontal, temmono-temporal). It is important to take into account, that children with cerebral palsy have disturbances of phonemic perception. Often, children do not distinguish between hearing sounds, cannot repeat component rows, allocate sounds in words. At dysarthria, there are violations of pronunciation of vowel and consonant sounds, tempo of speech, modulation of voice, breathing, phonation, as well as asynchronous breathing, alignment and articulation. As a result, we identified the main features and specifics of the speech development of children with cerebral palsy and described the conditions necessary for the full development of language. Language disturbances in children's cerebral palsy depend on the localization and severity of brain damage. Great importance in the mechanism of speech disorders has a pathology that limits the ability of movement and knowledge of the world.

Endovascular Treatment of Congenital Portosystemic Shunt: A Single-Center Prospective Study

Purpose: To design a prospective study on endovascular closure of congenital portosystemic shunts. The primary endpoint was to assess the safety of endovascular closure. The secondary endpoint was to evaluate the clinical, analytical and imaging outcomes of treatment. Methods: Fifteen patients (age range: 2 days to 21 years; 10 male) were referred to our center due to congenital portosystemic shunts. The following data were collected prior to treatment: age, sex, medical history, clinical and analytical data, urine trimethylaminuria, abdominal-US, and body-CT. The following data were collected at the time of intervention: anatomical and hemodynamic characteristics of the shunts, device used, and closure success. The following data were collected at various post-intervention time points: during hospital stay (to confirm shunt closure and detect complications) and at one year after (for clinical, analytical, and imaging purposes). Results: The treatment was successful in 12 participants, migration of the device was observed in two, while acute splanchnic thrombosis was observed in one. Off-label devices were used in attempting to close the side-to-side shunts, and success was achieved using Amplatzer™ Ductus-Occluder and Amplatzer™ Muscular-Vascular-Septal-Defect-Occluder. The main changes were: increased prothrombin activity (p=0.043); decreased AST, ALT, GGT, and bilirubin (p=0.007, p=0.056, p=0.036, p=0.013); thrombocytopenia resolution (p=0.131); expansion of portal veins (p=0.005); normalization of Doppler portal flow (100%); regression of liver nodules (p=0.001); ammonia normalization (p=0.003); and disappearance of trimethylaminuria (p=0.285). Conclusion: Endovascular closure is effective. Our results support the indication of endovascular closure for side-to-side shunts and for cases of congenital absence of portal vein.

Phytochemical Profile, Toxicity, and Pharmacological Potential of Peels from Four Species of Tropical Fruits

Tropical fruit peels are generally discarded as waste, yet they contain bioactive substances that could have various uses; in addition, their pharmacological potential remains unexplored. This study aims to characterize the phytochemical profile, toxicity, and pharmacological potential of methanol extracts obtained from the peels of the following tropical fruit species: Annona squamosa L. (purple sugar apple), Annona reticulata L. (custard apple), Chrysophyllum cainito L. (green star apple), and Melicoccus bijugatus Jacq. (mamoncillo). Methanol peel extracts were obtained by maceration. All extracts contained flavonoids, anthraquinones, and triterpenoids as determined by colorimetric methods. A. squamosa and C. cainito exhibited the highest content of total phenols as assayed by the Folin–Ciocalteu method. M. bijugatus showed the highest content of total sugars (fructose, glucose, and sucrose) as determined by high-performance liquid chromatography. A. squamosa and C. cainito presented the highest antioxidant capacities (according to 2,2′-diphenyl-1-picrylhydrazyl, 2,2′-azinobis(3-ethylbenzothiazoline−6-sulfonic acid, and cupric reducing antioxidant capacity assays), displayed moderate toxicity against HCT-116 cells, and increased the vinblastine susceptibility of MCF-7/Vin+. A. squamosa and M. bijugatus extracts demonstrated modulation of acetylcholinesterase activity, whereas those of A. reticulata showed anti-inflammatory activity by inhibiting protein denaturation. These results confirm that tropical fruit peels can be valuable sources of bioactive compounds, and our findings provide new information about their pharmacologic potential so that they can be used as raw material for the development of new drugs aimed at treating a variety of ailments.

El estudio de la historia se entiende aquí como un diálogo, una conversación entre diversas comunidades relacionadas entre sí; entre comunidades de nuestro pasado y las que coexisten en el presente desde donde se redescribe y discute la historia, el pasado que se busca comprender y potenciar de manera que se proyecte al futuro. Para que el diálogo sea posible, sin embargo, hay que elaborar una situación hermenéutica que permita, al ser consciente de los propios prejuicios, ponerlos a prueba, ampliar el horizonte e ir al encuentro del otro. Puestas en este marco, sólo una de las dos obras estudiadas participa de esta manera de entender la historia. Con la dosis de objetividad que permite el ser consciente de que le es parte del campo que se estudia, el autor de Pancho Villa encuentra en ese otro que fuera uno de los protagonistas de la Revolución Mexicana, a un interlocutor con el cual intercambiar la palabra que somos. En Cien años de confusión, en cambio, el autor concibe la historia como un monólogo; la modernidad, paradigma y prejuicio que respalda la obra, lleva en sí la imposibilidad del diálogo: la figura moderna del sujeto no encuentra en la historia otra cosa que no sea objeto.

The mediterranean diet’s effect on stroke risk

The Mediterranean diet has been growing in popularity as preventive medicine has made way in the healthcare community. Doctors are telling patients to adopt this diet due to its role in lowering stroke rates and other cardiovascular diseases in both Italy and Greece. These two countries have some of the lowest stroke numbers in the world, and experts believe this is the result of the diet and lifestyle of the people of that region. The diet includes foods such as fruits, vegetables, nuts, fish, and olive oil, along with less cholesterol, saturated fats, and trans fats. These specific foods and combinations of foods include properties that can prevent high blood pressure. We will explore the capabilities of the Mediterranean diet in preventing stroke, and the importance of preventive medicine in healthcare.
